Abstract

Background: Caries are one of the dental and oral health problems that are still the highest sequence today. The prevalence of caries in Indonesia gets 90% in the toddler population. A mother's knowledge about dental health will determine the mother's attitude towards children's dental health care, and low maternal knowledge is associated with high caries prevalence. The working area of the Bangsalsari Public Health Center is one of the areas in Jember that have a relatively high number of caries cases in toddlers, which is more than half of the number of toddlers experiencing caries. The study aims to analyse the relationship between knowledge and attitudes of mothers in the maintenance of dental health of toddlers in the work area of the Bangsalsari Public Health Center, Jember. Methods: This type of research is analytic observational with a cross-sectional approach. The research subjects were selected by a total sampling technique and obtained by as many as 126 mothers of toddlers. Data were collected using a knowledge and attitude questionnaire each containing 10 questions. The research data that have been obtained are then carried out with a nonparametric Spearman correlation test. Results: Spearman test shows a significant value of 0.000 (p<0.05), the correlation value obtained is r = 0.824 with a positive value. Conclusion: There is a significant relationship between knowledge and attitudes of mothers in dental health care for toddlers, and both variables have a unidirectional relationship with a very strong correlation level. Keywords : Attitudes of mothers, Dental health care, Knowledge, Toddlers

Abstract

Background:Regarding to the data generated by Basic Health Research in 2018, the prevalence of Indonesian people with dental and oral problems was amounted to 57.6%. Haruan fish scale chitosan has been shown to be capable of inhibiting growth and killing microorganisms that cause oral disease and has the potential to become a safe biomaterial. Toxicity testing was carried out to ensure safety of the chemical compounds before they can be used as drugs or during clinical trials. Purpose:This research was intended to analyze the toxicity of haruan fish scales (Channa striata) chitosan on BHK-21 fibroblast cells by utilizing the MTT assay method. Method:This research was classified as a true experimental research by adopting a post-test-only design with a control group design consisting of 6 groups. The treatment of haruan fish scale chitosan was given to 4 groups with concentrations of 25%, 50%, 75%, and 100%, with 2 control groups, specifically cell control and media control. The absorbance value was read by means of ELISA reader and the percentage of cell viability was subsequently calculated. The results of the average percentage of cell viability were then analyzed using SPSS to obtain the IC50 value. Result:The calculation of the average cell viability of each concentration was successfully obtained in this research. The average cell viability at 25% concentration was 2.106%, at 50% concentration was 12.01%, at 75% concentration was 77%, and at 100% concentration was 80.194%. The average percentage of cell viability data was further entered into the SPSS software and the value of IC50> 1000 µg/mL was successfully obtained. Conclusion: Haruan fish chitosan (Channa striata) is not toxic to BHK-21 fibroblas cell. Keywords : Channa striata , Fibroblast cell, Scale chitosan, Toxicity test.

Abstract

Background: Dental caries is a disease of the hard tissues of the teeth that most often occurs with a prevalence rate of about 45.3%. Dental caries is occuring because of the interaction between biofilms and carbohydrates from the food on the tooth surface (host). The caused by the biofilm formation is the presence of colonies of Streptococcus mutans bacteria. To reduce the accumulation of biofilms, mechanical and chemical methods can be used, namely using a toothbrush and mouthwash. Probiotics are widely used in dentistry because they have anti-plaque activity and form H2O2 which can reduce biofilm formation. Purpose: The purpose of this study was to determine the effect of the probiotic Streptococcus sanguinis on the formation of Streptococcus mutans biofilm in artificial saliva. Method: This research method was experimental analytic with post test only group design. The samples were 9 and divided into 3 groups, namely the probiotic group, Streptococcus sanguinis, the positive control group chlorhexidine and the negative placebo control group. Each group incubated in adhesion phase for 4 hours at 37°C. The formation of biofilms is measured by calculating the average results of Optical Density using an ELISA-reader. Data analysis was performed using the test One-way Anova. Results: The average values of optical density in the S. sanguinis, chlorhexidine and placebo groups, in sequence, were 0.374, 0.414, and 0.420. One-way Anova test obtained a significant value (p) of 0.014 (p<0.05) which indicates that there are significant differences in the probiotic group Streptococcus sanguinis, the chlorhexidine group, and the placebo group.  Conclusion: This study showed that the probiotic Streptococcus sanguinis had a significant impact on the formation of Streptococcus mutans biofilm in artificial saliva. Keywords :          Biofilm, Optical Density, Probiotic, Streptococcus mutans, Streptococcus sanguinis

