Abstract

One of the determinants of achieving quality control and cost control is the implementation of tiered referral health. This research was motivated by the absence of research on the implementation of referrals from FKTP to FKTL. Data from RSUP Dr. Wahidin Sudirohusodo’s annual report showed in 2015 and first half of 2016, >60% cases in class A hospital were level 1 severity case which should still be handle in class D and C hospitals. This affects the accumulation of patiens in tertiary hospitals and inefficient use of resources. It is a qualitative research with phenomenology approach to determine the implementation of a tiered referral system between advanced health facilities in JKN era in South Sulawesi. The results shows most of referral were made due to facilities limitation and inavailability of specialist.  There were referrals which made based on patients requests not based on clinical reasons. It is also found hospital staff who did not know about the existing of standard operational procedure of referral in the hospital. Some hospital staff has no knowledge on the referral regulation nor about hospital classification. This research is expected to be able to contribute to the development of science and can be used as input for hospitals and BPJS in an effort to improve the optimization of tiered referral services. Based on the results of the study it is recommended to improve facilities and infrastructure that can support the provision of referral services in hospitals.

Abstract

Health development starts from the smallest unit of society, namely the family. The Ministry of Health establishes an operational strategy for health development through the Healthy Indonesia Program with a Family Approach (PIS-PK). Data collection activities carried out in the PIS-PK aim to obtain health data for each family member by the Puskesmas. This study aims to determine the description of the index of healthy families in the working area of Puskesmas Batang I by using 12 indicators of healthy families related to the Health Promotion Program. This research is a quantitative study carried out in a descriptive observative manner, namely by analyzing the data narrative. Desa Kecapak has an index value of 0.08 (Unhealthy Family), Kelurahan Sambong has an index of 0.2 (Unhealthy Family), Kelurahan Proyonanggan Utara  has an index 0.13 (Unhealthy Family), and Kelurahan Proyonanggan Tengah has an index 0 , 21 (Unhealthy Family). Puskesmas Batang I work area has an index of 0.18 and is in the Unhealthy Family category. The families that were recorded had not had total coverage, which was still 54.2%. The health promotion program of the Batang I Puskesmas includes data collection on Clean dan Healthy Behavior/Perilaku Hidup Bersih dan Sehat (PHBS), training for posyandu cadres, self-awareness surveys/Survey Mawas Diri (SMD), and guidance for Alert Village/Desa Siaga.. The Health Promotion Program can refer to data on the healthy family index and PHBS by looking at the low indicator coverage of healthy families. Puskesmas can cooperate with educational institutions so that the number of families being recorded is maximized. Based on the healthy family index coverage figures, the health promotion program must synergize with the Disease Eradication Program, especially pulmonary TB disease, hypertension, and mental disorders. It is hoped that the development of posyandu and posbindu cadres can increase the coverage of the healthy family index, especially in increasing family knowledge about infectious and non-communicable diseases

Abstract

ABSTRACTBackground: Hospitals must be able to provide a guarantee of security and protection against the service impact that have fulfilled the quality standards to meet the community's right on the quality services. Hospital patient safety is a system where hospitals make patient care safer. Safety in service can be started from the accuracy of patient identification. The observational results show a mismatch in its implementation.Objectives: The purpose of this study is to determine the discrepancy in the implementation of inpatient identification on regulations, implementation, and obstacles in the implementation of inpatient identification in hospitals.Methodology: This research is a case study with a qualitative approach producing descriptive data. Research subjects consisted of Management, Staff, and Patients with purposive sampling techniques. The instruments used in this qualitative study were guidelines for tracking observation, in-depth interviews, and FGDs. Data analysis uses reduction, presentation, and drawing conclusions.Results: The implementation of inpatient identification in the hospital has been applied well. Based on the results of regulatory search, interviews, documentation, observations and simulations, it is obtained that the score achievement is with a value of 55 (91.67%) which means completed.Discussions: The implementation of inpatient identification in supporting patient safety programs can run well if there is an effort to build a patient safety culture in the hospital through support from the leadership, supervision and supervision, training and socialization, motivating staff and conducting monitoring and evaluation.Conclusion: Overall the implementation of inpatient identification at the hospital has been run well. There are policies that regulate, but the implementation of non-compliance is due to non-compliance staffs. There are some contents of the medical record sheet and the filling is not in accordance with standards and there is a limitation of identity label.

Abstract

Outpatient services hosted by the hospital have never been absent from public visits. In fact, every year an outpatient visitor is always increasing. This research intends to identify potential failure mode that can  inhibit of every flow of service in the outpatient care unit using the Failure Mode Effect Analysis (FMEA) method. Qualitative research plan using an observation survey approach and in-depth interviews with the outpatient service head Coordinator conducted in February 2020 on the hospital outpatient unit service process. The results of this study Indicate the potential failure mode that has the value of the RPN above the value of cut off point 180 as many as six out of ten failure modes. Firstly, the check is not on schedule (360), secondly, the patient lags a turn call order Check (270), third, Specific drug failure is not available (245), fourth, general patient protests with the price of the drug (224), fifth, the patient is void to poly (196), the sixth patient registrant online missed sequence number queue (180). Based on the results of the research, hospitals are expected to follow up with the results of this research by conducting a redesign of the process that occurs today using the FMEA to maintain service quality.

