Abstract

Background: Individual case management strategy is not suitable for reducing scabies transmission, especially in high prevalence settings. A community-based approach has been proposed to control scabies. We developed a mobile application, called DeSkab, to empower non-medical personnel in crowded populations e.g., boarding schools, and to aid the identification of skin lesions suggesting scabies based on the cardinal signs of scabies. Early treatment and prevention of scabies transmission is expected to follow this approach. This was the initial development of the DeSkab mobile application which aims to assess user acceptance. Methods: The DeSkab mobile application was designed using Java and XML, supported by Google's Android. The scabies detection and education features were the app’s key distinctive aspects. An online survey was performed, aimed at potential users, including boarding school teachers and healthcare workers. The survey was divided into four sections: application design and user-friendliness, early detection data entry and interpretation, education features, and user recommendations. Result: Overall, users' acceptance of this application was good. More than 70% of the users gave good feedback for the application. Using mobile health makes it easier for the users to find information about scabies and check whether their skin lesions are suggestive of scabies. Conclusion: This application is expected to help expand scabies detection, especially in crowded communities. Improvements must be made to the interface, data entry, and educational material for the app's next iteration. Further study is needed to determine how mobile health application can improve scabies detection in communities.

Abstract

Background: Scabies has been added to World Health Organization (WHO) list of neglected tropical disease in 2017. Various methods have been developed to control scabies in highly prevalent communities. In this study we conducted a diagnostic study to evaluate the performance of scabies detection by trained nonmedical personnel (NMP) using Deteksi Skabies (Deskab) instrument which has been validated for NMP. Methods: Eight NMPs in a boarding school were trained to detect scabies using DeSkab instrument. The NMPs diagnosis were compared to diagnosis of 10 medical doctors. The study was conducted in a religion-affiliated boarding school in West Java, Indonesia. Both examiners consecutively assessed boarding school students using DeSkab instrument and were blinded to each other findings. Results: Among 140 participants included in this study, scabies was confirmed by medical doctors in 60 participants. Diagnostic accuracy of NMPs examination is 72.14% [95% confidence interval (CI) 64.2-78.9], with sensitivity and specificity 67.42% (95% CI 57.13-76.26), and 80.32% (95% CI 67.54-88.98) respectively. The inter-rater agreement (Cohen’s kappa) for diagnosing scabies is 0.44. Conclusion: NMPs can be trained to detect scabies in their community with acceptable accuracy. Improving training are recommended to further improve the diagnosis skills and maintaining sustainable detection program.

Abstract

Background: Presenteeism is the presence of workers in the workplace without full working capacity due to health factors, but several studies also link it to organizational influence. The government staffing system in Indonesia has two types of work status (permanent and non-permanent) and policy changes related to increasing the use of Visual Display Terminal (VDT), will certainly affect worker presenteeism. This study was conducted to assess the risk factors presenteeism in workers using VDT in the Indonesian government office. Methods: This study was a cross-sectional observational study using a questionnaire method using the Stanford Presenteeism Scale (SPS-6) to assess presenteeism and its association with organizational and health risk factors in a multivariate manner. Computer Vision Syndrome (CVS) at one of associated factor, was assessed using Indonesian version of CVS Questionnaire (CVSQ). Analysis of this study used SPSS version 26. Results: Of the 201 research subjects, 44.28% of presenteeism was found. but complaints of computer vision syndrome (Positive CVSQ score) were found in almost all of the study subjects (84.5%). Risk factors for employment status and position have a strong relationship with presenteeism (p <0.001). Temporary worker status has a 7.08x risk (aOR=7.08 95% CI = 3.1-15.8) of experiencing presenteeism. Whereas for workers with a presenteeism risk position of 3.45x (aOR=3.45 95% CI = 1.3-8.5). Conclusion: Most government workers who use VDT continue to work even though they have health problems in the form of complaints of computer vision syndrome. Non permanent status (aOR=7.08 95% CI = 3.1-15.8) and low position (aOR=3.45 95% CI = 1.3-8.5) are determinant factors for presenteeism.

Abstract

Kecelakaan di lingkungan kerja, terutama di laboratorium, menekankan pentingnya tindakan keselamatan yang efektif dan kesadaran yang tinggi. Penelitian ini bertujuan untuk mengkaji hubungan antara lama pendidikan dan pengetahuan tentang Kesehatan dan Keselamatan Kerja (K3) terhadap kepatuhan mahasiswa kedokteran di laboratorium melalui studi cross-sectional di laboratorium Fakultas Kedokteran dan Ilmu Kesehatan UNTIRTA yang melibatkan 80 mahasiswa kedokteran. Hasil penelitian menunjukkan adanya hubungan yang signifikan antara lama pendidikan terhadap kepatuhan mahasiswa (p < 0,001; OR 49,8), sementara tidak ditemukan korelasi yang signifikan antara pengetahuan K3 dan kepatuhan mahasiswa (p = 1,00; OR 0,537). Temuan ini menegaskan bahwa lama pendidikan memiliki peran yang besar dalam meningkatkan kepatuhan terhadap protokol keselamatan, serta menunjukkan perlunya intervensi pendidikan yang lebih intensif untuk meningkatkan kesadaran keselamatan di tempat kerja.

