Abstract

Proper insurance system is the first target of millennium development. Indonesian Ministry of Health offers an insurance system for helping reduce infant and maternal mortality rates: The Jampersal. Reproductive health for women and children is the second targets of MDGs . It has been 1 year of Jampersal implementation, therefore it needs to evaluate the effectiveness, especially in a remote area like Ternate in Northern Maluku. The study was conducted by collecting secondary quantitative data and primary qualitative data from 7 Puskesmas in Ternate (out of 8) in August-November 2012. Data were collected from 15 nursery mothers who underwent Jampersal insurance, from 15 midwives, 8 medical doctors and 1 obsgyn specialist from all districts Puskesmas in Ternate. Quantitative data were analyzed by descriptive statistics and qualitative data were analyzed by content analysis. that Jampersal was significantly increased documentation of medical record on mother who delivers the baby in Puskesmas; who were also increased in number. Previously lots of mother delivers at home with traditional birth attendances without proper documentation on medical records. It means that indirectly, Jampersal should have been helping in controling problems on reproductive and child health. However, it is not yet clear that Jampersal actually helps to reduce infant and maternal mortality rate due to many obstacles following the Jampersal implementations.

Abstract

Latar belakang: Umpan balik sangat dibutuhkan dalam pendidikan sarjana dan pendidikan klinik. Umpan balik yang diberikan didalam pendidikan klinik digunakan untuk memberikan penguatan atas tindakan yang dilakukan atau memberikan koreksi terhadap kesalahan yang dilakukan dalam melakukan suatu tindakan perawatan kepada pasien. Umpan balik yang diberikan oleh dosen bervariasi antara satu dosen dengan dosen yang lainnya. Sehingga diperlukan pelatihan untuk pemberian umpan balik tersebut. Tujuan: Tujuan dari penelitian ini adalah untuk memberikan pelatihan pemberian umpan balik dosen pembimbing klinik kepada mahasiswa. Metode: Jenis penelitian ini adalah penelitian kuasi eksperimental. Subyek penelitian terdiri dari 32 dosen yang memenuhi kriteria inklusi, yang terdiri dari dokter gigi umum sebayak 25 orang, dokter gigi spesialis sebanyak 3 orang, dan dokter gigi S2 sebanyak 4 orang. Hasil: 96,6% menyatakan siap untuk memberikan umpan balik kepada mahasiswa setelah mengikuti pelatihan, pertanyaan kedua menyatakan waktu untuk pelatihan cukup sebesar 84,4%, pertanyaan ketiga menyatakan pelatihan umpan balik yang diberikan merupakan hal yang baru sebesar 84,4%, pertanyaan keempat menyatakan materi umpan balik dengan metode Pendleton merupakan hal yang baru sebesar 100%, dan pertanyaan kelima menyatakan materi yang diberikan cukup lengkap sebesar 96,9%. Kesimpulan: Pelatihan pemberian umpan balik dengan metode Pendleton kepada dosen pembimbing klinik di kedokteran gigi memberikan pengalaman dan pengetahuan yang baru untuk digunakan dalam pembimbingan fase klinik kepada mahasiswa.

Abstract

Research on the impacts of the ‘requirement system’ on student learning is still rare, however the system is still widely applied by Dental Schools in many countries. The major consequent of this system is the unpreparedness of students’ learning prior to presenting patients with particular complaints. Objective: This study aimed to explore the effect of the ‘requirement system’ on students’ learning strategy in Dental Education University of Jenderal Soedirman, Purwokerto, Indonesia. Methods: This was a qualitative-phenomenography study. The collection of data was through observations and interviews. The number of subjects was 13 students of the same batch in a clinical education level, determined by purposive sampling. Observations by 2 clinical teachers were done in advance and lasted for six weeks, followed by in-depth interviews. The analysis followed the phenomenography method. Results: Interviews revealed that application of the ‘requirement system’ had prompted the students to get the patients and to learn or not learn correspondingly to the specified cases. Students will have adequate preparation to learn if they are motivated to discuss with the teachers, having previous experiences, and if the patient is perceived to be special. Inadequate preparation of learning occurred when students felt tired, insufficient time between patients’ arrival and presentation in front of clinical teachers, and repetition of the case. Observations revealed that preparation for learning did not consequently lead to students’ performance in doing clinical work. ‘Well-done’ up to ‘less than expected’ performances were found in both single-cases as well repetition-cases. Conclusion: ‘Requirement system’ driven students’ preparation for learning. However, number of cases did not. Modifying the ‘requirement system’ and improving the quality of clinical supervision are two important things suggested by this study

