Abstract

ABSTRACT Measuring customer satisfaction is an important element in providing better, more efficient and more effective service. If a customer is dissatisfied with a service provided, then the service can be ascertained that it is ineffective and inefficient. This is especially important for public services. The implementation of services to the community, as carried out by the Imelda Indonesian Worker Hospital in Medan, needs to get attention, especially in the quality of service to suit the needs and expectations of the community. This study focuses on the quality of nursing services and patient satisfaction in the Sakura Room Hospital Imelda Hospital Medan inpatient. This study aims to determine the Nursing Care Services by Implementing Nurses with Patient Satisfaction in the Sakura Inpatient Room Imelda General Hospital Indonesian Workers Medan. This research uses descriptive correlation research with a cross sectional approach, while the data analysis used is the Spearman Spirit correlation statistical test. The population in this study were patients who were treated in the Sakura Room inpatient installation of RSU IPI Medan, namely 31 patients who were hospitalized. The number of samples of 31 respondents by means of purposive sampling. The results showed that the quality of nursing services at Inpatient Intalasi IPI Hospital was 20 respondents (64.5%), 6 respondents (19.4%) sufficient category and 5 people (16.1%) in poor category. The conclusion is that there is a relationship (r = 0.615) between closing nursing services and patient satisfaction, where the higher the quality of nursing service, the higher the patient satisfaction.

Abstract

Pembedahan atau tindakan operasi merupakan pengalaman yang sulit bagi hampir semua pasien. Berbagai kemungkinan yang buruk dapat terjadi dan membahayakan bagi pasien, baik ancaman aktual, potensial, integritas pada tubuh maupun jiwa seseorang sehingga seringkali pasien menunjukkan respon atau sikap cemas terutama saat menunggu berlangsungnya jadwal operasi yang disertai dengan terganggunya kualitas tidur pasien karena pasien sering terbangun di malam hari. Keadaan ini disebabkan karena pasien mengalami kecemasan baik dari ringan sampai dengan berat. Penelitian ini bertujuan untuk mengetahui pengaruh kecemasan terhadap kualitas tidur pada pasien dewasa pra operasi di RSU Imelda Pekerja Indonesia Tahun 2018. Penelitian ini merupakan penelitian Kuantitatif dengan pendekatan Korelasi. Sampel dalam penelitian ini merupakan pasien pra operasi, pengambilan sampel menggunakan Teknik Purposive Sampling sejumlah 74 responden. Uji analisis dalam penelitian ini menggunakan uji analaisi regresi liner sederhana. Setelah dilakukan penelitian terhadap kualitas tidur pasien dewasa pra operasi dapat hasil mayoritas responden mengalami Cemas Sedang 38 orang (51,3%), Cemas Ringan 24 orang (32,4%) dan Cemas Berat 12 orang (16,3%). Rata-rata responden mengalami kualitas tidur buruk 45 orang (60,8%) dan yang baik 29 orang (39,2%). Responden yang mengalami kecemasan sedang sampai berat diketahui mayoritas berjenis kelamim perempuan dan yang akan menjalanin Operasi Caesarea dan bedah Orthopedi Dari uji analis menunjukkan nilai Sig = 0,002< 0,005 sehingga dapat disimpulkan bahwa ada pengaruh kecemasan terhadap kualitas tidur pada pasien pra operasi di rsu imelda pekerja indonesia pada tahun 2018.

Abstract

Indonesia's target to eliminate TB in 2010 was not achieved and was pushed back to 2030. The achievement of the target for the percentage of TB detection and treatment in North Sumatra in 2016-2019 has been achieved but has decreased in 2020. There is also the problem of increasing cases of Drug Resistant TB (TB). RO) while TB RO service facilities are still limited in districts and cities. In this study, researchers describe the inhibiting factors of the TB Control Program, especially in North Sumatra and recommend solutions in resolving these inhibiting factors. The method in this study is a Case Study with a single holistic case study. Participants from this study were 9 (nine) people. The phenomena studied are the inhibiting factors in the TB control program and the sources that can be used to solve the problem. Data were collected by interview using a modified question instrument according to developments in the interview process. The inhibiting factors found were discussed using Learning Organization Theory to find a solution. The study found 5 (five) themes from the interviews. From the five themes, it was found that the inhibiting factors of the TB Control Program in North Sumatra, namely (1) the use of SITB which had not been widely socialized so that the data utilization was not optimal, (2) the lack of personnel to carry out direct observations of patient compliance with medication, (3 ) there is no policy that regulates the involvement of the private sector, (4) there is no systematic integrated planning governing cross-sectoral collaboration between TB program managers and other work units in government, (5) socialization of TCM utilization has not been optimal. Control of TB in North Sumatra stems from the implementation of evaluations that have not been optimal and the systematic and measurable program has not been structured that can involve various parties in supporting the management of the TB program. TB managers in North Sumatra need to develop programs that are able to involve various parties with periodic and measurable evaluations. The involvement of various parties with systematic and measurable programs is an opportunity that will support the success of the TB program in North Sumatra.

