Abstract. The beautiful fashion home industry is industry engaged in the production of knitted sweaters, clothes and skirts. The results of observations and interviews, there are product failures that occur in the company, namely defects with holes, wrinkles and stains. These defective products occur because the company does not implement quality control in every process. Therefore, it is necessary to identify the most dominant causal factors in each process using Statistical Quality Control (SQC) and Failure Modes and Effect Analysis (FMEA) methods. Based on data processing using SQC, the percentage of defects with holes is 59%, wrinkled 26%, and stains 15%. In addition, the SQC method obtained the root cause of the problem which is influenced by several factors, namely the operator is less skilled, the thread breaks easily, the needle breaks easily, the operator works in a hurry, the duration of pressing the product is too fast, the machine performance decreases, the lighting is dim, the operator does not clean the work table before starting the production process, the machine is dirty, the raw material has been stained from suppliers, the work area is dirty. The results of the FMEA analysis with the highest RPN is 180 for the potential cause of the thread breaking easily and the needle breaking easily, while the lowest is 75 for the potential cause of the dirty work area. The proposed improvement is to choose better thread quality and not break easily, and pay attention to the length of time the needle is used. Abstrak. Home industry indah fashion merupakan industri yang bergerak di bidang produksi rajut sweater, baju dan rok. Hasil observasi dan wawancara, terdapat kegagalan produk yang terjadi di perusahaan yaitu cacat berlubang, berkerut dan noda. Produk cacat tersebut terjadi karena perusahaan tidak menerapkan pengendalian kualitas di setiap prosesnya. Oleh karena itu, diperlukan identifikasi faktor penyebab kecacatan yang paling dominan pada setiap prosesnya dengan menggunakan metode Statistical Quality Control (SQC) dan Failure Modes and Effect Analysis (FMEA). Berdasarkan pengolahan data menggunakan SQC, diperoleh persentase cacat berlubang 59 %, berkerut 26%, dan noda 15%. Selain itu, dengan metode SQC diperoleh akar penyebab masalah yang dipengaruhi oleh beberapa faktor yaitu operator kurang terampil, benang mudah putus, jarum mudah patah, operator bekerja terburu-buru, durasi penekanan terhadap produk terlalu cepat, kinerja mesin menurun, pencahayaan redup, operator tidak membersihkan meja kerja sebelum memulai proses produksi, mesin kotor, bahan baku sudah terkena noda dari pemasok, area kerja kotor. Adapun hasil analisa FMEA dengan nilai RPN tertinggi yaitu sebesar 180 pada potential cause benang mudah putus dan jarum mudah patah, sedangkan nilai terendah yaitu 75 pada potential cause area kerja kotor. Perbaikan yang diusulkan yaitu memilih kualitas benang yang lebih baik dan tidak mudah putus, dan memperhatikan lama waktu penggunaan jarum.

