Abstract

Introduction: DM prevalence keeps rising, with 90-95% of cases being DMT2, which, if not treated properly, will cause various complications. Around 30-50% of DMT2 sufferers experience complications, with the proportion at Dr. Wahidin Sudirohusodo Hospital reaching 49.7%. High complications stem from uncontrolled factors, making optimal management key to prevention. Aim: Analyze risk factors for chronic complications in DMT2 patients at Dr. Wahidin Sudirohusodo Hospital. Method: Observational analytic with case control design. The population included DMT2 patients at Dr. Wahidin Sudirohusodo Hospital, with 158 samples (79 cases and 79 controls) selected using purposive sampling. Data were collected through interviews and medical records, and then analysed univariately, bivariately, and multivariately. Results: Multivariate statistical analysis with multiple logistic regression showed significant results for the variables age (3.62), duration of DM (4.83), medication compliance (3.26), blood sugar control (3.63), and stress (6.82). Conclusion: Age, duration of DM, medication compliance, blood sugar control, and stress are significant risk factors for complications, with stress as the most influential factor. Therefore, DMT2 patients need to manage stress, routinely control blood sugar, and comply with treatment to prevent more serious complications.

Abstract

The prevalence of type 2 diabetes mellitus continues to increase globally and has become a significant economic burden on health, making the identification of risk factors for chronic complications important in order to prevent long-term effects. This study aims to analyze risk factors for chronic complications in patients with type 2 DM at five community health centers in Makassar: Sudiang, Ballaparang, Kassi-kassi, Antara, and Paccerakkang. The study design used an analytical observational approach with a case-control approach. The total sample consisted of 100 respondents (50 cases and 50 controls) selected using purposive sampling. Data were collected through interviews and medical records, then analyzed univariately and bivariately (Odds Ratio/OR). The results of statistical analysis showed that age (OR = 2.524; 95% CI = 0.723-8.818), blood pressure (OR = 1.379; 95% CI = 0.628–3.029), GDP (OR = 3.802; 95% CI = 1.482–9.750), duration of DM (OR = 18.614; 95% CI = 6.782–51.087), treatment adherence (OR = 2.263; 95% CI = 1.013–5.052), stress (OR = 1.084; 95% CI = 0.494–2.377), and self-care for DM (OR = 2.495; 95% CI = 1.105–5.629). Based on these findings, patients with type 2 DM are advised to monitor their blood sugar levels regularly, improve adherence to therapy, and implement good self-care practices such as diet management, regular physical activity, and periodic health checkups to prevent chronic complications.

Abstract

Abstract

Introduction: Non-adherence among patients with chronic kidney disease (CKD) undergoing hemodialysis has been reported to range from 10% to nearly 60% and may lead to acute and chronic complications, reduced quality of life, and increased mortality risk. Variations in previous findings indicate that factors influencing adherence require further investigation within specific local contexts. This study aimed to analyze factors associated with adherence among hemodialysis patients at Dr. Wahidin Sudirohusodo General Hospital, Makassar. Methods: This study employed a quantitative cross-sectional design and was conducted at the hemodialysis unit of Dr. Wahidin Sudirohusodo General Hospital, Makassar, involving 103 patients selected through purposive sampling. Data were collected through structured interviews using validated questionnaires and review of medical records. Data were analyzed using the Chi-Square test and Fisher’s Exact Test. Ethical approval was obtained from the Health Research Ethics Committee of the Faculty of Public Health, Hasanuddin University (Approval Number: 3500/UN4.14.1/TP.01.02/2024). Results: The analysis showed significant associations between age (p=0.032), family support (p=0.025), and self-efficacy (p=0.025) with hemodialysis adherence. No significant associations were found between sex (p=0.447), education level (p=0.916), duration of hemodialysis (p=0.738), and anxiety level (p=0.738) with adherence. Self-efficacy was identified as the most strongly associated factor with patient adherence. Conclusion: Age, family support, and self-efficacy were significantly associated with hemodialysis adherence. These findings highlight the importance of psychosocial approaches, particularly strengthening self-efficacy and family support, in improving adherence among hemodialysis patients. Future research should employ longitudinal designs to better explore causal relationships.
