Abstract

Aims: Pleural effusion, characterized by the abnormal accumulation of pleural fluid, poses significant health risks and often necessitates interventions such as water seal drainage (WSD) placement. Diaphragmatic breathing exercises (DBE) are non-invasive techniques that may enhance pulmonary recovery and expedite WSD removal by optimizing respiratory mechanics. Despite the potential benefits, limited research has explored their effectiveness in patients with pleural effusion. Objective: This study aimed to evaluate the effectiveness of diaphragmatic breathing exercises in enhancing pulmonary recovery and expediting WSD removal in pleural effusion patients at Persahabatan Hospital, Jakarta, Indonesia. Methods: A quasi-experimental, single-group pretest-posttest design was employed. Seventeen adult patients diagnosed with pleural effusion undergoing WSD placement were recruited through purposive sampling. Participants performed DBE twice daily for 15 minutes over seven days. Pulmonary recovery and WSD outcomes were assessed using a validated Pulmonary Recovery Assessment Questionnaire. Data were analyzed using paired t-tests, with statistical significance set at p < 0.05. Results: The mean age of participants was 51.71 years (±10.12), with 64.7% female. Peak current respiration significantly improved from 103.53 L/min (±36.22) pre-intervention to 165.52 L/min (±75.28) post-intervention (p = 0.000). DBE also reduced the duration of WSD placement, indicating enhanced pulmonary recovery. Participants reported high feasibility and acceptability of the intervention. Conclusion: Diaphragmatic breathing exercises significantly improve pulmonary recovery and expedite WSD removal in pleural effusion patients. These findings highlight the potential of DBE as a cost-effective, non-invasive adjunct in respiratory rehabilitation. Further research with larger sample sizes and long-term follow-up is recommended to validate these results.

Abstract

Stretch marks manifest as hyperpigmented dermal lesions that commonly occur during pregnancy. These cutaneous changes frequently cause physical discomfort and psychological distress among postpartum women, leading to diminished self-esteem and social withdrawal. Nonpharmacological interventions, particularly olive oil application, are potentially effective management approaches. This study evaluated the efficacy of topical olive oil in reducing stretch mark visibility among postpartum women in the Karang Mulya Community Health Center service area in 2024. This quantitative investigation employed a quasi-experimental design with pre- and posttest assessments. The participants included 45 postpartum women on postpartum day 6 residing within the Karang Mulya Health Center catchment area in March 2024. A total sampling technique was used, and data collection relied on standardized observational assessment tools. Statistical analysis was performed via the Mann‒Whitney test (p<0.05). The baseline assessment revealed a mean stretch mark severity score of 3.87 (SD=1.407) before olive oil application, which decreased significantly to 1.07 (SD=0.799) after the intervention. The experimental group demonstrated a statistically significant improvement compared with the control group (p<0.001), confirming marked differences in the visibility of stretch marks following olive oil treatment. Topical olive oil application significantly reduces stretch mark visibility in postpartum women. These findings support the implementation of this technique as a safe and accessible nonpharmacological intervention in postpartum care protocols for stretch mark management.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is a chronic lung disease with progressive, irreversible and partially reversible airway obstruction. COPD includes chronic bronchitis and emphysema, or a combination. COPD patients generally complain of continuous shortness of breath and this will result in a decrease in the patient's quality of life and as a result the patient will go back and forth to the hospital. To prevent this, it is necessary to do breathing exercises. The aim of implementing EBNP is to implement Progressive Muscle Relaxation (PMR) and Deep Breathing Exercises (DBE) interventions to change the shortness of breath response in COPD patients in the form of a randomized controlled trial with a pre-post-test design. The sampling technique uses G-Power version 3.1.9.7 with T-test, one tail, effect size 0.53, alpha error 0.05, and power 0.80, a minimum of 18 is required plus a possibility of error of 10%, so the number of patients needed in this study is 20 at Persahabatan Hospital, East Jakarta. The implementation of PMR actions with 13 (thirteen) movements and DBE with 6 (six) movements is carried out every day for 5 weeks, from May 15 to June 16 2023. Method: The study design in this research is a case study involving 40 respondents. Respondents were divided into two groups, namely 20 people in the PMR group and 20 people in the DBE group. Evaluation is carried out every week using the dyspnoe-12 instrument. Results: This application shows that there is a difference in the response to shortness of breath between before and after the PMR procedure (P value 0.001) and the DBE procedure (P value = 0.001). Conclusion: The application of EBNP recommends the need for nurses to carry out PMR and DBE interventions to change the shortness of breath response in COPD patients.