Abstract. CV Kezka Diastama is a company engaged in textile services that produces motif headscarves, plain headscarves, mukena, jersey, t-shirts and others. The motif headscarf is a product that has the highest level of demand but in the production process the motif headscarf experiences high defects as well. The company only sets a standard defect of 2%, but the motif headscarf product in its production activities still has a defect rate ranging from 3.5% - 5.3%. This resulted in the company experiencing losses in raw materials, time and costs. Products that experience defects with an increasing number, will reduce quality and the company experiences losses in raw materials, time and costs. The company wants defects in motif headscarf products to be minimized. Therefore, it is necessary to improve the quality to minimize the number of defects in the product hijab motif. The research method for improving the quality of the motif headscarf is the Failure Mode and Effect Analysis (FMEA) method. Based on the results of FMEA analysis, it is known that the highest RPN values are machine damage of 245 and 280, incorrect fabric positioning of 210 and incorrect machine settings of 240 and 210. The highest RPN results will be taken corrective action. Abstrak. CV Kezka Diastama merupakan salah satu perusahaan bergerak di bidang jasa tekstil yang memproduksi pembuatan jilbab motif, jilbab polos, mukena, jersey, kaos dan lainnya. Jilbab motif merupakan produk yang memiliki tingkat permintaan yang paling tinggi namun dalam proses produksinya jilbab motif mengalami kecacatan yang tinggi juga. Perusahaan hanya menetapkan standar kecacatan sebesar 2%, namun produk jilbab motif dalam aktivitas produksinya masih memiliki tingkat kecacatan yang berkisar 3,5% - 5,3%. Hal tersebut mengakibatkan perusahaan mengalami kerugian bahan baku, waktu dan biaya. Produk yang mengalami kecacatang dengan jumlah yang terus meningkat, mak akan mengurangi kualitas dan perusahaan mengalami kerugian bahan baku, waktu dan biaya. Perusahaan menginginkan kecacatan pada produk jilbab motif dapat diminimasi. Oleh karena itu, perlu dilakukan perbaikan kualitas untuk meminimasi jumlah cacat pada produk jilbab motif. Metode penelitian perbaikan kualitas jilbab motif yaitu dengan metode Failure Mode and Effect Analysis (FMEA). Berdasarkan hasil analisis FMEA diketahui nilai RPN tertinggi yaitu kerusakan mesin sebesar 245 dan 280, penempatan posisi kain tidak benar sebesar 210 dan setting mesin tidak benar sebesar 240 dan 210. Hasil RPN tertinggi akan dilakukan tindakan perbaikan.

Abstract. PT X is a company engaged in manufacturing, namely producing bottled drinking water. The defects that occur exceed the predetermined standards with an average percentage of 3.11%. Efforts can be made to reduce product defects and improve product quality in the production process by using the Statistical Quality Control (SQC) method used to identify problems and Failure Mode and Effect Analysis (FMEA) aims to find and measure various risks that have the potential to cause failure. Based on the results of the analysis using the fishbone diagram to find the root cause of the defect is influenced by several factors, namely the first factor is human, lack of accuracy, negligence, not checking air pressure regularly, not checking the water hose on the machine and not paying attention to the cleanliness of the machine. The second factor is the machine, the temperature setting sensor error and the pressing sensor error. The third factor is that the raw materials used are not good such as the lid melting easily and the cup is too thin. The fourth factor is the method, the SOP is less detailed. Based on the calculation of the risk priority number (RPN) value, the highest result is the cause of the failure of the cup is too thin at 315 while the smallest value is the cause of the failure of the lid from the supplier at 168. Proposed improvements made to reduce defective products are by increasing supervision from supervisors, conducting regular maintenance, checking air pressure regularly, making SOPs more detailed and increasing workers' concern for machine cleanliness and cleaning machines regularly. Abstrak. PT. X merupakan perusahaan yang bergerak di bidang manufaktur yaitu memproduksi air minum dalam kemasan. Kecacatan yang terjadi melebihi standar yang telah ditetapkan dengan rata-rata presentase sebesar 3,11%. Upaya yang dapat dilakukan untuk mengurangi kecacatan produk dan meningkatkan kualitas produk pada proses produksi dengan menggunakan metode Statistical Quality Control (SQC) digunakan untuk mengidentifikasi permasalahan dan Failure Mode and Effect Analysis (FMEA) bertujuan untuk mencari dan mengukur berbagai resiko yang berpotensi menyebabkan kegagalan. Berdasarkan hasil analisa menggunakan fishbone diagram untuk mencari akar penyebab terjadinya kecacatan dipengaruhi oleh beberapa faktor yaitu faktor pertama manusia, kurang ketelitian, kelalaian, tidak mengecek tekanan udara secara berkala, tidak mengecek selang air pada mesin dan kurang memperhatikan kebersihan mesin. Faktor kedua yaitu mesin, sensor pengaturan suhu error dan sensor pengepressan error. Faktor ketiga yaitu bahan baku yang digunakan tidak bagus seperti lid mudah meleleh dan cup terlalu tipis. Faktor keempat yaitu metode, SOP kurang detail. Berdasarkan hasil perhitungan nilai risk priority number (RPN) didapatkan hasil tertinggi yaitu penyebab kegagalan cup terlalu tipis sebesar 315 sedangkan nilai terkecil yaitu penyebab kegagalan lid dari supplier sebesar 168. Usulan perbaikan yang dilakukan untuk mengurangi produk cacat yaitu dengan meningkatkan pengawasan dari supervisor, melakukan maintenance secara berkala, melakukan pengecekan tekanan udara secara berkala, membuat SOP lebih detail dan meningkatkan kepedulian pekerja terhadap kebersihan mesin dan melakukan pembersihan mesin secara berkala.

