Abstract

The National Health Insurance (Jaminan Kesehatan Nasional/JKN) is a nationwide program that applies social insurance principles to provide equitable access to healthcare services, including coverage for catastrophic illnesses that entail high medical expenditures. Despite its extensive coverage, the use of JKN for the management of devastating illness is influenced by various socioeconomic and health-related factors. This study aimed to identify determinants of JKN utilization for the management of catastrophic diseases in Indonesia. This systematic review was conducted using the PRISMA guidelines. Literature searches were performed across four electronic databases: PubMed, PubMed Central, Semantic Scholar, and Google Scholar. Articles were included if they discussed factors influencing the utilization of JKN or other national health insurance schemes for catastrophic illnesses. Exclusion criteria excluded studies that did not address catastrophic disease management or were not relevant to insurance utilization. The review focused on publications issued between 2015 and 2025. The initial search identified 678 articles, which were screened using title, abstract, and full-text reviews, as well as duplicate checks. A total of six articles met all eligibility criteria for final analysis. The included studies consistently showed that several factors supported JKN utilization in the management of catastrophic illness. These determinants comprised household income, family size, marital status, gender, age, medical costs, and the presence of chronic illness. Evidence indicated that low-income households, larger families, older individuals, and those with chronic diseases were more likely to rely on national health insurance due to high financial vulnerability. This review concluded that key socioeconomic and health-related factors influence the use of JKN for the management of catastrophic illness in Indonesia. Strengthening JKN policies and enhancing financial protection mechanisms are essential to reducing the economic burden of devastating diseases and ensuring the program's long-term sustainability.

Abstract

Background: Lean management, part of the Toyota Production System, is now applied across various industries to improve service quality. The implementation of lean in hospitals aims to enhance efficiency, reduce the length of hospital stays, and minimize medical errors, although the outcomes vary depending on factors such as leadership, training, and staff engagement. Purpose: To examine the improvement of the quality of outpatient services in Indonesian hospitals through lean healthcare management. Method: This article uses a PICO approach and follows the PRISMA guidelines, starting with a database search that yielded 256 articles. These were then screened based on inclusion and exclusion criteria, resulting in 12 relevant articles. The selection process included the removal of duplicate articles, screening of titles and abstracts, and full-text evaluation. Results: The implementation of lean management in Indonesian hospitals was carried out by observing and recording service flow from registration to payment, then analyzing service time based on Value-Added and Non-Value-Added Activities using Visual Stream Mapping or spaghetti diagrams, and discussing solutions through the 5 Whys and Fishbone Analysis methods. Conclusion: Lean management has been proven effective and suitable for application in hospital outpatient services to improve efficiency by eliminating waste and optimizing systems.   Keywords: Healthcare; Hospital; Lean Management; Outpatient.   Pendahuluan: Manajemen Lean, bagian dari Toyota Production System, kini diterapkan di berbagai industri untuk meningkatkan kualitas layanan. Implementasi manajemen lean di rumah sakit bertujuan untuk meningkatkan efisiensi, mengurangi lama pelayanan, dan meminimalkan kesalahan medis, meskipun hasilnya bervariasi tergantung pada faktor-faktor seperti kepemimpinan, pelatihan, dan keterlibatan staf. Tujuan: Untuk mengkaji peningkatan kualitas pelayanan rawat jalan di rumah sakit Indonesia melalui manajemen lean healthcare. Metode: Artikel ini menggunakan pendekatan PICO dan mengikuti pedoman PRISMA, dimulai dengan pencarian database yang menghasilkan 256 artikel. Artikel-artikel ini kemudian disaring berdasarkan kriteria inklusi dan eksklusi, menghasilkan 12 artikel yang relevan. Proses seleksi mencakup penghapusan artikel duplikat, penyaringan judul dan abstrak, serta evaluasi fulltext. Hasil: Implementasi manajemen lean di rumah sakit Indonesia dilakukan dengan mengamati dan mencatat alur layanan dari pendaftaran hingga pembayaran, kemudian menganalisis waktu layanan berdasarkan Aktivitas Bernilai Tambah (Value-Added) dan Aktivitas Tidak Bernilai Tambah (Non-Value-Added) menggunakan Visual Stream Mapping atau diagram spageti, dan mendiskusikan solusi melalui metode 5 Whys dan Analisis Fishbone. Simpulan: Manajemen lean terbukti efektif dan cocok untuk diterapkan di layanan rawat jalan rumah sakit untuk meningkatkan efisiensi dengan menghilangkan pemborosan (waste) dan mengoptimalkan sistem.   Kata Kunci: Healthcare; Manajemen Lean; Rawat Jalan; Rumah Sakit.

