Abstract

Background: Ultrasound-guided arterial cannulation has the potential to increase the total success rate and first attempt and reduce complications compared to conventional landmark palpation methods. This study aims to compare the success rate of radial artery cannula with palpation techniques and ultrasound guidance in patients undergoing major surgery under general anesthesia. Methods: This study is a randomized controlled trial singly. The inclusion criteria for this study were patients who underwent both emergency and elective surgery under general anesthesia, aged >18 years, with ASA I and II physical status. The study sample (60 people) was randomized by computer and divided into two groups (ultrasound guidance and palpation technique). The study will assess the success of the first insertion of the radial artery cannula in both groups. Data analysis was performed with SPSS 25 univariate and bivariate. Results: There is a significant relationship with the success of the first attempt at an ultrasound-guided cannula trial with a p-value of 0.045 with an odds ratio of 3.33 with a 95% confidence interval (0.998 – 11.139). Conclusion: Ultrasound-guided arterial cannulation has a better success rate and fewer complications than conventional palpation techniques.

Abstract

Background: The rRT-PCR examination is very effective as a diagnostic tool for COVID-19 but cannot be a full reference as an evaluation examination because rRT-PCR is able to capture the genetic material of the virus without ensuring that the virus is still alive or only dead particles. This study aims to explore the correlation between absolute lymphocyte values and the severity of COVID-19 at Dr. Mohammad Hoesin General Hospital Palembang. Methods: This study is an observational study involving 413 research subjects at Dr. Mohammad Hoesin General Hospital. Data analysis was carried out univariate and bivariate. Results: The absolute number of lymphocytes with the severity of COVID-19 has a negative correlation, with a strong correlation strength, which is expressed by the value of r = -0.617 and the degree of significance of p = 0.000. Conclusion: There is a correlation between absolute lymphocyte count and the severity of COVID-19 at Dr. Mohammad Hoesin General Hospital Palembang.

Abstract

Introduction: Endophthalmitis is the most severe ocular complication and often causes blindness. The incidence of endophthalmitis varies depending on the type of surgery performed, from 0.082% to 0.32%. The use of povidone-iodine and topical antibiotics on the conjunctiva has been reported to reduce the likelihood of postoperative infection. This study aimed to compare the effectiveness of levofloxacin ED 0.5% with povidone-iodine 2.5% in reducing the normal bacterial flora of the conjunctiva in intraocular preoperative procedures. Methods: Clinical trial experimental studies. A total of 34 research subjects participated in this study. The research subjects were grouped into treatment groups of 0.5% levofloxacin and 2.5% povidone-iodine. Analysis of differences in the number of bacterial colonies was carried out with the help of bivariate SPSS software. Results: Treatment with levofloxacin 0.5% eye drop was able to reduce the number of colonies by 78.11%, while treatment with povidone-iodine 2.5% was able to reduce the number of colonies by 68.57%. Conclusion: Levofloxacin 0.5% eye drop is superior in reducing the number of conjunctival bacterial colonies in intraocular preoperative procedures compared to 2.5% povidone-iodine.

Abstract

Introduction: Rupture of the Achilles tendon has become an increasingly common case in the last two decades with an incidence of 18% per 100,000 population. The controversy about the best treatment arises because the results are determined by the method of tendon repair and postoperative functional rehabilitation. In recent years, there is a great deal of interest in hyaluronic acid which is found in certain amounts in the soft tissue extracellular matrix and joint fluid. Several experimental studies have shown the application of hyaluronic acid (HA) to heal tendons and stimulate tendon healing and reduce adhesion formation. This research is to find out the effectiveness of injecting hyaluronic acid in behavior changes in mice that have modified Achilles tendon rupture in 21 mice divided into three groups according to their treatment, namely intramuscular HA (IM), Intralesional HA (IL) and NaCl in laboratory animals Sriwijaya University Faculty of Medicine 2019 by seeing changes in behavior every 7 days 3 times. Results: The group with HA-IL treatment had the highest cure success rate, which was 7 (58.3%) good grades, 5 (41.7%) good grades and 2 (100%) very good grades and 5 (33.3%) good and 2 (50%) extraordinary (change behavior 1, 2 and 3, respectively). Then HA-IM 5 (41.7%) Good grades, 7 (58%) Good grades, and 5 (33.3%) and 2 (50%) exceptional grades (change behavior 1, 2 and 3, respectively) and NaCL only have a value of 5 (33.3%) both for behavior change 3. Conclusion: There is a difference in effectiveness between administration of hyaluronic acid and without administration of hyaluronic acid to behavioral changes in repair of the Achilles tendon.

Abstract

Background: Platelet/lymphocyte ratio (PLR) is considered as predictor for chemotherapy response in locally advanced breast cancer undergoing neo-adjuvant chemotherapy. This study aims to evaluate the relationship between the platelet/lymphocyte ratio and neo-adjuvant chemotherapy response in patients with locally advanced breast cancer at Dr. Mohammad Hoesin General Hospital Palembang. Methods: This study is a prognostic test of the platelet/lymphocyte ratio accuracy to predict the response of neo-adjuvant chemotherapy in patients with locally advanced breast cancer. The study included 113 subjects. Results: From 113 patients, the PLR cut off point value was 206.83 with an under curve area (AUC) of 91.8%. The results of the PLR prognostic test were sensitivity of 98%, specificity of 87%, Positive Predictive Value of 95%, Negative Predictive Value of 93%, and 95% of accuracy. There was a significant relationship between PLR and chemotherapy response in patients with locally advanced breast cancer (p = 0.000). Conclusion: There was a significant relationship between PLR on chemotherapy response. PLR can be used as a predictor of neo-adjuvant chemotherapy response with a good degree of accuracy.

