Abstract

At the beginning of the growth, tooth inclination is mesioangular, then move gradually to come into contact with thedistal surface of the second molar and further commit to sliding movement parallel to second molar axis. This positionwill be retained to achieve the eruption way to oral cavity. However, the last decade reported incidence increasedimpaction of the third molar teeth in the lower jaw. This study was aimed to study the level of pre-eruption of mandibularthird molar in the population group aged 14-17 years to predict the risk of tooth impaction or not. Differentiatedpopulations on cluster I, age 14-15 years old; group II, 15.1-16 years old; and group III 16.1-17 years old. Researchcarried out by clinical observation and measurement of at Department of Radiology in Jember University with 48people selected based on predetermined criteria. X-ray projection was exposed by paralleling technique on the lowerthird molar teeth, left and right. The results showed 43.75% of 14-15 year old group had the third molar angle interval50-56°; 15.1-16 year age group, 80% have 57-70° angle interval, and 16.1-17 years age group, 46.67% has angularinterval 64-70°. It was concluded that a large interval of the third molar angle increase due to the increased of age,and the angle is different between groups based on gender.

Abstract

Objective: The purpose of this research is to determine the effect of Tq to re-epithelialization at the margin of tooth socket post-extraction in diabetic rats.Material and Methods: Used 27 rats were induced diabetic with streptozotocin (STZ) dose of 50 mg/kg body weight of rats which divided into 3 groups that has been stained with hematoxillin-eosin and cytokeratin 19. They are P1 treated with thymoquinone (Tq), P2 treated with metformin, and K without treatment. Each group consist of 9 rats were divided into 3 subgroups based on day of observation. They are 3rd, 7th and 10th day post-extraction. Re-epithelialization was observed at the margin of the tooth socket that has been stained with hematoxillin-eosin and cytokeratin 19Results: showed there was an improvement of re epithelialization percentage in the Tq treatment group compared with P2 and K groups.Conclusion: Thymoquinone extract may decrease blood glucose level and increasing re-epithelialization in margin of the tooth socket after extraction in diabetic rats.

Abstract

Background: One of the benign salivary gland tumors is Warthin’s tumor, which is a benign tumor consisting of a papillary cystic structure covered by a double epithelial layer cells and lymphoid stroma with germinal center. Several cases have reported the Warthin’s tumor transformation into a malignant tumor such as lymphoma that develops from their stromal. Expression of cytotoxic T-lymphocyte antigen 4 (CTLA-4) as part of the immune checkpoint when highly expressed leads to a more rapid development or progression of tumors. Purpose: To analyze CTLA-4 expression in Warthin’s tumors associated with the pathogenesis of its growth through an escape mechanism from immune checkpoints and analyze based on CTLA expression whether this marker has the potential to be used as immunotherapy by administering anti CTLA-4. Methods: The tissue sections slides of Warthin’s tumor (n=8) were stained with Hematoxylin Eosin and immunostained with Recombinant Anti-CTLA4 antibody [CAL49] (ab237712). The slide with positive CTLA-4 is shown as staining on the cell membrane and/or cytoplasm. Observations were carried out using Optilab. The result is presented as figures. Results: Tumor cells expressed of CTLA-4 show in cytoplasm and/or cell membranes of the epithelial and stromal components of Warthin’s lymphoid. CTLA-4 is expressed lymphoid stroma, which is associated with inhibition of T cell activity against tumor cells, while the exact mechanism of CTLA-4 expression in epithelial components is not known but is thought to induce tumorigenesis and inhibit apoptosis. Conclusion: CTLA-4 is expressed in epithelial and stromal cells of Warthin’s tumor and this expression indicates that Warthin’s tumor cell growth is through the escape mechanism of the CTLA-4 check point immune. Further research is necessary to investigate whether CTLA-4 expression in lymphoid stroma has relate to their transformation toward a malignant tumor of lymphoma.

