Abstract

The daily facts indicated that an individual is the key factor to achieve organization’s success. Every organization with its best performance is always related to its human resources’ balanced competency. The midwife has role, function, and competencies to provide maternal services for women. The midwife’s role is not only a doer, but also an organizer, an educator, and a researcher. Therefore, the midwife is expected to focus on prevention and health promotion aspect with basis of partnership, and community empowerment collaboration with other health workers in order to be readily prepare the health services for anyone who is in need.This was qualitative research method with case study approach. The informant was the midwives of Tambakrejo Community Health Center of Surabaya. According to the data of antenatal care attendance, trained birth attendance, and perinatal care, there were 859 pregnant women, 581 persons on phase of K1 (67.64%), and 551 persons on phase of K4 (64.14%). The birth attendances by trained birth attendants are 513 people (62.56%), while perinatal care recorded 570 people attending the health service (69.51%). The data indicated the performance by the midwives of Tambakrejo Community Health Center categorized in the lowest position among 63 community health centers in Surabaya. The result showed poor soft skill competency and poor hard skill competency on midwives. In conclusion, poor soft skill competency including personal competence and social competence should be trained and developed by obstetrics and gynecology specialist of Soewandhi Public Hospital of Surabaya. The study suggested Surabaya District Health Office needs to conduct training of technology information and computer in order to increase the service quality and to solved the problem related to hard skill competencies on technology and computer skill.

Abstract

 AbstrakPenyebab  utama  kematian  ibu di Kabupaten Pati dalam tiga tahun terakhir  adalah preeklampsia. Pada tahun 2016, 35% kasus kematian ibu disebabkan oleh preeklampsia. Penelitian ini bertujuan untuk menganalisis faktor-faktor risiko kejadian preeklampsia di Wilayah Kabupaten Pati. Jenis penelitian observasional analitik dengan rancangan kasus kontrol. Subjek sejumlah 40 kasus preeklampsia dan 120 kasus non preeklampsia  dipilih secara purposive. Pengumpulan data dilakukan dengan wawancara terstruktur menggunakan kuesioner. Data asupan makanan diperoleh dari  FFQ (Food Frequency Questionnaire). Analisis data dilakukan dengan chi-square dan regresi logistik ganda. Hasil penelitian menunjukkan faktor risiko terjadinya preeklampsia adalah faktor risiko internal : riwayat keluarga preeklampsia (OR=6,74; 95%CI:2,54-17,89, obesitas (OR=2,68; 95%CI:1,29-5,60), riwayat hipertensi (OR=7,4; 95%CI:2,91-18,82), stress  (OR=2,64; 95%CI:1,26-5,51), dan faktor risiko eksternal: asupan anti oksidan yang  rendah (OR=3,71; 95%CI:1,23-11,19), asupan natrium berlebih (OR=2,54; 95%CI:1,14-5.67), paparan asap rokok (OR=2,43; 95%CI:1,14-5,17).  Analisis multivariat menunjukkan riwayat  keluarga preeklampsia  (OR=5,32; 95%CI:1,63-17,40), obesitas (OR=4,01; 95%CI:1,60-10,09, riwayat hipertensi (OR=7,38; 95%CI:2,32-23,45), stress atau kecemasan (OR=3,30; 95%CI:1,31-8,33), asupan anti oksidan yang rendah (OR=4,22; 95%CI:1,13-15,74) secara bersama-sama merupakan faktor risiko kejadian preeklampsia. Variabel yang bukan merupakan faktor risiko preeklampsia adalah usia ibu, paritas, dan penambahan berat badan berlebihan selama kehamilan. Disimpulkan bahwa faktor risiko yang paling kuat adalah riwayat hipertensi ibu sebelum kehamilan. Disarankan agar memasukkan riwayat hipertensi, riwayat keluarga preeklampsia, obesitas, dan stres dalam anamnese  pelayanan antenatal untuk mendeteksi dini preeklampsia.

