Abstract

Background: Survived from COVID-19 infection, some patients yet have residual symptoms. Multi-organ and mechanisms of disease can be involved. The data regarding echocardiographic dimension and function of the cardiac in the COVID-19 survivors remains scarce. Method: This was a descriptive cross-sectional study that involves a total of 63 subjects. Subjects were employees and medical residents at National Cardiovascular Center Harapan Kita, who previously get infected by COVID-19. Each subject was examined transthoracic echocardiography once at the time of recruitment. Echocardiographic parameters obtained in this study included dimension and systolic function of the left ventricle and right ventricle, global longitudinal strain by 2D speckle tracking echocardiography, and myocardial work index. Result: More than a half of the subjects experienced persistent symptoms after recovery from COVID-19 infection and mainly was fatigue (33.3%). The timing of data acquisition on the median was 32 days after the negative of the COVID-19 test result. 2D echocardiography measurement of left ventricle indicated mean of end-diastolic diameter and end-systolic diameter was 45 mm and 27 mm, respectively. The mean ejection fraction (EF) of the left ventricle by Simpson’s biplane method was 61%. The median of tricuspid annular plane systolic excursion (TAPSE) parameter was 23 mm and the fractional area change (FAC) parameter was 39%. The mean of global longitudinal strain (GLS) was -19.6%. Conclusion: After recovery from COVID-19 infection, some survivors may have post-acute infectious consequences of COVID-19 such as fatigue, dyspnea, and malaise. However, echocardiographic findings in those patients with mild symptoms, including 2D echocardiography, myocardial strain analysis, and myocardial work index, indicate normal dimension and systolic function in both left ventricle and right ventricle.

Abstract

Abstract Background: Heart valve disease is still a significant health burden in the world, including Indonesia. The postoperative outcome of mitral valve surgery is influenced by many things, including decreased right ventricular (RV) function, which is the most common complication. Several studies have shown that decreased RV function after mitral valve surgery is associated with long-term outcomes. TAPSE is a routine and easy measurement of RV systolic function. A decrease in TAPSE after cardiac surgery is common because of the effects of pericardiotomy, and does not necessarily reflect a decrease in RV ejection fraction (RVEF). Regardless of whether postoperative TAPSE values ​​indicate right ventricular systolic function or only due to the effects of pericardiotomy, it is still not clear whether postoperative TAPSE values ​​have a prognostic value to long-term mortality after mitral valve surgery. Therefore, the objective of this study is to obtain information regarding the relationship of TAPSE echocardiographic parameters after mitral valve surgery with long-term mortality. Methods: This is a retrospective cohort study, looking at the effect of TAPSE on outcome after mitral valve surgery. The analysis starts from the starting point of the study when the patient was discharged alive from the hospital after mitral valve surgery (operation period January 2016 – February 2017) to the end point of the study, which was June 30th, 2021 and the observed outcome was mortality from any cause. Results: Of the 266 study subjects, 11 subjects died within 4-5 years after mitral valve surgery, the mortality is 4%. Bivariate analysis was performed on several factors and no relationship was found between the analyzed variables and mortality. Conclusion: There is no relationship between mortality and TAPSE after mitral valve surgery. Keywords: TAPSE, Mitral Valve Surgery, Mortality