Abstract. The development of the food industry, especially Bandung City experienced growth and resulted in competition. PD Alam Makmur is a company in the food sector, the problem faced is the decline in sales of meatball products due to companies that do not keep up with the times and experience a decrease in revenue. This research is expected to overcome the problems experienced by PD Alam Makmur, by carrying out business development strategies in order to increase sales of PD Alam Makmur meatball products. By identification using the 7P Marketing Mix (Product, Price, Place, Promotion, People, Process, Phusical Evidence), then determining internal and external factors based on 7P variables. Then a business development strategy was designed using the SWOT (Strength, Weakness, Opportunity, Threats) method, resulting in a strategy that can increase sales of PD Alam Makmur meatball products with the QSPM (Quantitative Strategic Planning Matrix) matrix. The results of the study can provide information based on internal and external factors, aspects that can affect PD Alam Makmur's business development strategy based on internal factors, namely optimizing meatball products with varied variants offered. If based on external factors is to use social media to get a wider market share. The design of business development strategies that can be done by PD Alam Makmur is to implement growth and development strategies. The alternative strategy chosen based on the QSPM matrix is to form a team that focuses on selling halal-certified meatball products online in order to expand market share. Abstrak. Perkembangan industri makanan, khususnya Kota Bandung mengalami pertumbuhan dan mengakibatkan persaingan. PD Alam Makmur merupakan perusahaan dalam bidang makanan, permasalahan yang dihadapi yaitu menurunnya penjualan produk bakso akibat perusahaan yang tidak mengikuti perkembangan jaman dan mengalami penurunan pendapatan. Penelitian ini diharapkan dapat mengatasi permasalahan yang dialami PD Alam Makmur, dengan melakukan strategi pengembangan usaha agar dapat meningkatkan penjualan produk bakso PD Alam Makmur. Dengan indentifikasi menggunakan Bauran Pemasaran 7P (Product, Price, Place, Promotion, People, Process, Phusical Evidence), lalu dilakukan penentuan faktor internal dan eksternal berdasarkan variabel 7P. Kemudian dilakukan perancangan strategi pengembangan usaha menggunakan metode SWOT (Strength, Weakness, Opportunity, Threats), menghasilkan strategi yang dapat meningkatkan penjualan produk bakso PD Alam Makmur dengan matriks QSPM (Quantitative Strategic Planning Matriks). Hasil dari penelitian dapat memberikan informasi berdasarkan faktor internal dan eksternal, aspek yang dapat mempengaruhi strategi pengembangan usaha PD Alam Makmur berdasarkan faktor internal yaitu mengoptimalkan produk bakso dengan varian yang ditawarkan bervariasi. Jika berdasarkan faktor eksternal adalah memanfaatkan media sosial agar mendapat pangsa pasar yang lebih luas. Perancangan strategi pengembangan usaha yang dapat dilakukan PD Alam Makmur adalah menerapkan strategi pertumbuhan dan pembangunan. Adapun alternatif strategi terpilih berdasarkan matriks QSPM yaitu dengan membentuk tim yang berfokus untuk melakukan penjualan produk bakso bersertifikat halal secara online agar dapat memperluas pangsa pasar.