Abstract

Proporsi populasi remaja di Indonesia mencapai seperempat dari total penduduk. Menurut Riset Kesehatan Dasar Tahun 2010, angka ini cukup besar dan perlu ada upaya yang spesifik dalam meningkatkan derajat kesehatan remaja. Beberapa penelitian menunjukkan bahwa remaja saat ini sedang menghadapi globalisasi dan berpotensi melakukan perilaku berisiko. Penanganan permasalahan kesehatan remaja di Indonesia diupayakan oleh pemerintah melalui kerja sama lintas sektoral, pelayanan kesehatan dasar, dan pola intervensi. Harapannya strategi yang diterapkan telah disesuaikan dengan kebutuhan tahapan proses tumbuh kembang remaja.Penelitian ini merupakan penelitian deskriptif kualitatif. Tujuan dari penelitian ini adalah mengidentifikan persepsi remaja dalam keterlibatannya dalam pelaksanaan program kesehatan remaja dan mengidentifikasi kebutuhan dan harapan remaja terhadap pelaksanaan program kesehatan remaja. Subjek penelitian ini terdiri dari 2 kelompok, yaitu kelompok peer educator dan kelompok user (sasaran program). Remaja dalam penelitian berasal dari berbagai setting promosi kesehatan, yaitu remaja sekolah dan remaja di luar sekolah. Remaja di luar sekolah berasal dari remaja komunitas dan remaja jalanan. Pengumpulan data diperoleh dengan menggunakan teknik wawancara mendalam dan focus group discussion (FGD).Hasil penelitian menunjukkan keterlibatan kelompok peer educator masih cenderung pasif karena wewenangnya hanya sebatas pada tahap pelaksanaan program, harapannya peer educator dilibatkan mulai dari perencanaan program. Sedangkan keterlibatan kelompok remaja di luar sekolah cenderung lebih sangat pasif bahkan ada yang belum terpapar program. Program kesehatan remaja seharusnya melibatkan remaja dari berbagai setting dan disesuaikan dengan kebutuhan kapasitas remaja. Remaja berharap dapat dilibatkan mulai dari tahap perencanaan hingga evaluasi program karena remaja sebagai prime mover dalam keberhasilan program kesehatan remaja.

Abstract

Maternal mortality of Surabaya, in 2017, reached 79.40 per 100,000 live births due to direct causes by 77.2%. There are many programs to reduce maternal mortality and infant mortality such as improving delivery services in the health facility. In 2008, the partnership program between midwives and traditional birth attendants had developed to increase the access of quality maternal services for mothers and babies. However, in 2017 the delivery practice with traditional birth attendants has remained active in the area of Bulak Banteng Community Health Center, Surabaya.      This study aimed to analyze the women’s decision selecting the traditional birth attendants for their delivery practice by exploring knowledge factor, belief factor, health service access factor, and family factor in Bulak Banteng Community Health Center of Surabaya. The research was a qualitative study using the approach of case study including in-depth interview and exploring secondary data. The sample was the mothers who delivered the baby with the traditional birth attendant from a previous year ago, and the midwife of health community center. The variable from this study were knowledge factor, belief factor, health service access factor, and family factor. The study used the analytical method consisted of data collecting, data reduction, and verification.      The result showed less knowledge factor of healthy and safe delivery in the community. The belief factor and family factor held an important role in the selection of the traditional birth attendants during birth attendance, while the factor of health service access showed nothing related to the selection process. There is a need to implement health education for pregnant women, husband, family, and people with an aim of giving the understanding about healthy and safe birth attendance. In conclude, the selection on traditional birth attendant was influenced by knowledge factor, belief factor, and family factor.

Abstract

Based on the medical records of the Banyumanik Hospital, the number of visits in the Banyumanik Hospital dental clinic in 2015 was 766 patients (90%), increasing in 2016 to 781 (91%), but decreased in 2017 to 724 patients (85%) one factor the cause is the quality of service. This study aimed  to analyze the effect of quality outpatient services on patient satisfaction in dental polyclinics in hospitals. Banyumanik Semarang. This was quantitative research with cross sectional design. The number of samples in this study were 54 respondents. Research variables were 5 dimensions of service quality. The results of the study showed the dimensions of the quality of outpatient care in dental poly: Real Evidence had a poor result of 68.5, reliability had poor results of 32 respondents 59.3%, poor responsiveness 40.7%, poor guarantees 59.3%, poor empathy at 55.6%, and less patient satisfaction at 44.4%. In the results of multivariate analysis that the responsiveness variable has the highest significance of 0.039 with the value of Exp (B) 4,566. The conclusion of this study was that the dimensions of service quality namely Real Evidence, Responsiveness, and Empathy affect patient satisfaction, while the guarantee does not affect the satisfaction of outpatients in the Dental Polyclinic Hospital. Abstract

