Abstract

Postpartum hemorrhage (PPH) is a critical obstetric emergency and a major contributor to maternal mortality. Understanding risk factors across demographic groups is essential for guiding clinical surveillance. This study aimed to analyze the association between demographic and clinical factors, including maternal hemoglobin level, age, parity, weight, neonatal birth weight, and clinical causes of PPH based on the 4T framework (Tone, Tissue, Trauma, Thrombin), and the incidence of PPH among postpartum women. An observational cross-sectional study was conducted on 40 postpartum women who delivered vaginally at a Midwife Independent Practice (PMB) in Banda Aceh, Indonesia, between February and September 2021. Total sampling was applied. Data were collected through direct observation and clinical records. PPH was defined as estimated blood loss ≥500 mL within two hours after delivery. Statistical analysis included chi-square tests and odds ratio (OR) calculations. The prevalence of PPH in this sample was 42.5%. Perineal rupture was the most common clinical finding (77.5%), followed by retained placenta (12.5%) and uterine atony (10%). A significant association was found between clinical causes and the incidence of PPH (p = 0.001). Maternal anemia (Hb <12 g/dL) was significantly associated with PPH (p = 0.018; OR = 7.5), indicating a high-risk subgroup. Other demographic factors, age, parity, maternal weight, and neonatal birth weight were not significantly associated with PPH (p > 0.05). PPH was significantly associated with clinical causes, particularly uterine atony and retained placenta, as well as maternal anemia. These findings support the need for routine antenatal hemoglobin screening and strengthened postpartum monitoring in midwife-led clinical settings to improve early detection and response to bleeding risks.

Abstract

Malnutrition, in all its forms, includes undernutrition (wasting, stunting, underweight), vitamin or mineral deficiencies, overweight, obesity, and non-communicable diseases caused by diet. Nearly half of deaths in children under 5 years of age are caused by malnutrition. The active role of cadres in early detection of malnutrition and stimulation of growth and development in children at the Integrated Health Post (Posyandu) contributes to reducing the prevalence of malnutrition and thus improving the quality of health. This community service activity trained Posyandu cadres to monitor the nutritional status of toddlers in the Blang Mangat Lhokseumawe Community Health Center (Puskesmas) working area. The problem-solving method was carried out through education, training on measuring nutritional status and filling out the Health Card (KMS). Activities included outreach, pre-tests, group discussions, training, post-tests, and mentoring. Cadres were trained to measure the height and weight, upper arm circumference, and head circumference of toddlers, and to fill out the KMS. The assessment results revealed that the cadres' understanding of how to measure nutritional status and fill out the Health Card (KMS) improved significantly, with 100% coverage, and 80% of them were able to carry out the nutritional status measurements correctly. Observations during the cadre skills training at the integrated health post (Posyandu) showed that all cadres were able to accurately measure the nutritional status of toddlers. This demonstrates active participation and the practical application of the knowledge gained during the training. Improving cadre skills through ongoing training is crucial for monitoring toddler nutritional status.

Abstract

Background:  The transformation of health services through the rebranding of Integrated Health Posts (Posyandu) as a community-friendly service positioned Posyandu as the primary hub for community-based health services, with a lifecycle approach, from pregnant women to the elderly. However, in Aceh Province, Posyandu's functioning still faces challenges such as limited funding, unequal capacity of cadres, and the suboptimal institutional role of the Posyandu Operational Working Group (Pokjanal) and Working Group (Pokja). Purpose: To compile policy recommendations in the form of a policy brief regarding the formation and strengthening of regulations for Pokjanal and Pokja Posyandu at the district/city level in Aceh. Methods: The research employed a strategic review approach based on a policy brief. Data were collected through focus group discussions (FGDs) and in-depth interviews with stakeholders, including the Ministry of Home Affairs, the Ministry of Health, the Health Office, the Community Empowerment Office, the Family Welfare Movement (PKK), the Integrated Health Service Post (Posyandu) working group (Pokjanal), and the National Population and Family Planning Board (BKKBN). Data were analyzed qualitatively through thematic synthesis to formulate policy recommendations. Results:  The findings show that 54% of Posyandu in Aceh are not yet active. Posyandu working groups at the district/city and sub-district levels are not yet functioning optimally, while Posyandu working groups at the village level are still limited. Synthesis of the results of FGDs and interviews resulted in nine policy recommendations, including: socialization of Posyandu's position as a Village Community Institution; affirmation of Posyandu's function as a forum for health and social activities; strengthening the role of working groups in services; increasing the capacity of cadres based on 25 basic competencies; structuring the management structure through Village Regulations/Village Decrees; strengthening lifecycle-based and cross-sectoral services; harmonizing regulations following Law No. 17/2023 concerning Health; and regulating cadre terms of office and incentives. Conclusion: Optimizing Posyandu in Aceh requires derivative regulations at the district/city level for the formation and strengthening of Pokjanal-Pokja, accompanied by clear institutional support and increasing the capacity of Posyandu cadres so that integrated health services can be sustainable.

Abstract

The main problem facing Indonesia today is its still-high population growth rate. This problem can be addressed using effective, long-term contraceptives. One recommended and considered effective contraceptive program is the Long-Term Contraceptive Method, which is an implant. The number of long-term contraceptive acceptors in Indonesia is still low, namely below 5%, including in the Regency of Aceh Besar. The purpose of this study was to provide midwifery care to the implant acceptor, namely Mrs. C (25 years old), in the working area of the Peukan Bada Community Health Center (Puskesmas), Aceh Besar Regency, and document this using the midwifery documentation method (SOAP). Midwifery care for the implant acceptor was provided three times: pre-insertion, insertion, and post-inser
