Abstract

The COVID-19 pandemic has had an impact on health services for breast cancer patients. During the COVID-19 pandemic, there were new government regulations, namely PSBB or large-scale social restrictions. During the PSBB, activities in public facilities are limited to only urgent interests. Therefore, breast cancer patients have difficulties during examination and treatment during the COVID-19 pandemic. The obstacles for breast cancer patients are difficulties in mobilizing to the hospital, health protocol regulations such as maintaining distance, discomfort when wearing masks and swabs before treatment, the emergence of fear and anxiety, expensive transportation costs during examination and treatment, and psychological disorders. So the impact is delay in examination (diagnostic delay) and delay in treatment (treatment delay) of breast cancer patients. This study uses a qualitative descriptive method with an in-depth interview approach that is guided by semi-structured questions and triangulates the perspectives of patients, patients' families, and health workers. The study was conducted from November 2021 to August 2022. The sample used was 27 respondents by purposive sampling. Analysis using condensed text system. There are breast cancer patients who express fear of seeking treatment during the COVID-19 pandemic because of the increasing number of pandemic cases and the stigma of being "covided". However, there are patients who express that they are not afraid to seek treatment during the pandemic because they have an optimistic and strong faith. Patients have problems seeking treatment during the pandemic due swab before treatment, feeling uncomfortable when wearing a mask, and transportation costs constraints. Patients experience diagnostic and treatment delays during the pandemic due to fear of being infected with the corona virus, lack of knowledge about symptoms and treatment of breast cancer, belief in alternative medicine, shyness to carry out initial examinations, fear of medical action, and queue lists. Based on the results of the study, it can be concluded that there are internal and external factors that influence the COVID-19 pandemic on diagnostic delay and treatment delay in breast cancer patients

Abstract

Gangguan pendengaran atau tuli merupakan salah satu masalah yang cukup serius dan banyak terjadi di seluruh negara di dunia. Gangguan pendengaran adalah hilangnya kemampuan untuk mendengar bunyi dalam cakupan frekuensi yang normal untuk didengar . Salah satu penyebabnya adalah pemakaian obat-obat ototoksik seperti streptomisin yang dapat merusak stria vaskularis. Selain tuli konduksi dan sensorineural, dapat juga terjadi tuli campuran. Tuli campuran adalah tuli baik konduktif maupun sensorineural akibat disfungsi konduksi udara maupun konduksi tulang. Pengabdian ini berupa penyuluhan pada semua pasien di Rumah Sakit Islam Siti Rahmah pada semua usia tentang gangguan pendengaran

Abstract

Latar belakang: Pemeriksaan fungsi tiroid pada bayi baru lahir merupakan langkah penting dalam deteksi dini gangguan tiroid, terutama hipotiroidisme kongenital yang berpotensi menyebabkan keterlambatan perkembangan fisik dan mental. Di RSI Siti Rahmah Padang, program pengabdian masyarakat ini dilaksanakan untuk meningkatkan deteksi dini dan intervensi terhadap gangguan fungsi tiroid pada neonatus, dengan tujuan meminimalkan dampak jangka panjang terhadap kualitas hidup dan perkembangan anak. Tujuan: Program ini bertujuan untuk mengevaluasi fungsi tiroid pada bayi baru lahir melalui pemeriksaan laboratorium, serta meningkatkan kesadaran orang tua dan tenaga kesehatan mengenai pentingnya skrining dini. Data yang diperoleh diharapkan dapat menjadi dasar dalam pengembangan strategi kesehatan neonatus yang lebih komprehensif dan berkelanjutan. Metode: Penelitian dilaksanakan dengan metode skrining menggunakan sampel darah yang diambil melalui teknik tusuk tumit. Analisis laboratorium difokuskan pada pengukuran konsentrasi Thyroid Stimulating Hormone (TSH) dengan perbandingan terhadap nilai referensi standar. Selain itu, program ini juga mengintegrasikan kegiatan penyuluhan kepada orang tua dan tenaga kesehatan mengenai tanda-tanda gangguan fungsi tiroid serta pentingnya deteksi dini. Kesimpulan: Program pengabdian masyarakat ini membuktikan bahwa pemeriksaan fungsi tiroid pada bayi baru lahir merupakan upaya strategis dalam mendeteksi dini gangguan tiroid, yang esensial untuk mencegah komplikasi perkembangan. Keterlibatan berbagai stakeholder, baik tenaga kesehatan maupun masyarakat, merupakan kunci sukses dalam penerapan program skrining yang berkelanjutan di lingkungan RSI Siti Rahmah Padang.