Abstract

The purpose of the study was to discuss the causes of malnutrition in TB patients: A case study in Medan Indonesia, this research method is a case study from Luck, Jackson and Usher (2005), the results of this study are the factors that cause malnutrition in TB patients found from this study. are (1) elderly and pre-elderly TB patients, (2) TB patients with Diabetes Mellitus, (3) TB patients with HIV, (4) TB patients with treatment failure. The data from this study indicate the need for an assessment of the nutritional status of patients when TB is diagnosed. This is to increase awareness of the nutritional status of the patient since the patient was diagnosed with TB. In accordance with the results of this study, the assessment really needs to be emphasized in cases of TB with the elderly, TB with HIV, TB with Diabetes Mellitus and TB with treatment failure.Abstrak: Tujuan penelitian untuk membahas penyebab malnutrisi pada pasien TB: Studi kasus di Medan Indonesia, metode penelitian ini adalah studi kasus dari Luck, Jackson and Usher (2005), hasil penelitian ini adalah faktor-faktor penyebab malnutrisi pada pasien TB ditemukan dari penelitian ini. adalah (1) pasien TB lansia dan pra-lansia, (2) pasien TB dengan Diabetes Mellitus, (3) pasien TB dengan HIV, (4) pasien TB dengan kegagalan pengobatan. Data dari penelitian ini menunjukkan perlunya penilaian status gizi pasien saat didiagnosis TB. Hal ini untuk meningkatkan kesadaran akan status gizi pasien sejak pasien terdiagnosis TB. Sesuai dengan hasil penelitian ini, pengkajian sangat perlu ditekankan pada kasus TB dengan usia lanjut, TB dengan HIV, TB dengan Diabetes Mellitus dan TB dengan kegagalan pengobatan.

Abstract

High maternal mortality rate (MMR) in Indonesia is an indicator to be controlled.  The goal is to achieve less than 70 per 100,000 live births by 2030.  Preeclampsia is one problem induced the maternal deaths. It has been acknowledged there is some differences preeclampsia risk factors between regions. In this study, the reseracher want to identify the risk factors in preeclampsia cases in Medan, Indonesia. The study was restropective study using case study design.  The unit analysis was medical record patients with preeclampsia who admitted to one private hospital in Medan, fom January until June 2021. The risk factors found in this study did not support previous studies. The maternal age and maternal parity found not different between groups of patients with preeclampsia. However, the older age and more parity had more perinatal problems. Other conditions found in the patiens were anaemia, diabetes mellitus, hepatitis B, hypertension heart diseases with hypoalbumin, and HIV.  Mostly patients had severe preeclampsia, but no maternal death found.  There were neonati problems recorded, which were intrauterine fetal retardation, intrauterine fetal death and premature. It is recommended to give education to the pregnant women to do their antenatal care periodically, especially with previous hystory of preeclampsia or with hypertension problem and young girl about anemia especially with ferrous deficiency problem and the adequate nutrition to be consumed.  It also a good idea to have blood pressure monitoring device at home and know to use it in order to monitor the blood pressure more often in pregnancy period.

Abstract

Background: Tuberculosis (TB) remains a significant public health challenge in Indonesia, with the country experiencing one of the highest numbers of lost cases in TB management. Therefore, there is a need to identify the underlying reasons for this problem. Objective: This study aimed to explore the experiences of TB-diagnosed patients and their families during the time of diagnosis and while undergoing the TB medication program. Methods: This study employed a qualitative descriptive-interpretive approach. The study was conducted in government community health centers (CHC) from May 2022 to July 2022. A total of 22 participants, consisting of 12 TB-diagnosed patients and ten family members, were included in the study. Data were collected through focus group discussions and analyzed thematically. Results: Five themes were developed: (1) Delay in tuberculosis diagnosis, (2) Delay in starting TB treatment, (3) High willingness of patients and their families to recover, (4) Understanding that TB is an infectious disease, and (5) Factors affecting patient recovery. Conclusion: The study findings might contribute to the National TB elimination program. It is recommended that all health workers practicing in the community should be involved in the TB program to improve its management. Collaboration between multiple sectors in the community can also provide an advantage in solving TB problems by increasing new case detection. Additionally, it is suggested that all nurses working with TB patients establish rapport with health cadres and patients’ families to enhance medication adherence in patients.

