Abstract

Pressure ulcers can cause various problems for clients and hospitals. For patients, decubitus can cause extreme pain and discomfort, increase the length of stay (LOS), and quality indicators in the hospital cannot be achieved. The chronology of various factors means that a less standard decubitus assessment instrument will cause decubitus to continue to occur. This study aims to analyze the instruments used to prevent pressure ulcers in ICU patients. This study reviews eight relevant articles from 2019-2024. The results of a literature search of 2,662 journals resulted in a total of 15 journals being reviewed with the results of several studies using different pressure ulcer prevention instruments, including the Braden Scale, PUPrep, PUP Instrument, PIPGAI, COMHON, CFA, VALENT, PUKAT, KPUP, and RAPS, all instruments are used as decubitus detection instruments.

Abstract

Emergency Departments are critical healthcare units where efficient triage plays an important role in ensuring optimal patient care. The Emergency Severity Index (ESI) is a widely used triage system designed to improve prioritization and resource allocation. Proper use of ESI-based triage will improve the quality of care. Objective: This literature review aims to analyze the impact of ESI-based triage optimization on Emergency Department service quality. Methods: A systematic search was conducted using Google Scholar, PubMed, ResearchGate, and SciSpace databases, using the keywords: Emergency Severity Index (ESI), Emergency Department, Triage, using the PCC (Population, Concept, Context) approach. The article selection process used PRISMA which was then subjected to critical appraisal. The initial search was conducted through Google Scholar (5,521 articles), PubMed (2,019 articles), ResearchGate (881 articles), and SciSpace (99 articles). The process of removing duplicate articles left 8,151 articles, the initial screening of 8,125 articles was eliminated, so only 26 articles entered the further screening stage. From this process, 16 articles did not meet the criteria and were excluded. Finally, 10 articles that met the inclusion criteria were selected and reviewed. Results: The findings showed that ESI optimization improved patient flow, reduced waiting time, and improved resource utilization, leading to better clinical outcomes and patient satisfaction. Conclusion: implementation of an optimized ESI-based triage system contributes significantly to the efficiency and quality of Emergency Department care, emphasizing the need for consistent training and protocol adherence.

Abstract

Hypertension is a leading contributor to cardiovascular morbidity and mortality. Non-pharmacological strategies such as back massage and music relaxation have been proposed to attenuate sympathetic activation and stabilize blood pressure. Grounded in relaxation theory and stress physiology, this review synthesizes current evidence on the effectiveness of these interventions in patients with hypertension. A systematic review was conducted in accordance with PRISMA guidelines across national and international databases (2020–2025). The research question was structured using the PICO framework. Quasi-experimental and randomized controlled trials involving hypertensive patients and reporting pre–post or between-group changes in blood pressure were eligible. Of 312 records identified, 97 duplicates were removed. After title–abstract screening of 215 records and full-text assessment of 69 articles, 15 studies met the inclusion criteria and were narratively synthesized. Most studies demonstrated statistically significant reductions in systolic and diastolic blood pressure following back massage, music therapy, or combined relaxation approaches. However, notable heterogeneity was observed in intervention protocols, duration, comparators, and control for confounding factors. Back massage and music relaxation appear to be safe, complementary interventions with promising potential to reduce blood pressure among individuals with hypertension. Nonetheless, rigorously designed randomized controlled trials with standardized protocols and longer follow-up are warranted to strengthen the evidence base.

Abstract

Benign Prostatic Hyperplasia (BPH) is common in men and worsens with age. Transurethral Resection of the Prostate (TURP) is a common treatment but may cause post-operative pain. Non-pharmacological interventions such as warm and cold compresses are often used to reduce pain. Data from KHZ Musthafa Regional Hospital, Tasikmalaya Regency, recorded 302 BPH cases between April and June. This study aimed to compare the effectiveness of warm and cold compresses in reducing post-operative pain in TURP patients. A quasi-experimental pretest-posttest control group design was used. The population consisted of 101 TURP patients over three months. Purposive sampling selected 60 participants, divided into Group 1 (warm compress, n = 30) and Group 2 (cold compress, n = 30). Warm compresses were applied using a hot water bottle, and cold compresses using ice water; temperatures were monitored with a thermometer. Pain was measured using the Numerical Rating Scale (NRS) before and after intervention. Data were analyzed using central tendency, paired t-tests, and independent sample t-tests. Participants were aged 61–70 years, mostly with junior or senior high school education, and worked in private sectors or as employees. Warm compresses reduced pain from 5.53 (moderate) to 3.07 (mild), and cold compresses from 5.63 to 3.90. Both interventions significantly reduced pain (p = 0.000), with warm compresses more effective than cold (p = 0.003). Warm compresses are more effective than cold compresses in reducing post-TURP pain, though both significantly decrease pain levels.