Abstract

COVID-19 has become a global pandemic in 2020. As of May 25, 2020 there were 22,750 confirmed cases of COVID-19 in Indonesia. Prevention and control of COVID-19 needs to be carried out by the village government which is in direct contact with the community. Supported by the issuance of Permendes No. 6 of 2020 concerning the Implementation Regulations for the COVID-19 Response Village Program. Meteseh Village is one of the villages in Kendal Regency that has a risk of spreading COVID-19. This research is a qualitative descriptive study using in-depth interviews using purposive technique with informants selected based on criteria and literature study. The results show that the implementation of the COVID-19 Response Village Program in Meteseh Village has not been running optimally due to unclear implementation standards in related policies, limited human resource capacity in program planning and resource management, especially in the aspect of controlling COVID-19 and the community attitudes that do not apply health protocols influenced by social, economic, political conditions. Therefore, Meteseh Village is advised to increase the role of village midwives to provide education about the handling of COVID-19 to program implementers, increase the participation of health workers, religious leaders and community leaders in Meteseh Village to assist in handling COVID-19, continue to provide informations of COVID-19 and conduct evaluation activities of program implementation including make a complete written report about program implementation.

Abstract

Background UNICEF and WHO estimate that if all babies were breastfed for at least the first six months of their lives, morbidity and malnutrition rates would significantly decrease worldwide. both of these organizations promote the policy of initiating early breastfeeding by campaigning for a baby-friendly hospital policy, one of which encourages early breastfeeding initiation to be carried out. The aim of our study is to review the available evidence regarding the positive effects of early breastfeeding initiation on the success of exclusive breastfeeding, to suggest the most appropriate strategy to support it.Research Methods The research taken was research published in 2000-2019, obtained through major databases such as PubMed, Google Schoolar, and then examined to obtain original documents. Research searches related to the effectiveness of early initiation initiation (IMD) and the factors that influence it use the main terms "early initiation of breastfeeding", "effectivity" and "determinants of". A total of 31 literature conclusions and results of research in making this review. The 31 selected literatures have met the specified inclusion criteria.The Result Giving breastfeeding initiation is best done no later than 1 hour postpartum. IMD (initiation of early breastfeeding) is effective in stimulating milk production, producing antibody protection, reducing postpartum maternal bleeding, and in the long term affecting the duration of breastfeeding. IMD has also been studied as having the effect of reducing neonatal mortality by preventing morbidity such as diarrhea, pneumonia, UTI, meningitis and neonatal sepsis. Factors that influence the success of IMD include geographical, socioeconomic, individual health, education and maternal knowledge about IMD, duration of labor, maternal employment, hospital policy, support from health workers, labor delivery, gestational age, and maternal health conditions.Conclusions Breastfeeding initiation has been extensively studied increasing the success of exclusive breastfeeding and contributing to the reduction in infant mortality caused by infection. However, the success rate of breastfeeding initiation in several developing countries in the world still varies. Many factors influence the success of breastfeeding initiation, which still need to involve health promotion, because the implementation of breastfeeding initiation needs to involve many parties, in addition to the general public as well as to health workers who are tasked with helping in the maternity unit.

Abstract

Perawatan kehamilan tidak diinginkan pada remaja merupakan salah satu upaya untuk menurunkan angka kematian ibu dan bayi. Remaja yang mengalami kehamilan tidak diinginkan biasanya terlambat untuk melakukan perawatan kehamilan. Ketidakpatuhan dalam perawatan kehamilan dapat berdampak pada janin dan remaja itu sendiri. Penelitian bertujuan untuk menganalisis faktor perilaku perawatan kehamilan tidak diinginkan pada remaja di kecamatan Karangrayung Kabupaten Grobogan dilihat dari penyebab terjadinya kehamilan tidak diinginkan, penyesuaian diri remaja, penyesuaian sosial remaja dengan kehamilan tidak diinginkan, praktek perawatan kehamilan dan faktor – faktor yang mempengaruhi praktek perawatan kehamilan. Penelitian dilakukan dengan metode kualitatif dengan pendekatan fenomenologi melalui wawancara mendalam. Subyek penelitian adalah remaja dengan kehamilan tidak diinginkan dan memenuhi kriteria inklusi sebanyak 6 orang yang dipilih secara purposive sampling. Informan triangulasi penelitian adalah orang tua remaja hamil, teman dekat, tetangga, bidan Puskesmas, Kepala sekolah dan guru BP. Hasil penelitian menunjukkan bahwa penyebab kehamilan tidak diinginkan pada remaja yaitu gaya berpacaran dan pergaulan yang kurang sehat serta kurangnya perhatian dari orang tua. Penyesuaian diri, remaja sudah menerima dan memutuskan melanjutkan kehamilan karena adanya dukungan orang tua dan teman dekat. Dalam penyesuaian sosial, remaja berupaya tetap berbaur dan bersikap ramah dengan tetangga, walaupun remaja dan keluarga menjadi bahan pembicaraan para tetangga. Praktek perawatan kehamilan masih kurang sehingga belum terpenuhinya dengan baik kebutuhan nutrisi dan personal hygiene. Faktor yang sangat berpengaruh pada remaja hamil dalam perawatan kehamilan yaitu adanya dukungan dari keluarga serta dukungan petugas kesehatan. Saran untuk petugas kesehatan dapat meningkatkan pengetahuan tentang perawatan kehamilan melalui program – program promotif dan preventif seperti pendampingan pada remaja hamil. Untuk remaja, dapat bertanggungjawab atas keputusan yang sudah diambil, membiasakan diri untuk berbaur dengan masyarakat dan memilih teman yang baik. Untuk orang tua dapat membentengi anak, memberi dukungan, bimbingan dan arahan pada anak meski sudah terjadi kehamilan yang tidak diinginkan. Untuk masyarakat dapat membantu dan merangkul remaja dengan kehamilan tidak diinginkan. Bagi sekolah, memberikan bimbingan konseling dan pengarahan kepada siswa tentang kesehatan kenakalan remaja khususnya mengenai kehamilan tidak diinginkan pada remaja.

