Abstract

Background: Labour pain causes hyperventilation, increasing the need for oxygen and blood pressure and reducing intestinal motility and urinary bladder. These conditions will interfere with uterine contractions, which can cause uterine inertia, prolonged labour, inadequate baby oxygenation that results in foetal distress, and maternal and/or foetal death. if labour pain is not treated. Aromatherapy is a therapeutic action that uses essential oils. which is beneficial in improving one's physical and psychological condition to be better. Each essential oil has unique pharmacological effects, such as antibacterial, antiviral, sedative, and stimulates the adrenals. Methods The type of research used in this research is quantitative with a quasi-experimental research design. The research design used was a two-group prepost-test design, a research activity that gave an initial test (pretest) before treatment, after treatment, and then a final test (post-test). The population in this study were all 29 primigravida mothers. The sample was 21 mothers with primigravida. The independent variable in this study was lemongrass aromatherapy, which was measured using standard operating procedures for aromatherapy. The dependent variable in this study was the reduction in pain measured using the NRS scale. The data analysis technique used was univariate data analysis using frequency descriptions and bivariate statistical tests using Wilcoxon signed. Results: The value of p = 0.002 (α < 0.05) was obtained. Therefore, it was concluded that there were differences in pain changes before and after the lemongrass aromatherapy group. Giving lemongrass aromatherapy to pain in the first stage of labour is effective. Conclusion: A woman giving birth during the first active phase needs support to maintain her motivation to birth normally and cope with any discomfort she may experience. Lemongrass aromatherapy, given as a gift, can be used to ease the discomfort of labor, improving both the mother's experience and her ability to trust the process.

Abstract

Highlights: 1. Auricular acupressure at the Shenmen point is effective in reducing anxiety. 2. Auricular acupressure is more practical than body acupressure since it does not interfere with expectant mothers' mobilization. 3. As a non-pharmacological method, auricular acupressure is cost-effective, efficient, and effective in overcoming maternal anxiety during labor Abstract Anxiety is a common psychological symptom in expectant mothers, particularly among primigravidae during labor. There is a link between excessive anxiety and prolonged labor. One of the options to manage anxiety is the use of non-pharmacological methods. Numerous studies have investigated the effectiveness of auricular acupressure to treat anxiety, but few have examined its effect on anxiety biomarkers. Thus, the objective of this study was to analyze the effect of auricular acupressure targeting the Shenmen point on the levels of β-endorphins and anxiety among primigravidae during the first stage of labor. This experimental study used a pretest-posttest control group design. The research subjects were primigravidae in the first stage of labor at the independent midwife practice of Yefi Marliandiani in Surabaya, Indonesia. Among 40 primigravidae, 34 individuals met the inclusion criteria and were then equally divided into treatment and control groups using a simple randomization. The treatment group received auricular acupressure at the Shenmen point every 30 minutes for a duration of 5 minutes, while the control group practiced deep breathing relaxation. The anxiety levels were assessed using the Hamilton Anxiety Rating Scale. The β-endorphin levels were measured by drawing 3 cc of venous blood before (4 cm cervix dilation) and after (9–10 cm cervix dilation) treatment. The obtained data were analyzed using paired- and independent-samples t-tests. A decrease in the mean anxiety level was observed in the treatment group (18.59±7.52) relative to the control group (19.76±7.11), albeit not significant (p=0.64). Simultaneously, the average β-endorphin levels of the treatment group (355.4±224.7) increased compared to the control group (225.1±127.5), with a significant difference observed between the two groups (p=0.04). In conclusion, while both auricular acupressure at the Shenmen point and deep breathing relaxation are effective in reducing anxiety, the former is more prominent in its ability to increase β-endorphin levels.

Abstract

Childbirth is a physiological process that is influenced by maternal, familial, and environmental support. Primigravida women often experience heightened anxiety due to lack of childbirth experience. Increased anxiety can stimulate cortisol release, potentially leading to labor complications such as impaired uterine contractions and prolonged labor. One non-pharmacological approach to reduce anxiety during labor is acupressure. While ear acupressure has been shown to reduce labor pain, evidence regarding its effect on anxiety remains limited. This study aimed to examine the effectiveness of ear acupressure at the Shen Men point in reducing anxiety during the early stage of labor. A quasi-experimental one-group pre-test–post-test design was used, involving 31 primigravida women in early labor selected through convenience sampling. Anxiety levels were measured before and after the intervention using the Anxiety Assessment Scale for Women in Labor (AASWL). Before the intervention, 67.70% of participants reported anxiety levels above 50%. After ear acupressure, more than half of the participants (54.80%) continued to report anxiety levels above 50%. The mean anxiety score decreased from 25.71 before the intervention to 16.48 after, showing an average reduction of 9.23 points. However, statistical analysis using the Wilcoxon signed-rank test yielded a p-value of 0.157, indicating that the reduction was not statistically significant. In conclusion, ear acupressure at the Shen Men point was not shown to have a significant effect on reducing anxiety during the first stage of labor in primigravida women.