Abstract. PT. Hariff Daya Tunggal Engineering is a company engaged in telecommunications infrastructure, system planning and work on several telecommunication projects, control systems and IT Based Systems. The market strategy applied by the company is Engineering To Order (ETO). Currently PT. Hariff Daya Tunggal Engineering was faced with the problem of defects in SIMON 2000 products that exceeded established policy standards. Therefore, the Statistical Quality Control (SQC) and Failure Mode and Effect Analysis (FMEA) methods to assess defects that occur in the company.This study was conducted to minimize disability that occurs by applying the SQC and FMEA methods. The stages carried out in this study are using 7 SQC tools, then analyzed using FMEA and providing proposals using 5W + 1H based on the highest Risk Priorty Number (RPN) value. The results obtained using the SQC and FMEA methods have defects that must be prioritized, namely poor solder and badfit defects. Both disabilities have the highest RPN values compared to other disabilities. The handling that can be done by the company is to procure closed container racks, display visualization, and procurement of Quality Control divisions of each process, then make Operational Standards (SOPs) as a reference for operators when carrying out the process. Strategi pasar yang diterapkan oleh perusahaan yaitu Engineering To Order (ETO). Saat ini PT. Hariff Daya Tunggal Engineering dihadapkan dengan permasalahan yaitu terjadinya kecacatan pada produk SIMON 2000 yang melebihi standar kebijakan yang telah ditetapkan perusahaan. Oleh karena itu, perusahaan dapat menggunakan Statistical Quality Control (SQC) dan Failure Mode and Effect Analayisis (FMEA) untuk meminimalisir kecacatan yang terjadi di perusahaan.Penelitian ini dilakukan untuk menimalisir kecacatan yang terjadi dengan menerapkan metode SQC dan FMEA. Tahapan yang dilakukan dalam penelitian ini yaitu dengan menggunakan 7 alat SQC, kemudian dianalisis dengan menggunakan FMEA serta memberikan usulan dengan menggunakan 5W+1H berdasarkan nilai Risk Priorty Number (RPN) tertinggi. Hasil yang diperoleh dengan menggunakan metode SQC dan FMEA terdapat kecacatan yang harus diprioritaskan yaitu cacat poor solder dan badfit. Kedua kecacatan tersebut memiliki nilai RPN tertinggi dibanding kecacatan lainya.

Abstract. PT. Poly Jaya Medikal is a company engaged in the pharmaceutical and healthcare equipment industry. The company's sales market extends beyond the domestic market, covering global markets as well. Among the products manufactured by the company is the Wheelchair, which is the most frequently ordered product compared to others. Direct observations at the company revealed that the production process still results in defective products. Defects in wheelchair products include issues such as stuck wheels, dented frames, and deflated hand supports. Continuous negligence of these defects will have adverse effects on the company. Besides losing customers, the company may face bankruptcy. This research employs Statistical Quality Control to identify the most common types of defects and Failure Modes Effect Analysis (FMEA) as a tool for in-depth identification. Frame dents were found to be the most prevalent type of defect, accounting for 49% of defects, with the highest Risk Priority Number (RPN) attributed to the absence of a fixed assembly Standard Operating Procedure (SOP). A suggested improvement solution is to design an assembly SOP on the production floor, aiming to assist workers in performing their tasks more effectively. Abstrak. PT. Poly Jaya Medikal merupakan sebuah perusahaan yang bergerak di bidang industri farmasi dan juga peralatan Kesehatan. Pasar penjualan dari perusahaan tidak hanya berada didalam negeri, tetapi sudah mencakup pasar global. Produk-produk yang dibuat oleh perusahaan diantaranya adalah Kursi Roda. Kursi roda menjadi produk yang paling banyak di pesan jika dibandingkan dengan produk lainnya. Pengamatan secara langsung dilakukan di perusahaan dan didapatkan bahwa dalam produksinya, perusahaan masih menghasilkan produk cacat. Produk cacat pada kursi roda juga ditemui dengan jenis kecacatan seperti roda macet, rangka penyok, dan tumpuan tangan kempis. Kecacatan yang terus menerus dibbiarkan akan berdampak buruk pada perusahaan, selain perusahaan nantinya akan kehilangan pelanggan, perusahaan juga akan menglami kebangkrutan. Penelitian ini menggunakan metode Statistical Quality Control untuk mengidentifikasi jenis kecacatan yang paling banyak terjadi, dan Failures Modes Effect Analysis (FMEA) sebagai alat untuk melakukan identifikasi secara mendalam. Kecacatan jenis rangka penyok memiliki nilai lebih tinggi terjadi dengan rasio kecacatan sebesar 49% dan nilai RPN terbesar pada tidak adanya SOP perakitan yang tetap. Solusi perbaikan yang dapat diusulkan adalah dengan merancang sebuah SOP perakitan pada lantai produksi dan diharapkan dapat membantu para pekerja bekerja dengan lebih baik.

