Abstract

Upaya mewujudkan mutu pelayanan rawat jalan, manajemen RS “X” Malang memiliki standar operasional prosedur (SOP). Namun, SOP alur pelayanan rawat jalan tidak dilaksanakan di polklinik spesialis maupun umum. Dampaknya terdapat terdapat 83 pasien tidak membayar biaya tindakan dan pemeriksaan tambahan selama 4 bulan. Tujuan dari penelitian ini adalah untuk mengetahui akar masalah tidak dijalankan SOP alur pelayanan rawat jalan dan mencari solusinya. Jenis penelitian adalah deskriptif pendekatan kualitatifdilakukan dengan cara observasi, wawancara, dan Focus Group Discussion (FGD). Hasil penelitian menunjukkan bahwa petugas tidak sepakat dengan SOP alur pelayanan rawat jalan, dokumen tidak lengkap, petugas lupa urut-urutan SOP, diagram alur pelayanan tidak ada, jarak poli dengan kasir jauh. Kesimpulan dari penelitian ini adalah belum ada kesepakatan antara petugas dan manajemen menjadi alasan utama tidak dijalankan SOP alur pelayanan rawat jalan. Petugas menghendaki adanya perubahan SOP alur pelayanan rawat jalan sesuai dengan kondisi lingkungan rumah sakit saat ini. Saran yang dapat dterapkan adalah penambahan petugas pengantar pasien ke kasir.Impact of Implementation Not Comply SOP Flow Outpatient Hospital "X" MalangAbstractOutpatient installation in UMM hospital is main installation because it tends to increase the number of visits every year. As a result of not being implemented SOPs outpatient services flow have impacted their patients do not pay the costs of medical treatment and additional examinations. The financial statements of RS UMM for 4 months there were 83 patients did not pay the costs of medical treatment  and additional examination. The purpose of this study is to investigate the implementation of SOPs outpatient services flow in UMM hospital. This research was conducted with a qualitative descriptive approach. Search root of the problem by means of observation, interviews, and a Focus Group Discussion (FGD). The results showed that outpatient SOPs services are not implemented, there are no file / document SOPs outpatient services flow in the relevant units, some officers in polyclinic do not know SOPs outpatient services flow, and SOPs has never been evaluated. The conclusion of the research is need to be evaluated SOPs outpatient service flow in accordance with the condition of hospital environment at this time.

Abstract

Pelaporan kinerja rawat inap merupakan komponen penting dalam evaluasi dan perencanaan rumah sakit. Studi pendahuluan menunjukkan rendahnya kinerja pelaporan rawat inap oleh kepala ruang karena lemahnya pemahanan tentang cara penghitungan dan pelaporan indikator kinerja rumah sakit. Tujuan dari penelitian ini adalah mencari faktor-faktor yang mempengaruhi pemahaman kepala ruang rawat inap dalam menghitung dan melaporkan indikator kinerja efisiensi ruang, mengidentifikasi solusi dan evaluasi dampak solusi tersebut. Metode yang digunakan dalam penelitian ini adalah deskriptif kualitatif dengan melakukan wawancara tidak terstruktur kepada kepala bidang keperawatan, 5 kepala ruang serta 3 perawat di rawat inap. Hasil penelitian menunjukkan bahwa kepala ruang rawat inap tidak mengetahui apa saja indikator efisiensi rumah sakit dan definisinya, perhitungan kinerja bangsal tidak terdeskripsikan dalam Tupoksi (Tugas Pokok dan Fungsi) kepala ruang, tidak ada panduan cara menghitung indikator efisiensi rumah sakit khususnya BOR (Bed Occupancy Rate) di unit rawat inap, belum ada kebijakan dari rumah sakit untuk kepala ruang rawat inap melaporkan kinerja efisiensi ruangan perbulan/pertahun, belum ada sosialisasi dan pelatihan, tidak adanya evaluasi kinerja manajemen bangsal setiap bulan dari rumah sakit. Alternatif solusi yang dipilih rumah sakit untuk mengatasi semua penyebab adalah sosialisasi dan pelatihan tentang perhitungan indikator kinerja ruang kepada petugas rumah sakit. Dampak dari sosialisasi tersebut adalah para kepala ruangan dapat menghitung dan melaporkan indikator kinerja efisiensi dimasing-masing ruangan. Dapat disimpulkan lemahnya pemahaman kepala ruang tentang indikator kinerja karena tidak adanya pelatihan dan panduan yang disebabkan tidak ada dukungan kebijakan dan anggaran.Kata Kunci: Indikator efisiensi rumah sakit, kepala ruang, pemahaman, rawat inap

Abstract

Berdasarkan data di RS X di Malang, sejak diterapkannya INA-CBG pada tahun 2009 masih terdapat ketidaksesuaian diagnosis dengan kode ICD-10. Tujuan penelitian ini untuk mengetahui peran faktor-faktor yang mempengaruhi ketepatan penulisan koding diagnosis ICD-10 oleh tenaga medis RS X di Malang. Desain penelitian ini menggunakan desain penelitian deskriptif kualitatif. Informan penelitian ini berjumlah 5 orang dengan menggunakan teknik wawancara mendalam (In depth interview) dan Focus Group Discussion (FGD). Hasil penelitian menunjukkan lemahnya pengetahuan, pemahaman dan sikap Dokter terhadap pentingnya akurasi koding diagnosis sesuai ICD 10 dalam manajemen di rumah sakit, meskipun sudah terpapar dengan pelatihan dan sosialisasi yang didukung dengan adanya prosedur standar. Rumah sakit perlu memperkuat pemahaman dan sikap positif Dokter terhadap akurasi koding diagnosis sesuai ICD yang didukung dengan mekanisme reward and punishment yang jelas.Kata Kunci: Akurasi koding diagnosis,dokter,  ICD 10, pengetahuan dan sikap