Abstract

Introduction: Breast cancer is one of the most common neoplasm occurs in women. Neoadjuvant Chemotherapy (NACT) is widely used in Locally Advanced Breast Cancer (LABC) and Inflammatory Breast Cancer (IBC), to reduce tumor size (down-sizing), and further allowing breast-conserving surgery to be performed. Accurate markers are needed to prevent impractical chemotherapy that allows patients to receive definitive surgery in a timely manner. Previous study showed neutrophil/lymphocyte ratio (NLR) was superior in predicting long-term outcomes over PLR. This study aims to evaluate the relationship between the NLR value and neoadjuvant chemotherapy response in patients with locally advanced breast cancer at Dr. Mohammad Hoesin General Hospital Palembang. Methods: This study is a prognostic test of the NLR accuracy to predict the response of neoadjuvant chemotherapy in patients with locally advanced breast cancer. The study included 113 patients as study subjects. Results: The obtained NLR cut off point value was ≤ 2.92 with an area under the ROC curve (AUC) of 92.2%. The diagnostic measures of the NLR prognostic test had sensitivity of 93%, specificity of 93%, Positive Predictive Value of 97%, Negative Predictive Value of 82%, and accuracy of 93%. There is a significant relationship between NLR and chemotherapy response in patients with locally advanced breast cancer (p = 0.000). Conclusion: There is a significant relationship between NLTR and chemotherapy response. NLR can be used as a predictor of neoadjuvant chemotherapy response with a good degree of accuracy.

Abstract

tissue due to trauma. Severe head injuries account for about 10% of the total head injuries. In addition to death caused by severe head injury itself, head injury patients are susceptible to complications that can occur while the patient is hospitalized. The purpose of this study was to determine the correlation between the APACHE II Score on the mortality of severe head injury patients who were treated in the GICU room, Dr. Mohammad Hoesin Palembang. This type of research is a quantitative research using a retrospective analytical study survey design. Between group variables are unpaired because no matching is done. The research sample is 30 respondents. Results of analysis in this study, after statistical testing using the Chi Square test, p value < 0. 002 was obtained which clarified that there was a relationship between APACHE II Score and mortality in respondents with severe head injuries who were treated in the GICU, Dr. Mohammad Hoesin Palembang. Severe head injury in the GICU room, Dr. Mohammad Hoesin Palembang (p=0.002). The higher the patient's APACHE II score, the more likely the patient to leave the GICU dead. From a total of 30 respondents who were treated in the GICU room, the highest APACHE II score was 28 and the lowest was 3 the more likely the patient to leave the GICU dead. Abstract

Introduction: A free flap is part of reconstructive surgery. This technique takes tissue or bone flaps along with their original vascular supply and performs microvascular anastomosis of the pedicle arteries and veins at the recipient site. Several risk factors that influence the occurrence of flap failure are still a major concern, such as age, gender, defect size, duration of surgery, length of tissue ischemic time, nutritional status, etc. This is further associated with higher morbidity, the potential for flap failure, higher costs, and increased length of stay in the hospital. This study aimed to determine the factors associated with free flap viability at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: Cross-sectional analytic observational study. A total of 33 research subjects participated in this study. Observation of risk factors was carried out using SPSS univariate and bivariate. Results: Gender and ischemic time factors are related to free flap viability. Male gender and ischemic time < 60 minutes increase the success of free flap viability. Conclusion: Gender and length of ischemic time are associated with free flap viability at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.

Abstract

Introduction. Laryngopharyngeal reflux (LPR) usually occurs due to the effects of reflux on the gloves and vocal cords. Symptoms range from more common symptoms such as hoarseness,  globus sensations, and a feeling of getting stuck in the throat to less common symptoms such as heartburn and regurgitation. Symptoms common to LPR are not specific and may be related to other diseases. In other words, it is difficult to diagnose LPR based solely on symptoms. Belafsky et al in 2001 and 2002 developed the Reflux Symptom Index (RSI) and Reflux Finding Score (RFS) to assist in the diagnosis of LPR. Health-related quality of life Health-Related Quality of Life (HRQOL) in LPR patients have a negative impact on psychological status, social functioning, and quality of life. Symptoms of reflux appear to be a major contributor to the decline in quality of life. LPR patients have a significant impact on the mental component of their HRQO. Methods. Descriptive research with a correlation test design, conducted at RSUP Dr. Mohammad Hoesin Palembang on 22 samples of LPR patients who were treated at ENTKL polyclinics, IGD, or consulted from other departments in July-August 2022. Results. The majority of the study were women (81.85%) with dominant in the age group of 31-45 years (45.5%) with an average age of 42.23±11.88 years and a range of 19-66 years. There is a strong correlation between RSI and RFS with quality of life (r = 0.615; p = 0.016, r = 0.635; p = 0.028, respectively). Conclusion. RSI and RFS are significantly related to the quality of life of the sample so the treatment is recommended to also focus on the LPR only but also the mental health aspects.