Abstract. CV. Patma Jaya Textile is a textile company focused on making woven sarongs. This woven sarong is intended to meet the needs of society, especially Muslims throughout the world. The company is committed to customer satisfaction and product quality, but faces the challenge of increasing the number of defective products in its woven sarongs. The average defect reached 2.36% in 1 year, exceeding the previous tolerance limit (1.5%). Identification of problems using Fault Tree Analysis shows human, machine, material and environmental factors as causes of defects. The Failure Mode and Effect Analysis method revealed that the largest RPM occurred in potential failure of the surface of the hollow cover, caused by operators who did not follow the company's SOP. Proposed improvements involve increased supervision by the Head of Production, the addition of artificial light at work stations, and training for operators. These steps are expected to reduce defective products. (English) Abstrak. CV. Patma Jaya Textile adalah perusahaan tekstil fokus pada pembuatan sarung tenun. Sarung tenun ini ditujukan untuk memenuhi kebutuhan masyarakat, terutama warga Muslim di seluruh dunia. Perusahaan berkomitmen pada kepuasan pelanggan dan kualitas produk, namun menghadapi tantangan peningkatan jumlah produk cacat pada sarung tenun. Rata-rata cacat mencapai 2,36% dalam 1 tahun, melampaui batas toleransi sebelumnya (1,5%). Identifikasi masalah menggunakan Fault Tree Analysis menunjukkan faktor manusia, mesin, material, dan lingkungan sebagai penyebab kecacatan. Metode Failure Mode and Effect Analysis mengungkap RPM terbesar terjadi pada kegagalan potensial permukaan sarung berlubang, disebabkan oleh operator yang tidak mengikuti SOP perusahaan. Usulan perbaikan melibatkan peningkatan pengawasan oleh Kepala Produksi, penambahan cahaya buatan di stasiun kerja, dan pelatihan untuk operator. Abstract. UD. Mandiri Jaya is a company engaged in the business of trading furniture and electronics which was etablished since 2012. UD. Mandiri Jaya also produces around 120 to 150 units of sofa every month. Currently UD. Mandiri Jaya is experiencing a problem, namely the occurrence of defective sofa products that exceed the actual limit of the predetermined defect tolerance. Therefore, UD. Mandiri Jaya needs to make improvements to improve product quality by using the Statistical Quality Control (SQC) and Failure Mode and Effects Analysis (FMEA) methods. The stages carried out in this study are identifying the causes of defects using tools from SQC including checksheets, process flow diagrams, histograms, pareto diagrams, scatter diagrams, and cause and effect diagrams. After that, the causes of defects that have been identified are analyzed using FMEA through severity, occurrence, detection, calculating the Risk Priority Number (RPN) value, prioritizing defects based on the RPN value, and taking action to reduce defects so that the highest RPN value is obtained. The results obtained from the SQC and FMEA methods, then proposed improvements using 5W + 1H based on the RPN value. The results obtained are that there are defects that have the highest RPN value to be prioritized in improvement including no SOP, the environment is not cleaned regularly, and the quality of the fabric is not good. Abstrak. UD. Mandiri Jaya merupakan perusahaan yang bergerak dalam bidang usaha perdagangan furniture dan elektronik yang berdiri sejak tahun 2012. UD. Mandiri Jaya juga memproduksi sofa sekitar 120 sampai 150 unit setiap bulannya. Saat ini UD. Mandiri Jaya mengalami permasalahan yaitu terjadinya kecacatan produk sofa yang melewati batas aktual toleransi kecacatan yang telah ditentukan. Maka dari itu, UD. Mandiri Jaya perlu melakukan perbaikan untuk meningkatkan kuliatas produk dengan menggunakan metode Statistical Quality Control (SQC) dan Failure Mode and Effects