Abstract

Program pendampingan oleh gasurkes (petugas surveilans Kesehatan) di Kota Semarang telah dilaksananakan oleh Dinas Kesehatan yang bertujuan untuk mengurangi angka kematian ibu yang masih cukup tinggi. Pendampingan yang dilakukan oleh gasurkes tumpang tindih dengan pendampingan oleh kader dan PKK sehingga membuat ibu merasa tidak nyaman. Oleh karena itu perlu dilakukan evaluasi program pendampingan ibu hamil terkait tingkat kepuasan ibu hamil terhadap pelayanan yang diterima.Desain penelitian ini cross sectional dengan pendekatan kuantitatif. Populasi penelitian adalah ibu hamil di Puskesmas Pandanaran dan Puskesmas Kedungmundu. Subjek penelitian 34 ibu hamil dari wilayah kerja Puskesmas Pandanaran dan 76 ibu hamil dari wilayah kerja Puskesmas Kedungmundu. Data dikumpulkan dengan observasi dan wawancara menggunakan kuesioner terstruktur. Pengolahan dan analisis data menggunakan SmartPLS.Hasil penelitian menunjukkan bahwa tidak ada pengaruh pendidikan (T-statistik: 0,076), pekerjaan (T-statistik: 0,100) dan pengetahuan (T-statistik: 0,807) terhadap kepuasan ibu hamil dalam program pendampingan. Kualitas pelayanan berpengaruh terhadap kepuasan ibu hamil dalam program pendampingan, T-statistik: 27,850 (>1,96).Disarakan untuk melaksanakan program pendampingan secara terus-menerus karena memiliki dampak yang baik dan positif untuk ibu hamil dan juga dapat mendeteksi dini komplikasi selama masa kehamilan. Selain itu gasurkes diharapkan dapat lebih mudah dihubungi saat ibu hamil membutuhkan dan  lebih merespon terhadap keluhan yang disampaikan.

Universitas DiponegoroFakultas Kesehatan MasyarakatProgram Studi Magister Ilmu Kesehatan MasyarakatKonsentrasi Sistem Informasi Manajemen Kesehatan2017 ABSTRAK Fikri Faidul JihadPengembangan Sistem Informasi Administrasi Penggunaan Vaksin Berbasis Web untuk Mendukung Pengelolaan Imunisasi di Dinas Kesehatan Kabupaten Aceh Baratxvii + 137 halaman + 28 tabel + 41 gambar + 15 lampiran  Imunisasi terbukti efektif dalam mencegah berbagai penyakit yang dapat dicegah dengan imunisasi (PD3i). Mekanisme penyelenggaraan imunisasi termasuk pengelolaan rantai vaksin, pencatatan dan pelaporan serta supervisi dan bimbingan teknis. Agar pelaksanaan program imunisasi berhasil, maka semua unsur harus berjalan dengan baik, termasuk manajemen penggunaan vaksin yang bertujuan terhadap peningkatan pengelolaan administrasi vaksin imunisasi. Tujuan dari penelitian ini adalah untuk mengembangkan sistem informasi administrasi penggunaan vaksin untuk mendukung pengelolaan imunisasi di Dinas Kesehatan Kabupaten Aceh Barat.Jenis penelitian yang digunakan merupakan Pra-Eksperimental Design (One-Group Pretest-Postest Design). Pengembangan sistem menggunakan metode FAST (Framework for the Application of System Techniques). Subjek penelitian terdiri dari 7 responden untuk Analisa kebutuhan pengembangan sistem informasi dan 15 responden untuk uji coba sistem informasi. Hasil Analisis data secara deskriptif menunjukkan bahwa nlai rerata tertimbang secara keseluruhan kualitas informasi sebelum pengembangan sistem sebesar 2,27 dan setelah pengembangan sistem 3,36. Terdapat peningkatan nilai rerata sesudah pengembangan sistem sebesar 1,09 artinya terdapat perbedaan kelengkapan, kemudahan, kesesuaian, keakuratan, ketepatan waktu sebelum dan sesudah pengembangan sistem.Perlu adanya dukungan dan komitmen dari Dinas Kesehatan Kabupaten Aceh Barat untuk memanfaatkan sistem informasi ini sebagai sumber informasi untuk mendukung pengelolaan administrasi vaksin imunisasi di Dinas Kesehatan Kabupaten Aceh Barat. Kata kunci       : Sistem Informasi, administrasi vaksin, Dinas Kesehatan Kabupaten Aceh BaratKepustakaan   : 42 (2001-2015)

