Abstract

Background: Individual case management strategy is not suitable for reducing scabies transmission, especially in high prevalence settings. A community-based approach has been proposed to control scabies. We developed a mobile application, called DeSkab, to empower non-medical personnel in crowded populations e.g., boarding schools, and to aid the identification of skin lesions suggesting scabies based on the cardinal signs of scabies. Early treatment and prevention of scabies transmission is expected to follow this approach. This was the initial development of the DeSkab mobile application which aims to assess user acceptance. Methods: The DeSkab mobile application was designed using Java and XML, supported by Google's Android. The scabies detection and education features were the app’s key distinctive aspects. An online survey was performed, aimed at potential users, including boarding school teachers and healthcare workers. The survey was divided into four sections: application design and user-friendliness, early detection data entry and interpretation, education features, and user recommendations. Result: Overall, users' acceptance of this application was good. More than 70% of the users gave good feedback for the application. Using mobile health makes it easier for the users to find information about scabies and check whether their skin lesions are suggestive of scabies. Conclusion: This application is expected to help expand scabies detection, especially in crowded communities. Improvements must be made to the interface, data entry, and educational material for the app's next iteration. Further study is needed to determine how mobile health application can improve scabies detection in communities.

Abstract

Background: Scabies has been added to World Health Organization (WHO) list of neglected tropical disease in 2017. Various methods have been developed to control scabies in highly prevalent communities. In this study we conducted a diagnostic study to evaluate the performance of scabies detection by trained nonmedical personnel (NMP) using Deteksi Skabies (Deskab) instrument which has been validated for NMP. Methods: Eight NMPs in a boarding school were trained to detect scabies using DeSkab instrument. The NMPs diagnosis were compared to diagnosis of 10 medical doctors. The study was conducted in a religion-affiliated boarding school in West Java, Indonesia. Both examiners consecutively assessed boarding school students using DeSkab instrument and were blinded to each other findings. Results: Among 140 participants included in this study, scabies was confirmed by medical doctors in 60 participants. Diagnostic accuracy of NMPs examination is 72.14% [95% confidence interval (CI) 64.2-78.9], with sensitivity and specificity 67.42% (95% CI 57.13-76.26), and 80.32% (95% CI 67.54-88.98) respectively. The inter-rater agreement (Cohen’s kappa) for diagnosing scabies is 0.44. Conclusion: NMPs can be trained to detect scabies in their community with acceptable accuracy. Improving training are recommended to further improve the diagnosis skills and maintaining sustainable detection program.

Abstract

Kecelakaan di lingkungan kerja, terutama di laboratorium, menekankan pentingnya tindakan keselamatan yang efektif dan kesadaran yang tinggi. Penelitian ini bertujuan untuk mengkaji hubungan antara lama pendidikan dan pengetahuan tentang Kesehatan dan Keselamatan Kerja (K3) terhadap kepatuhan mahasiswa kedokteran di laboratorium melalui studi cross-sectional di laboratorium Fakultas Kedokteran dan Ilmu Kesehatan UNTIRTA yang melibatkan 80 mahasiswa kedokteran. Hasil penelitian menunjukkan adanya hubungan yang signifikan antara lama pendidikan terhadap kepatuhan mahasiswa (p < 0,001; OR 49,8), sementara tidak ditemukan korelasi yang signifikan antara pengetahuan K3 dan kepatuhan mahasiswa (p = 1,00; OR 0,537). Temuan ini menegaskan bahwa lama pendidikan memiliki peran yang besar dalam meningkatkan kepatuhan terhadap protokol keselamatan, serta menunjukkan perlunya intervensi pendidikan yang lebih intensif untuk meningkatkan kesadaran keselamatan di tempat kerja.

Abstract

Preclinical medical students have a big role as agents of change to raise awareness of the pandemic caused by Coronavirus Disease 2019 (COVID-19) infection in family, friends, and society. This study aims to determine the final stage of preclinical medical students' knowledge, attitudes, and practices (KAP) toward COVID-19 infection. This study used a cross-sectional method that was conducted on 213 final-stage preclinical medical students in a public university in Depok, Indonesia using an online validated KAP Questionnaire that measures age, gender, domicile, class, and level of KAP toward COVID-19 infection. Data were analyzed using SPSS 21. The p-value <0.05 indicates statistical significance. The result KAP of students toward COVID-19 infection showed a good level of knowledge (73.7%), positive attitude (96.2%), and positive Practice (83.6). %). There was no relationship between the level of knowledge and attitudes toward COVID-19 infection because a p-value of 0.211 (p >0.05) was obtained. Meanwhile, there was a relationship between the level of knowledge and practice toward COVID-19 infection with a p-value = 0.044 (p <0.05). The results revealed that the majority of preclinical medical students had favorable attitudes, had adequate knowledge of COVID-19, and used effective practices to stop the transmission of the disease.