Abstract

Top-quality maternal health care requires midwifery and medical students to work together in multidisciplinary interprofessional education (IPE). Achieving IPE learning competencies requires interprofessional assessment methods; one such is portfolio. Study on portfolio as interprofessional assessment is limited. To evaluate interprofessional core competencies illustrated by IPE portfolios. A qualitative design using content analysis to evaluate portfolios of midwifery and medical students attending three weeks’ interprofessional learning in an Indonesian university. Sixty portfolios were analyzed for four IPE core competencies. Fifteen open-ended questionnaires were collected to confirm the data and further explore issues. Four interprofessional core competencies were illustrated in portfolios. While only three portfolios contained objective evidence of learning, over two-thirds of students could plan appropriate, concrete work based on interprofessional learning. The results indicate that the portfolio assesses interprofessional learning with student reflections that illustrate the achievement of four IPE core competencies. The validity of this competency achievement is also supported and confirmed by the evidence of learning and subsequent learning plans. Furthermore, portfolios also can encourage the students to prepare a concrete and appropriate work plan or study plan for students’ interprofessional learning.

Abstract

Employability skills as non technical skills are important for effective and successful individual participation in the workplace. The main aims of the research were to identify important employability skills needed by graduates of Public Health Study Program Universitas Ahmad Dahlan (PHSP UAD) and to assess the achievement of the employability skills development that has been carried out by PHSP UAD. This study used a mixed method research with convergent design. Quantitative research was conducted with survey to graduates. Qualitative research was conducted with structured interviews to employers. Both data were analyzed separately and interpreted concurrently. Graduates and employers of PHSP UAD in broad outline were in line on the important employability skills needed in the workplace and the assessment of the achievement. Several employability skills consistently were considered important. The achievement of employability skills that were related to public health knowledge and its application was high, meanwhile the achievement of several important employability skills which are needed in the work place in the 21st were low. Employability skills needed for graduates of PHSP UAD were formulated. Curriculum revision is recommended to integrate employability skills into core activities to promote the development of employability skills of PHSP UAD graduates.

Abstract

Teori belajar terdiri dari teori belajar behaviourism, sosial kognitif, dan konstruktivisme. Sistem asrama merupakan pendidikan informal yang mendukung pembelajaran formal. Dengan sistem asrama, mahasiswa diharapkan dapat mandiri dalam hal belajar. Tujuan penelitian ini adalah untuk mengetahui aplikasi teori belajar di asrama berkaitan dengan kemandirian belajar mahasiswa. Penelitian ini menggunakan metode kualitatif dengan pendekatan etnografi. Subjek penelitian berjumlah 14 orang. Pengambilan data menggunakan teknik observasi dan wawancara. Hasil penelitian ini adalah aplikasi teori behaviorisme dengan adanya peraturan terkait kehidupan asrama serta adanya pelanggaran terhadap peraturan di asrama, tetapi punishment kurang tegas. Aplikasi teori sosial kognitif terlihat dominan dengan adanya pengasuh asrama yang menjadi role model, mahasiswa belajar dengan mengamati dan mencontoh, interaksi antar mahasiswa dalam belajar, pengaturan belajar sendiri oleh mahasiswa. Aplikasi teori konstruktivisme adalah pengasuh asrama memfasilitasi mahasiswa, adanya interaksi dan kolaborasi antara pengasuh asrama dan mahasiswa maupun antar mahasiswa, dan adanya pembelajaran sesuai konteks keperawatan.  Abstract Application of Learning Theory related to Independent Learning of the Students. Learning theory consists of behaviorism, social cognitive and constructivism. Boarding school is an informal educational supports formal learning. With a dormitory system, students are expected to be independent in their learning process. The purpose of this study was to determine the application of learning theory related to the student learning independence. This study used a qualitative method with an ethnographic approach. There were 14 subjects. The data was collected by observation and interview techniques. The result of this research was application of the theory of behaviorism with the relevant regulations of dormitory life and a violation of the rules in the dorm, but the punishment was less firm. Application of social cognitive theory was dominant in the presence of dormitory staff as a role model, students learn by observing and imitating, the interaction between students in learning, self regulatory in learning. Application of the theory of constructivism was shown by dormitory staff facilitating to students, the interaction and collaboration between dormitory staff and student and among students and the learning context related to nursing. Keywords: dormitory, independent learning, learning theory