Abstract

Background: Head and neck cancer is a malignancy originating from the upper aerodigestive tract. Head and neck cancer patients undergoing radiotherapy using brachytherapy, 2DRT, 3CDRT, and IMRT techniques. Patients undergoing radiotherapy will experience several acute side effects consisting of mucositis, dysphagia, xerostomia, and dermatitis. This is influenced by the technique and dose given. This study aims to grade and time the acute side effects of radiotherapy for head and neck cancer at the Andalas University Hospital Padang. Methods: A quantitative descriptive method with a cross-sectional approach is used. Secondary data with a total sample of 88 patients. This research uses medical record data and is analyzed using statistical analysis methods. Results: This study found that the most head and neck cancer patients who underwent radiotherapy were male 57 (64.8%) patients, nasopharyngeal subsite 41 (46.6%) patients with non-keratinizing nasopharyngeal carcinoma 32 (36.4%) patients. The most common radiotherapy technique for head and neck cancer was IMRT in 58 (65.9%) patients with a dose of 61-70 Gy in 78 (88.6%) patients and the most common stage was IV A in 51 (58%) patients. Patients did not undergo chemotherapy treatment in 51 (58%) and surgery in 69 (78.4%). Body mass index (BMI) before radiotherapy was normal in 74 (84.1%) patients and after radiotherapy became underweight in 57 (64.8%) patients resulting in weight loss 1 kg in 83 (94.3%) patients. The highest grade of radiotherapy side effects was grade 1 which consisted of mucositis 84 (95.5%), dysphagia 83 (94.3%), xerostomia 78 (88.6%), and dermatitis 78 (88.6%) patients. The most frequent timing of mucositis side effects occurred in the 3rd week in 40 (45%) patients, dysphagia in the 3rd week in 41 (47%) patients, xerostomia in the 3rd week in 33 (38%) patients, and dermatitis at week 6 in 24 (27%) patients. Conclusions: Patients with head and neck cancer who undergo radiotherapy from week 1 to week 7 will experience side effects, including mucositis, dysphagia, xerostomia, and dermatitis. The average side effect appears in the 3rd week with grade 1.

Abstract

Background: Breast cancer is experiencing the most significant increase in cases among all cancers worldwide. Approximately 65% to 75 % of advanced breast cancer patients will experience bone metastases. CT scan is the most affordable and accessible method for evaluating bone metastases. This study aims to describe the characteristics of CT scan examination of bone metastases in breast cancer patients in the radiotherapy installation at Universitas Andalas Hospital from 2019 until 2021. Methods: A total of 51 patients with breast cancer who had bone metastases were included in this quantitative descriptive study, using a total sampling approach. The clinical data was taken from the medical records at the radiotherapy installation at Andalas University Hospital. The collected data was processed and analyzed using SPSS statistical software. Results: Based on the results, the mean age of patients was 49 years. The most histopathological type was invasive carcinoma mammae of no special type (57%). The most common site for bone metastases is osteo vertebrae (35%). Mixed lesions were the most frequent bone metastatic finding (51%), with fractures occurring in 27% of cases. Conclusions: The results of this study are breast cancer with bone metastases occurring on average at the age of 49 years, histopathological type the most is invasive carcinoma mammae of no special type. Breast cancer with bone metastases is most common in Osteo Vertebrae with mixed lesions as well as pathological fracture with a low percentage.

Abstract

Background: Diagnostic and treatment delays are contributing factors to the increasing incidence and mortality of breast cancer worldwide, including in Indonesia. The study aimed to discern the underlying factors contributing to diagnostic and treatment delays in breast cancer patients.  Methods: This study uses a descriptive method. Primary data collection through guided interviews using questionnaires to 40 breast cancer patients who underwent treatment. This study was conducted at the University Andalas Hospital from January 2022 to September 2022. Results: The breast cancer diagnostic delay is caused by poor knowledge (52.5%), negative attitudes and behaviors (57.5%), a strong fear of breast cancer (55.0%), good social support (65.0%), and the impact of the Coronavirus Disease 2019 (COVID-19) pandemic on breast cancer examination (75.0%). The breast cancer treatment delay is due to poor knowledge (52.5%), positive attitudes and behaviors (52.5%), a strong fear of breast cancer treatment (55.0%), good social support (67.5%), the impact of the COVID-19 pandemic on breast cancer treatment (75.0%). Conclusions: The overview of factors causing diagnostic delay and treatment delays in breast cancer patients is dominated by negative categories, particularly in terms of knowledge, attitudes and behaviors, fear, and the COVID-19 pandemic. Only social support is positive, falling into a good category, as well as the attitudes and behaviors related to breast cancer treatment, which are also categorized as good.

Abstract

Phantoms play an essential role in medical physics, particularly for radiation dose verification, yet the high cost of commercial standard phantoms often limits their availability. This study aimed to reconstruct a thoracic phantom as a replica of the human chest cavity using simple and low-cost materials. The reconstruction process began with evaluating the density values of the selected materials: cadaver bone was used as a substitute for human bone, styrofoam for lung tissue, and plasticine for soft tissues such as muscle and fat. Density measurements showed that cadaver bone ranged from +350 HU to +1000 HU, styrofoam from –700 HU to –950 HU, and plasticine from +39 HU to +234 HU. The skeletal-based reconstruction successfully represented the anatomical alignment of thoracic organs, as confirmed by CT simulator imaging. These findings indicate that the reconstructed thoracic phantom provides a feasible and cost-effective alternative to expensive commercial phantoms and can be effectively utilized for radiation dose verification in medical physics applications.
