Abstract

Adolescent pregnancy and child marriage is a common public health problem worldwide.  It is one of the key issues concerning reproductive health of women not only in developing countries but also in developed countries, including Indonesia.  The international community is also aware that the problem of Adolescent pregnancy and child marriage is a very serious problem. The general implication is that women and children will be at risk in various aspects, related to unwanted marriages, forced sexual intercourse, pregnancy at a very young age, as well as an increased risk of transmission of HIV infection, other sexually transmitted diseases, and cervical cancer. The purpose of the article is to review current situation and issues on adolescent pregnancy and child marriage in Indonesia.

Abstract

Pendahuluan: Terapi KNF terdiri atas radioterapi, kemoterapi, maupun kombinasi keduanya. Penilaian terhadap perubahan ukuran tumor menjadi hal yang penting dalam evaluasi klinis terapi kanker. Terdapat kriteria respon tumor yang telah distandarisasi, salah satunya yaitu kriteria WHO. Respon terapi dikelompokkan menjadi 4 kategori yaitu complete response (CR), partial response (PR), progressive disease (PD), dan stable disease (SD). Penelitian ini bertujuan untuk mengetahui gambaran terapi dan respon terapi karsinoma nasofaring di RSUP Dr. M. Djamil Padang. Metode: Penelitian ini merupakan penelitian observasional deskriptif dengan pendekatan retrospektif yang mengambil data dari bagian rekam medik dan poliklinik THT-KL RSUP Dr. M. Djamil Padang pada periode Januari 2018 – Desember 2020. Hasil: Hasil didapatkan 28 pasien KNF yang telah selesai menjalani seluruh rangkaian terapi. Dari 28 pasien, 67,8% adalah laki-laki. Kelompok usia paling banyak terdapat pada rentang usia 50-59 tahun (35,7%), berasal dari suku Minang (93%) dengan tingkat pendidikan paling banyak adalah SMA (78,5%). Sebagian besar pasien didiagnosis dengan stadium IV (57,2%) dengan jenis KNF WHO tipe 3 (82,2%). Pilihan terapi yang paling banyak diberikan pada pasien KNF yaitu kemoterapi neoadjuvan yang dilanjutkan dengan radioterapi (64,3%). Kesimpulan: Hasil respon terapi dari semua pasien KNF yang telah menjalani rangkaian pengobatan adalah CR (46,4%), PR (46,4%), PD (7,2%), dan tidak terdapat pasien dengan SD.

Abstract

Background: Post anaesthetic shivering (PAS) is a fasciculation of the face, jaw, or head or the occurrence of muscle hyperactivity so that the body can produce heat after anesthesia. Multiple factors can influence the incidence of PAS including age, sex, ASA physical status, body mass index, type of anesthesia, duration of surgery, room temperature, preoperative body temperature (<36,5⁰C), and type of surgery.  Objective : The purpose of this study was to describe the incidence of PAS in post-spinal anesthesia caesarean section patients at RSIA Siti Hawa Padang Method: This research is a descriptive observational study using accidental sampling technique and obtained a total sample of 52 patients Result: The result showed that 25 of 52 patients (48,1%) experienced PAS. Most PAS events were experienced by moderate grade (25%), the age group >35 years (60%), underweight body mass index (62,5%), ASA II status (52,2%), preoperative temperature in the hypothermia group (51,4%), postoperative temperature in the normothermia group (66,7%), and operation duration >30 minutes (57,1%) Conclusion: Most of the patients did not experience PAS and most of PAS experience was moderate grade.

Abstract

Background: Dysmenorrhea is a menstrual pain that is often experienced by young women, with a global prevalence of 16.8-81%. In Indonesia, the prevalence reaches 54.89%, with adolescent girls being the largest group. Treatment generally uses analgesics, but it has side effects. Yoga as a non-pharmacological therapy can reduce pain through muscle relaxation. Purpose: Analyzing the effect of yoga therapy on the intensity of the degree of dysmenorrhea in female students of the Class of 2018 Undergraduate Medical Education Study Program, Faculty of Medicine, Andalas University. Methods: This research uses a pure experimental design with a pre-test, post-test, and only group design, with a sample size of 32 people. The research instrument used in the study was a pre-test and post-test questionnaire, which contained VAS scores and guidelines regarding yoga movements. The questionnaire contains a Visual Analog Scale (VAS), which consists of a vertical scale from 0 to 10 cm. Results:  The results showed there were no differences related to age. As many as 18% of respondents usually took analgesics, while 81.3% did not take analgesics to treat dysmenorrhea before yoga therapy. Meanwhile, after yoga therapy, 5 out of 6 respondents who usually consumed analgesics in previous menstrual cycles no longer consumed analgesics, and one respondent still consumed analgesics. Bivariate analysis showed a value of p=0.00 (p<0.05), which means there is a relationship between the provision of yoga therapy and the degree of dysmenorrhea. Where providing yoga therapy can reduce the intensity of pain in respondents who experience dysmenorrhea. Conclusion: This research concludes that there is a significant relationship between the provision of yoga therapy and the degree of dysmenorrhea.

Abstract

Since its first launched in 2014, many hospitals claimed to have financial loss due to inequality between hospital real tariff and BPJS payment. It is important to identify the characteristics and trends of caesarean deliveries in hospitals prior to measuring the efficiency level under INA-CBGs system. However, until now there is yet to be a descriptive study to understand this issue comprehensively. To fill this research gap, this study aimed to compare caesarean section incidents and costs between private and public hospitals in West Sumatra, Indonesia. This study is expected to describe real situation related to caesarean cases and explore the preference of patients in the province for caesarean deliveries. This study was retrospective and cross-sectional design of caesarean section in all West Sumatran hospitals under INA-CBGs system. The data used was taken from BPJS region West Sumatra-Riau-Jambi for period 2016 to 2018. During that period, almost 59 thousand caesarean section were performed, of which 64% in private hospitals and 36% in public hospitals. Within three years, caesarean cases in private hospitals were almost doubled while public hospitals showed a decreasing trend. In 2018, three quarter of caesarean costs was paid by BPJS to private hospitals while public hospitals only received a quarter of total costs.