Abstract

Background: Myocardial dysfunction is increasingly recognized as a complication of COVID-19 infection, with implications for patient prognosis and long-term cardiovascular health. Longitudinal strain, measured via echocardiography, is a sensitive marker of myocardial function that may provide valuable insights into cardiac involvement in COVID-19 patients. This study aimed to assess myocardial dysfunction using longitudinal strain analysis in COVID-19 patients, correlating clinical symptoms and laboratory results at admission and four months post-treatment. Methods : This study compared clinical and laboratory parameters in COVID-19 patients post-recovery with and without myocardial dysfunction. Adult COVID-19 survivors were included if they were hospitalized and met certain criteria. Independent variables included clinical factors and laboratory factors at admission, while the dependent variable was myocardial dysfunction assessed through longitudinal strain of the left and right ventricles on speckle tracking echocardiography. The study was conducted at the Harapan Kita Heart and Blood Vessel Center (RSJPDHK - FKUI)/Department of Cardiology and Vascular Medicine, Faculty of Medicine, University of Indonesia, Jakarta. Results: This study examined the cardiovascular health of 162 participants three months after getting infected with COVID-19. Those with comorbidities had the lowest LV GLS levels. Admission factors like obesity, SpO2, and PaO2 levels were linked to decreased LV GLS levels. These findings suggest that these admission factors may predict the progression of COVID-19 syndrome and its implications on cardiovascular health. Conclusion: COVID-19 patients with cardiovascular comorbidities have lower LV-GLS values. CAD status during admission affects LV GLS values 3-6 months after COVID-19 infection, indicating myocardial dysfunction. Basal lateral LV-GLS correlates with obesity status, SpO2, and PaO2 during admission. Closely monitor COVID-19 patients with cardiovascular comorbidities and recognize the implications of CAD status on myocardial function post-infection.

Abstract

Character development in Islamic education is an integral part of shaping a generation with strong faith and high morals. The thoughts of Imam Al-Ghazali, as one of the leading figures in the intellectual tradition of Islam, provide a strong foundation for the development of the concept of "Insan Kamil" or the perfect human. This article aims to analyze the patterns of character development according to Imam Al-Ghazali's thoughts, focusing on efforts towards the transformation of Insan Kamil within the context of Islamic education. The research method employed is a descriptive-analytical approach to examine various works and concepts proposed by Imam Al-Ghazali regarding character development and the concept of Insan Kamil. Analysis is conducted on Al-Ghazali's classical texts and modern interpretations to understand their relevance and applicability in today's educational context. Findings from the study indicate that the concept of character development according to Imam Al-Ghazali provides a robust foundation for holistic and sustainable Islamic education. Principles taught by Al-Ghazali, such as emphasis on noble ethics, simplicity, and honesty, can effectively be integrated to shape individuals who are not only intellectually capable but also morally upright in everyday life.

Abstract

The issue of gender equality in Islam is one of the central themes in contemporary Islamic studies. Normatively, Islamic teachings affirm the principle of justice and the equality of human dignity without gender discrimination. The Qur'an and Hadith contain values that support respect for women in spiritual, social, and political spheres. However, in practice, the application of gender equality in contemporary society often faces challenges, ranging from cultural conservatism to excessive liberalism. This paper analyzes the principles of gender equality in Islam and their application in the modern social context, emphasizing the importance of maintaining balance to avoid falling into extreme mindsets that contradict Islamic values themselves. A proportional approach between normative texts and social realities is necessary to actualize a form of gender equality that is just, ethical, and not excessive. Thus, Islam remains relevant in building a society that equally respects the dignity of both men and women without compromising the principles of Sharia.

Abstract

Pendekatan normatif-teologis dalam studi Islam merupakan pendekatan yang fundamental dalam memahami dan menerapkan ajaran agama Islam. Pendekatan ini menekankan pada kebenaran mutlak ajaran Islam yang dipandang berasal dari wahyu ilahi, termasuk Al-Qur'an dan Hadis. Artikel ini bertujuan untuk menggali dan menganalisis bagaimana pendekatan normatif-teologis digunakan dalam menafsirkan ajaran Islam, serta dampaknya terhadap praktik keagamaan dan pemikiran umat Islam secara umum. Penelitian ini menggunakan metode analisis literatur dan studi pustaka untuk mengeksplorasi berbagai teks-teks suci dan literatur Islam yang relevan, serta mengumpulkan pandangan dari para ahli studi agama dan pemikiran Islam. Hasil kajian menunjukkan bahwa pendekatan normatif-teologis memberikan landasan yang kuat bagi umat Islam dalam mempertahankan identitas keagamaan mereka dan memelihara keaslian ajaran Islam. Namun, pendekatan ini juga dapat menimbulkan tantangan dalam menghadapi dinamika sosial dan kemajuan zaman yang sering kali tidak tercakup secara eksplisit dalam teks-teks suci.
