Abstract

Stroke is part of Cerebro Vascular Disease (CVD), which is any form of blood circulation disorder affecting the brain or due to pathological processes in the blood vessels. Acute infarction stroke is caused by blockage or occlusion of the small cerebral arteries caused by blood clots (thrombus) originating from excessive clotting of platelets which must be given antiplatelet agents. Indonesia has biodiversity, including a diversity of plants that can be used as traditional medicine. One of the medicinal plants that contains natural ingredients that have the potential to have biological activity as an antiplatelet is papaya leaves (Noviar, 2023). Flavonoid compounds, steroids and tannins in free form and tannin-protein complexes have anti-aggregation properties. This research uses 2 methods, namely survey and sampling methods to determine the scope of empowerment and education in the form of counseling and assistance. Apart from that, another method used is an educational method in the form of counseling and assistance to provide an understanding of the use of papaya leaves to prevent the risk of blood vessel blockages. The results of research from 18 cadres showed that based on the Wilcoxon test, a sig value of 0.000 <α (0.05) was obtained. Thus, it can be concluded that the pre-test scores and post-test scores have a significant difference in scores with an increase in scores for cadres of 22.66% or 40.02%. Meanwhile, the results of the Paired T Test obtained a sig value of 0.000 <α (0.05), thus it can be concluded that the platelet aggregation value before and after administration of papaya leaves had a significant difference with a decrease in value of 9.94% or 13.87%. This shows that papaya leaves are effective in reducing platelet aggregation to prevent the risk of blood vessel blockages

Abstract

Background: Covid-19 was first discovered in late 2019 in Wuhan, China caused by the coronavirus-2 or SARS-Cov-2. This virus causes a cytokine storm characterized by hyperferritinemia and increased IL-6 levels. Most Covid-19 patients require ventilator assistance due to severe pneumonia, alveoli damage, lung parenchyma damage, and the occurrence of Acute Respiratory Distress Syndrome (ARDS) characterized by decreased oxygen saturation. Objective: to find a correlation between Ferritin levels and IL-6 levels on oxygen saturation. Methods: The type of research used is descriptive. The study population and samples were all patients of Advent Hospital Bandung who were admitted to the ICU room infected with Covid-19 with a positive PCR value with a CT value <40 and examined for Ferritin, IL-6 and oxygen saturation parameters in the period January-December 2021. And using the Spearman correlation statistical test. Results: It is known that the average Ferritin level is 1,528.04 ng/mL, the average IL-6 level is 238.35 pg/mL and the average oxygen saturation value has an average value of 90.1%, and there is a correlation between Ferritin levels and oxygen saturation of 29.7%, but there is no correlation between IL-6 levels and oxygen saturation. Conclusion: there is a correlation between Ferritin levels and oxygen saturation which has a weak level of relationship. Suggestion: Ferritin is recommended as an initial examination to indicate the good and bad condition of Covid-19 patients and can help clinicians in better and faster management of Covid-19 patients, because there is a relationship with oxygen saturation values.

Abstract

Background: Thalassemia is a disease caused by a deficiency or loss of synthesis of one or more normal globin chains. In diseases that require blood transfusions, such as Transfusion Dependent Thalassemia (TDT), it is very important to examine the blood type to determine the blood to be transfused. The ABO blood type system antigen or secretor, is found in body fluids in the form of dissolved antigens, including in saliva and urine. Interference can be caused by the frequency of transfusion, which can change the secretor content in the saliva and urine of thalassemia patients, causing false negatives in the examination of secretor status. Objectives: This study aims to determine the effect of transfusion frequency on the titer of secretor status in saliva and urine specimens of Thalassemia patients. Materials and Methods: The type of study is quasi-experimental, Agglutination-Inhibition method. The research sample was 23 Thalassemia major patients. 15 patients are secretor patients, with a frequency of 1x transfusion a month were 7 people (47%), a frequency of 2x transfusion a month were 5 people (33%), and 4x transfusion a month were 3 people (20%). Results: The results of the study on Thalassemia patients obtained from saliva were titers between 1/8 to 1/256. From urine, the results were non-secretors. In contrast to previous studies, which stated that urinary is still detected but four times weaker than saliva. Based on the ANOVA statistical calculation, sig. 0.909 (p>0.05) was obtained. Conclusions: The conclusion is that there is no effect of transfusion frequency on the titer of secretor status in saliva or urine.

