Abstract

Abstract: Multi Drugs Resistant Tuberculosis (MDR TB) is tubercolusis with the least resistance to rifampicin and isoniazid. The progresivity and side effects of MDR TB treatment can be monitored through clinical, imaging, and laboratory evaluation. Monocyte Lymphocyte Ratio (MLR), Neutrophil Lymphocyte Ratio (NLR), and Platelet Lymphocyte Ratio (PLR) are the inflammatory markers used as biomarkers of treatment progress in MDR TB. The purpose of this research was to determine the difference of MLR, NLR, and PLR value pre and post MDR TB therapy and to determine the best prognostic biomarker in the evaluation of MDR TB treatment progress at Ulin General Hospital in January-December 2017. This research was an analytic observational study with a cross-sectional design. The sample was 17 patients selected based on the inclusion and exclusion criteria. Results obtained from paired T-tests showed the decreasing of MLR, NLR, PLR value in MDR TB patients receiving TB therapy. PLR had a more role as inflammatory biomarker of MDR TB treatment with p = 0.000, compared to MLR (ρ = 0.01), and NLR ( ρ = 0.006). Abstract

Abstract: Physical exercise in patients with heart failure can decrease the level of proinflammatory biomarkers andincrease functional capacity. Circuit training is one of the most advantageous exercise models because it improves cardiorespiratory fitness and muscle strength.This study aimed to investigate the effect of circuit training on TNF alphaandsix minutes walk test in patients with chronic heart failure in RSUP Dr. Kariadi. Twenty-six stable chronic heart failure with reduced ejection fraction patients were randomized into exercise group that received circuit training in the rehabilitation center of Kariadi Hospital for a month and control group. TNF- α levels as a inflammatory biomarker and distance of six minute walk test as a functional capacity parameter were taken before and after the exercise period.Nine-teensubjects completed the study without any significant side effects.There was no significant difference in TNF-α levels before and after treatment between treatment groups and control groups (p=0,513). The treatment group with circuit training showed a greater increase in distance of six minute walk test than the control group (p=0,034).It was concluded that circuit training in patients with chronic heart failure can increase distance of six minute walk test, but no changes in TNF alpha levels. Keywords: Circuit training, TNF-a, six minute walk test

Abstract

Abstract: A Maisonneuve’s Fracture is a fracture of proximal third fibula associated with a disruption of the distal tibiofibular syndesmosis, the interosseous membrane and associated injuries (eg, fracture of the medial malleolus, fracture of the posterior malleolus, and rupture of the deltoid ligament). The mechanism for the fracture is an external rotation force to the ankle with transmission of the force through the interosseous membrane which causes a proximal fibular fracture.In a very rare form, we can found an unusual pattern of the level of the involved fibula, such as middle third of fibula. A 19 year old male  presented to us with pain and swelling of his left ankle after felt while playing futsal. Physical examination showed deformity, swelling and tenderness on his left ankle. Range of motion was decreased. On imaging, plain radiographs of left ankle showed dislocation of the ankle, with disruption of syndesmotic and fracture comminutive middle third of fibula. We performed operation which are consists of three procedures; closed reduction, percutaneous fixation of syndesmotic by using a single transfixing screw, and plate-screw for the fibula. Normal range of motion is achieved well after 8 weeks, without pain on full weightbearing. The pattern of the fracture-dislocation of the ankle discussed is very rare. The mechanism of the injury is a twisting motion of the upperbody inward, while the foot is planted, resulting a more higher energy impacted to the ankle and fibula. Abstract