Abstract

Background. The clinical oral cavity characteristics of children with intellectual disabilities include a high prevalence of periodontal disease and dental caries. South Kalimantan is a wetland or marsh that contains acidic water with a pH of 3.5-4.5. The acidic condition causes tooth demineralization, which can lead to caries. Oral health education through special needs children dental health education (DHE) interactive video is needed. Objective. Analysing the effectiveness of dental health education (DHE) video media for special needs children living in wetlands during the Covid-19 pandemic on the oral hygiene status of children with intellectual disabilities. Methods. The study used a quasi-experimental design with a pretest, posttest without control design and using a simple random sampling technique. The samples were 52 children with intellectual disabilities who were subjected to the OHI-S examination before watching a DHE video for children with intellectual disabilities made by the author as a pretest. Afterward, the children were instructed to watch a 5-minute video made by the author using the Makaton method. Posttest involved OHIS reexamination after watching the video for 20 consecutive days. Data were analysed using the Wilcoxon statistical test. Results. There were 29 children with fair OHI-S (55.8%), 15 with poor OHI-S (28.8%), and 8 with good OHI-S (15.4%). The result of the Wilcoxon test indicated a value of 0.000 (p < 0.05). Conclusion. OHI-S score before the DHE video treatment was fair and improved to good after the DHE video. Abstract

Background: Children's oral health is influenced by several factors related to their mother's condition. Mothers who have poor oral health generally have high anxiety about dentists and will rarely visit dental health services so their children will have poor oral health and oral problems will arise, especially caries. This has a negative impact on the quality of life of children. This study aims to determine the effect of maternal dental anxiety and child caries status on the quality of life of school children in an elementary school in Medan City. Methods: A cross-sectional design was conducted with 71 students, 6 until 7 years-old elementary schools in Medan. Maternal dental anxiety was measured using a Dental Anxiety Scale (DAS) questionnaire and children's quality of life was measured using a Parental Perception Questionnaire (PPQ). The child's caries status was assessed by taking with a smartphone camera and calculating the presence or absence of caries in each child. Results: Caries prevalence in children was 29.6%, maternal dental anxiety 88.7% low, 2.8% high anxiety. The mean score of maternal anxiety was 10.04± 3.46 and the child's quality of life score was 22.48±13. There is a significant effect between caries status and oral symptoms (p-value= 0.012) and  functional limitations (p value = 0.039). Conclusion: Mothers who have high anxiety about dental care will ignore visits to the doctor. If there are dental and oral problems such as caries in children, this condition becomes protracted and affects the quality of life of children. Keywords: Caries, Dental anxiety, Maternal, Oral health, Quality of life

Abstract

Background: Dental caries is a common chronic infectious disease of dental hard tissue resulting from the interaction between tooth structure, the microbial biofilm formed on the tooth surface, dietary as well as salivary influences. Streptococcus mutans are considered to be the main bacteria that play role in the formation of cariogenic biofilms. Chlorhexidine gluconate 0,2% is a commonly used mouthwash and considered as the gold standard. The long-term use of chlorhexidine gluconate 0,2% may cause side effects so an alternative mouthwash with natural ingredients is needed. Kelakai leaves extract contains bioactive compounds which has antimicrobial effects such as flavonoids, alkaloids, tannins and saponins which can be used as an alternative mouthwash for chlorhexidine gluconate 0.2%. Objective: The purpose of this study was to observe the antibacterial effectiveness of kelakai leaves extract with concentrations of 3.125%, 6.25%, 12.5%, 25%, 50%, 75% and 100% with chlorhexidine gluconate 0.2% on growth of Streptococcus mutans bacteria. Methods: This study conducted through true experimental laboratories with a post-test only research design with control group design. This study used 9 treatments with 5 repetitions so that there were a total of 45 samples.  The treatment group was kelakai leaves extract with concentrations of 3.125%, 6.25%, 12.5%, 25%, 50%, 75% and 100% with two control groups namely chlorhexidine gluconate 0.2% and distilled water against Streptococcus mutans. Results: The MIC of kelakai leaves extract was set at a concentration of 12.5% and the MBC at a concentration of 50%. Conclusion: Kelakai leaves extract concentrations 50%, 75% and 100% has antibacterial effectiveness which equivalent to chlorhexidine gluconate 0,2% against Streptococcus mutans bacteria.Keywords: Antibacterial, Stenochlaena palustris leaves extract, Streptococcus mutans