Abstract

Latar Belakang: Survei vektor leptosirosis pada lokasi KLB yang dilakukan oleh Dinas Kesehatan Kabupaten Bondowoso Tahun 2021 di Kecamatan Bondowoso, Kelurahan Tamansari RT/RW 13/05 menunjukkan success trap tikus sebesar 34,04%. Penelitian ini bertujuan mengetahui determinan aspek lingkungan baik lingkungan fisik maupun sosial (perilaku) yang berisiko terjadinya penularan Leptospira sp dari tikus terkonfirmasi di Kabupaten Bondowoso, Kelurahan Tamansari RT/RW 13/05 pasca KLB leptospirosis.Metode: Penelitian ini merupakan penelitian analitik observasional kuantitatif dengan desain rancang cross sectional di Kelurahan Tamansari RT.13/05 Kecamatan Bondowoso. Populasi dan sampel penelitian sebanyak 47 rumah di lokasi pasca KLB dengan mengamati empat variabel aspek lingkungan yang berisiko terjadinya penularan bakteri Leptospira sp. Data dianalisis mempergunakan software SPSS 22.0 untuk univariat, bivariat, dan multivariat dengan regresi logistik.Hasil: Hasil uji chi-square antara determinan empat aspek lingkungan yang berisiko (kondisi fisik selokan, keadaan selokan pada musim penghujan, kondisi tempat pengumpulan sampah, kondisi rumah) terhadap keberadaan bakteri Leptospira sp dari tikus yang tertangkap menunjukkan hubungan yang signifikan. Adapun hasil analisis regresi logistik menunjukkan ada satu aspek yang tidak berpengaruh yakni keadaan selokan pada musim penghujan yang meluber di pekarangan/halaman rumah.Simpulan: aspek lingkungan fisik terbukti berisiko terjadinya penularan Leptospira sp dari tikus terkonfirmasi di Kabupaten Bondowoso ABSTRACTTitle: Determinants of Environmental Aspects at Risk for Transmission of Leptospira sp Bacteria from Rats Confirmed in Bondowoso RegencyBackground: The vector survey of outbreak locations conducted by the Bondowoso District Health Office in 2021 in Bondowoso District, Tamansari Village RT/RW 13/05 showed a success trap of 34,04% rats. This study aims to determine the determinants of environmental aspects, both physical and social (behavioral) that are at risk of transmission of Leptospira sp from confirmed rats in Bondowoso Regency, Tamansari Village RT/RW 13/05 after the outbreak of leptospirosis. Methods: This research is a quantitative observational analytic study with a cross sectional design in Tamansari Village, RT.13/05, Bondowoso District. The population and sample of the study were 47 houses in post of outbreak locations by observing four variables of environmental aspects that were at risk of transmission of Leptospira sp. Data were analyzed using SPSS 22.0 software for univariate, bivariate, and multivariate with logistic regression. Results: The results of the chi-square test between the determinants of four environmental aspects at risk (ditch conditions, ditch conditions in the rainy season, conditions of garbage collection sites, house conditions) to the presence of Leptospira sp bacteria from captured mice showed a significant relationship. The results of the logistic regression analysis showed that there was one aspect that had no effect, namely the condition of the sewers in the rainy season which overflowed in the yard/yard of the house. Conclusion: aspects of the physical environmental proved to be at risk of transmission of Leptospira sp from confirmed rats in Bondowoso Regency.

Abstract

Tobacco sorting officers in Jember Regency are still sorting tobacco leaves manually using hand muscles which can cause Musculo Skeletal Disorders (MSDs) complaints.One of the MSDs registered at the International Labor Organization (ILO) is Carpal Tunnel Syndrome (CTS) which is a disorder resulting from compression of the median nerve in the carpal tunnel at the wrist with the main symptom being tingling, pain radiating to the fingers which can be accompanied by numbness, weakness. muscles, stiffness, and possible muscle atrophy. This study aims to determine the factors associated with CTS supported by Phalen's test. This research is expected to provide information to workers and the public regarding health problems that can be experienced by tobacco sorting workers as well as being a consideration for increasing health insurance for these workers. This research was conducted using a cross-sectional survey design at PTPN A and PT B, Jember Regency with a sample calculation using the Slovin formula and obtained 246 respondents.The data analysis technique used was univariate SPSS analysis of frequency distribution, bivariate Chi Square analysis and SEM PLS multivariate analysis. research results show that education had an effect on the symptom severity scale, age had an influence on the symptom severity scale, age had an effect on the functional status scale and age had an influence on the Phalen's test. Conclusion:Age is the dominant factor affecting the symptom severity scale, functional status scale and Phalen's test. Abstract