Abstract: The most People with Schizophrenia (PwS) experience a decline in social function caused by various factors. Clinical symptoms of PwS affect the social function of PwS, especially negative symptoms. The purpose of this study is to analyze the effect of negative symptoms on the social function of people with schizophrenia in the community. This is a cross-sectional study. Subjects of the study were people with schizophrenia aged 18-56 years who will follow Community-Based Rehabilitation activities in 10 areas of puskesmas in Yogyakarta Special Region. The study was conducted in February 2017. The social function and negative symptoms of PwS were measured using a validated measuring instrument. Hypothesis test using Anova test. The subjects of the study were obtained by purposive sampling technique of 100 PwS that fulfilled inclusion and exclusion criteria. Subjects with a PSP score of 0-30 (poor: someone who intensively needs care and social support) of 8 people (8%) had negative PANSS score of 28.50 ± 6.78. Subjects with a score of 31-70 (moderate: someone who occasionally needs care and social support) of 57 people (57%) had negative PANSS of 20.65 ± 8.18. Subjects with a score of 71-100 (mild: someone with mild disturbance, but able to perform individual functions) of 35 subjects (35%) had negative PANSS score of 12.49 ± 5.98. PwS which has a negative value of PANSS is greater, has a worse social function. The results showed that the negative symptoms had an effect on the social function of people with schizophrenia which was statistically significant (p <0.05). Negative symptoms affect the social function of PwS. PwS that has a high negative symptom score, has a worse social function. Keywords: social function, schizophrenia, negative symptoms

Abstract

Abstract: Primary tumours of musculoskeletal tissues are relatively uncommomn, they represent only 1% of all malignancies in all ages groups, but the incidence is arising each year. Many medical research and experiments were developed to found appropriate methods of treatment for tumor patients. Mortality rates of malignant musculoskeletal tumor patients still higher than benign cases. The aim of this study was to describe the incidence, age and sex distribution, location and histology of benign and malignant musculoskeletal tumors diagnosed and/or treated at a tertiary referral Orthopaedic Department serving the East Java. This was a retrospective analysis of prospectively collected data of medical records of all patients diagnosed and/or treated for any type of musculoskeletal tumor at Orthopaedic and Traumatology Department of Saiful Anwar Hospital, in the period from January 2011 to December 2017. This study resulted there were 531 cases of musculoskeletal tumor, within the characteristic: the peak of incidence occurs in the patient older than 60; the highest case was Osteosarcoma (92 cases); and breast metastases (40 cases) distributed highest mortality rates for metastastic bone tumor patients. Result of this study was people at age 60 and older have higher risk for Muskulokeletal Tumor, and Osteosarcoma was the most frequent case.  Keywords: Musculoskeletal tumor, data based system, incidence, East Java

Abstract

Abstract: Chronic inflammation contributes to the occurrence of various metabolic diseases and even cancer. Chronic inflammation results from excessive response of free radicals. Free radicals are triggered by various agents and oxidation processes in the body as Reactive Oxygen Stress (ROS). The high level of free radicals (oxidants), without adequate intake of antioxidants will lead to oxidative stress conditions. Oxidative stress triggers persistent of inflammation. The formation of ROS is difficult to avoid because it can be generated from cellular metabolic processes. Therefore, we should intake adequate of antioxidants and avoid the various agents induces ROS in everyday life. Keywords : ROS, Stress Oxidative, Inflammation

Abstract

Abstract: Indonesia has the greatest number of smokers in Southeast Asia dominated by male consume kretek cigarette. Cigarette’s smoke is source of free radicals that can cause oxidative stress to sperm and lead it to infertility. Red rice bran extract has lot of potential antioxidants to stop oxidatif stress. This study was experimental within 30 days. The 25 Sprague dawley male rats divided into 5 groups: K1 wasn’t treated, K2,P1,P2, and P3 exposed to smokes of 2 kretek cigarettes, given 96% ethanol extract of red rice bran dosage 100 mg/Kg (P1), 200 mg/Kg (P2) and 400 mg/Kg (P3). Spermatozoa number and viability was observed. Data tested with One Way Anova. There was significant effect from red rice bran extract toward sperm number and viability (p=0,00). Average spermatozoa number was 91.90±7.72 (K1), 39.68±7,51 (K2), 79.88±8.63 (P1), 86.40±10.5 (P2), 86.00±5.78 (P3). Average viability was  65.00±6.85 (K1), 29.6±5.85 (K2), 51.4±3.50 (P1), 60.00±6.67 (P2), 61,00±2.91 (P3). The increasing of number and viability has been achived at dose 100mg/Kg and best at 400 mg/KgBB. The 96% ethanol extract of  red rice bran can prevent the decreasing number and viability of rat spermatozoa exposed by kretek cigarette. Keywords: Clove cigarette, spermatozoa, rice bran extract