Abstract

ABSTRACTBackground: Indonesia National Health Insurance also regulates referral system, which is intended to increase effectiveness and efficiency of resources. There is a need to optimize tiered referral system: primary, secondary, and tertiary so that it runs effectively and efficiently. Tiered referral for maternal cases are still frequently done in Bengkulu City, especially on emergency cases that cannot be handled by primary health center, a lot of expectant mother are aged <20 and >35 which become risk factor in pregnancy, and patients’ wish to undergo USG exam which is unavailable at PHC. The high frequency of referral among National Health Insurance participants will cause an increase on referral facility utilization, which in turn will increase cost in referral health facility.Objective: To analyze implementation of tiered referral system on maternal cases in National Health Insurance participants in 3 PHC of Bengkulu City.Methods: This study use quantitative design using case studies. Study use primary and secondary data. Primary data collected from observation and in depth interview with medical staff in PHC and patients.Results: From observation and direct interview with medical staffs, patients, and management or doctors from 3 PHC used as sample, 4 aspects among other are found, including staffs perception about health workers availability, drugs availability, medical instrument availability, and availability of health facility are often become obstacle, proven by observation of medicines and instruments that available in PHC. Meanwhile no obstacle found in severity level and access to hospital.Conclusion: Perception of medical staffs in terms of understanding of diagnosis and severity level and also access to hospital is not the main factors. On the contrary, staffs’ perception about human resources, drug, instruments, and health facilities still need support from involved parties in addressing these obstacles found in primary health center. Keywords: Tiered referral, maternal emergency, National Health Insurance ABSTRAKLatar Belakang: Didalam BPJS diatur juga system rujukan kesehatan, yang merupakan manajemen utilisasi pelayanan kesehatan untuk meningkatkan efektifitas dan efisiensi sumber daya, dapat ditinjau dari penyedia kesehatan maupun penerima kesehatan. Perlunya optimalisasi kepatuhan pelaksanaan sistem rujukan berjenjang: primer, sekunder dan tersier agar efektifitas dan efisiensi berjalan optimal. Proses rujukan berjenjang pada pasien maternal di kota Bengkulu masih tinggi, terutama kasus kegawatdaruratan maternal hal ini tidak dapat ditangani oleh fasilitas kesehatan tingkat pertama dan masih adanya ibu hamil yang berusia < 20 tahun dan ibu-ibu hamil usia> 35 tahun yag menjadi resiko tinggi dari kehamilan tersebut dan juga keinginan dari pasien untuk melakukan USG yang tidak tersedia di puskesmas. Tingginya rujukan pasien BPJS akan berdampak pada peningkatan pemanfaatan fasilitas pelayanan tingkat lanjutan, maka akibatnya akan terjadi pembengkakan biaya pelayanan kesehatan pada fasilitas kesehatan lanjutan. Tujuan: Menganalisis pelaksanaan rujukan berjenjang FKTP kasus kegawatdaruratan maternal peserta BPJS Kesehatan pada 3 Puskesmas perawatan di Kota Bengkulu.Metode: Rancangan penelitian yang digunakan adalah kualitatif bersifat case studies. Penelitian ini menggunakan data primer dan data skunder. Data primer diperoleh melalui observasi dan wawancara mendalam (in-depth interview) kepada petugas medis di puskesmas dan pasien. Aspek yang dikaji pada penelitian ini yaitu Severity level, ketersediaan sumber daya manusia kesehatan, ketersediaan obat-obatan, ketersediaan alat-alat medis, ketersediaan fasilitas kesehatan dan akses menuju rumah sakit.Hasil: Dari hasil observasi dan wawancara langsung dengan petugas medis, pasien dan pihak manajemen atau dokter dari 3 puskesmas menjadi tempat penelitian di peroleh informasi bahwa dari ke 6 (enam) aspek tersebut ada 4 aspek antara lain persepsi petugas tentang ketersediaan sumber daya manusia kesehatan, ketersediaan obat-obatan, ketersediaan alat-alat medis, ketersediaan fasilitas kesehatan menjadi kendala yang sering dan di buktikan oleh hasil observasi obat-obatan dan alat-alat yang tersedia di puskesmas. Sedang kan pada aspek severity level dan Akses menuju RS tidak ditemukannya kendala.Kesimpulan: Persepsi petugas medis dilihat dari pemahaman diagnosa dan severity level dan akses menuju RS tidak mengalami kendala. Sedangkan pada persepsi petugas mengenai SDM, Ketersediaan obat-obatan, ketersediaan alat- alat kesehatan dan Fasilitas kesehatan masih diperlukan adanya bantuan dari berbagai pihak yang terkait dalam memperbaiki untuk melengkapi kendala dihadapi di fasilitas kesehatan tingkat pertama. Kata Kunci : Rujukan berjenjang, Kegawatdaruratan maternal, BPJS