Primary Health Centre (PHC) is one of the public institution which have important role to provide health care services to the community. According to Regulation of Semarang Major no. 13 year 2016 about  Governance Pattern of  Local Public Service Institution  (BLUD) of Puskesmas, since 2017 PHC in Semarang transformed into BLUD PHC which enable PHC to be more flexible, productive, efficient, and effective on managing their resources and finance. This research objective was to identify patient satisfaction of BLUD PHC in Semarang. This was quantitative research with cross-sectional design and research variables are tangible, reliability, responsiveness, assurance, empaty, dan patient’s satisfaction. Respondents were 140 patients of BLUD PHC in Semarang City: Bulu Lor, Gayamsari, Pudakpayung, Bangetayu, Halmahera, Kedungmundu, dan Mangkang. Result showed that most of the patient satisfied with the health care services in PHC. From all of the indicators, more than 80% respondents stated that they were satisfied with the services in PHC. The highest score was on responsiveness aspect, which 95.0% patient scored high on the PHC responsiveness. It could be because PHC already maximize their resources to provide high quality health care services.

Abstract

Diterapkannya sistem implementasi verifikasi digital klaim (Vedika) oleh BPJS Kesehatan bagi fasilitas kesehatan rujukan tingkat lanjutan (FKRTL) mengakibatkan terjadinya penarikan petugas/pekerja BPJS Kesehatan di rumah sakit pemberi FKRTL. Akibat dari diterapkannya sistem ini, unit BPJS Kesehatan Centre yang sebelumnya ada di RSUD Ungaran dan memiliki fungsi sebagai unit penanganan masalah pelayanan kesehatan peserta BPJS Kesehatan menjadi hilang. Hal ini berpengaruh kepada proses pelayanan kesehatan di RSUD Ungaran terutama dalam hal penanganan keluhan pelayanan kesehatan dan pemberian informasi perihal kepesertaan BPJS, dimana hilangnya unit BPJS Kesehatan Centre ini berdampak kepada timbulnya keluhan pelayanan kesehatan oleh pasien peserta BPJS Kesehatan. Di RSUD Ungaran, fungsi unit BPJS Kesehatan Centre ini digantikan oleh unit Rekam Medis. Tujuan penelitian ini adalah untuk menganalisa beban unit Rekam Medis di RSUD Ungaran paska pemberlakuan sistem Vedika.Metode penelitian bersifat kualitatif disajikan dalam bentuk deskriptif untuk menggambarkan secara obyektif perilaku, persepsi, motivasi, dan tindakan.Dengan menggunakan analisa perspektif relatives advantage, compatibility dan complexity, hasil menunjukkan bahwa beban kerja unit Rekam Medis di RSUD menjadi bertambah karena dibebankan tugas pokok dan fungsi yang sama dengan unit BPJS Kesehatan Centre.RSUD Ungaran memerlukan solusi kebijakan berupa pembentukan unit baru untuk menangani masalah keluhan pelayanan kesehatan dan pemberian informasi terkait kepesertaan BPJS Kesehatan untuk memastikan hak dan kewajiban pasien peserta BPJS Kesehatan terpenuhi.

