Abstract

Health is one of the basic human needs and is a right for every citizen that is protected by law. The form of the State's efforts in providing health services is by launching the National Health Insurance Program (JKN) through the Social Security Administration Agency. In practice, many cases experience a deficit related to costs incurred by the Hospital and costs obtained through INA-CBG's claims. The purpose of this study was to see if there were any adverse differences in the Total Knee Replacement case carried out at Siti Fatimah Hospital. This research was quantitative, descriptive research in hospital. Data collection was a total of 19 patients suffering from knee Osteoarthritis and who had to do Total Knee Replacement at Siti Fatimah Hospital from 2020-2023. The results of the study found that in every case of Total Knee Replacement the hospital always experienced a deficit. In average national healthcare system only reimbursed half of our cost. Further evaluation of INA-CBGs reimbursement scheme is necessary to ensure hospital to achieve ideal cost recovery when patients are treated.

Abstract

dr. Iskak Tulungagung hospital made a healthcare service innovation during Covid-19 pandemic. This innovation called as hospital without wall to reduce the Covid-19 transmission. This program provides overall health services integrating pre-, intra-, inter-, and post-hospital care services through the Public Safety Center (PSC) platform. Authors examine the implementation of a hospital without walls in post-hospital aspect by using Donabedian's Model to improve service quality. The independent variables are facility and waiting time. The dependent variable is satisfaction. The intervening variable is patient perception. Assessment using a Likert scale questionnaire with a value range of 1-5. Descriptive data analysis used SPSS 25 and multivariate analysis used SMARTPLS 3.0. Our 96 respondents are very satisfied with this service but had to wait for a long time until the nurse arrived at home. Our analysis showed that facility variable affects the waiting time so that is affects patient satisfaction. This program has good structure so it makes a good process and satisfaction. Waiting time significantly associated with facility.

Abstract

Mitra Delima Hospital as one of the health facilities in providing its services is required to carry out the management and development of SIMRS in accordance with Minister of Health Regulation Number 82 of 2013 with the aim of supporting and improving the health service process at the hospital. The aim of this research is to analyze user satisfaction of the Hospital Management Information System (SIMRS) using the End User Computing Satisfaction (EUCS) method at Mitra Delima Hospital. This type of quantitative research, the research sample used was 35 health workers at Mitra Delima Hospital. The instrument uses a questionnaire and the analysis technique uses linier regression analysis. The research results showed that some respondents were very satisfied with the use of the Hospital Management Information System (SIMRS) using the End User Computing Satisfaction (EUCS) method at Mitra Delima Hospital. The regression analysis showed that content (p value 0,001) and timeliness (p value 0,03) influence the user satisfaction,  while the other three which is accuracy, appearance and ease of use have no influence on user satisfaction. In the simultaneously analysis showed that there is an influence between all independents variables to users satisfaction (p value 0,00; Rsquare  0,935)at Mitra Delima Hospital. It can be concluded that overall respondents are satisfied with the use of the Hospital Management Information System (SIMRS) using the End User Computing Satisfaction (EUCS) method at Mitra Delima Hospital. Mitra Delima Hospital can maintain and increase the level of satisfaction with using SIMRS using the EUCS method, where it can provide additional training to officers in using SIMRS.