Abstract

Community development in Lio Village performed by Faculty of Medicine Universitas Indonesia students aimed to improve the health status of villagers. Because economy correlates with health, an economic intervention may increase the health status of the people. Based on this, an entrepreneurship program was initiated as a part of a community development program. This study is qualitative study that was conducted between July 2020 and February 2021. The intervention consisted of asynchronous webinar sessions, workshops, and business competition. Data were gathered from post-test questionnaires through a Google form. The winners were chosen according to the business ideas and given prizes. Then, a follow-up interview was conducted to measure the benefits of the intervention. For the results, more than 50% of the participants understood the theory by answering correct questions in the post-evaluation forms. The participants also reported feeling happier and having better mental health. Of the 88 medical students who volunteered in the event, 78.2% felt a tremendously beneficial experience of community empowerment and 21.8% felt they have experienced community empowerment. In conclusion, remote-online community development intervention may be a solution during the COVID-19 pandemic. By acknowledging the importance of the economic aspect to health and improving aspects that influence health holistically, both economic and health states of the people may be improved.

Abstract

To raise awareness and understanding of maternal and children’s health, a team from Faculty of Medicine, Universitas Indonesia, Indonesia, conducted a program called Tinggi Cerdas. It is a community development program in Kampung Lio, Depok, West Java. This article evaluates the impacts of activity related to the participant’s awareness and knowledge about maternal and children’s health. The program was developed and conducted as an online program due to the COVID-19 pandemic situation, from September to December 2021. The program was developed with four different topics through online communication, equipped with an educational booklet for the mothers and a PrimaKu® application for health workers. Such approaches were employed to provide the community with better health awareness and understanding, as well as an instrument for the health workers to monitor the condition of the participants. Community health workers (CHWs) were given a workshop about the application to help them in raising mothers’ awareness to monitor their children’s growth and development. To examine the impacts of the program, pre- and post-tests were used to evaluate the impact on the participants’ knowledge—the mothers’ and CHWs’. The result of the program’s assessment suggests that mothers’ knowledge and awareness about children’s and maternal health are improved, particularly when the application was also utilized by the CHWs in Kampung Lio. The participants are satisfied with the program, implying the program should be repeated. Despite the limitations, health education for mothers and CHWs could be carried out successfully. Insights and reflections are provided in this study to encourage readers to develop a more effective program in the future.

Abstract

Purpose – Good student readiness is needed, especially for medical students who are required to lifelong learning. This study aims to assess the relationship between student readiness related to LMS and the summative test score of the Primary Healthcare System module (SPKP) for students of the Faculty of Medicine at the University of Indonesia.Methods – This research is a cross-sectional design. A total of 191 respondents from fourth-year medical students of the Primary Healthcare System module, the Faculty of Medicine, at the University of Indonesia, participated in this research. The questionnaire consists of 5 dimensions and 10 questions with answers rated one to three. The measure of readiness is determined by the median value. The data were analyzed using the chi-square test. Findings – The findings revealed that 51.3% of students are ready to accept learning at LMS. The most obtained SPKP module summative score was B-, with a total of 31.4%. It was found that there was a relationship between student readiness related to the learning management system and a summative test score, with a P-value of 0.04 for the primary healthcare system module in medical students at the University of Indonesia. Research Implications – This research contributes to universities, lecturers that it is necessary to conduct technical system training at the beginning of the semester, provide easily accessible interactive modules, and strengthen infrastructure to support independent learning.

Abstract

Introduction. Anal fistula is a pathological condition that connects the anorectal mucosa to the perianal skin, requiring various types of surgical procedures. However, surgery often causes postoperative pain. The use of methylene blue is being studied as a potential analgetic adjuvant in anal fistula surgery. Previous studies have shown that methylene blue can effectively reduce postoperative pain in various types of anorectal surgery. This study aims to determine methylene blue's clinical efficacy as an analgetic adjuvant. Methods. A double-blind, randomized controlled trial was conducted. Subjects consisting of subjects with simple anal fistula to undergo fistulectomy were randomly allocated into two groups. The first group received adjuvant analgesics in the form of methylene blue and NSAIDs, while the second group only received NSAIDs. Methylene blue 4 mL 1% was administered subcutaneously to the edge of the wound, sprayed on the surgery site, and intravenously administered ketorolac 3 x 30 mg. Randomization was done using the double-blind method. Pain levels were evaluated on the first, second, third, and seventh postoperative days using a visual analog scale (VAS). Results. Thirty-four subjects were enrolled (17 subjects in each group). Significant differences in pain levels (VAS values) were recorded between the group receiving the combination of methylene blue and NSAIDs compared to the group receiving only NSAIDs on days 1 to 3 after surgery (p <0.05). This difference was no longer significant between the two groups on the seventh postoperative day (p >0.05). Conclusion. The use of methylene blue in subjects undergoing anal fistula surgery as an adjuvant analgesic along with NSAIDs may reduce the intensity of pain better than NSAIDs alone.
