Abstract

Introduction : Diabetes mellitus (DM) has been considered as a ‘‘prothrombotic state’ with enhanced platelet reactivity. Platelet activity and aggregation potential, which are essential components of thrombogenesis and atherosclerosis, can be conveniently estimated by measuring mean platelet volume (MPV) as part of whole blood count.This study was aimed to determine platelet indices [MPV, platelet distribution width (PDW) and platelet-large cell ratio (P-LCR)] between type 2 diabetics with and without microvascular complication.Methods : This study included 70 subjects with type 2 DM (40 with one or more microvascular complications) and 30 without microvascular complications. Anticoagulated blood (ethylene diamine tetracetic acid) was collected and analyzed in an automated blood cell counter for platelet count and indices.Results : In our study found the MPV, PDW and P-LCR were all significantly higher in subjects without microvascular complication compared to the subjects with microvascular complication [ 9.97 (SD 0.68) vs 9.81 (SD 0.76) fl ; 11.81 (SD 1.55) vs 11.44 (SD 1.50) fl ; 24.93 (SD 4.72) vs 23.79 (SD 4.88) % (P >0.05) ], respectively.Conclusion : There was no significant differences in platelet indices (MPV, PDW, and P-LCR) between subjects with and without microvascular complication.Keywords : mean platelet volume; platelet distribution width; platelet-large cell ratio; diabetes melitus; microvascular complication

Abstract

COVID-19 is caused by a novel coronavirus known as SARS-CoV-2, a single chain RNA virus with a particle size of 120-160 nm. Covid-19 can affect almost all age groups, despite, the elderly and people who have records of chronic disease (co-morbid) have the risk to be affected more often and with worse complications from this disease. Diabetes mellitus (DM) is one of the risk factors for enhancing the severity of COVID-19 infection. A 26 years old man, was referred to the hospital with symptoms of shortness of breath, fever, and cough. The patient has no history of comorbidities, but his father has newly type-2 diabetes mellitus (T2DM). The patient was examined, with ad random blood sugar of 220 mg/dL and a chest radiograph suggesting bronchopneumonia. The patient was then examined for fasting and postprandial glucose, HbA1c, and diagnosed with DM, the patient's throat swab showed a positive COVID-19 result. Patients were then treated with antiviral, antibiotic, regulated insulin, and his blood sugar was monitored. The patient's blood sugar is stable and the patient discharge after 34 days of hospitalization.

Abstract

Background: Insulin Resistance (IR) causes a metabolic disorder including hyperglycemia, high blood pressure, and dyslipidemia. The purpose of the study was to assess the relationship of Triglycerides and Glucose Index (Tyg) decline relationship with lipid profile after bay leaf extract treatment in dyslipidemia patients. Method: Fifteen dyslipidemia patients were administered 2x200 mg salam leaf extract capsules for 30 days. Criteria dyslipidemia is defined as one of the following: total cholesterol level ≥ 240 mg/dL, triglyceride levels ≥150 mg/dL, LDL cholesterol levels ≥140 mg/dL, HDL cholesterol levels <40 mg/dL, or the use of lipid-lowering drugs. Blood samples lipid profile for biochemical tests were drawn in the morning after fasting 12 h. Anthropometric BMI and waist circumference measures were taken using standard procedures. The TG/HDL-C ratio was calculated by dividing the serum concentration of TG by HDL-C measured in mg/dL. TyG index was calculated based on formula: Ln [TG (mg/dL) x FG (mg/dL). Result: After 30 days of salam leaf extract treatment, there was a significant correlation of TyG reduction with parameters TC, LDL-C, TG, TG/HD ratio (p: 0.025*, p: 0.001*, p: 0.013*, sequentially) Conclusion: There is a significant correlation between Tyg reduction with TC, TG, and TG/HDL, which leads to a decrease in cardiovascular risk in dyslipidemia patients.