Abstract

Preclinical medical students have a big role as agents of change to raise awareness of the pandemic caused by Coronavirus Disease 2019 (COVID-19) infection in family, friends, and society. This study aims to determine the final stage of preclinical medical students' knowledge, attitudes, and practices (KAP) toward COVID-19 infection. This study used a cross-sectional method that was conducted on 213 final-stage preclinical medical students in a public university in Depok, Indonesia using an online validated KAP Questionnaire that measures age, gender, domicile, class, and level of KAP toward COVID-19 infection. Data were analyzed using SPSS 21. The p-value <0.05 indicates statistical significance. The result KAP of students toward COVID-19 infection showed a good level of knowledge (73.7%), positive attitude (96.2%), and positive Practice (83.6). %). There was no relationship between the level of knowledge and attitudes toward COVID-19 infection because a p-value of 0.211 (p >0.05) was obtained. Meanwhile, there was a relationship between the level of knowledge and practice toward COVID-19 infection with a p-value = 0.044 (p <0.05). The results revealed that the majority of preclinical medical students had favorable attitudes, had adequate knowledge of COVID-19, and used effective practices to stop the transmission of the disease.

Abstract

Scabies elimination is always challenging, as treatment alone is not sufficient to stop the transmission. To stop the transmission in places such as boarding schools, a community-based approach and cooperation with the teacher and managerial team are important. This study aims to evaluate the effectiveness of scabies detection training for non-medical personnel (NMP) in improving knowledge about scabies. This cross-sectional study was conducted in an Islamic boarding school in Bogor. NMPs in boarding school were trained to detect and prevent scabies in their communities. The training consists of expert lectures, group discussions and simulation training. All the participants were required to fill out the pre-test and post-test questionnaires. In 60 participants, there was a significant improvement from pre-test to post-test scores (p<0.001), and 85% showed an improvement in post-test. The knowledge about scabies' symptoms and signs had the best improvement, while the knowledge improvement about the elimination of mites and scabies prevention was still poor after the training. The improvement of scabies knowledge after training was significant. Training models focused on NMP knowledge in recognizing scabies for early detection in the community have the potential to be used by collaboration of health practitioners and boarding school management to eradicate the scabies problem. Keywords: boarding school, community empowerment, non-medical personnel, scabies detection, scabies training.   Pelatihan Skabies untuk Meningkatkan Pengetahuan Tenaga Non-Medis pada Populasi Risiko Tinggi di Indonesia Abstrak Program eliminasi skabies selalu menantang, karena tata laksana yang hanya pemberian obat tidak cukup untuk menghentikan transmisi.Untuk menghentikan transmisi di tempat padat penghuni seperti sekolah asrama, pendekatan berbasis komunitas dan kerja sama dengan guru serta manajemen sekolah menjadi penting. Studi ini bertujuan untuk mengevaluasi efektivitas pelatihan deteksi dini skabies bagi tenaga non medis (TNM) dalam meningkatkan pengetahuan mengenai skabies. Studi ini menggunakan desain potong lintang yang dilakukan di pesantren di Bogor. Tenaga non-medis yang berasal dari pesantren dilatih untuk mendeteksi dan mencegah skabies di komunitasnya. Pelatihan terdiri atas kuliah dari para ahli, diskusi kelompok dan simulasi. Semua peserta diminta untuk mengisi kuesioner pre-test dan post-test. Dari 60 peserta, terdapat peningkatan bermakna antara skor pre-test dan post-test (p<0,001), 85% menunjukkan peningkatan pada skor post-test. Pengetahuan terkait gejala dan tanda skabies memiliki peningkatan terbaik, sedangkan peningkatan mengenai eliminasi tungau dan pencegahan skabies masih kurang setelah pelatihan. Pengetahuan skabies meningkat bermakna. Model pelatihan yang berfokus pada TNM dalam mengenali skabies untuk deteksi dini di komunitas sangat potensial untuk berkolaborasi dengan tenaga kesehatan dan manajemen pesantren untuk eradikasi skabies. Kata kunci: sekolah asrama, pemberdayaan masyarakat, tenaga non medis, deteksi skabies, pelatihan skabies.

Abstract

Background: Diabetic foot ulcer is a problem in patients with diabetes mellitus (DM). One adjuvant therapy that can enhance wound healing is Low-Level Laser Therapy (LLLT), but there is no established guideline regarding the dosage. In Indonesia, there has been no study comparing the energy density of LLLT on diabetic ulcer healingMethods: This is an experimental study on 28 subjects with a randomized diabetic foot ulcer. Group A received standard treatment of ulcer and LLLT 5J/cm2. Group B received standard treatment of ulcer and LLLT 10J/cm2.Results: The difference in wound size between group A and group B were 4.15 mm2 and 7.5 mm2 (p=0.178). The healing rate of group A and group B were 4.15 (-10-34.5) mm2/4 weeks and 7.5 (-2.8-34) mm2/4weeks (p=0.168).Conclusions: There was no statistically significant difference between the group receiving LLLT 5J/cm2 or 10 J/cm2 in diabetic foot ulcer healing.