Abstract

Promotion and decision making from customer have positive relation. Marketing promotion mix is one of a promotion communication tools in a company. Preeliminary study in  Muhammadiyah Malang University Hospital (UMM Hospital) showed that patient visitation from other medical specialistic is still low. This study objectives are to know the factors of low patient visitation to other medical specialistic in UMM Hospital. Methodology used in this study are observation, focus group discussion (FGD), documents review, and interview. The study result shows that there are no monitoring and evaluation that has been done to promotion activities, therefore promotion activities are not maximal. Solution that agreed to UMM Hospital are making guideline and standard operational of procedurs (SOP) of monitoring and evaluation of promotion activities, and quistionnare to patient visitation in UMM Hospital.

Abstract

The quality of health services is the degree of perfection of health services in accordance with professional standards and service standards. The quality of health services can be measured by five quality dimensions, namely reliability, assurance, tangible, empathy, responsiveness. The ultimate goal of the quality of health services is the creation of patient satisfaction. This study aims to determine the relationship between the quality of health services and the satisfaction of inpatients class III JKN participants at Prof. Dr. Soekandar Hospital at Mojosari. The research design is analytical observational with a cross sectional approach. The sample is 201 inpatients. Hypothesis testing using Kendall's tau test. Based on the results of the analysis, it was found that p = 0.000 which stated that there was a relationship between the quality of health services and the satisfaction of class III inpatients of JKN participants. This shows that the quality of health services greatly affects the satisfaction of inpatients. If the quality of health services provided is good, the patient will feel satisfied. The study concluded that there was a relationship between the quality of health services and the level of satisfaction of inpatients. Good service quality will result in a high level of satisfaction.

Abstract

The current implementation of JKN brings major changes in the health care and financing system, especially in hospitals. The payment system in hospitals has changed, which previously used a fee-for-service system, was changed to a prospective payment system using the INA-CBGs rate and BPJS Kesehatan was appointed as the implementing institution for the program. The INA-CBG's payment system was chosen because it is possible to control health costs, encourage quality services according to standards, limit unnecessary or excessive health services and encourage providers to carry out cost containment. This study aims to determine the impact of the implementation of the JKN program on health costs in FKTL. This research is a literature review (narrative review) by conductinga literature review of 12 journals. Journals were obtained from Google Scholar and PubMed searches with the keywords Impact of JKN on Healthcare Costs in FKTL OR Hospitals which were selected with the criteria of national and international journals with good reputation and indexed by SINTA and SCOPUS which were published no later than 2016. the purpose of this study and passed the selection. The results of the analysis show that the impact of JKN, especially on health costs, has an effect on the pattern ofJKN utilization by the community, patient satisfaction, hospital efficiency, Gross Profit Margin and Net Profit Margin of hospitals, and has an effect on hospital income. It can be concluded that there has been an influence on FKTL since the JKN era entered Indonesia.

Abstract

The absence of medication errors is a sign of successful implementation of patient safety management, particularly safe treatment. The quality indicator data report at the Pharmacy Installation of a private hospital in Malang in 2020 showed a number of drug administration errors in the inpatient unit. This study aimed to explore the incidence and causes of medication errors in the inpatient unit. The study was conducted at a private hospital in Malang in August-September 2021. The study design was descriptive with data collected through focus group discussions (FGD) with five inpatient pharmacy staff, four inpatient nurses, and the head of the medical support. The study results indicated no clear regulation, especially the Standard Operational Procedure (SOP) for administering drugs to inpatients, which becomes a latent cause of medication errors. Hospitals need to develop SOPs for administering drugs to inpatients according to the 7 Pillars, followed by socialization to nurses in inpatients and periodic monitoring.

Abstract

Pendahuluan: Katarak ialah keadaan dimana lensa mata yang biasanya jernih menjadi keruh. Sebanyak 81,2% dari 8 juta orang Indonesia yang mengalami gangguan penglihatan pada tahun 2017 disebabkan oleh katarak. Pajanan kronis sinar (UV) adalah salah satu dari banyak faktor risiko di tempat kerja dimana Indonesia adalah salah satu negara dengan iklim tropis. Risiko penyakit akibat kerja meningkat di sub sektor perkebunan oleh karena pajanan sinar UV. Rendahnya pendidikan di masyarakat berpengaruh terhadap kemampuan pemahaman dan tingkat kesadaran akan penyakit katarak yang rendah. Metode: Desain penelitian ini adalah observasional analitik dengan pendekatan cross sectional dengan pengambilan data menggunakan kuesioner yang kemudian dianalisis secara deskriptif dan analitik melalui software IBM SPSS versi ke-23 dengan menggunakan Uji Statistik Pearson Chi-Square dengan total responden sebanyak 46 pekerja Kebun Teh Wonosari Kabupaten Malang. Hasil: Tidak didapatkan pengaruh pengetahuan paparan sinar UV terhadap gejala katarak pada pekerja Kebun Teh Wonosari Kabupaten Malang dengan nilai p-value = 0,668 (p>0,05) Kesimpulan: Tingkat pengetahuan paparan sinar ultraviolet tidak memberikan pengaruh terhadap gejala katarak pada pekerja Kebun Teh Wonosari Kabupaten Malang karena pada responden dengan kategori pengetahuan rendah tidak ditemukan gejala katarak dengan frekuensi yaitu 38 orang dan 1 orang dengan gejala katarak sehingga pengetahuan tidak berpengaruh terhadap gejala katarak.
