Abstract

Radiation protection aims to safeguard patients by applying the principles of justification, optimization, and limitation, ensuring radiation doses remain as low as possible without compromising image quality. Pediatric patients exhibit high radiosensitivity because their body cells are actively developing, making them more susceptible to the biological effects of radiation. Pediatric emergencies often require rapid radiological examinations, such as in cases of suspected diaphragmatic eventration. Suboptimal use of exposure factors (kVp, mAs) and collimation settings can increase radiation doses received by pediatric patients. This study aims to describe the Entrance Surface Dose (ESD) values, analyze the influence of technical factors on ESD, and evaluate compliance with IAEA Diagnostic Reference Level (DRL) standards. A mixed-methods approach combining qualitative and quantitative methods was employed. The study subject was a 3-day-old pediatric patient undergoing an OMD (esophagogastroduodenal) examination during an emergency at the Radiology Department of Sleman Regional General Hospital (RSUD Sleman). Data were collected between September 2025 and June 2026 through observation and interviews with four informants: one radiologist, two radiographers, and one medical physicist; quantitative data were derived from Dose Area Product (DAP) values. The OMD examination involved five exposures—AP (first), AP (second), LPO, LPO (repeat), and RPO—resulting in a total ESD of 0.389 mGy. The total DAP was 508.7 mGy·cm², which remains below the IAEA DRL reference value of 760 mGy·cm² for the 0-year age group. Factors such as kVp, mAs, SID, and collimation area were found to directly influence ESD values. The use of a 42 × 42 cm collimation field was excessively large and suboptimal for a pediatric patient.

Abstract

A perianal fistula is an abnormal tract connecting the anal canal to the skin surrounding the anus. Fistulography is used to visualize the fistula tract, thereby aiding in diagnosis. At the Radiology Department of Banyumas Regional General Hospital (RSUD Banyumas), the procedure differs from standard theory; specifically, PA and lateral projections are used, no markers are placed on the preliminary image, and undiluted iodine-based contrast media is utilized. This study aims to examine the fistulography procedure for perianal fistula cases at RSUD Banyumas, including the rationale for the chosen projections, the omission of markers, and the use of undiluted contrast media. A descriptive qualitative method with a case study approach was employed from December 2025 to May 2026. Data collection involved observation, interviews, documentation review, and a literature study. Research subjects included three radiographers, one radiologist, one referring physician, and one radiology nurse. Data were analyzed narratively and compared against established theory. The study found that fistulography for perianal fistula at RSUD Banyumas is performed without special patient preparation. Equipment and materials included a DR X-ray unit, apron, 50 cc syringe, Abocath catheter, gloves, gauze, Betadine, and Hexiol (300 mgI/mL) iodine contrast medium (50 cc). The contrast medium is administered incrementally through the Abocath catheter. The examination involves a plain pelvic radiograph (PA projection) followed by post-contrast images in both PA and lateral projections. These projections were selected to visualize the fistula's tract and depth, with the PA projection serving to minimize radiation exposure to the gonads. Markers are not used because the fistula's location is identified via the placement of the Abocath catheter. Using undiluted contrast medium provides higher opacity, ensuring the fistula tract is clearly visualized.

Abstract

Congenital Talipes Equino Varus (CTEV) is a congenital foot deformity that is commonly found in pediatric patients, with a fairly high prevalence in Indonesia. Radiographic examination of the foot becomes important to assess the degree of deformity and assist in therapy planning. The projections used in cases of Congenital Talipes Equino Varus (CTEV) include the anterior-posterior (AP) projection (Kite Method), lateral projection (Kite Method), and axial dorsoplantar projection (Kandel Method). The objective of this study is to identify the examination techniques for pediatric feet with clinical congenital talipes equino varus (CTEV) and to understand the reasons for using the anteroposterior (AP) and lateral (Kite method) projections. This research uses a qualitative method with a case study approach. Data were collected through observation, interviews, and documentation on five pediatric patients diagnosed with CTEV who underwent foot radiographic examinations at the Radiology Installation of Islamic Hospital Kendal. Data analysis was conducted to compare the radiographic examination techniques applied at RSI Kendal with the recommended radiographic projection standards in the literature, as well as to assess their impact on image quality and diagnostic accuracy. Results radiographic examination of the feet in pediatric patients with CTEV at RSI Kendal was performed without any special preparation, other than removing metal objects from the area being examined. The equipment used includes a CR X-ray machine, 24x30 cm cassette, computer, printer, and image reader. The examination was conducted with two projections, namely anteroposterior (AP) and lateral, both with the patient in a supine position and the X-ray beam perpendicular to the cassette. The FFD used was 100 cm with an exposure factor of 4 mAs and 40-50 kVp. This technique is deemed sufficient to produce diagnostic images in cases of CTEV. Conclusions, pediatric foot examination techniques with two projections (AP and lateral) are deemed effective in establishing the diagnosis of CTEV and producing optimal diagnostic images. The aim of using the anteroposterior (AP) projection is to assess the talo first metatarsal angle and the talocalcaneal angle, while the lateral projection helps to evaluate the laterocalcaneal angle. To achieve better visualization of the foot anatomy, it is advisable to add an axial dorsoplantar projection to enhance the quality of visualization of the anatomical structures of the foot.

Abstract

This study aims to examine the technique of Shoulder Joint radiographic examination in trauma cases carried out at PKU Muhammadiyah Hospital Karanganyar. The method used is qualitative descriptive with a case study approach through direct observation, interviews with radiographers, and documentation of examination procedures. The results showed that the radiographic examination was carried out using Posterior Antero Thorax (AP) projection without special preparation, except for the removal of metal objects around the examination area. This projection was chosen because it was able to assess the condition of the shoulder and thorax simultaneously, allowing comparisons between the healthy side and the traumatized side, as well as detecting abnormalities in the costae and lungs with minimal radiation exposure. Although the AP projections provide an overview, this study highlights the importance of developing a specific Standard Operating Procedure (SPO) for the examination of the Shoulder Joint in trauma cases to improve diagnostic consistency and accuracy. The study also recommends the use of additional projections such as the Scapula Y view and Transthoracic Lateral as a complement to obtain a more detailed visualization of the structure of the shoulder joint and surrounding tissues. These findings are expected to be a reference in the development of radiography protocols for shoulder trauma in health care facilities, as well as improve the quality of radiology diagnostic services as a whole.
