Abstract

Pendahuluan: Nyeri punggung bawah atau low back pain (LBP) adalah gangguan kesehatan yang sering dan memilki dampak sosial ekonomi yang berat. Kurang lebih 40% dari nyeri punggung bawah diakibatkan oleh herniasi diskus. Selain herniasi diskus, perubahan endplate tipe Modic juga telah diasosiasikan dengan kejadian nyeri punggung bawah. Meskipun memiliki insidensi yang relatif tinggi dan sering dihubungkan dengan LBP, hubungan antara perubahan endplate tipe Modic dengan penyebab lain LBP seperti herniasi diskus masih belum diketahui dengan jelas. Penelitian ini bertujuan untuk mengetahui hubungan antara derajat herniasi diskus dengan masing-masing tipe perubahan endplate Modic pada pasien yang menjalani pemeriksaan MRI lumbosakral. Metode: Penelitian ini dilakukan dengan desain observasi potong lintang menggunakan data sekunder pada 488 sampel. Derajat herniasi diskus intervertebrais dan tipe perubahan endplate Modic dinilai menggunakan pemeriksaan MRI lumbosakral. Hasil: Derajat herniasi diskus yang paling banyak ditemukan adalah protrusio sebanyak 199 sampel (40,8%) diikuti dengan bulging diskus sebanyak 154 sampel (31,6%). Tipe perubahan Modic yang paling banyak ditemukan pada level terjadinya herniasi diskus adalah perubahan Modic tipe II sebanyak 168 sampel (34,4%). Menggunakan analisa Chi-Square, didapatkan hubungan yang bermakna antara derajat herniasi diskus dengan tipe perubahan Modic dengan nilai p<0,001. Simpulan: Perubahan Modic tipe 1 lebih sering terjadi pada bulging diskus, sedangkan perubahan Modic tipe 2 dan 3 lebih banyak didapatkan pada derajat herniasi yang lebih berat seperti protrusio, ekstrusio dan sekuesterasi.

Abstract

Rabies is a neurological disease with fatal impacts because it has a high mortality rate. Animals, primarily dogs, mediate the spread of rabies. Clinical findings of rabies are divided into two categories: classic rabies (furious type) and paralytic type. Furious rabies has many cardinal features, such as fluctuating consciousness, aerophobia or hydrophobia, inspiratory spasm, and autonomic dysfunction. Ascending paralysis with a lower motor neuron lesion is the initial sign of paralytic rabies. The rabies virus invades and lives in neurons. It is virulent. Besides clinical findings, the diagnostic approach can involve radiology, microbiology, and histopathology. There are not only symptomatic treatments for rabies patients but also vaccines, immunoglobulin, and antivirals to decrease the progressivity of the disease. Vaccination programs and animal control have become essential in reducing rabies cases in Indonesia.

Abstract

Insulin resistance, a precursor to diabetes mellitus and metabolic syndrome, has increased in prevalence in recent decades. Grip strength reflects muscle strength and is thought to provide an overview of the condition of insulin sensitivity. The purpose of this study was to evaluate the association between insulin resistance and handgrip strength in male and female Indonesian adults. This cross-sectional study recruited 169 adult participants, 71 of the subjects were males and 98 of the subjects were females. Handgrip strength, insulin, fasting glucose, HOMA-IR, and OGTT were measured, then the association between handgrip strength and insulin resistance was assessed. Linear regression analysis showed that in male subjects, relative handgrip strength was negatively associated with log HOMA-IR (univariate: B= -0.703, beta= -0.323, p=0.006; multivariate: B= -0.687, beta= -0.316, p=0.012) while no association was found in female subjects (univariate: B= -0.192, beta= -0.136, p=0.183; multivariate: B= -0.172, beta= -0.121, p=0.262). The optimal cut-off value of relative handgrip strength to assess insulin resistance is 0.45 (sensitivity 73.3%, specificity 73.1%) in the male group and 0.34 (sensitivity 68.4%, specificity 61.0%) in the female group. The weaker the handgrip strength in males, the higher the HOMA-IR value. In females, no significant correlation showed between handgrip strength and insulin resistance.

Abstract

This study was aimed to identify the effect of Al-Quran Murottal therapy on sleep quality of non-hemorrhagic stroke patients. This was a quasi design experiment with a control group, involving 18 respondents as the result of simple randomization. Samples were divided into two groups: intervention and control, who met the inclusion and exclusion criteria. Sleep quality was measured by the Pittsburg Sleep Quality Index (PSQI). This study showed that there was a significant difference in sleep quality before and after Al-Quran Murottal therapy (p-value 0,018) even though the result was not significant in the control group (p-value 1,000). There was a significant effect of Al-Quran Murottal therapy for the sleep quality of non-hemorrhagic stroke patients.