Abstract

ABSTRACTBackground: Maternal deaths as a global public health problem and urgent to be addressed through the launch of the Safe Motherhood program. UN Resolution on universal health coverage (Universal Health Coverage) is an important resolution and urged all countries to develop a health system with equitable access and affordable cost. This is one of the programs to reduce MMR and IMR. Based on research in three countries, namely Burkina Faso, Ghana and Tanzania also had a strong effort to improve the quality of maternal and neonatal health (MNH) In addition, these three countries are also striving to improve the performance and motivation of the provider. In Indonesia, according to Ministry of Health Decree No. 59 Year 2014 About the standard of health care, require midwives in cooperation with BPJS through a network of family doctors who have been appointed to make the deal as one of the Government's aim to improve maternal and child health (MCH).Methods: This study is a qualitative research design of phenomenology. Data collection is carried out by the method of in depth review or directly using the guidelines of unstructured interviews, and open questions midwives practice independently as a unit of analysis. Samples or informants taken up to a certain saturation or have reached sufficient number until there is no more data that needs to be explored. Triangulation of data include interview with the chairman of the Indonesian Midwives Association (IBI). Midwife verifiers and family doctor.Results: The results showed that midwives have a bad perception of the delivery package BPJS today. The reason is partly that the mechanism of the claims made package, complicated claim procedures and disbursement process long and low birth rates. So the motivation BPM in cooperation with the current BPJS is relatively small. The phenomenon that researchers have found that there are several midwife, although still tied to cooperation with BPJS but not serving patients with BPJS. Other phenomena that is the BPM want to directly contracted by with BPJS without a network of family doctors. This is due to lack of socialization of BPJS for strengthening primary care program that is currently being initiated by the government.Conclusions: The perception of Independent Midwife Practice against BPJS delivery package is still bad. Although the objective of the government is quite good, but there is still need for evaluation and dissemination as an effort to strengthen the primary care for BPM in the sustainability of cooperation in networks of family doctors in the hope that the package delivery mechanism is not paid in package and increase of the delivery service tarif rates between 800 thousand to 1.5 million IDR. Keywords: Independent Midwife Practice, Delivery Package, BPJS ABSTRAKLatar Belakang: Kematian ibu sebagai masalah kesehatan masyarakat global dan mendesak untuk segera ditanggulangi melalui peluncuran program Safe Motherhood. Resolusi PBB pada cakupan kesehatan universal (Universal Health Coverage) pada bulan desember 2012, yang menggaris bawahi bahwa UHC merupakan resolusi yang penting dan mendesak pada semua negara untuk mengembangkan system kesehatan dengan akses yang adil dan biaya yang terjangkau. Hal ini merupakan salah satu program untuk menurunkan AKI dan AKB. Berdasarkan penelitian di tiga Negara yaitu Burkina Faso, Ghana dan Tanzania juga memiliki upaya yang kuat untuk meningkatkan kualitas kesehatan ibu dan bayi (MNH) Selain itu ketiga negara tersebut juga berjuang untuk meningkatkan kinerja dan motivasi para provider. Di Indonesia, berdasarkan Keputusan Menteri Kesehatan Nomor 59 Tahun 2014 Tentang standar pelayanan kesehatan, mewajibkan para bidan bekerjasama dengan BPJS melalui jejaring dokter keluarga yang telah ditunjuk untuk membuat kesepakatan sebagai salah satu tujuan pemerintah untuk meningkatkan pelayanan kesehatan ibu dan anak (KIA).Metode: Penelitian ini merupakan penelitian kualitatif dengan rancangan fenomenologi,. Pengambilan data dilaksanakan dengan metode wawancara mendalam secara langsung menggunakan pedoman wawancara tidak terstruktur, dan pertanyaan terbuka kepada para bidan praktek mandiri sebagai unit analisis. Sampel atau informan di ambil sampai dengan saturasi tertentu atau telah mencapai kecukupan hingga tidak ada lagi data yang perlu digali. Sebagai triangulasi yang dipilih antara lain ketua IBI, Bidan Koordinator/verifikator dasar dan Dokter keluarga.Hasil: Dari hasil penelitian menunjukkan bahwa para bidan memiliki persepsi yang buruk terhadap paket persalinan BPJS saat ini. Meskipun jumlah paketnya lebih besar dari Jampersal, namun tidak semua paket bisa diklaimkan. Penyebabnya antara lain yaitu mekanisme klaim yang dibuat perpaket, prosedur klaimnya rumit dan proses pencairannya lama serta rendahnya tarif persalinan. Sehingga motivasi Bidan Praktek Mandiri (BPM) didalam kerjasama dengan BPJS saat ini relatif kurang. Fenomena yang peneliti temukan yaitu ada beberapa bidan yang meskipun masih terikat kerjasama dengan BPJS tetapi tidak melayani pasien persalinan dengan BPJS. Hal ini terkait dengan rumitnya prosedur paket persalinan BPJS sehingga bidan enggan untuk mengklaim ke BPJS. Fenomena lainnya yaitu para BPM ingin bisa langsung bekerjasama dengan BPJS tanpa melalui jejaring dokter keluarga. Hal ini akibat kurangnya sosialisasi dari BPJS untuk program penguatan layanan primer yang saat ini sedang dicanangkan oleh pemerintah.Kesimpulan: Persepsi Bidan Praktek Mandiri terhadap paket persalinan BPJS masih buruk. Untuk keberlanjutan menjadi provider di dalam jejaring dokter keluarga para BPM masih ingin tetap melanjutkan namun dengan harapan agar mekanisme paket persalinan tidak di buat perpaket dan ada peningkatan tarif persalinan antara 800 ribu sampai 1.5 juta rupiah. Kata Kunci: Persepsi, Bidan Praktek Mandiri, Paket Persalinan, BPJS