Abstract

Latar belakang: Duplikasi nomor rekam medis merupakan masalah yang sering terjadi di puskesmas, salah satunya adalah Puskesmas Labruk. Berdasarkan hasil pendahuluan, dari 2.501 sampel berkas rekam medis didapatkan 10,3% berkas yang terduplikasi. Dampak dari duplikasi ini adalah tidak lengkapnya kronologi penyakit pasien, penggunaan map dan formulir yang berlebihan. Tujuan dari penelitian ini adalah untuk menganalisis penyebab duplikasi nomor rekam medis berdasarkan dukungan organisasi dan dukungan manajemen di Puskesmas Labruk.Metode: Jenis penelitian adalah kualitatif dengan teknik pengumpulan data yang digunakan adalah wawancara dan observasi. Objek penelitian adalah berkas rekam medis dan subjek penelitian adalah petugas pendaftaran, kepala rekam medis dan kepala puskesmas.Hasil: Hasil penelitian menunjukkan bahwa faktor yang menyebabkan terjadinya duplikasi dari dukungan organisasi adalah tidak adanya SOP, tidak adanya KIUP, dan nomor rekam medis tidak dibuat otomatis pada SIMPUS. Faktor yang menyebabkan duplikasi dari dukungan manajemen adalah tidak dilakukannya evaluasi dan diskusi untuk mengatasi masalah duplikasi.Simpulan: Dukungan organisasi dan dukungan manajemen adalah penyebab dari terjadinya duplikasi, apabila terdapat kekurangan pada dukungan organisasi ataupun manajemen maka duplikasi akan terus meningkat.

Abstract

Imposing large-scale social restrictions has resulted in a transition of activity patterns in society. This change results in many activities that must be done at home, so that housewives try to adjust quickly to the situation. This demand raises mental health problems, which can also affect physical health, so that other people to provide support can be important value to housewives. This study aims to analyze the effect of social support and adaptation processes on the mental health status of housewives, including levels of depression, levels of anxiety disorders, and levels of stress because of large-scale social restrictions. This research is a survey research with quantitative methods and using a cross-sectional design. Respondents of this study were 100 housewives who were taken using accidental sampling. Collecting data using a questionnaire and analyzed using the Rank Spearman test. The results showed a relationship between social support with the level of depression (p-value = 0.014) and the level of anxiety disorders (p-value = 0.030), as well as the adaptation process with the level of depression (p-value = 0.002). There is a relationship between social support with the level of depression and the level of anxiety disorders in housewives, and the adaptation process with the level of depression in housewives. It is recommended that the East Jakarta City Government develop a program that involves selected communities to become mental health counselors. In addition, the puskesmas can make innovations in mental health efforts for families and optimize the role of mental health cadres.

Abstract

Performance appraisal for employee usually based on the job description that has been designed by organization. Therefore, the result of employee’s performance appraisal will be considered based on employee’s capability to complete all the tasks from their job description. The objective of this research is to analyze and identify the effect of compensation benefit, organizational commitment and organizational culture towards employee’s performance in YARSI Hospital. The method of this research is using causality design method based on time dimension, one short study; the measurement towards dependent and independent variable at the same interval. The sampling technic in this research using Cluster random Sampling. Analyzed unit is data analysis using Regression. The result of this partial test revealed that employee’s performance affected to organizational commitment, compensation benefit, organizational culture and the result of this simultaneous test showed that the most significant effect is compensation benefit. Surprisingly, Researcher finds that in YARSI Hospital spread over the spiritual values proved that all employee remain working hard even though the compensation benefit system has not been implemented at the finest condition.