Analysis (FMEA). Tahapan yang dilakukan dalam penelitian ini yaitu mengidentifikasi penyebab kecacatan menggunakan tools dari SQC diantaranya checksheet, diagram alir proses, histogram, diagram pareto, diagram pencar, dan diagram sebab akibat. Setelah itu, penyebab kecacatan yang telah diidentifikasi dianalisis menggunakan FMEA melalui severity, occurrence, detection, menghitung nilai Risk Priority Number (RPN), mengurutkan prioritas kecacatan berdasarkan nilai RPN, dan mengambil tindakan untuk mengurangi kecacatan sehingga didapatkan nilai RPN tertinggi. Hasil yang telah didapat dari metode SQC dan FMEA, kemudian dilakukan usulan perbaikan menggunakan 5W+1H berdasarkan nilai RPN. Hasil yang diperoleh yaitu terdapat cacat yang memiliki nilai RPN tertinggi untuk menjadi prioritas dalam perbaikan diantaranya tidak ada SOP, lingkungan tidak dibersikan secara berkala, dan kualitas kain yang kurang baik.

Abstract. UMKM X is an UMKM that operates in the digital print industry with the products produced, namely jersey products. These products are produced through work stations, namely design work stations, print work stations, cutting work stations, press work stations, and finishing work stations and packaging and sanding. X MSMEs are currently experiencing problems, namely the presence of defective products which result in losses for these MSMEs because these defects cannot be reworked. Based on data on production numbers and data on the number of defects at X MSMEs, it is known that the average percentage of defects during 2022 is 10.28%, this shows that the high number of defects that occur in jersey products. Defects that occur at the press work station are shading defects, defects exposed to other colors, defects in color not entering and pinch defects. The defect that occurs at the printing workstation is the ink spillage defect. Defects that occur in jersey products can be subject to quality improvement using the Fault Tree Analysis (FTA) method to determine the causes of defects that occur and Failure Mode and Effect Analysis (FMEA) to design repairs and produce risk values for the causes of defects that are prioritized for repair or value. Abstrak. UMKM X yang bergerak di bidang industri digital print dengan produk yang dihasilkan yaitu produk jersey. Produk tersebut diproduksi dengan melewati stasiun kerja yaitu stasiun kerja desain, stasiun kerja cetak, stasiun kerja potong, stasiun kerja press, dan stasiun kerja finishing dan packaging serta pengamplasan. UMKM X saat ini sedang mengalami permasalahan yaitu adanya produk cacat yang mengakibatkan kerugian bagi UMKM ini karena kecacatan tersebut tidak bisa dikerjakan ulang. Berdasarkan data jumlah produksi dan data jumlah cacat pada UMKM X diketahui persentase cacat rata-rata selama tahun 2022 sebesar 10,28%, hal tersebut menunjukkan bahwa tingginya kecacatan yang terjadi pada produk jersey. Kecacatan yang terjadi pada stasiun kerja press adalah cacat shading, cacat terkena warna lain, cacat warna tidak masuk, dan cacat pinch. Kecacatan yang terjadi pada stasiun kerja cetak adalah cacat tinta meleber. Kecacatan yang terjadi pada produk jersey dapat dilakukan perbaikan kualitas dengan menggunakan metode Fault Tree Analysis (FTA) untuk mengetahui penyebab kecacatan yang terjadi dan Failure Mode and Effect Analysis (FMEA) untuk merancang perbaikan dan menghasilkan nilai risiko penyebab kecacatan yang diprioritaskan untuk dilakukan perbaikan atau nilai RPN tertinggi pada proses dari suatu produk. Perancangan perbaikan dibuat menggunakan konsep bertanya 5W + 1H yang dilakukan berdasarkan hasil FMEA yang sudah dihitung. Perbaikan yang dilakukan di antaranya adalah memasukkan pelatihan untuk operator yang berpengalaman dan operator yang baru, membuat SOP untuk stasiun kerja press, memasang HEPA Filter di stasiun kerja press, dan memasang dehumidifier di stasiun kerja print.