Background: Cocoa pod husk (CPH) of Theobroma cacao L. contains tannins, alkaloids, saponins, and flavonoids triterpenoids that have antibacterial, anti-inflammatory, and antioxidant activities that can accelerate wound healing and alveolar bone regeneration. Purpose: This study aimed to analyze the effect of CPH extract gel on the number of fibroblasts, osteoblasts, and osteoclasts in Wistar rats’ sockets after tooth extraction.  Methods: The samples used were 36 male Wistar rats divided into three groups: the negative control group (placebo gel), the positive control group (Alveogyl; Septodont, UK), and the treatment group (100 mg/ml CPH extract gel), which in each application was given for three, seven, and fourteen days on the mandibular first molar post-extraction socket. The samples were sacrificed and tissue was processed on days 4, 8, and 15. The amount of fibroblasts, osteoblasts, and osteoclasts was identified and determined by the Image J software. Results: The Mann-Whitney U analysis showed significant differences (p<0.05) between groups. Fibroblasts and osteoblasts in the treatment group were significantly higher than the control groups. Osteoclasts in the treatment group were significantly lower than the control group. Conclusion: Ethanol extract gel of CPH significantly increased fibroblasts on days 3 and 5 and osteoblasts on days 7 and 14 and decreased osteoclasts on day 14 in post-extraction socket of Wistar rats.

Abstract

Jember Regency is the 2nd Regency after Surabaya City in finding the highest number of TB cases in East Java Province, with 5,244 adult cases and 478 pediatric TB cases found. Finding TB cases in children is an important component in controlling TB because the number of children aged less than 14 years constitutes 12% of the population. The government's efforts to control childhood TB by providing TPT (TPT) as an effort to strengthen the management of latent tuberculosis infection (ILTB) are still low. Toddlers in close contact with Drug Sensitive Tuberculosis (TB SO) are a group at risk of being infected with TB so they need to get TPT. This study aims to determine and analyze the driving factors, enabling factors, reinforcing factors and the role of the Medication Supervisor (PMO) on medication adherence in the TPT program for toddlers in close contact with TB in Jember Regency. The method in this research uses the Explanatory Research type, which is a type of research that aims to analyze the relationship or influence between variables through hypothesis testing. The results of this research indicate that the success factors of the TPT administration program, namely driving factors, enabling factors, reinforcing factors and the role of the Medication Supervisor (PMO) are proven to influence medication adherence in the TPT administration program for toddlers in close contact. It’s known that the factors contributed mainly to the successness of Tuberculosis Prevention Therapy are family’s support and the rile of the the Medication Supervisor (PMO).

Abstract

Background: Unknowingly, many tuberculosis (TB) patients visit dentists for examinations. TB can be detected in dental practices, appearing as an oral manifestation that is rarely diagnosed without TB symptoms. Purpose: To describe the development of advanced technology-based oral tuberculosis detection: a comprehensive review of recent innovations to improve the accuracy and effectiveness of tuberculosis (TB) diagnosis and control. Method: This study is a narrative review aimed at providing a comprehensive understanding of oral tuberculosis (TB) detection and the role of advanced technologies, such as Artificial Intelligence (AI) and the Internet of Things (IoT), in improving diagnostic accuracy and effectiveness. This method was conducted by collecting and compiling various research findings, case reports, and scientific studies relevant to the topic of oral TB and technological innovation in the medical world. Results: This study found that asymptomatic TB is often found in some patients, but they complain of unusual changes in the oral cavity, such as ulcers, osteomyelitis, and sore throat. Conclusion: Diagnosing TB lesions and disease using the latest technology is fast, effective, and accurate to determine appropriate treatment immediately.   Keywords: Accuracy; Advanced Technology; Innovation; Oral Tuberculosis Detection; Tuberculosis.   Pendahuluan: Tanpa disadari, banyak pasien tuberculosis (TB) yang datang ke dokter gigi untuk memeriksakan diri. Penyakit TB dapat ditemukan di praktik dokter gigi yang muncul sebagai manifestasi oral yang jarang sekali terdiagnosis tanpa gejala TB. Tujuan: Untuk memaparkan pengembangan deteksi tuberkulosis oral berbasis teknologi canggih: kajian komperhensif atas inovasi terkini dalam meningkatkan akurasi dan efektivitas diagnosis serta pengendalian penularan tuberkulosis (TB). Metode: Penelitian ini merupakan sebuah ulasan naratif (narrative review) yang bertujuan untuk menyajikan pemahaman komperhensif mengenai deteksi tuberculosis (TB) oral serta peran teknologi canggih, seperti Artificial Intelligence (AI) dan Internet of Things (IoT), dalam meningkatkan akurasi dan efektivitas diagnosis. Metode ini dilakukan dengan mengumpulkan dan menyusun berbagai hasil penelitian, laporan kasus, serta kajian ilmiah yang relevan dengan topik TB oral dan inovasi teknologi dalam dunia medis. Hasil: Penelitian ini menemukan bahwa, TB asimtomatik sering ditemukan pada beberapa pasien, hanya saja mereka mengeluhkan adanya perubahan yang tidak biasa pada rongga mulut pasien, seperti ulkus, oesteomielitis, dan sakit tengggorokan. Simpulan: Diagnosis lesi dan penyakit TB menggunakan teknologi terkini secara cepat, efektif, dan akurat untuk menentukan pengobatan yang tepat dengan segera.   Kata Kunci: Akurasi; Deteksi Tuberculosis Oral; Inovasi; Teknologi Canggih; Tuberculosis.