Abstract

Hemodialysis is one of the treatments that can be carried out in patients with chronic kidney failure where hemodialysis can be a replacement therapy for kidney function. However, a long period of undergoing hemodialysis can affect a person's psychosocial factors due to dependence and limitations in carrying out activities. The aim of this study was to determine the relationship between the length of time undergoing hemodialysis and the depression status of chronic kidney failure patients at the Imelda Workers Indonesia General Hospital, Medan. This type of research is quantitative research with correlational methods and a cross-sectional approach design. This research was conducted in July-August 2023 in the Hemodialysis Room at the Imelda General Hospital for Indonesian Workers in Medan. This research used a Probability sampling technique using the Multistage Sampling method, namely the stratified sampling method in the first stage then the Quota sampling method in the next stage. second, so that the total research sample is 60 respondents. The instrument used in this research is a questionnaire that refers to the Beck Depression Inventory (BDI-II) to determine depression status. Data analysis used univariate and bivariate analysis with statistical tests (Chi-Square Test) in the SPSS application. The research results in this study show the results of the Chi-Square statistical test with a significant (p) value of 0.001 or p<0.05, meaning it exists. There is a relationship between the length of time undergoing hemodialysis and depression status in chronic kidney failure patients at the Imelda Workers Indonesia General Hospital, Medan. The conclusion that we got was that the tendency for depression increased in clients who had just undergone hemodialysis. The longer you undergo hemodialysis, the lower your depression status.

Abstract

Tuberculosis (TB) is one of the biggest global health problems after HIV (Human Immunodeficiency Virus). Tuberculosis is caused by Mycobacterium Tuberculosis and spreads through breathing. The aim of this research is to determine the source of information for health workers in private primary health facilities (Klinik Pratama) in the working area of ??the Terjun Community Health Center, Paya Pasir Village, Medan City. The research method used is a qualitative descriptive research type. The location of this research was carried out at the Pratama Clinic in the Working Area of ??the Terjun Community Health Center, Paya Pasir Village, Medan City. This research was carried out from March to August 2023. The number of participants in this research was 8 health workers who worked at the Pratama clinic. Research data was collected through in-depth interviews with participants and analyzed thematically. The research results consist of 3 themes, namely: 1 (sources of information for health workers in providing Tuberculosis services), 2 (Level of knowledge of health workers regarding tuberculosis services), 3 (Selection of health workers who receive Tuberculosis training). The conclusions from this research are: 1. Health workers at the Pratama Clinic get more information sources from the internet and posters. 2. The level of knowledge of health workers regarding the signs and symptoms of TB is sufficient, but the TB service facilities at the Pratama clinic are not yet satisfactory. 3. The health service visited the clinic but not all health workers took part in tuberculosis training, only those in the tuberculosis program took part (PIC TBC).

Abstract

Tuberculosis (TB) is a type of infectious disease which is one of the ten main causes of world health problems because it has negative impacts including death. The aim of this research is to find out how prepared the private primary health facility Pratama Clinic is for TB services. The research method used is descriptive qualitative research. The location of this research was carried out at four Pratama Clinics in the working area of the Terjun Community Health Center, Paya Pasir Village, Medan City. This research was carried out from March to August 2023. The number of participants in this research were 8 health workers who worked at the Pratama Clinic. Research data was collected through in-depth interviews with participants and analyzed thematically. The research results consist of 5 (five) themes, namely: (1) tuberculosis examinations have been carried out, but sputum examinations are still carried out at community health centers, (2) examination results take a long time due to the lack of equipment, (3) tuberculosis training is only attended by personnel certain health conditions and (4) patients do not return to the clinic to get tuberculosis results because the patient's understanding of tuberculosis is still lacking. The conclusion of this research is that the Pratama clinic is ready to treat TB patients and the clinic only provides pots for suspected sputum from patients, the limited sputum examination tools mean that you have to wait for the results within two to three days, the Pratama clinic has already attended training but those who do training only certain people and patient understanding about tuberculosis is still lacking.

Abstract

Nosocomial infection of Mycobacterium tuberculosis is a serious challenge in hospitals due to the high risk of transmission to healthcare workers due to direct exposure to tuberculosis patients. This study aims to explore the experiences of healthcare workers in preventing nosocomial Mycobacterium tuberculosis infections at Imelda Buruh Indonesia General Hospital, Medan. This study used a descriptive qualitative research type with a phenomenological approach, involving six participants selected through a purposive sampling technique. Data were collected through in-depth interviews and analyzed using the Colaizzi method. The results revealed four themes, namely: healthcare workers' understanding of nosocomial Mycobacterium tuberculosis, healthcare workers' experiences in preventing nosocomial Mycobacterium tuberculosis infections, challenges faced by healthcare workers in preventing nosocomial Mycobacterium tuberculosis infections, healthcare workers' suggestions for improving policies for preventing nosocomial Mycobacterium tuberculosis infections. The conclusion of this study is that healthcare workers have understood how tuberculosis is transmitted and implemented preventive measures such as the use of masks and patient education. However, limited isolation room facilities, lack of standard personal protective equipment, and low patient and family compliance with protocols are significant obstacles
