Abstract

Jaminan Kesehatan Nasional (JKN) merupakan program pemerintah yang bertujuan untuk memberikan perlindungan kesehatan kepada seluruh rakyat Indonesia. Program ini telah memberikan dampak positif bagi masyarakat, salah satunya adalah meningkatnya akses masyarakat terhadap pelayanan kesehatan. Namun, masih terdapat beberapa kendala dalam pelaksanaan program JKN, salah satunya adalah masih kurangnya pengetahuan dan keterampilan kader JKN dalam memberikan informasi dan pelayanan kepada masyarakat. Hal ini dapat menghambat keberhasilan program JKN dalam mencapai tujuannya. Tujuan: meningkatkan keterampilan berkomunikasi bagi kader JKN di Kabupaten Kediri. Metode: Kegiatan pengabdian masyarakat ini dilaksanakan dengan metode pelatihan dan pendampingan dengan materi komunikasi interpersonal, komunikasi publik, dan materi Jaminan Kesehatan Nasional. Pendampingan dilaksanakan selama 1 hari dengan tujuan untuk memberikan kesempatan kepada kader JKN untuk mempraktikkan keterampilan berkomunikasi yang telah mereka pelajari. Hasil: Kegiatan pengabdian masyarakat ini berhasil meningkatkan keterampilan berkomunikasi bagi kader JKN. Hal ini dapat dilihat dari hasil evaluasi yang menunjukkan bahwa peserta pelatihan dapat menerapkan keterampilan berkomunikasi yang telah mereka pelajari dalam memberikan informasi dan pelayanan kepada masyarakat. Kesimpulan: Kegiatan pengabdian masyarakat ini telah memberikan manfaat bagi para peserta dan dapat meningkatkan kualitas pelayanan JKN di Kabupaten Kediri. Namun, perlu adanya upaya untuk meningkatkan keterampilan berkomunikasi bagi kader JKN secara berkelanjutan.

Abstract

Indonesia has ranked third among countries with the highest number of smokers in Asia because the price of cigarettes in those countries is still affordable. The strategy to make the price of cigarettes is not affordable is increasing cigarette excise tax. This instrument is considered the most effective way to control cigarette consumption. The purpose of this study was to find out how the public perceive the increase of the cigarette excise tax to cover the National Health Insurance (NHI) deficit and to identify the factors which affect such perception. This study used mobile phone survey and the sample were 1000 respondents. The list of potential respondent’s mobile phone numbers was selected using the systematic random sampling method with an interval of 100,000 to 200,000. The result showed that 87.9% respondents agree to increase cigarette price so that the children do not start to smoke. The majority of respondents (86.2%) also agree to increase the price of cigarettes to finance the JKN deficit. Perceptions of respondents who agreed to increase the price of cigarettes to prevent smoking initiation in children also tended to accede the increase in cigarette prices for financing the JKN deficit.

Abstract

The Chronic Disease Management Program (Prolanis) is an effort to improve the quality of life for chronic disease sufferers through regular health monitoring and proper management. However, the level of adherence of Prolanis participants to follow the program remains a major challenge. One way to improve participant adherence is by involving health cadres who act as a link between participants and healthcare facilities. This Community Service activity aims to implement education and training for health cadres to increase participant adherence in following Prolanis. The results of the Community Service activity show an increase in cadres' knowledge about Prolanis and their skills in communicating with participants. In addition, cadres were educated to carry out measurements of height, weight, Body Mass Index (BMI), as well as blood sugar and blood pressure measurements. It can be concluded that training health cadres plays an important role in improving Prolanis participants' adherence, and this activity is expected to support the success of Prolanis implementation.

Abstract

Type 2 Diabetes Mellitus (T2DM) represents a growing global health challenge with significant clinical and economic implications, notably in Indonesia. The burden is intensified by renal and cardiovascular complications, especially among National Health Insurance beneficiaries. This retrospective study evaluated the direct medical costs of hospitalized T2DM patients with such complications at a type B hospital from 2019 to 2022. Costs were analyzed from both healthcare provider and payer perspectives, covering medical services, medical support services, pharmaceuticals, and other support services. Of the 192 patients reviewed, most were male (57.8%), aged 45–64 years (72.4%), with hospital stays of 1–5 days (90,1%) and severity level E-4-10-I (100%). Renal complications incur higher treatment costs than cardiovascular complications, with medical support services dominating renal costs and medical services such as doctor and nursing procedures, examinations, and visits contributing most to cardiovascular costs. These results highlight the urgent need for preventive measures, early diagnosis, and innovative treatments to optimize T2DM management and reduce associated economic burdens.

Abstract

Primary health care (PHC) has one of the largest funding sources, capitation, which BPJS Healthcare Security transfers monthly. Capitation fund receipts were frequently insufficient due to failure to meet performance-based capitation/Kapitasi Berbasis Kinerja (KBK) targets, including for PHC in Malang City, Indonesia. This study aimed to examine the management and utilization of capitation funds through a KBK mechanism. This descriptive study used a qualitative approach supported by quantitative data. This study was conducted from July to August 2023 and purposely selected nine informants from BPJS Healthcare Security, the local health office, and PHC. The interviews were transcribed verbatim and analyzed using a thematic analysis approach. Three themes were identified from data analysis: revenue allocation, utilization allocation, and budget expenditure appropriateness. The use of capitation follows the Mayor's regulations, which allocate service and operational costs. However, remaining capitation funds always occur due to unavoidable conditions. Separating accounts between capitation and other funding sources is necessary to track capitation utilization. Therefore, BPJS Healthcare Security needs to participate in capitation reporting and monitoring.
