Background: There are many National Health Insurance (NHI) participants who do not yet understand the mechanism of NHI services, including clients who are hospitalized. The phenomena that exist on the field, that the client will enlist the help of health professionals who are always in the ward, especially nurses, to provide information on all matters, including NHI. On the other hand, nurses have a major role in caring for clients, based on their duties and functions.
Purpose : This study aims to determine if there is expansion of the role of nurses as care giver and advokator in the inpatient unit, era NHI program.
Methods : This research is a descriptive study of the role of nurses as care giver and advokator in inpatient era NHI program, employing triangulation method. Respondents were 64 inpatient clients registered as NHI participants in the Hospital Semarang assigned by proportional simple random sampling, and four nurses.
Result : The results showed that there are expanding the role of nurses as care giver and advocator in the inpatient unit, era NHI program.
Conclusion : With the implementation of NHI program, the roles of nurses as care giver and advokator has expanded in the inpatient unit. BPJS/NHI parties need to disseminate information relating to NHI to nurses, as well as the need to be given compensatory reward for nurses

The raising of diabetes melitus has increased complication on diabetes, one of that occurence is the wound on patients’ foot (diabetic foot ulcer). There are three supporting factors to diabetic foot ulcer likes neurotic disturbance, infection, and flow of blood disturbance. Researchs showed the capability comparison of hydrogel in debridement on necrotic tissue with debridemen enzimatic, hydrogel has better result in debridement and granulation on the tissue grow faster. The purpose of this research is to find out the efectiveness of healing wound by using Normal Saline(NaCl 0,9%) and hydrogel for diabetes melitus patinets’ with ulcer.This research is a quantitave one with experimental design. Population of this research are all of the diabetes melitus patients’ with ulcer. Method of sampling is using purposive sampling which collected 30 respondent foe each group. This data prepared with independent t – test, but abnormality distribution of data, become use alternative analize is using Mann-Whitney U-test.The result of data analize by using Mann-Whitney U-test and 5% signification, with ρ = 0,000 with Z result : 6,482 and mean rank 45,08:15,92 (3:1) it means that hydrogel is more effective than normal saline in healing wound for diabetes melitus patinets’ with ulcer at District of Semarang Hospital. The mean repaired ulcer with hydrogel decrease mean 10-13 point and normal saline decrease 2-3 point on 9 days (Bates Jansen scale).Conclusion this woundcare with hydrogel 3 poit more effectiveness than normal saline on regenetaion tissues. This advice make hydrogel for woundcare diabetic ulcer or more ulcers.

Background: COPD is a pulmonary disease characterized by air obstruction in respiratory tract from bronchi to the bronchioles, which continuously occur progressively, and irreversible, and related with the inflammatory response due to toxic gases (cigarette smoke, factory pollution and vehicle pollution) so it can cause a desaturation oxygen (SpO2). This impact will caused in concentration decreased, functional activity reduction, hypoxemia, hypoxia until cyanosis. To prevent this impact, there are two non-pharmacological methods that can be applied, a tripod position and pursed lips breathing exercise. Purpose: To determine the effectiveness of tripod position and pursed lips breathing to enhance oxygen saturation in patients with COPD.Methods: The research used queasy experimental with pre-posttest design. This research used total sampling that have 34 samples COPD patient which is include in inclusion criteria in working area of Puskesmas Padangsari Semarang. The samples are divided into 2 treatment groups (tripod position groups and pursed lips breathing groups) that gives 15 minutes therapy 3 times within 3 continuous days. This research will begins from February until March 2019. Statistical test to determine the affectivity of each groups using non-parametric test (Mann-Whitney test). Results: the result of this research shows differences effectiveness between tripod position and pursed lips breathing with p value 0.00 with mean deviation before and after therapy of tripod position 0.29 and pursed lips breathing 1.23.Conclusion: in this research shows that pursed lips breathing is most effective to enhance oxygen saturation in COPD patient compare with tripod position.