Abstract

ABSTRAK Latar Belakang Saat ini semakin banyak pemeriksaan laboratorium yang menggunakan peralatan otomatis, tentunya sangat menunjang untuk hasil laboratorium yang cepat dan akurat. Salah satu teknologi yang digunakan untuk transportasi sampel yang digunakan di Advent Hospital Bandung adalah Sistem Tabung Pneumatik (STP). STP merupakan bagian integral dari fasilitas medis besar yang menyediakan interkoneksi cepat antar unit di rumah sakit dan sering digunakan untuk mengangkut sampel darah. Prinsip kerja STP adalah sistem dengan vakum parsial melalui jaringan pipa, mendorong wadah silinder sehingga tempat material yang dikirim menyerupai kapsul dapat meluncur di sepanjang jalur pipa dengan kecepatan tinggi. Tujuan : Penelitian ini bertujuan untuk mengetahui apakah ada pengaruh waktu tunggu dan jarak yang ditempuh oleh sistem tabung pneumatik terhadap pemeriksaan PT. Metode : Penelitian ini bersifat deskriptif komparatif yang membandingkan nilai pemeriksaan PT pada sampel pemeriksaan. Sepuluh orang diambil darah venanya, masing-masing 8 tabung vacutainer dengan antikoagulan natrium sitrat 3,2%. Semua tabung vacutainer dikirim ke laboratorium oleh petugas pengiriman. Satu tabung dikirim langsung dan tiga tabung vacutainer lainnya dikirim dari laboratorium ke unit perawatan lantai 1, 4 dan 6 dengan jarak pneumatik 50 meter, 100 meter dan 200 meter, dan empat tabung lainnya dikirim dengan cara yang sama setelah waktu tunggu 1 jam. Pemeriksaan PT dilakukan secara bersamaan menggunakan koagulator metode magnetis mekanik. Hasil : Data penelitian diolah secara statistik menggunakan uji GLM, hasil uji diperoleh nilai Fcount = 0,973 dan Sig. = 0,458 > α (0,05). Kesimpulan : Hasil penelitian ini adalah tidak ada pengaruh yang signifikan antara waktu tunggu dan jarak yang ditempuh oleh STP terhadap pemeriksaan PT. Abstract

Background: Platelet Rich Plasma is a biological product in the form of autologous plasma produced from the centrifugation process of blood specimens and has a platelet concentration above normal values. Things that can affect the results of PRP are blood collection, centrifugation speed, centrifugation time and temperature and the use of anticoagulants. Aims: The purpose of this study was to determine the average and see the effect of sodium citrate anticoagulant and ACD-A anticoagulant by using a shelf life of 0 hours, 4 hours and 8 hours in PRP on platelet count. Methods: This type of research is a pseudo-experiment with a Statistica Group Comparison research design. The number of samples in this study were 6 people. PRP research data on platelet count was processed statistically with the General Linear Model (GLM) method. The Results: The average data of PRP on platelet count using sodium citrate anticoagulant with a shelf life of 0 hours 533.333 cell/µL; 4 hours 527.167 cell/µL; 8 hours 190.167 cell/µL. Average data of PRP on platelet count using ACD-A anticoagulant with a shelf life of 0 hours 986.333 cell/µL; 4 hours 976.500 cell/µL; 8 hours 634.833 cell/µL. The data results for 0 hour and 4 hour shelf life on anticoagulants Sodium Citrate and ACD-A have a Sig value of 0.646 0.05 while 8 hour shelf life has a Sig value of 0.000 0.05. Conclusion: No effect of 0 hour and 4 hour shelf life while there is an effect of 8 hour shelf life and there is an effect of anticoagulants Sodium Citrate and ACD-A in PRP on platelet count.

