Abstract

Background: Diabetes Mellitus (DM) is a chronic metabolic disease that requires self-management education to prevent complications. Diabetes Self Management Education (DSME) is used as an effective method in improving the independent care of DM patients when discharged from the hospital. There are still many methods of delivering health education in several hospitals using poster / leaflet print media that are placed in strategic locations. Another method that can be done is to use the WhatsApp application.Purpose: This study aims to determine the feasibility of health education media about DSME through the WhatsApp application on Diabetes Mellitus type II clients. Methods: The method in this study uses Research and Development (RD) by using the ADDIE model development method (Analysis, Design, Development, Implementation, Evaluation). The subjects of this study were conducted by 3 experts (health promotion experts, nursing practitioners, and surgical medical nurses) and 10 clients with type II DM. In this study using a validation sheet from 3 experts by providing an assessment of the media used by researchers includes 3 aspects, including: aspects of the material, aspects of the content, and aspects of appearance. Results: The internal validity test was carried out by 3 experts with an average percentage rating on the material aspect of 93.3%, the content aspect of 95% and the display aspect of 91.6%. External validity test results obtained from trials limited to 10 clients with an average percentage of results in 3 aspects, among others: 96.5% material aspects, 95.5% understanding aspects and 96% display aspects. Conclusion: Submission of DSME through the WhatsApp application is considered very feasible to be used as a medium for delivering material for type II DM clients because it can improve understanding of self-management when discharged from the hospital.

Sample of the study was diabetes mellitus type 2 patients (n=52) divided into 2 groups using non probability sampling with purposive sampling. The instrument used SDSCA questionnaire by Kusniawati (2011) (r = 0,361, alpha cronbach ‘s = 0,80). Data analysis used dependent and independent t test. Results: Data analysis used independent t test obtained p value 0.019, indicating there was a significant differences and the results of the Δmean DSME group was 18,50 and the Δmean CBIA-DM group was 15.61, so that DSME was more effective than CBIA-DM.Conclusion: DSME is more effective to improving self care in people with diabetes mellitus. It is recommended that the Puskesmas can implement the DSME intervention conducting socialization through prolanist activities.

Abstract

Background: Tuberculosis is an infectious disease caused by the bacterium Mycobacterium tuberculosis. Tuberculosis treatment programs are long and cause some side effects that make patients not adherent to treatment so that family support is needed.Purpose: This study aims to discuss family support with medication adherence in patients with pulmonary tuberculosis. Methods: The type of research that studies the literature using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) method. This literature study took 17 electronic journals from 112 international and national electronic journals namely pubmed, google scholar, and one search published between 2014 and 2019 with research inclusion criteria, namely articles containing family support and medication adherence in patients with pulmonary tuberculosis. Results: Most of the literature states that family support is in the good category and has high medication adherence. The most extended family support given to pulmonary tuberculosis patients is informational support and emotional support, while the least support provided is instrumental support. The most dominant adherence factors are the patient's desire to recover or good patient motivation, the role of the family or good family support, and the role of the health worker as a PMO. The relationship of family support with medication adherence in patients with pulmonary tuberculosis showed a significant relationship. Conclusion: The literature states that the better the family support given to patients with pulmonary tuberculosis, the higher the compliance to take medication for these patients. Family support can improve treatment success in patients with pulmonary TB.

Abstract

Background: Chronic Kidney Failure (CKD) is a kidney function disorder that occurs gradually, cannot be recovered, and describes a decrease in urine production which results in an accumulation of fluid in the body. This can disrupt their fluid and electrolyte balance. In this regard, in Indonesia there are still many phenomena of non-compliance with fluid monitoring that occur. If this is left unchecked, it will result in a condition of overload or excess fluid in the patient. Fluid management is needed to fulfill the patient's fluid and electrolyte needs, so that changes in body weight and the results of the calculation of fluid balance can be controlled and become an illustration of meeting these needs.Purpose: to explain nursing care to meet fluid and electrolyte needs in chronic kidney disease patients. Methods: nursing care was carried out on patients with a diagnosis of chronic kidney disease who have impaired fluid and electrolyte requirements. The nursing intervention used to overcome this problem was fluid management. Data were collected from interviews and observations and further analysis was carried out. Results: After three days nursing intervention, the results showed that the client's level of shortness of breath decreased, the blood pressure level was close to normal, fluid calculation close to balance, and the client's weight seemed stable. Conclusion: fluid management was effective for monitoring fluid intake and output of chronic kidney disease patient.