Background: Chronic Kidney Failure (CKD) is a kidney function disorder that occurs gradually, cannot be recovered, and describes a decrease in urine production which results in an accumulation of fluid in the body. This can disrupt their fluid and electrolyte balance. In this regard, in Indonesia there are still many phenomena of non-compliance with fluid monitoring that occur. If this is left unchecked, it will result in a condition of overload or excess fluid in the patient. Fluid management is needed to fulfill the patient's fluid and electrolyte needs, so that changes in body weight and the results of the calculation of fluid balance can be controlled and become an illustration of meeting these needs.Purpose: to explain nursing care to meet fluid and electrolyte needs in chronic kidney disease patients.Methods: nursing care was carried out on patients with a diagnosis of chronic kidney disease who have impaired fluid and electrolyte requirements. The nursing intervention used to overcome this problem was fluid management. Data were collected from interviews and observations and further analysis was carried out.Results: After three days nursing intervention, the results showed that the client's level of shortness of breath decreased, the blood pressure level was close to normal, fluid calculation close to balance, and the client's weight seemed stable.Conclusion: fluid management was effective for monitoring fluid intake and output of chronic kidney disease patient.

Self-management can be used as an effort to deal with chronic diseases, especially diabetes mellitus (DM). Consistent self-management can control the instability of blood sugar levels, minimize complications and improve the quality of life of sufferers. The purpose of the activity is to carry out assistance, education and self-management training of Diabetes Mellitus (DM). The method used is lectures and demonstrations using the medium of successful self-management books diabtes melitus and audiovisual on health education self-management diabetes mellitus. The target is 89 people, the number is divided into 2 groups, namely 49 elderly people with DM and 40 health cadres. Health cadres are only given education and socialization of the use of books to provide support to the elderly. The activity was carried out during two meetings, on the first day participants took blood sugar measurements during (GDS) and pretested by filling out a quality of life questionnaire (QOL). Then given material about self-management in people with DM. At the meeting, the two elderly participants conducted a posttest by filling out a quality of life questionnaire (QOL). The results of the activity showed that there was an improvement in the quality of life with a total score of >39 with a good quality of life from 35% to 47% after participating in diabetes mellitus self-management health education activities so that it could be concluded that there was an improvement in quality of life. This activity can help the elderly in recognizing and managing diabetes mellitus. ABSTRAK Manajemen diri dapat digunakan sebagai upaya untuk menangani penyakit kronis terutama diabetes melitus (DM). Manajemen diri yang dilakukan secara konsisten dapat mengontrol ketidakstabilan kadar gula darah, meminimalkan komplikasi dan meningkatkan kualitas hidup penderitanya. Tujuan kegiatan adalah melaksanakan pendampingan, edukasi dan pelatihan manajemen diri Diabetes Mellitus (DM). Metode yang digunakan adalah ceramah dan demosntrasi dengan menggunakan media buku sukses manajemen diri diabtes melitus dan audiovisual tentang pendidikan kesehatan manajemen diri diabetes melitus. Sasaran sebanyak 89 orang, Jumlah tersebut dibagi menjadi 2 kelompok yaitu 49 lansia dengan DM dan 40 kader kesehatan. Kader kesehatan hanya diberikan edukasi dan sosialisasi penggunaan buku untuk memberikan dukungan pada lansia. Kegiatan dilaksanakan selama dua kali pertemuan, pada hari pertama peserta dilakukan pengukuran gula darah sewaktu (GDS) dan melakukan pretest dengan mengisi kuesioner kualitas hidup (QOL). Kemudian diberikan materi tentang manajemen diri pada penderita DM. pada pertemuan kedua peserta lansia melakukan posttest dengan mengisi kuesioner kualitas hidup (QOL). Hasil kegiatan menunjukkan terdapat peningkatan kualitas hidup dengan total skor >39 dengan kualitas hidup baik dari 35% menjadi 47% setelah mengikuti kegiatan pendidikan kesehatan manajemen diri diabetes melitus sehingga dapat disimpulkan terjadi peningkatan kualitas hidup. Kegiatan ini dapat membantu lansia dalam mengenali dan mengelola penyakit diabetes melitus.