Abstract

ABSTRACT: Background: Postpartum thyroiditis (PPT) is a thyroid dysfunctionn syndrome which is temporary or permanent that occurs 1 year after giving birth or abortion. The occurrence of postpartum thyroiditis is 5 – 10% of postpartum women in the world. It is caused by an inflammation of the thyroid gland which leads to a destruction of the thyroid follicle and proteolysis of thyroglobulin. Case presentation: A woman, 31 years old, was admitted to the hospital complaining an enlargement on her neck. The patient said her the enlargement keeps getting larger and she also complained a feeling of something stuck in the throat. The patient also complained a hoarse voice. The patient has just given birth of her second child 5 months ago. From a hematologic examination, radiologic, and blood culture, the patient was diagnosed with Postpartum Thyroiditis. The patient was given Euthyrox therapy and will undergo repeated evaluation after 1 months of the therapy. Conclusion: We reported a case of Postpartum Thyroiditis that was treated comprehensively, and showed a good prognosis.

Abstract

Background: Individuals with diabetes mellitus (DM), hypertension, and severe obesity (BMI > 40 kg/m2) are more likely to be infected and are at a higher risk for complications and death from COVID-19. The purpose of the study was to evaluate association of Covid-19 severity with blood glucose profile in type 2 diabetes mellitus patients. Method: The study was conducted from June 2021 – September 2021 in the isolation inpatient room of RSUP. H. Adam Malik Medan, and the research samples were all newly patients diagnosed COVID-19 with T2DM comorbidity. The inclusion criteria were all patients aged ≥18 years, COVID-19 group:moderate, severe, and critical degrees, with comorbid T2DM. HOMA-IR was calculated based on fasting plasma insulin and fasting plasma glucose. Results:. There are significant differences in fasting insulin levels and insulin resistance (HOMA-IR) based on severity of Covid-19 infection. There are significant differences on fasting plasma insulin and HOMA-IR between medium degree vs critical degree, and severe degree vs critical degree (p < 0.01). Conclusion: There are significant association between Covid-19 severity with fasting insulin resistance (HOMA-IR) in type 2 Diabetes Mellitus patients, but there aren’t significant association between Covid-19 severity with fasting plasma glucose, post prandial glucose, or HbA1c.

Abstract

Background. C-reactive protein (CRP) is a phylogenetically highly conserved plasma protein that participates in the systemic response to inflammation and its plasma concentration increases during inflammatory states. The purpose of the study was to determine the correlation between CRP and blood sugar profile and lipid profile in obesity undergoing lifestyle modification for 3 months. Method. The study was conducted on hospital nurses with BMI >25 (obesity) who were selected voluntarily. Before making lifestyle modifications (diet and physical exercise) for 3 months, patients are given lighting, understanding, and training on lifestyle modifications. Patients were examined for BMI, FPG, PPG, HbA1c, LDL, HDL, and ApoB before and after 3 months of lifestyle modification. Result. Lifestyle modification for 3 months had been performed on 20 patients with a BMI of 30.9±4.15 kg /m2 (obesity). There is a significant correlation between CRP and PPG, HbA1C, LDLC, and ApoB (p < 0.005). Conclusion. There is a significant correlation between CRP and glucose profile and lipid profile on obesity undergoing lifestyle modification for 3 months.

Abstract

Background: The use of herbal medicine has long been practiced throughout the world, and the production and processing of herbal medicines are constantly being improved to treat various diseases. The aim of this study was to assess the correlation between low density lipoprotein cholesterol and certain lipid profiles in the treatment of bay leaf extract (syzgium polyanthum (wight) walp) to dyslipidemia patients. Method: We recruit dyslipidemia patients, and the search project was given an explanation and asked to give written consent (informed consent) to participate in the research. Then anamnes is carried out and examination:measurement of BMI, measurement of weist circumference, after being satisfied for 10-12 hours, the patient then took blood samples by the laboratory, to examine hs-CRP levels, lipid profiles (total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides), routine blood, kidney function (ureum, creatinine). Blood samples is taken before and after 30 days study. Bay leaf extract 150 mg is taken two times perday . Result: Subjects who participate of the study were 15 peoples as a treatment group and ages 47.47±11,507 years old. There is signifikant differences in cholesterol total, LDL-C and Apo=B (all, p < 0,01). There is a correlation between LDL-C and Total Cholesrerol, Triglyceride, FPG and Apo-B (all, p < 0.05). Conclusion: In this study, bay leaf administration 2x150 mg for 30 days, improved lipid profiles of total cholesterol, LDL, and ApoB in dyslipidemic patients. Bay leaf extract administration has the potential to prevent and improve cardiovascular disease in dyslipidemic patients