Abstract

Background: Oral health is an integral part of overall health. In pre-elderly and elderly individuals, cognitive decline and low education levels often lead to low awareness and knowledge about maintaining dental and oral hygiene. One way to increase knowledge is through education using leaflets. Purpose: To determine the effectiveness of dental and oral health promotion using leaflets on knowledge levels in pre-elderly and elderly individuals. Method: This study used a quasi-experimental design with a one-group pre-test post-test on 36 pre-elderly and elderly respondents selected using a total sampling technique. The research instrument was an eight-item questionnaire, and data were analyzed using the Wilcoxon Signed Rank test. Results: The average knowledge score of respondents increased from 4.53 before the intervention to 6.50 after the leaflet education. The Wilcoxon test showed a p-value of 0.000 (p<0.05), indicating a significant difference between knowledge before and after the intervention. Conclusion: Education using leaflets is effective in increasing knowledge about dental and oral health among pre-elderly and elderly people because it conveys information in an engaging, clear, and easy-to-understand manner. Recommendation: A combination of leaflets with an audiovisual approach and improved practices, such as brushing and denture care, and follow-up evaluation to measure long-term behavior changes are needed.   Keywords: Dental and Oral Health; Elderly; Knowledge; Health Promotion; Media Leaflets.   Pendahuluan: Kesehatan gigi dan mulut merupakan bagian integral dari kesehatan tubuh secara keseluruhan. Pada pra lansia dan lansia, penurunan fungsi kognitif dan rendahnya tingkat pendidikan sering menyebabkan rendahnya pengetahuan serta kesadaran dalam menjaga kebersihan gigi dan mulut. Salah satu upaya peningkatan pengetahuan dapat dilakukan melalui penyuluhan menggunakan media leaflet. Tujuan: Untuk mengetahui efektivitas promosi kesehatan gigi dan mulut menggunakan media leaflet terhadap tingkat pengetahuan pada pra lansia dan lansia. Metode: Penelitian ini menggunakan desain kuasi eksperimental dengan rancangan one group pre-test post-test pada 36 responden pra lansia dan lansia yang dipilih dengan teknik total sampling. Instrumen penelitian berupa kuesioner delapan butir pertanyaan, dan data dianalisis menggunakan uji Wilcoxon Signed Rank. Hasil: Rata-rata skor pengetahuan responden meningkat dari 4.53 sebelum intervensi menjadi 6.50 setelah diberikan penyuluhan menggunakan leaflet. Hasil uji Wilcoxon menunjukkan nilai p 0.000 (p<0.05), yang berarti terdapat perbedaan signifikan antara pengetahuan sebelum dan sesudah intervensi. Simpulan: Penyuluhan menggunakan media leaflet efektif dalam meningkatkan pengetahuan pra lansia dan lansia tentang kesehatan gigi dan mulut karena mampu menyampaikan informasi secara menarik, jelas, dan mudah dipahami. Saran: Perlu adanya kombinasi leaflet dengan pendekatan audiovisual, dan demonstrasi praktik, seperti cara menyikat gigi atau perawatan gigi tiruan, serta evaluasi tindak lanjut untuk mengukur perubahan perilaku jangka panjang.   Kata Kunci:Kesehatan Gigi dan Mulut; Lansia; Media Leaflet; Pengetahuan; Promosi Kesehatan.

Abstract

This study is a systematic literature review aimed at identifying and analyzing the influence of social media exposure on the health perceptions and behaviors of adolescents from various family backgrounds in Indonesia, including those from broken homes. The review used a quantitative-focused approach, analyzing six primary research articles published between 2015 and 2025. Findings reveal that social media significantly shapes adolescents’ understanding of physical, mental, and social health. Exposure to educational and positive content is associated with improved health awareness and behavior among the general adolescent population. In contrast, excessive engagement with toxic or unrealistic content contributes to increased stress, distorted self-perception, and unhealthy lifestyles. For adolescents from broken homes, social media serves as a psychological coping space and a source for meaning-making. However, it also carries risks if not supported by adequate digital literacy. These findings highlight the need for inclusive, empathetic, and context-sensitive digital health communication strategies and the development of supportive online communities for vulnerable youth. The review recommends future research with broader coverage and longitudinal designs to deepen the understanding of how social media influences adolescent health across different family structures.

Abstract

Tuberculosis prevention remains a significant challenge in high-risk boarding school settings due to overcrowding, prolonged close contact, and limited institutional health governance. At the same time, existing Research has largely emphasised epidemiological and individual-level factors with limited attention to contextual and institutional dynamics, particularly among adolescents in congregate educational environments. This qualitative exploratory study aimed to examine barriers and enablers to tuberculosis prevention from the perspectives of adolescents, caregivers, school administrators, and health professionals, using in-depth interviews and focus group discussions with purposively selected participants involved in boarding school health management and tuberculosis prevention. Thematic analysis revealed that the availability of basic environmental facilities, caregiving support, and strong institutional cohesion served as key enablers of tuberculosis prevention; however, these were constrained by overcrowded living conditions, the absence of formal tuberculosis-specific procedures, tuberculosis-related stigma among adolescents, limited financial and health system support, informal governance practices, and fear of social consequences, which collectively hindered early reporting and preventive behaviors. Overall, tuberculosis prevention in boarding schools is shaped by a complex interplay of environmental, institutional, and sociocultural factors, underscoring the need for context-sensitive, institution-based prevention strategies that address stigma, strengthen governance, and enhance collaboration with health systems to reduce tuberculosis risk among adolescents in boarding schools.