Abstract

Instalasi Gawat Darurat (IGD) memerlukan sistem triase yang akurat untuk menjamin keselamatan pasien dan mempercepat response time. Penerapan Emergency Severity Index (ESI) sebagai sistem triase lima level diharapkan mampu meningkatkan ketepatan prioritas pelayanan, namun implementasinya belum optimal. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan ketepatan implementasi triase model ESI di ruang IGD. Penelitian ini bersifat retrospektif, dengan desain penelitian cross sectional. Penelitian ini dilakukan di Ruang IGD pada Bulan September-Desember 2025. Jumlah populasi 40 orang perawat IGD, dengan pengambilan sampel menggunakan teknik total sampling. Alat pengumpulan data menggunakan kuesioner dan lembar observasi. Analisis yang digunakan adalah distribusi frekuensi dan uji statistik chi square untuk menganalisis hubungan antara variabel terikat dengan variabel bebas. Hasil penelitian ini menyimpulkan bahwa: Ada hubungan yang signifikan antara tingkat pengetahuan perawat dengan ketepatan pelaksanaan Triase Model sistem ESI (p value 0,002<0,05); Ada hubungan yang signifikan antara lamanya bekerja perawat dengan ketepatan pelaksanaan Triase Model sistem ESI (p value 0,003<0,05);  Ada hubungan yang signifikan antara keterampilan perawat dengan ketepatan pelaksanaan Triase Model sistem ESI (p value 0,011<0,05); Ada hubungan yang signifikan antara pelatihan perawat dengan ketepatan pelaksanaan Triase Model sistem ESI (p value 0,033<0,05); Ada hubungan yang signifikan antara persepsi beban kerja perawat dengan ketepatan pelaksanaan Triase Model sistem ESI (p value 0,000<0,05); Penelitian ini merekomendasikan agar meningkatkan kompetensi seluruh perawat IGD, khususnya melalui pelatihan BTCLS dan Triase Model sistem ESI.

Abstract

This study examined the relationship between self-care management and hospitalization frequency among patients receiving maintenance hemodialysis. A quantitative empirical approach with a descriptive correlational cross-sectional design was applied to 99 patients undergoing routine hemodialysis. Self-care management was measured using a validated questionnaire, while hospitalization frequency during the previous year was obtained from medical records. Descriptive analysis demonstrated that a substantial proportion of patients exhibited moderate to high levels of self-care management. Bivariate analysis revealed a statistically significant association between self-care management and hospitalization frequency, indicating that patients with higher self-care capacity experienced fewer hospital admissions. These findings suggest that effective self-care behaviors, including adherence to diet and fluid restriction, vascular access care, and stress management, contribute to greater clinical stability. Integration with existing literature confirms that self-care management operates as a protective behavioral mechanism influencing objective health outcomes in chronic kidney disease. Strengthening self-care management through structured nursing education and supportive interventions may reduce preventable hospitalizations and improve healthcare efficiency for hemodialysis populations. The results provide empirical support for incorporating self-care management as a core component of chronic renal care strategies.

Abstract

Burnout is a condition of physical, emotional, and mental exhaustion frequently experienced by nurses due to high work demands, particularly in outpatient units with large patient volumes and rapid service intensity. This condition can affect nurses’ performance, attitudes, and interactions in providing nursing care, thereby potentially reducing the quality of services perceived by patients. This study aimed to determine the relationship between burnout among outpatient nurses and the quality of patient services. This study employed a quantitative method with a cross-sectional design. The research population consisted of outpatient nurses and patients, with a sample of 20 nurses and 100 patients selected using purposive sampling techniques. Data were collected using a nurse burnout questionnaire measuring emotional exhaustion, depersonalization, and personal accomplishment, as well as a patient service quality questionnaire based on the dimensions of reliability, responsiveness, assurance, empathy, and tangibles. Data analysis was conducted using univariate and bivariate analyses with the Gamma test. The results showed that most outpatient nurses experienced a moderate level of burnout, while the majority of patients perceived the quality of services as high. Bivariate analysis indicated a significant relationship between nurses’ burnout levels and the quality of outpatient services. Lower levels of nurse burnout were associated with better quality of services perceived by patients.