Abstract

Patient safety is a system that makes patient care in the hospital safer and prevent injuries caused by mistakes due to carrying out an action or not taking the action that should be taken. This study aimed to analyze the influence of patient safety culture, factors that influence and availability of resources on prevention of adverse events in the installation of inpatient services at RS X Semarang.This type of research was quantitative with a cross sectional approach. The sample was all members of the population consisting of midwives and nurses at RS X  as many as 31 employees. Data collected using structured questionnaire. Data analyzed by Chi Square test and Logistic Regression.The results showed that the majority of respondents had a supportive patient safety culture (80.6%), half of the respondents had influential factors (51.6%), half of the respondents had the availability of good resources (51.6%), and make efforts to prevent KTD (71%). There was sigificant relationship between patient safety culture and efforts to prevent KTD (p value = 0.004), there was no significant relationship between availability of resources and efforts to prevent KTD (p value = 0.113), and there was an influence between the patient safety culture on efforts to prevent KTD (p value = 0.012); OR = 0.031; CI = 0.002 - 0.467. The patient safety culture has been shown to have an influence on efforts to prevent KTD.

Abstract

Patient safety incidents at a hospital still occur at almost 92.1% near-miss events, 4.3% harmful incidents, 2.4% no-harm incidents and 1.2% unintended events. At the General District Hospital of dr. Soediran Mangun Sumarso, the hospital manager has not understood the function of reporting system so that there is no follow up. Even, blaming culture still happens, and not all health workers care about incident reporting. Establishing a patient safety culture is the first step to implement a patient safety program. This study was intended to analyze a patient safety culture among health workers.This study was conducted at the General District Hospital of dr. Soediran Mangun Sumarso which is a type-B hospital. This study was analytic with a cross-sectional design. It involved 228 health workers as respondents, used Kruskal Wallis for the analysis and adopted instruments from the Hospital Survey on Patient Safety Culture (HSOPSC). The findings explained that communication openness dimension is in the middle category. Teamwork in the units is good, and the overall perception is moderate. Staffing is moderate, so id non-punitive response. The reporting frequency is moderate with 43.8% respondents who said not reporting any incidents for the past 12 months. The organizational learning is good, and the staff expectation is moderate. The level of patient safety is good, and the feedback is moderate. Transfer and transition are good. The p-values for staff expectation, organizational learning, unit tea work, openness, and feedback are 0.001, 0.017, 0.004, 0.000, and 0.039 respectively. Whereas the non-punitive response has a p-value of 0.001, staffing has a p-value of 0.005, and management support results in a p-value of 0.001. Teamwork among units has a p-value of 0.001 and transfer and transition performs p-value of 0.004.There are some differences among health workers in terms of communication openness with a p-value of 0.000.

Abstract

Preliminary study in Hospital S showed that incomplete medical records still found and caused late claims. Based on these findings, this study analysed the medical record management system to support the National Health Insurance claim of inpatients at S Hospital.This was qualitative study using cross-sectional approach. The key informants are Hospital Director, Doctors, Coding Staffs, Casemix Staffs, Case Manager, and Internal Verificators. Whereas, the triangulation informants included Owner, Internal Auditor Unit, and Medical Committee. The data collection was done through in-depth interview and observation. Results showed that the number of human resources was enough, but the socialization about medical records to doctors was still impaired.  The funding was sufficient while the infrastructure and facilities were not advanced to support the claim verification. Medical records were available and easy to fill in. Guidelines and Standard Operating Procedures were available, but doctors’ compliance with medical records was lacking. The management of medical record in supporting the claims was not well-implemented because the doctors do not have time to fill the medical resume and surgery report. The claim auditors have regularly controlled, inspected, and followed up the medical records.It is recommended that doctors should be given intense socialization about medical records and instructed to communicate with each other to fill in the medical records on time. The hospital is also recommend to do utilization review on the completeness of medical records to support the National Health and give repressive punishment to doctors who do not fill the medical records.

Abstract

The preliminary study at the Ken Saras Hospital showed the nurses have verbal errors, long admission, and treatment incompliance with the standard. This problem was assumed to be related with the room coordinator's leadership style. Hence, this study identified the effect of the coordinator's leadership style towards nurses responsiveness on the healthcare services at the Ken Saras hospital.This was an observational and Quantitative study using a cross sectional design. There were 70 nurses as samples who are incharge of giving treatment and having been trained about service excellence. The dependent variable is nurses' responsiveness, and the ondependent one is room coordinator's transformational leadership. The validity and reability of the instruments for this study have been tested at the same-type hospital. The data were analyzed gradually starting from univariate analysis, bivariate analysis using chi-square, and multivariate analysis using logistic regression test.The univariate analysis performed that the lroom coordinator's leadership was  good, but it lacked in  inspirational motivation and individual consideration. Whereas, the bivariate analysis found there is a relationship between the sub-variable of idealized influence  and nurses' response on excellence service. Finally, the multivariate analysis further noticed that all variable of transformational leadership style have no significant effect towards nurses’ responsiveness on service execellence It is suggested that the management of Ken Saras hospital improve the leadershiop skill of the room coordinator. especially of the aspects of inspirational motivation and individualized consideration. also, the management should consider the candidate's characteristics choosen to be a room coordinator