Abstract

Background: Subclinical hypothyroidism (SH) is a very common disorder in the general population. In patients with known hypothyroidism or hypothyroidism who have been undergoing treatment, a TSH test should be included in the preoperative assessment to determine the adequacy of treatment and to ensure that thyroid therapy is optimized before surgery. Case Presentation: A 39 years old woman complains about a weakness that has been 4 years interfering with activities. Shortness of breath if doing strenuous activities, walking long distances, and climbing stairs, and shortness of breath decreases after rest. Sore throat found since 1 week of hospitalization, and fever for 1 day. A lump in the neck is not found. Previously, the patient had been treated for 8 days in the Department of Cardiology and planned the installation of a temporary pacemaker due to bradycardia that persisted until cardiac arrest. Conclusion: Subclinical hypothyroid patients undergoing pre-procedure intervention are predisposed to anemia, electrophysiologic disturbances, and hypotension, all of which can precipitate cardiovascular collapse. The goal of therapy in the perioperative patient with thyroid dysfunction centers around the attempt to normalize hormone levels before surgical intervention whenever possible and, when that is not feasible, to use other measures that will maximize hemodynamic stability and prevent decompensation.

Abstract

Introduction: Diabetes mellitus (DM) is inappropriately elevated blood glucose levels. Diabetic foot ulcer (DFU) is the most common complication of diabetes mellitus. Many research studies was found that neutrophil lymphocyte ratio (NLR) have positive correlation with HBA1C, blood glucose, and outcome of diabetic foot ulcer. Methods: The literature searches using online databases such as PubMed, ScienceDirect Elsevier, and Google Schoolar. Result: After doing an online search, we found 39 articles were relevant for this review topic. Analysis: Many research studies have found increased HbA1C (≥6,5) and increased blood glucose (random blood glucose ≥ 200mg/dL and/or fasting blood glucose ≥126 mg/dL) in patients with diabetes mellitus have a positive correlation with increased NLR. And there was increased NLR values in patients with diabetic foot ulcer and NLR values was higher (NLR≥6) in non-healing ulcers than in healing ulcers. Conclusion: NLR can be used as screening tool for determining evaluation of diabetes mellitus and as screening tool for determining outcome of diabetic foot ulcers.

Abstract

Background: Diabetes mellitus (DM) and tuberculosis (TB) are major global health concerns with a bidirectional relationship that exacerbates clinical outcomes. Indonesia faces a high prevalence of both diseases; however, specific risk factors contributing to the incidence of Type 2 DM (T2DM) among pulmonary TB patients remain underexplored, particularly in North Sumatra. This study aims to identify risk factors associated with T2DMin patients with pulmonary TB. Method: A retrospective case-control study was conducted involving 80 participants, comprising 40 pulmonary TB patients with T2DM and 40 without DM. Data were obtained from electronic medical records and analyzed using Chi-square tests, Fisher’s Exact Test, and multivariate logistic regression. Result Educational level (OR = 5.001; p = 0.027), occupational status (OR = 3.500; p = 0.038), and nutritional status based on body mass index (OR = 3.498; p = 0.017) were significantly associated with the incidence of T2DM in pulmonary TB patients. Result: No significant associations were found for age, gender, radiographic findings, or smoking habits. Higher educational level, employment in the private sector, and having a normal to overweight BMI are key risk factors for T2DM among pulmonary TB patients.