Tirah baring dalam waktu yang lama beresiko memperbesar resiko decubitus pada penderita stroke sehingga harus dilakukan pencegahan agar tidak menyebabkan kenaikan grade pada luka dan menimbulkan komplikasi. Upaya untuk mencegah dan menjaga integritas kulit yang mengalami immobilisasi lama dengan cara melakukan massage effleurage secara berlahan yang diselingi dengan Virgin Coconut Oil (VCO). Penelitian ini bertujuan untuk mengetahui adakah pengaruh massage dengan Virgin Coconut Oil (VCO) terhadap pencegahan decubitus pada pasien stroke non hemoragik. Desain penelitian menggunakan quasi-experinmental dengan desain penelitian pretest dan post test with cintrol group. Dengan metode total sampling dan masing-masing kelompok intervensi dan kontrol berjumlah 15 responden. Variabel independent penelitian ini adalah Massage Effleurage dan Virgin Coconut Oil (VCO) Variabel dependen penelitian ini adalah Dekubitus dan Stroke Non Hemoragik. Pengukuran risiko dekubitus dengan skala braden Data diperoleh dengan cara wawancara, pemberian Massage effleurage dengan Virgin Coconut Oil (VCO) diberikan setiap 1-2 kali sehari selama 7 hari. hasil penelitian menunjukan bahwa terdapat pengaruh massage effleurage dengan Virgin Coconut Oil (VCO) terhadap pencegahan risiko dekubitus pada pasien stroke non hemoragik dengan hasil uji dependent t test pada kelompok intervensi menunjukan p volue 0,000 ( p<0,05). Serta hasil uji independent t test pada post test kelompok intervensi dan kontrol menunjukan p volue 0,000 (p< 0,05) kesimpulan pada penelitian ini adalah ada pengaruh massage effleurage dengan Virgin Coconut Oil (VCO) terhadap pencegahan risiko dekubitus pada pasien stroke non hemoragik.

Pasien dengan gangguan ginjal kronik perlu mengubah gaya hidup mereka terutama berfokus pada asupan cairan. Ketidakpatuhan terhadap pembatasan asupan cairan yang disarankan menyebabkan memburuknya kondisi dengan cepat. Penelitian ini bertujuan untuk menganalisis faktor yang mempengaruhi kepatuhan pembatasan asupan cairan pada pasien gagal ginjal kronis. Penelitian ini merupakan studi deskriptif korelasi dengan besar sampel sebanyak 65 orang yang menjalani hemodialisa di Rumah Sakit Islam Kota Semarang. Partisipan mengisi kuesioner yang diadaptasi dari kuesioner ESRD-AQ yang dianalisis dengan uji regresi linier sederhana. Hasil penelitian diperoleh bahwa responden yang tidak patuh dalam pembatasan asupan cairan sebanyak 63,1%. Pengetahuan (r=2,94, p-value=0,018) dan dukungan keluarga (r=3,15, p-value=0,01) berhubungan secara signifikan dengan kepatuhan pembatasan asupan cairan. Sedangkan umur, jenis kelamin, pendidikan, lama menjalani hemodialisa tidak terdapat hubungan dengan kepatuhan pembatasan asupan cairan. Pengetahuan dan dukungan keluarga merpakan faktor yang mempengaruhi kepatuhan pembatasan asupan cairan pada pasien gagal ginjal kronik, karena itu diperlukan peningkatan pengetahuan keluarga untuk membantu pembatasan kelebihan cairan pada pasien gangguan ginjal kronik.

Background: critical patients often experience sleep disturbances, namely the time that is too short to sleep. Changes in the characteristics and sleep time of critical patients can cause worsening of the disease and increase the risk of death. Sleep in critical patients has a very important function in the repair mechanism of damaged tissue cells (natural healing mechanism).Purpose: to determine the relationship between disease response factors, environmental factors, and nursing procedure factors on the sleep quality of patientsMethods: this cross-sectional study took place at a tertiary hospital, from March to May 2023, including 50 adults. We collected demographic and clinical data and utilized the Richards-Campbell Sleep Questionnaire (RCSQ) and the Sleep Disorders Factors Questionnaire (KFGT) scale to evaluate sleep qualityResults: the study showed a significant relationship between physical factors and sleep quality in patients with sig. 0.001 (sig. 0.05) and shows a strong relationship (p value = 0.84).
