Abstract

Abstrak Indonesia merupakan negara kedua di dunia dengan insidensi Tuberkulosis (TB) terbanyak setelah India, sehingga Indonesia termasuk high-burden country dalam penyakit TB. Angka keberhasilan pengobatan TB di Indonesia baru mencapai 84% pada tahun 2016 sehingga perlu ditingkatkan. Inovasi yang banyak dikembangkan untuk meningkatkan kepatuhan pasien dalam pengobatan TB adalah metode pengingat dengan menggunakan media elektronik, salah satunya melalui SMS mengingat pengguna aktif telepon genggam di Indonesia cukup banyak, yaitu mencapai 70,2 juta untuk smartphone pada tahun 2017 dan terus meningkat. Tujuannya untuk mengetahui pengaruh sistem pengingat melalui pesan singkat (text-messaging reminder system) dalam kepatuhan pasien terhadap pengobatan TB. Pencarian literatur dilakukan pada beberapa pangkalan data jurnal ilmiah kedokteran besar seperti Pubmed, Cochrane, EBSCOhost, dan Science Direct. Artikel disaring sesuai desain yang dibutuhkan untuk tinjauan intervensi, kriteria inklusi, dan kriteria eksklusi. Telaah kritis menggunakan metode intervensi sesuai Oxford Center for Evidence Based Medicine 2011. Telaah kritis dilakukan pada dua artikel terpilih dengan metode RCT dan telaah sistematis. Pada studi RCT terhadap 2207 pasien di Pakistan, diperoleh penggunaan sistem pengingat SMS tidak berpengaruh terhadap kepatuhan pengobatan TB. Pada telaah sistematis yang meninjau empat artikel, diperoleh kesimpulan bahwa sistem SMS mampu meningkatkan kepatuhan pengobatan TB. Namun, signifikansinya masih diragukan karena studi yang diinklusi merupakan studi dengan tingkatan bukti rendah berdasarkan kategori GRADE. Pengaruh metode pengingat berupa SMS terhadap kepatuhan pengobatan TB masih inkonklusif. Diperlukan penelitian lebih lanjut dengan populasi besar serta desain penelitian yang sesuai (RCT). Kata kunci: Tuberkulosis, SMS, Sistem Pengingat, Kepatuhan, Pengobatan Abstract Indonesia ranked second in the world for its Tuberculosis (TB) incidence. Therefore, Indonesia is included in the list of high burden countries for TB. TB treatment success rate in Indonesia for 2016 is 84%, but this number still needs to be increased. Many innovations using electronic devices such as handphones are developed to increase patient’s adherence to TB treatment. One of the easiest applicable methods is through SMS. Indonesia is also a developing country with developing technology usage, with 70.2 million active smartphone users in 2017. To determine whether a text messaging reminder system can increase adherence in patients with Tuberculosis treatment. Literature searching was conducted in large medical journal databases such as Pubmed, Cochrane, EBSCOhost, and Science Direct. The articles are selected by considering the study designs that correlate with the intervention appraisal method, inclusions, and exclusions criteria. Intervention type appraisal was conducted using the guideline of the Oxford Center for Evidence-Based Medicine 2011. Appraisals were made for two chosen articles: one with the RCT method, and the other with a systematic review method. RCT study conducted in Pakistan showed that text messaging reminder system does not affect adherence in patients with TB treatment. The other study that reviews four articles concludes that a text messaging reminder system may increase adherence in TB patients undergoing treatment. But, the significance is still doubted as the studies included in this review are of low evidence level based on the GRADE category. The impact of the text messaging reminder system in increasing treatment adherence of TB patients is still inconclusive. Abstract

Dokter Layanan Primer (DLP) or Primary Care Physician (PCP) is a newly introduced term by the Indonesian government in 2013 since the enforcement of Medical Education Law 20/2013. DLP is a physician who solidifies his/ her education and career in primary care. They have postgraduate medical training in primary care and are experts in this field. In most countries, to be a generalist physician practising at primary care facilities such as health centres and primary care clinics, medical school graduates have to take postgraduate medical training to be proficient in terms of knowledge and skills in primary care services. Family medicine is the main body of knowledge of the primary care postgraduate training program in those countries even though their graduates are called differently among countries. These physicians are called family doctors or family physicians in the United States of America (USA), general practitioners (GPs) in Commonwealth countries, huisarts in the Netherlands. Abstract