Abstract

Hemodialysis is the primary therapy for patients with chronic kidney disease to maintain survival. The success of hemodialysis therapy is influenced by several factors, including comorbidities and hemodialysis adequacy. Comorbid conditions may increase the risk of complications, while hemodialysis adequacy plays an important role in determining therapeutic effectiveness and patient prognosis. This study aimed to determine the relationship between comorbidities and hemodialysis adequacy with survival rates among hemodialysis patients at Sultan Agung Islamic Hospital, Semarang. This quantitative study employed a cross-sectional design. A total of 175 hemodialysis patients were selected using purposive sampling. Survival analysis was performed using the Kaplan–Meier method to evaluate differences in survival rates based on comorbidities and hemodialysis adequacy. The results showed that most respondents had diabetes mellitus as a comorbidity (64.6%), adequate hemodialysis adequacy (58.9%), and were alive during the observation period (65.1%). Kaplan–Meier analysis demonstrated no significant difference in survival rates based on comorbidities, whereas a significant difference in survival rates was observed based on hemodialysis adequacy. Comorbidities were not associated with survival rate, while hemodialysis adequacy was significantly associated with survival rate among hemodialysis patients. Adequate hemodialysis is essential to improve patient survival.

Abstract

Patients with Acute Coronary Syndrome (ACS) post Percutaneous Coronary Intervention (PCI) often still experience chest pain due to inflammatory processes or psychological factors. Unmanaged pain can increase cardiac workload. The Benson relaxation technique is a non-pharmacological intervention that combines deep breathing techniques with an element of faith (spiritual) to induce a relaxation response. This study aims to determine the effect of the Benson relaxation technique on reducing chest pain levels in ACS patients after PCI at Referral hospitals in Central Java Province. This study used a one-group pretest-posttest design with a sample of 36 respondents selected by purposive sampling. Pain measurement used the Numeric Rating Scale (NRS) before and after the intervention. The Benson relaxation technique was administered for 10–15 minutes per session. Data analysis used the Wilcoxon Signed Ranks Test.Before the intervention, all respondents (100%) experienced moderate chest pain (mean=5.11). After the intervention, there was a significant reduction, with 61.1% of respondents falling into the mild pain category and 38.9% remaining in the moderate category (mean=3.39). Statistical test results showed a significant difference between pain scores before and after the intervention (Z=-5.500; p=0.000).The Benson relaxation technique is proven effective in reducing chest pain levels in ACS patients post PCI in the Emergency Department

Abstract

Asthma is a chronic disease that can cause respiratory problems and affect a patient's quality of life. One of the most common symptoms of asthma is increased respiratory rate. The combination of the Benson and Fowler positions has been shown to be effective in reducing stress and increasing lung capacity. The purpose of this study was to determine the effect of the Benson and Fowler positions on respiratory rate in asthma patients. This was a descriptive observational study. The sample used in this study was 18 respondents. Data collection was carried out using a respiratory frequency observation sheet. The data obtained were statistically processed using the Wilcoxon test formula. The characteristics of asthma patient respondents at Sultan Agung Islamic Hospital, Semarang, were predominantly female (11 respondents (61.1%), predominantly aged 8 respondents (44.4%) and 46-55 years old (12 respondents (66.7%), predominantly employed as laborers (12 respondents (66.7%), predominantly educated as high school (9 respondents (50.0%), predominantly diagnosed as allergic asthma (11 respondents (61.1%), and predominantly underwent therapy for more than 6 months (16 respondents (88.9%). The mean respiratory rate (RR) in asthma patients before the intervention was 24.78 ± 1.263. The mean respiratory rate (RR) in asthma patients after the intervention was 19.50 ± 2.121). There is a combination of Benson and Fowler positions on respiratory rate in asthma patients at Sultan Agung Islamic Hospital, Semarang (p-value < 0.05).
