Abstract

Objective: This study was conducted to analyze the prognostic factors for orchidectomy in testicular torsion patients. Material & Methods: From January 2001 to December 2021, 100 cases of testicular torsion cases were retrospectively collected. Data were described and analyzed based on clinical factors (age, duration of symptoms, scrotal swelling, high-riding testis, Prehn test, cremasteric reflex, leukocytosis, degree of twisting) and non-clinical factors (distance from hospital, duration of travel to hospital, education level and employment status) for each surgical procedure. Then logistic regression analysis was performed. Results: There were 88 testicular torsion patients (88%) underwent orchidectomy and 12 patients (12%) underwent orchidopexy. There was no significant relationship between age, duration of symptoms, scrotal swelling, Prehn test, cremasteric reflex, leukocytosis, distance from hospital, duration of travel to hospital, education level and employment status with orchidectomy (p>0.05). Significant prognostic factors for orchidectomy in testicular torsion patients were high-riding testis (OR = 0.06; 95% CI = 0.01-0.37; p = 0.002) and degree of twisting (OR = 0.04; 95% CI = 0.006-0.24; p = 0.000). Conclusion: High-riding testis and degree of twisting are prognostic factors for orchidectomy in testicular torsion patients. Keywords: Orchidectomy, prognostic factors, testicular torsion.

Abstract

Introduction. This research was conducted to investigate the association between urinary tract stones and the occurrence of urinary tract infections in patients receiving treatment at Saiful Anwar General Hospital in Malang.Methods. This research constitutes an analytical observational study employing a case-control research design conducted at Saiful Anwar General Hospital in Malang from 2015 to 2020.Results. This study comprised a sample size of 104 patients. In the group of patients with urinary tract stones, the highest occurrence was among men (77.3%), particularly those aged between 30-60 years (59.1%). In the group of patients with UTIs, males had the highest prevalence (67.3%), with a significant portion at the age of 50 (55.8%). The Chi-Square analysis investigating the association between urinary tract stones and the presence of urinary tract infections resulted in a p-value of 0.008, accompanied by an Odd Ratio of 4.566.Conclusions. A notable association has been identified between urinary tract stones and the presence of urinary tract infections.

Abstract

Introduction: Benign Prostate Hyperplasia (BPH) is an enlarged prostate gland that causes problems in micturition and results in disruption of urine flow. Factors that cause prostate enlargement from a combination of chronic and progressive urine retention, high bladder pressure and ureterohydronephrosis can cause progressive kidney injury. The obstructive process changes the cells and physiological conditions in the bladder muscle and collagen contributes to increasing bladder pressure and the inability to empty the bladder and ends up causing kidney failure. This study was to determine the prevalence of chronic kidney disease (CKD) in BPH patients undergoing transurethral resection of the prostate (TURP) surgery.Method: Using non-probability sampling method with consecutive sampling technique. Data were analyzed by univariate analysis using data frequency tables and bivariate analysis using the chi-square test.Result: Of the total 178 BPH patients, 15 of them suffered from CKD (8.4%). BPH prevalence was highest in the age category 61-70 (39.3%), the majority had prostate volume greater than 50 ml (59%), the most common symptoms were severe lower urinary tract symptoms (LUTS) (83.1%) and urine retention (74.7%), some of them were accompanied by comorbidities such as hypertension (33.7%). The most common indication for surgery in patients who underwent TURP was recurrent retention (79.2%). The chi-square test showed an association between BPH and the incidence of CKD.Conclusion: The prevalence of CKD at all stages in BPH patients who underwent TURP surgery was 8.4%. The most influential factors for the occurrence of CKD in BPH patients are prostate volume 30-40 ml, prostate volume 41-50 ml, and men aged 71-80 years.