Abstract

Pregnancy and childbirth services are needed to provide comfort to reduce the level of anxiety in facing labour among pregnant women. Gentle birth is a method of childbirth that is calm, gentle, polite, and utilizes all natural elements in the human body. This study aimed to determine the effect of Gentle Birth preparation on the level of anxiety in facing labor among primigravidae pregnant women.This study was a quasi experimental with Pre-test – Post-test Control Group Design conducted to 90 pregnant women. The samples were selected using the Purposive Sampling technique, 45 were assigned to the intervention group and 45 were assigned to the control group. Data collected with structured questionnaire. Data analysed with paired t test to determine differences in scores between before and after intervention and independent t test to determine differences in the level of anxiety between groups.The results showed that there was no difference in the level of anxiety before the administration of Gentle birth preparation between the intervention group and the control group (p>0.05), there was a difference in the level of anxiety after the administration of Gentle birth preparation between the intervention group and the control group (p<0.05). The difference can be seen from the decrease in the difference between anxiety scores after the administration of Gentle birth preparation in the intervention group which decreased by -11.4, while in the control group it decreased by -2.13. It can be concluded that there was an effect of Gentle Birth the preparation on the level of anxiety in facing labor among primigravidae pregnant women

Abstract

The prevalence of diabetes mellitus (DM) will continue to increase throughout the years. A way to achieve normal blood sugar levels is through adherence to medications. Nonadherence to medications in DM patients can result in high blood sugar levels and increase the risk of complications. This study was conducted to determine factors affecting medication adherence among DM patients in Puskesmas Pancoran Mas Depok in March – April 2019. This was an observational analytic study using a cross-sectional method. MMAS-8, DKQ-24 and family support questionnaires were used to collect the data along with the demographic data. Chi-square was used to analyze the data. The factors affecting medical adherence included patients’ education level (OR=2.325; 95%CI=1.034-5.224), gender (OR=4.200 95%CI=1.699-10.380), diabetes duration (OR = 2.426 95%CI=1.019-5.775), number of drugs taken (OR=3.680 95%CI=1.604-8.445), dosing frequency (OR=3.350 95%CI=1.283-8.749), and patient’s knowledge (OR=2.668 95%CI=1.135-6.276). Logistic regression analyzes showed that gender is the most powerful factors in affecting medication adherence.

Abstract

Cancellation of elective surgery causes harm to the hospital and patients so it needs to be prevented by using a risk management approach. The purpose of this research is to evaluate the control system for canceling elective operations in hospitals after studying the FMEA method (Failure Mode Effects Analysis).This was qualitative research design presented in explanatory design. The main informants in this study included elements of management and practitioners in the hospital. Triangulation informants included patients and head of the room. Data were collected by in-depth interview and observation techniques. The researcher analyzes using content analysisThe results of the study showed that cancellation of elective surgery was mostly due to the health condition of the patient. Cancellation of human resource factors is caused by doctors who experience fatigue. Cancellation because the operating room is caused because the operating schedule is not appropriate. Cancellation due to device factors due to unavailability of implants.This study recommends increasing the range of preoperative studies, and increasing coordination for equalizing the workload of operators. While suggestions for hospitals are in the form of adding operator  adding operating devices and repairing SIRS for markers of operations.

Abstract

RSI PKU Muhammadiyah Pekajangan Hospital always tries to develop existing services. The hospital must maintain services in order to satisfy all parties including its employees. The hospital sets the standard of employee satisfaction at 80%. The employee satisfaction survey continued to improve from 62.4% in 2015, 63.93% in 2016, to 72.29% in 2017 but has not yet reached the target. The research aims to analyze the factors that influence job satisfaction of RSI PKU Muhammadiyah Pekajangan Hospital employees.This type of research is quantitative with analytic methods and cross sectional study design. The number of samples is 172 people. The sample selection is done by optimum allocation stratified sampling. Data processing was performed by bifariate analysis using the Chi Square Test and multifariate analysis using multiple logistic regression tests.Employee satisfaction has reached 84.3%, exceeding the target set by the hospital. The work it self, awards / achievements, and coworkers have a significant relationship and influence on job satisfaction. Promotion, family support, payroll and benefits, work environment, and hospital policies have a relationship but the effect is less significant on job satisfaction. Leadership has no relationship with job satisfaction. The most influential factor on employee job satisfaction is the award / achievement factor with a Sig. = 0,000 and OR = 29,124.The award / achievement factor is the main factor so that the hospital needs to develop a management system that can adopt better awards / achievements with performance as a reference. Perceptions of payroll / benefits factors are also not good so it needs intervention. Giving awards in the form of additional payroll / benefits is expected to help increase job satisfaction.