Abstract

Background: Blood services in Indonesia, including processing and storing donor blood, aim to ensure blood quality remains optimal. Washed Red Cell blood components are used for patients with severe allergies due to repeated transfusions, have a limited shelf life and are susceptible to secondary infections. Aims: Blood services in Indonesia, including processing and storing donor blood, aim to ensure blood quality remains optimal. Washed Red Cell blood components are used for patients with severe allergies due to repeated transfusions, have a limited shelf life and are susceptible to secondary infections. Methods: This research design uses a quasi-experiment with serial time measurements. Samples were taken from one bag of WRC donor blood from the Bandung City PMI Blood Transfusion Unit. Specimens were stored at room temperature (20-25°C) and refrigerator temperature (4-6°C) for 0, 3, 6, and 9 hours, and the number of erythrocytes was measured using a Haematology Analyzer. The results of the study showed that the average number of erythrocytes stored at room temperature was 7.54×10⁶/µL at the 0 hour examination, 6.37×10⁶/µL at the 3 hour examination, 5.40×10⁶/µL at the 6 hour examination. and at the 9 hour examination 4.56×10⁶/µL. Meanwhile, the average number of erythrocytes stored at refrigerator temperature was at the 0 hour examination 7.65×10⁶/µL, at the 3 hour examination 7.43×10⁶/µL, at the 6 hour examination 7.25×10⁶/µL, and at 9 hour examination 7.02×10⁶/µL. The results of the General Linear Model (GLM) statistical test show a sig value = 0.000 0.05. Conclusions: temperature and storage time have a significant influence on the number of erythrocytes.

Abstract

Background: Erythrocyte Sedimentation Rate (ESR) is an examination that aims to determine the speed of erythrocytes settling in the blood measured for 1 hour. The examination of ESR performed within 2 hours at most. However, the examination is often not performed immediately due to several reasons that cause delays for several hours. The room temperature during the examination affects the examination. The temperature in West Java has the highest average reaching 32°C. Aims: To determine an effect of EDTA blood storage time and room temperature on the examination of ESR. Method: This research is included in the type of quasi eksperimental research by involving variations in blood storage time immediately, 4 hours, and 5 hours and room temperature during examination is room temperature 20-25oC and temperature 29-32oC. Result: The result of this study showed that there was a significant effect of the variable EDTA blood storage time and room temperature on the examination of ESR which is indicated by the value of Sig. 0,00 (Sig. 0,05). This is because EDTA blood stored more than 2 hours after blood collection will affect the shape of the cells and the temperature can affect the settling speed. Conclusion: Based on the results of statistical tests, it can be concluded that there is an effect of EDTA blood storage time and room temperature on the examination of ESR

Abstract

Chronic Myeloid Leukemia (CML) merupakan suatu kondisi yang ditandai oleh proliferasi abnormal sel hematopoietik yang dapat terjadi pada berbagai kelompok usia. Deteksi Breakpoint Cluster Region-Abelson (BCR-ABL) penting untuk mengidentifikasi keberadaan Kromosom Philadelphia (Ph 1 chr). Pemeriksaan Real-Time Polymerase Chain Reaction (Real-Time PCR) merupakan gold standar untuk deteksi BCR-ABL, namun memiliki keterbatasan seperti waktu pengerjaan lama dan biaya tinggi. Tes Cepat Molekuler (TCM) melalui platform GenXpert menawarkan alternatif yang lebih cepat dan praktis. Penelitian ini bertujuan mengevaluasi kinerja TCM BCR-ABL dibandingkan Real-Time PCR. Desain penelitian menggunakan pendekatan cross sectional dengan 54 sampel yang diperoleh melalui teknik consecutive sampling berdasarkan rumus Lemeshow. Hasil penelitian menunjukkan sensitivitas 91%, spesifisitas 90%, positive predictive value (PPV) 94%, dan negative predictive value (NPV) 86%. Uji Wilcoxon menunjukkan tidak ada perbedaan signifikan antara kedua metode (p=0,100). Berdasarkan hasil penelitian tersebut, TCM memiliki kinerja diagnostik yang mendekati Real-Time PCR dan berpotensi digunakan sebagai alternatif pemeriksaan cepat dan efisien pada deteksi CML.