The UI Makara Satellite Clinic (KSUI) is a university clinic that serves UI residents and the public as well as a vehicle for  education  and  health  research  in  primary  services.  It has been  a  BPJS  Outpatient  Service  Clinic  (KPRJ)  since October  2018.  It  is  called  a Satellite  Clinic  because  it  is planned  to  become  a  satellite  for  university  hospitals  in the academic health system at the University of Indonesia. Currently, KSUI is located on the Depok campus and the Salemba campus. In its daily services at the Depok campus, KSUI provides  services  for  doctors,  dentists,  nursing, pharmacy,  laboratories,  action  rooms, and psychological counseling.KSUI’s mission is not the same as an ordinary outpatient clinic, but because KSUI is a health service facilitated by the university, KSUI has a mission to maintain the health of the academic community, and not only treat them when they fall  ill.  Therefore,  in  addition  to  services that  respond  to patients coming to the clinic, KSUI also organizes periodic health check services, facilitates Posbindu activities in the faculties and office buildings on campus, and participates in medical care at any gathering or sports activities provided on campus.With  the  Covid-19  case  being  found  in  Indonesia,  KSUI decided  to  change  its service  method  as  quickly  and  as firmly  as  possible  from  face-to-face  services  to online services,  following  the  Chancellor’s  Decree  to  close  the campus  from  face-to-face  learning  and  other  student activities.  With  the  return  of  the  UI  dormitory students and  the  implementation  of  work  from  home  (WFH)  for lecturers  and education  staff,  KSUI  must  provide  safe services during a pandemic and can be accessed by service users  without  having  to  come  to  campus.  Therefore,  this article is a brief piece of information that has been done by KSUI when it decided that services should be carried out online.  It  is  not  easy  for  the  officer,  because  it  has  never been done, and it is not easy for the patient.

Abstract

Background: According to the World Health Organization, healthy aging is the process of developing and maintaining functional abilities that make the elderly happy. The increase in the elderly population requires more attention. In particular, health services at the primary health care level face problems related to the limited capacity of overall health services, especially in terms of health promotion and preventive health issues. It is necessary to improve the quality of health care services for the elderly to prevent greater health problems among the elderly population.Objectives: Understand the need to provide holistic health services for healthy aging and use their capabilities, and strengthen cooperation among health professionals in achieving healthy aging.Discusion: Primary health care is pointed out that primary health care should provide comprehensive services in a holistic manner to support a healthy aging process. Therefore, a well-structured, integrated, and cross-industry collaborative primary care system is needed. The system should include changes in professional behavior, coordination of care, and participation of patients' families and communities in comprehensive health care. This can be achieved through inter-professional education, continuous training and education of primary health care professionals, as well as primary health care services and cross-level health care technology innovation.Conclusions: Healthy aging is not just the absence of disease. Everyone in health and social care at all levels can play a role to help improve healthy aging. To make the elderly healthy, starting from the prevention of young health problems, it requires collaboration between health workers, primary health care and other health service levels, and health care that cooperates with patients, families, and communities.Keywords: healthy aging, primary care, preventive, health worker

Abstract

The transformation of basic health services that was proclaimed as part of the transformation of health services by the Minister of Health of the Republic of Indonesia in 2022 is a dream come true for me. Even though the World Health Organization (WHO) has been advocating for it since 2008 in its report Primary Care Now More Than Ever, Indonesia finally made it happen when the Ministry of Health was not led by a doctor. The emphasis on 'not working as a doctor' as the leader of the Ministry of Health seems to be 'beneficial' for primary health services in Indonesia, which so far has been under the shadow of the 'non-specialist and specialist' bureaucratic level of management which is equated with 'primary services and non-primary services'. So far, the terms and understanding of the government, BPJS, and doctors regarding medical specialist are essentially referral services, not primary services. It is as if a doctor working in primary care is considered not an expert. But if they become an expert, then they should be transferred to another place of work.

Abstract

From the Epidemiology Triangle, we can see that a person’s health status is influenced by 3 factors: host, agent, and the environment. The environment plays the biggest role as the cause of health problems, one of which is the family environment. Family can become a supporting factor or inhibiting factor in the successful management of patient’s cases. Therefore, a primary care physician/family physician needs to conduct family meetings to discuss and agree on solutions with the patient’s family by paying attention to inhibiting and supporting factors and find common solutions for the benefit of the patient.