Abstract

Background: Elderly populations have increased in line with the increased life expectancy. Currently, the balance of disease burden on the elderly has shifted towards chronic disease. Cardiovascular disease, diabetes, cognitive impairment and depression are chronic diseases that cause suffering and death in elderly populations. Therefore, it is necessary to conduct a comprehensive and multidimensional health assessment as a preventive effort to reduce morbidity in the elderly.Objective: The general objective of this research was to obtain a description of elderly health status at the Posyandu for the elderly using a geriatric assessment called the ABCDE (Ability, Balance, Cognitive, Diseases, Emotion) method.Methods: This research used a cross-sectional design. The subjects were elderly persons ≥ 60 years old who participated in the Posyandu for the elderly and who had fulfilled inclusion and exclusion criteria. The estimated minimum sample size was 242 selected using stratified random sampling. The statistical analysis in this research employed univariate, and bivariate analysis using Chi square tests.Results: Elderly participants had light dependency (34.2%) with average activities of daily living (ADL) value of 19.39 ± 1.32, positive Romberg test 3.7%, and Sharpened Romberg Test 50%. The prevalence of cognitive impairment was 43.4%, while the average score of Mini Mental State Examination was 23.31 ± 6.08, hypertension prevalence 73.14%, hypercholesterolemia 13.22%, and diabetes mellitus 10.33%. Elderly who had depression and probable depression were 12.81%, consisting of mild depression 11.57% and moderate depression 1.24%. Balance disorders had a statistically significant correlation with ADL (p = 0.012; Ratio Prevalence (RP) = 1.422: 95% CI), cognitive impairment (p = 0.039; RP = 1.31: 95% CI), and depression (p = 0.034; RP = 1.63: 95% CI).Conclusion: The prevalences of hypertension, cognitive impairment, and balance disorder on sharpened Romberg test were high. There were statistically significant correlations between balance disorder and ADL, cognitive function, and depression. These findings support the application of geriatric assessment by using ABCDE which can be applied in primary care setting for elderly people.