Abstract

The World Health Organization estimates that in 2030, there will be a shortage of nurses, especially in developing countries, due to job dissatisfaction. Human resources in hospitals are dominated by nurses, so it is important to pay attention to nurse job satisfaction. Diponegoro National Hospital must focus on nurse job satisfaction because there has been  an increase in nurses' resignations over last three years (2019–2021). The study was conducted to determine the factors associated with job satisfaction among nurses at the Diponegoro National Hospital. In this cross-sectional study, 97 nurses at Diponegoro Hospital were surveyed using a total sampling technique. Univariate analysis was presented with a frequency distribution table and bivariate analysis was based on chi-square statistics. The results showed there was a relationship between job satisfaction and awards (p = 0.010), promotion opportunities (p = 0.003), and hospital policies (p = 0.042). There was no association between job satisfaction and compensation (p = 0.527), interpersonal relationships (p = 0.057), or job security (p = 0.093). In conclusion, it was found that awards, promotions, and hospital policies are related to job satisfaction among nurses at Diponegoro National Hospital. It is expected that there will be improvements in the nurse reward program, improvements in the promotion system, evaluating promotion policy socialization, providing training and education needed by nurses for career advancement, evaluating the policy socialization system, evaluating the compensation system, and efforts to minimize interpersonal conflicts in the hospital.

Abstract

According to Law Number 36 of 2009 concerning Health, Article 5, paragraph 2 states that everyone has an equal right to access resources in the health sector and obtain safe, quality, and affordable health services. Health services in Primary Health Care involve two parties, so Assessing the quality of service should involve both the perspective of the Primary health care and the patients. This study aims to measure the aspects most needed by the community in outpatient health services at Pprimary Health Care using service quality indicators. The study uses a quantitative descriptive approach with a factor analysis design. The study was conducted at the Kamaipura Main Primary Health Care, Sigi Regency, Central Sulawesi Province, in 2022. The variables in this study include tangibility, responsiveness, reliability, assurance, and empathy. Data collection was carried out through observations, with the population and sample consisting of all patients who used outpatient services at the Kamaipura Primary Health Care totaling 228 people. Outpatients have two levels of needs divided into two components. Variables are grouped into components based on the highest correlation. The first component consists of variables such as reliability, assurance, tangible, and responsiveness. Variables in the first component can be prioritized as follows: 1) Responsiveness, 2) Tangible, 3) Assurance, 4) Reliability. In conclusion All aspects of healthcare services, such as Reliability, Assurance, Tangible, Empathy, and Responsiveness, are primary needs for outpatient patients. The analysis indicates that Responsiveness, Tangible, Assurance, and Reliability need improvement.

Abstract

The Sibela Surakarta Health Center has used the Management Information System of Health Center (SIMPUS) which is integrated in all sections as a flow of patient services starting from registration, polyclinic, laboratory, and pharmacy. During its implementation, SIMPUS at the Sibela Surakarta Health Center experienced various problems in data processing, one of which was a delay in the data input process to the computer caused by delays from program holders to SIMPUS officers or network delays. The purpose of this study was to analyze the satisfaction of health workers with SIMPUS at the Sibela Surakarta Health Center using the E-Servqual method. This research is a quantitative descriptive study with a cross-sectional research design using the E-Servqual method. The number of research subjects was 31 people, using quota sampling, and interviews with satisfaction questionnaires. Health workers were very satisfied with efficiency (gap value=0.01), not satisfied with reliability (gap value= -0.14), satisfied with compliance (gap value=0.00), very satisfied with privacy (gap value=0.02), dissatisfied with power responsive (gap value=-0.09), and dissatisfied with the contact (gap value=-0.08).Keywords: Satisfaction, Health Workers, SIMPUS.

Abstract

Evaluation of user satisfaction of Electronic Medical Record (EMR) system users can be considered by hospitals in developing EMR systems.  Tugurejo Hospital has implemented EMR both outpatient and inpatient. Quantitative research is conducted to determine user satisfaction. The respondents were 67 users of EMR from doctors and nurses. EMR user satisfaction according to Delone and McLean's IT/IS Success Model theory was analyzed using SMART-PLS to determine how strong the influence of system quality, information quality and service quality on EMR user satisfaction. The results showed that user satisfaction was influenced by system quality (T-stat = 2.96), information quality (T-stat = 1.66), service quality (T-stat = 1.84). There is a mutual influence between system quality, information quality and service quality on user satisfaction with strong categories (R-square = 0.748).