Abstract

 Abstract: One phase of wound healing is the inflammatory phase. Haruan fish extract has shown to accelerate the healing process up because it has anti-inflammatory effects. Toman fish originates from the same genus as the Haruan fish, but its anti-inflammatory effect remains unknown. It was done to analyzed anti-inflammatory effects of Toman fish extract. This study was true experimental design with posttest-only control design. Twenty-seven male mice of Balb-C strain were divided into 3 groups. Incision wound of 1 cm was made along the back of the mice. Nine mice in each group were sacrificed on 3rd days, then histopathology examination was conducted with Haematoxylin eosin. There were significant differences between treatment group given Toman fish extract and positive control group given Haruan fish extract, and also compared to the negative control group in PMN cell examination. There were significant differences between treatment group given Toman fish extract and positive control group given Haruan fish extract compared to negative controls on the number of macrophage cells. Toman fish extract could lower the number of PMN cells and was able to increase the number of macrophages on the 3rd days. Abstract

Abstract: Preeclampsia is a syndrome characterized by an increase in blood pressure and proteinuria that appears in the second trimester of pregnancy which is always recovered in the postnatal period. Preeclampsia can be influenced by many factors, but the analysis of risk factors for preeclampsia in Dr. Murjani Hospital has never been done, especially in relation to the pregnant women age, history of pregnancy, abortion, and stages of hypertension. Thus, this research needs to be done. This research was analytical study with cross sectional approach, which aims to explain the relationship between the age of pregnant women and the stages of hypertension with preeclampsia. Research subjects were patients who came to Dr. Murjani Hospital on 28 May-9 June 2018. Data were collected from the Medical Record section and analyzed by t-test to compare the average of two groups (normal and preeclampsia). Chi Square test with 95% confidence interval was used to find out the relationship between two variables. The results showed that there was no relationship between the age of pregnant mother with preeclampsia incident in Dr. Murjani Hospital, but the stages of hypertension is related with preeclampsia incident in Dr. Murjani Hospital. Keywords: preeclampsia, stage of hypertension, the age of pregnant women

Abstract

Abstract: Chronic Kidney Disease (CKD) is the main cause of hemodialysis therapy. This research aims to study the characteristic of the Chronic Kidney Disease patients who routinely undergo hemodialysis (HD) therapy. Observational research with the cross-sectional design is conducted to the chronic kidney disease patients routinely treated with hemodialysis in the Dr Tjitrowardojo Hospital in Purworejo. The respondents’ characteristics include demography, vital sign, chronic kidney disease causes, lifestyle, pain, and insomnia. Product Moment Pearson correlation test is used to test the data. The research’s result shows that the majority of the respondents are men 22 people (61.1%), aged 40-55 years old 17 people (38,9%), High School graduate 18 people (50%), working 25 people (69,4%), and are married 34 people (88,9%). The majority of the respondents do not smoke 33 people (91,7%), do not drink coffee 25 people (69,4%), do not exercise 32 people (88,9%), and suffered CKD due to hypertension 25 people (69,4%). The majority of the respondents, 24 people, have undergone HD for 1-5 years and every HD treatment, the duration is 4,5 hours. They have low Hb level (< 12 gr/dl), normal albumin level (3,5 – 5,2 g/dl), suffered moderate pain and insomnia (28 people). Respondents’ characteristics correlated strongly are the habit of drinking medicine, pain and insomnia. The conclusion is a smoking habit, pain, and insomnia is the closest characteristics linked to chronic kidney disease patients who routinely undergo hemodialysis.  Keywords: Characteristic, Chronic Kidney Disease, Hemodialysis, Pain, Insomnia