Abstract

Introduction: Nausea and vomiting (NV) remain as side effects of chemotherapy, which has a detrimental effect on patients’ quality of life (QOL) and treatment adherence. This study aimed to determine the effect of chemotherapy-induced NV (CINV) on the QOL of patients with ovarian cancer treated with chemotherapy at Dr. Cipto Mangunkusumo Hospital (CMH). Methods: We conducted a cross-sectional study in patients with ovarian cancer receiving first-line chemotherapy regimens of carboplatin and paclitaxel. The European Organisation for Research and Treatment helped translate the Quality-of-Life Questionnaire-OV28 (QLQ-OV28) into Indonesian and tested it in a preliminary study. The approved Indonesian version of the QLQ-OV28, with the Quality-of-Life Questionnaire-C30 (QLQ-C30), was then used to evaluate QOL before and 1 week after chemotherapy. Results: Several symptom scales increased, whereas function scales decreased. Moreover, 72.5% had an increase in the NV symptom scale, whereas 67.5% had a decrease in patients’ scale after chemotherapy. CINV had a significant partial effect on reducing QOL (p = 0.047 and y = 12.208–0.432). Conclusions: CINV has a significant influence on lowering the QOL of patients with ovarian cancer undergoing first-line chemotherapy regimens with carboplatin and paclitaxel in CMH.

Abstract

Brachiocephalic arteriovenous fistula (AVF) is the recommended vascular access for long-term hemodialysis. However, AVF failure rates remain high, particularly among patients with diabetes mellitus (DM). Diabetes contributes to vascular structural changes, such as increased intima-media thickness (IMT) and reduced volume flow (VF), which may affect AVF maturation success. To compare changes in IMT, brachial artery volume flow, and draining vein volume flow in chronic kidney disease (CKD) patients with and without DM who underwent brachiocephalic AVF creation. This was a retrospective cohort study conducted at Cipto Mangunkusumo National General Hospital between April and June 2025. A total of 42 CKD patients were enrolled and divided into two groups: 21 with DM and 21 without DM. IMT and volume flow were measured using Doppler ultrasound at weeks 0, 2, and 6 after AVF creation. Data were analyzed using the Mann-Whitney test and independent T-test, depending on data distribution. Forty two participants met the inclusion criteria. No significant differences were found between the DM and non-DM groups regarding changes in brachial artery IMT (p = 0.918), brachial artery volume flow (p = 0.538), or draining vein volume flow (p = 0.642) at week 6. Although both groups showed a trend of increased VF from week 0 to week 6, the differences were not statistically significant. There were no significant differences in changes in IMT, brachial artery volume flow, or draining vein volume flow between CKD patients with and without DM following brachiocephalic AVF creation.

Abstract

The prevalence of Chronic Venous Insufficiency (CVI) continues to increase globally, with approximately 30-40% of the adult population experiencing varicose veins as an early manifestation of venous insufficiency. To date, there are few studies that compare endovenous microwave ablation (EMA) with conventional management of high ligation and stripping (HLS) in terms of quality of life in CVI patients. This study aimed to assess the difference in quality of life of post-operative CVI patients using the CIVIQ-20 questionnaire, by comparing patients who were treated with EMA versus HLS. This prospective cohort study involved 40 CVI patients from RSCM, Hermina Bekasi Hospital, and Hermina Depok Hospital in the May-June 2025 period. Data was collected using questionnaires. Bivariate analysis with Chi-Square test, Fischer's exact test, Independent T-test, and Mann-Whitney U test. Overall, the mean change (?) of CIVIQ score for EMA vs. HLS was 12.82 ± 6.97 vs. 14.33 ± 6.78 (p = 0.334). In patients ? 60 years of age, the ? CIVIQ score for HLS vs. EMA was 16.17 ± 5.67 vs. 10.89 ± 2.67 (p = 0.029). In female patients, the ? CIVIQ score for HLS vs. EMA was 16.63 ± 5.32 vs. 10.23 ± 4.21 (p = 0.006). No significant difference in ? CIVIQ score between HLS and EMA was found in patients < 60 years of age, male, or patients with obesity compared to normoweight. In 60?year-old CVI patients and female CVI patients, management with HLS resulted in a significantly higher CIVIQ-20 ? score than EMA. This showed a better quality of life in both groups of patients when treated with HLS.