Abstract

Stunting merupakan permasalahn gizi kronis yang berdampak pada kegagalan pertumbuhan pada balita, sehingga balita terseabut cenderung lebih pendek daripada usianya. Pemantauan tumbuh kembang anak dapat diupayakan dengan adanya kegiatan posyandu balita. Peranan kader posyandu disamping untuk memotivasi orangtua akan kesadaran pentingnya memantau pengukuran tinggi dan berat badan balitanya, juga mempunyai peranan dalam proses pencatatan dan pelaporan hasil penimbangan balita kepada pihak puskesmas. Dengan kerjasama yang baik antara masyarakat, kader posyandu, dan puskesmas ini dapat menghasilkan data-data yang akurat, tepat, cepat, dan benar.Tujuan dari penelitian ini yaitu melakukan pengembangan sistem informasi pencatatan dan pelaporan status gizi balita stunting untuk mendukung monitoring dan penatalaksanaan status gizi balita.Metode yang digunakan dalam pengembangan sistem yaitu dengan FAST, dan menggunakan desain penelitian Action Research dengan pendekatan cross sectional. Proses pengumpulan data dilakukan dengan observasi dan wawancara mendalam. Subjek penelitian yaitu 1 petugas kia dan gizi puskesmas, 1 bidan Pembina wilayah, dan 13 kader posyandu balita kelurahan Gajahmungkur. Perancangan desain input, posesn, dan output disesuaikan dengan kebutuhan dari pengguna sistem informasi.Dapat disimpulkan bahwa pengembangan sistem informasi pencatatan dan pelaporan status gizi balita stunting “mozita” dapat membantu mengatasi permasalah yang muncul, dengan dapat diakses secara online sehingga dapat membantu proses pemasukan data dan pelaporan secara akurat, tepat, cepat, dan benar.

Abstract

In 2016 from 7 Puskesmas in Tarakan City, 4 (four) Puskesmas (58%) sent their reports above the 5th of each month. There is still a mismatch of the number of source data with the results of recapitulation such as the number of DHF cases in Tarakan DHO with the number of DHF cases in the Ministry of Health Republic of Indonesia SKDR as well as the incompatibility of the reporting format by recording surveillance staff. The purpose of this study was to develop a Routine Data Quality Assessment (RDQA) instrument for DHF Epidemiological Surveillance in Tarakan City Health Office. The type of research used is Research and Development. The total subjects of the study were 9 people with details of 7 data management officers in the Puskesmas in the Tarakan DKK working area and 1 person in charge of the DHF Program in Tarakan DKK. The steps in this study, namely: (1) Potential and problems, (2) Data Collection (3) Product Design (4) Design Validation (5) Design Revision (6) Product Trial (7) Product Revision (8) Trial Use. The results of this study are that data quality assessment instruments have been developed according to RDQA and routine data quality assessments by the Ministry of Health with R & D research methods modified with eight indicators namely timeliness, data availability, data completeness, monitoring and evaluation unit capabilities, reporting indicators and guidelines, data collection and reporting format, data management process, linkages with the national reporting system and the use of data for decision making. The results of the assessment of the instruments developed, namely the aspect of content feasibility has an average value of 81%, the feasibility aspect of presentation is 78% and language assessment aspects 81%. Based on the results of the assessment of the three aspects assessed in the development of the RDQA instrument, a good conclusion is reached, but there are general recommendations given both by the Chair of the DHF Program in the DHO and the Epidemiological Surveillance Data Management Officer in DHO and Puskesmas, namely the use of modules and manuals RDQA instrument.

Abstract

The Gasurkes System "Go Bumil" is an application system developed to improve the search performance of pregnant women by village level maternal and child health surveillance officers in the Semarang City area. Usability evaluation on the "Go Bumil" application system needs to be done to find out the usefulness of the application system for users.This type of research uses a pre-experimental design in a single group. Data collection used the PSSUQ questionnaire (Post Study System Usability Questionnaire). The research subjects were 12 respondents from urban KIA gasurkes, 104 respondents to pregnant women and postpartum mothers. The object of research is the "Go Pregnancy" application. Data processing methods use descriptive analysis using a weighted average.The results of the usability evaluation of the "Go Bumil" application with respondents to KIA gaskes, pregnant and postpartum mothers as a whole were satisfied with the application system developed. The overall results of the overall assessment on kelurahan gas, pregnant women and postpartum mothers in the category of system usefulness = 6.02 (range values 6.00 - 6.99 state respondents agree), information quality = 5.99 (range of values 5, 00 - 5.99 states the respondent somewhat agrees), interface quality = 5.99 (range values 5,00 - 5,99 state respondents somewhat agree) and overall satisfaction = 6.01 (range values 6,00 - 6,99 state respondent agreed). Based on the evaluation of the system usefulness, information quality, interface quality and overall satisfaction categories, it can be concluded that the respondents were satisfied with the "Go Bumil" application.