Abstract

The strategy to reduce the high infant mortality rate (IMR) is by placing midwives in the village. One of the tasks of village midwives is the prevention of complications through early detection, which is indicated by the performance of midwives based on neonatal coverage. Situation awareness is an important component of decision making that has a significant impact on the performance of the village midwife's duties, so the purpose of this study was to analyze the effect of the role of situation awareness on the performance of the village midwife's duties in detecting problems in young infants. Quantitative research was designed using a cross sectional study. All village midwives became the population and were determined based on the results of a sample calculation of 151 village midwives who would be taken proportionally. The research variables were situation awareness and task performance. The instrument used a questionnaire and analyzed using Simple Linear Regression. The results explained that the consistent value of task performance was 43.041. B situation awareness value (regression coefficient) 0.917. The regression coefficient is positive that the direction of the influence of situation awareness on task performance is positive. Decision making in simple linear regression is 0.000 <0.05 with the t value showing 8.256> from the t table of 1.97, which means that situation awareness has an influence on the performance of the village midwife's duties.  Situation awareness is a predictor that is able to influence task performance for village midwives in providing care to neonates in early detection of problems in young infants.

Abstract

Job satisfaction is a positive emotional condition as a result of job appraisal. Spirituality possessed by a doctor is an inner condition that grows because it interprets work in his life. This study aims to examine and explain the relationship between job satisfaction and physician performance, which is moderated by spirituality. From the questionnaires that were distributed, a total of 150 respondents were obtained consisting of general practitioners (71 people), doctors with Masters degrees (65 people), specialist doctors (9 people) and doctors with Doctoral education as many as 5 people. The doctors have worked for < 5 years (63 people), 5-10 years (32 people), 10-20 years (31 people) and > 20 years (24 people). The results of the construct validity test (job satisfaction) obtained data that there were 5 valid statement indicators. While the validity of spirituality obtained 4 valid data, and on the performance, parameters obtained 6 valid performance indicators. The results of the analysis obtained data that job satisfaction has a positive significant effect on the performance of doctors (P = 0.000). Spirituality has a positive significant effect on doctors' performance with a significance level of P=0.000. However, spirituality as a moderating variable has a negative effect, which means it reduces the effect of job satisfaction on performance. The spirituality factor reduces job satisfaction. It can be logically explained that job satisfaction is not solely a determinant of performance. Spirituality reflects the vocation of dedication as a doctor who has special knowledge to help human health and humanity. Spiritually, working doctors do not pursue job satisfaction merely, but there is a spirituality of dedication that aims to help save the souls of fellow human beings.

Abstract

The accuracy of the disease diagnoses code is determined by the accuracy of writing the diagnoses. The result of observation in 10 inpatient medical record documents at the TNI-AD 05.08.04 Hospital Lawang, 5 documents were found with inaccurate codes (50%) with 3 incorrectly written diagnoses. The purpose of this study was to analyze the effect of the accuracy in writing disease diagnoses to the accuracy of the diagnoses code for inpatients at the TNI-AD 05.08.04 Hospital, Lawang. This methode of research was an analytic observational with a cross-sectional approach. The number of sample analyze were 100 inpatient medical record documents which hospitalitaion from March to August 2022 and using simple random sampling technique by lottery. The accuracy of writing diseases diagnoses of inpatient was an independent variable and the accuracy of the diagnoses code for the inpatient at the TNI-AD 05.08.04 Hospital, Lawang was dependent variable in this study. The results showed the accuracy in writing disease diagnoses was 70% and the accuracy of the disease diagnoses code was 45%. The analytic test with Chi Square showed significantly the effect of accuracy in writing diseases diagnoses to the accuracy of diagnoses code for inpatient at the TNI-AD 05.08.04 Hospital Lawang with a p-value = 0.000 (<0.05). The accuracy in writing diseases diagnoses by doctor that refer to medical terminology of ICD-10 will producted an accurate diagnoses codes.