Abstract

Background: One of the benign tumors of the salivary glands is Warthin's Tumor (WT), which consists of cystic bilayer papillary epithelial cells accompanied by the presence of a lymphoid stroma. Several cases have been reported to turn malignant. One of the markers developed to identify tumor proteins is fibrinogen-like protein 1 (FGL1). Along with lymphocyte-activation gene 3 (LAG-3), FGL1 establishes an immune checkpoint pathway that plays a role in the mechanism of tumor immune escape. Not much has been reported regarding FGL1 expression in WT, but some studies have reported that its expression is associated with tumor growth. Purpose: This study aims to analyze the location of FGL1 expression in WT and its relation to the mechanism of tumor immune release through the interaction between FGL1 and LAG-3. Methods: Cases of WT (n = 11) and breast cancer (n = 1) were used as positive controls. All cases were stained with hematoxylin-eosin and Recombinant Anti-FGL1 antibody. The FGL1 expression was observed in the cell membrane, cytoplasm, and lymphoid stroma. The results are presented in the form of figures. Results: All cases of WT expressed FGL1 in the cell membrane, cytoplasm, and lymphoid stroma. Its expression in the cell membrane and cytoplasm is possibly related to the process of tumorigenesis and the increasing size of the lesion. Additionally, its expression is seen in the lymphoid stroma, which is closely related to immune escape by inhibiting lymphocytes against tumor cells. Conclusion: Warthin's tumor cells express FGL1, and this expression plays a role in tumor immune escape mechanisms and tumor growth.

Abstract

Background: Human Immunodeficiency Virus (HIV) infection causes Acquired Immune Deficiency Syndrome (AIDS), a collection of symptoms arising from the deteriorating immunity. The number of people living with HIV worldwide continues to increase year-by-year. Those infected with HIV need antiretroviral (ARV) therapy to suppress the amount of HIV in the body. Purpose: This study aimed to investigate the determinants of adherence in people living with HIV/AIDS (PLHIV) undergoing ARV therapy in Situbondo Regency. Methods: This study employed a quantitative analytic observational approach with a cross-sectional design. Data analysis involved both descriptive and multivariate analysis. The population for this study consisted of 328 individuals, from which 212 were selected as samples, all being PLHIV on ART as of April 2022, sourced from ARV treatment services in Situbondo district. Results: The research analysis was based on predisposing factors, measured using three indicators: work, knowledge, and belief in ARVs. Supporting aspects were measured by two factors: ownership of health insurance and adherence counselling. Reinforcing factors were measured using two indicators: family support and stigma in the community. Conclusion: The study concluded that predisposing and reinforcing factors significantly influenced PLHIV adherence to antiretroviral therapy. However, supporting factors did not show a significant influence on adherence in PLHIV undergoing antiretroviral therapy in the Situbondo District.