Abstract

Cancer is a disease caused by genetic changes that cause abnormal and uncontrolled cell growth. One of the cancers with an enormous growth in the number of cases is bladder cancer, with the number of new cases in 2020 amounting to 573,000 cases in the world. This number is supported by inadequate early detection modalities that cannot reach the wider community. The Detection On The Spot Bladder Cancer (DOTS Bca) innovation aims to reduce the prevalence of bladder cancer and becomes an affordable early bladder cancer detection tool that can be used repeatedly and has high sensitivity. DOTS BCa detects the 47 kiloDalton epitope protein biomarker in the patient's urine. This tool uses Carbon Quantum Dots (CDs) as a semiconductor material, which will be analyzed for the level of fluorescence when it binds to a 47 kiloDalton epitope. The innovation design method includes literature study, design, tool making and testing. The early detection system test was carried out by comparing the CDs fluorescence results with the bladder cancer diagnostic test and obtained an accuracy of 85% with sensitivity 90% and specificity 80%. This tool is analyzed using fluorescence resonance energy transfer, integrated application programming interface, and fuzzy logic classification, which can work non-invasively.

Abstract

Background: Orchidectomy is a surgical androgen deprivation therapy (ADT) for prostate cancer patients to achieve castrate testosterone levels. Selective androgen receptor modulators (SARMs) are used to mitigate the adverse effects of ADT, including elevated risk of osteoporosis, and reduced skeletal muscle mass. Rad140 is a novel SARMs that has high affinity for the androgen receptors. This study was conducted to determine the effects of SARM Rad140 on the number of osteoblasts, osteoclasts, muscle fiber cross-sectional area (CSA), muscle size, and number of myonuclei in rats underwent orchidectomy.Materials and Methods: An experimental study was conducted using a randomized post-test only control group design. Following orchidectomy, SARM Rad140 was administered orally for six weeks at various doses. Osteoblasts, osteoclasts, muscle fiber CSA, muscle size, and number of myonuclei were measured. Quantitative analysis was performed using one-way ANOVA.Results: There were significant differences in the effects of SARM Rad140 at different doses on osteoblast and osteoclast cells. At higher doses, the osteoblast cell counts in rats tended to increase, while the osteoclast counts tended to decline. The treatment group also showed significant results in the CSA of the gastrocnemius muscle fibers, as well as in the number of myonuclei of the gastrocnemius muscle.Conclusion: SARM Rad140 significantly increased the number of osteoblasts, muscle fiber CSA, and gastrocnemius muscle myonuclei, while decreasing osteoclasts. SARM Rad140 is a promising therapy for osteoporosis and muscle weakening due to ADT.Keywords: SARM Rad140, orchidectomy, osteoblasts, osteoclasts, muscle fiber cross sectional area, myonuclei

Abstract

Objective: This case report presents a 60-year-old woman with intravesical ureterocele complicated by severe right upper-moiety hydronephrosis. Case(s) Presentation: A 60-year-old woman came with the chief complaint of right flank pain radiating to the umbilicus and groin accompanied by nausea, vomiting and decreased appetite. Ultrasound examination showed severe hydronephrosis of the right kidney. Abdominal CT revealed a double collecting system with severe hydroureteronephrosis at the upper pelvicalyceal system and the presence of an intravesical ureterocele. Cystoscopy and right RPG of upper and lower moiety procedures were performed and showed right severe hydroureteronephrosis with stenosis of right upper moiety ureteral orifice and ureterocele, followed by uroterocele incision and upper moiety ureteral re-implantation. No further complaints or complications were found after discharge from the hospital. Discussion: Adult ureterocele is a rare congenital anomaly often detected incidentally or due to complications like obstruction and infection. Diagnosis relies on imaging modalities such as ultrasonography, intravenous urography, and CT urography, while treatment options range from minimally invasive endoscopic decompression to surgical excision, depending on severity and associated complications. Conclusion: Ureterocele is a rare congenital urological abnormality found in non-Caucasians. Due to the rarity of the case, a thorough examination is required to establish this dia\gnosis and the choice of the appropriate treatment procedure. The managements include incision of ureterocele and ureteroneocystostomy.