Abstract. Industrial development has experienced quite a significant increase. Standards or product quality characteristics have been determined, but this does not rule out the potential for products that do not comply with the standards. Several types of defects result in the product not being able to be used as well as losses in terms of time and costs, where the defects are in the form of thread threads that do not match, no closing pull, thread threads that do not match, coloring list is not neat. Based on existing defect data, the author was asked to research this problem to reduce defective products which resulted in production schedules not being on time. The problems that occur in the company include many products with visual and functional defects. So a feasible method is needed to solve this problem. The method used to further identify the problems above is by using Statistical Quality Control (SQC) and to make recommendations for improvement using the Failure Mode and Effect Analysis (FMEA) method. The causes of defects are seen from human factors, methods, materials, machines and tools. Based on the results of the RPN calculation, the highest one gets top priority for improvements. Of these priorities, the one with the highest RPN value is 360. Based on the highest RPN value, namely the product capacity is too small, a proposal for improvement is given to the cause of the defect by increasing the size of the product container at the molding work station by the company. Abstrak. Perkembangan perindustrian cukup mengalami peningkatan yang signifikan. Standar ataupun karakteristik mutu produk sudah ditetapkan, tetapi hal tersebut tak menutup potensi terdapat produk yang tak sesuai terhadap standar. Beberapa jenis kecacatan yang mengakibatkan produk tidak bisa digunakan serta kerugian dari segi waktu dan biaya, di mana kecacatan tersebut berupa drat ulir tidak sesuai, tarikan penutup tidak ada, drat gerigi tidak sesuai, pewarnaan list tidak rapi. Berdasarkan data kecacatan yang ada, penulis diminta untuk meneliti permasalahan tersebut guna mengurangi produk cacat yang mengakibatkan jadwal produksi tidak tepat waktu. Permasalahan yang terjadi di perusahaan adalah terdapat banyak produk dengan kecacatan visual maupun fungsional. Maka, diperlukan suatu metode yang layak untuk menyelesaikan permasalahan tersebut. Metode yang digunakan untuk mengidentifikasi lebih lanjut mengenai permasalahan di atas yaitu dengan menggunakan Statistical Quality Control (SQC) dan untuk membuat usulan perbaikan menggunakan metode Failure Mode and Effect Analysis (FMEA). Penyebab kecacatan dilihat dari faktor manusia, metode, bahan, mesin, dan alat. Berdasarkan hasil perhitungan Risk Priority Number (RPN), nilai tertinggi mendapatkan prioritas utama untuk dilakukan perbaikan. Dari prioritas tersebut, yang memiliki nilai RPN tertinggi yaitu sebesar 360. Berdasarkan nilai RPN tertinggi, yaitu tampungan produk yang terlalu kecil, diberikan usulan perbaikan untuk penyebab terjadinya kecacatan dengan melakukan penambahan ukuran tampungan produk pada stasiun kerja moulding oleh pihak perusahaan.