Abstract

Background: Oral lichen planus (OLP) is a chronic inflammatory autoimmune oral mucosa disease. The ulcerative type of OLP presents as erythematous ulceration with mucosal erosions surrounding whitish striae. The aetiology of OLP is unknown, but can be predisposed by psychological factors. Purpose: Reporting ulcerative oral lichen planus predisposed by psychological stress. Case: A 36 years old woman complained very painful ulcer in the right buccal mucosa and a grey-purple lower lip lesion for a year. It had been treated with topical corticosteroids, antibiotics, tretinoin, and vitamins but did not heal. Extraoral examination on the lower vermillion showed bluish-purple macules, diffuse and irregular borders, rough and painful. Intraoral examination in the right buccal mucosa showed an irregular yellowish-white ulcer with an erythematous area and white striae. Case Management: Screening for psychological stress by DASS 42 showed moderate stress and severe anxiety. Autofluorescence examination showed no malignant transformation sign. A complete blood count test showed low neutrophil and lymphocyte count. ANA test was normal.  Systemic Methyl Prednisolone 8 mg was prescribed 2 tabs twice a day and tapered off after the lesion resolved within 1 week. Supportive treatments include Benzydamine HCl and multivitamins. Discussion: Stress, anxiety, and depression were possible factors of OLP mediated by the Hypothalamic-pituitary-adrenal (HPA) axis system and the Sympathetic-adrenal-medullary (SAM) system. Specific mechanisms are mediated by Antigen-specific CD8+ (CTLs) that are activated with the help of CD4+ T-cells. Non-specific mechanism mediated by MMP-9 activation. They led to the apoptosis of keratinocytes. Systemic corticosteroids are an important treatment for oral lichen planus that do not responded to topical steroids.  Conclusion: Oral Lichen Planus is a chronic inflammatory disease that affects the oral mucosa and can be predisposed by psychological stress. Corticosteroid is very effective to treat oral lichen planus, especially the erosive-ulcerative type.  Keywords: Oral Lichen Planus, Psychological Stress, Ulcerative

Abstract

Background: A public health center is one of the primary health services that is obliged to provide health services during the covid-19 pandemic. At that period, dentists in the public health center must continue to provide services to patients according technical instructions. Changes in the service system during the pandemic has different perceptions of service quality which had an impact on patient satisfaction. Objective: The purpose of this study was to describe patient satisfaction of dental services in Banjarmasin during the covid-19 pandemic. Methods: This research is descriptive with a cross-sectional design. The population in this study were all people who came to the dental service of the public health center in Banjarmasin. The number of samples in this study was 86 samples taken by accidental sampling. Results: The results of this study showed that the reliable dimension there was 42 person (49%) who said reliable dimension is very satisfied, 52 person (61%) who said assurances dimension is very satisfied, 42 person (49%) who said tangible dimension is very satisfied, 43 person (50%) who said empathy dimension is very satisfied, and 53 person (61%) who said responsiveness dimension is very satisfied. Conclusion: It can be concluded that most patients are very satisfied with the health services provided by dentists in public health center Banjarmasin during the covid-19 pandemic because most of them applied service quality with 5 dimensions (assurance, empathy, reliable, responsiveness and tangible). Keywords : Assurance, Emphaty, Reliable, Responsiveness, Tangible.

Abstract

Background: South Kalimantan Province is one of the regions in Indonesia with the highest use of dentures with a percentage of 3.3%. Dentures should be cleaned frequently to help maintain their quality, particularly using a denture cleanser. The most widely-used denture cleanser in the community is alkaline peroxide. Alkaline peroxide can effectively inhibit the growth of Candida albicans on dentures. However, long-term use of alkaline peroxide may lead to discoloration of the acrylic resin denture base. The 30% of kelakai leaves extract may be used as an alternative to denture cleanser, because it is recognized to have antifungal properties and is expected to be capable of minimizing the effect of discoloration. Objective: To analyze the immersion effect of 30% kelakai leaves extract on discoloration of heat cured acrylic resin base. Methods: This research was a true experiment with a pretest and posttest design with a control group using a heat cured acrylic resin sample with a diameter of 15mm x 2mm thick. A total of 21 samples were divided into 3 groups of immersion, namely 30% kelakai leaves extract, alkaline peroxide, and aquadest. Samples were measured before and after immersion for 5 days to assess discoloration using a series of digital analysis tools and the CIELAB system. The value obtained is entered in the equation  Results: One Way ANOVA test and Post Hoc Bonferroni statistical test showed that there was a significant difference (p<0.05) between the 30% kelakai leaves extract (7.04 ± 0.31), alkaline peroxide (3.01 ± 0.29), and aquadest (2.31 ± 0.63). Each immersion group indicated significantly different results. Conclusion: There was a change in the color of the heat cured acrylic resin base after immersion in 30% kelakai leaf extract. Keywords: Discoloration, Heat cured acrylic resin, Digital analysis, Stenochlaena palustris leaf, System CIELAB.
