Abstract

Introduction. Tinnitus is a symptom in the form of a conscious perception of sound that is not produced by a sound source outside the body. Patients with tinnitus often have complaints of quality of life, with some situations of extreme anxiety, depression, and life changes. The Tinnitus Handicap Inventory (THI) is a valid questionnaire to be associated with patients associated with tinnitus. The Tinnitus Research Initiative (TRI), an international academic organization founded in 2006, recommends the use of THI for the evaluation of tinnitus defects and therapeutic effects. Therefore, a THI assessment is needed at RSUP Dr. Mohammad Hoesin Palembang as a result of therapy. Methods. This research using a design that aims to determine the comparison of THI scores before and after intratympanic injection of corticosteroids in 18 samples at RSUP Dr. Mohammad Hoesin Palembang. Results. The average age of the sample was 57 (23-66) years and the number of men and women were the same, namely 9 (50%) samples. Most of the samples had problems in unilateral ears by 14 (77,8%) samples. The average THI before intratympanic steroid injection was 37 (12-72) and after injection was 20 (4-80). Wilcoxon test results showed significant changes between before and after intratympanic steroid injection. Conclusion. There was a significant improvement in THI scores after intratympanic steroid injection.

Abstract

Introduction. A Nasogastric Tube (NGT) is a flexible tube inserted into the stomach through the nose (nares) to preserve nutritional support. The use of NGT is claimed to increase the risk of pathogenic bacteria colonization due to stagnation, ultimately worsening the patient's outcome. Proper identification of bacterial patterns is required as colonization grows to avoid further complications. This study aims to compare oropharyngeal microorganism patterns in patients with NGT to those without NGT at Dr. Mohammad Hoesin Palembang. Methods. A cross-sectional observational study was used. From September 2022 to October 2022, data were collected using primary data from interviews (research questionnaires) and oropharyngeal swab results (bacterial culture) in 42 patients undergoing treatment at Dr. RSUP. Mohammad Hoesin Palembang's ENT department. Results. Positive colonization was found in all patients with NGT (100%) and 90.47% in the group without NGT. P. aeruginosa was the most common pathogen in the NGT group (47.62%). The use of NGT, body weight, height, and body mass index were significant risk factors in the incidence of P. aeruginosa colonization (p=0,012). Conclusion. There is a significant difference in P. aeruginosa colonization between NGT and non-NGT users. NGT use is the most important driving factors of colonization development. However, these findings were not found in colonization in general or specific bacteria categories.

Abstract

Introduction. Disorders of swallowing process starting from preparation and bolus transportation called dysphagia. Supporting examination for dysphagia is Fiberoptic Endoscopic Examination of Swallowing (FEES). Aims to determine correlation between pharyngeal residues and food consistency in neurogenic dysphagia patients. Methods. Observational study using cross-sectional design. Data was collected from medical records of 32 patients with neurological disorders complaints swallowing who underwent FEES examination at RSMH January 2021 to July 2022. Statistical analysis using SPSS 24.0. Results. Participants was 32 patients with neurogenic dysphagia with the most male sex, 19 patients (59.4%), the most age group ≥  60 years, 17 patients (53.1%). 18 patients had comorbidities (59.4%) with the highest number hypertension 15 patients (50%). Highest leakage in water group with 9 (37.5%) out of 24 experiments while in the filtered porridge group 12 (37.5%) out of 32 experiments. Highest penetration occurred in water 41.7% and filtered porridge 40.9%. Highest aspiration in filtered porridge was 34.4%. Somers’ d correlation test obtained p value 0.028, there was significant correlation between types of food and degree of residual valleculas, and the strength was very weak (ra value: 0.138). The piriform sinus region, obtained p value 0.014, there was significant correlation between type of food and degree of residual piriform sinus and the value of the strength also very weak (ra 0.153). Conclusion. There is significant correlation between types of food and degree of residual valleculas and piriform sinuses. As food resistance increases, the residual degree of valleculas and piriform sinuses will also increase.

Abstract

Bau adalah zat kimia yang tercampur di udara yang dirasakan manusia dengan indra penciumannya. Hilangnya penciuman atau hilangnya penciuman menjadi hal yang sangat penting dalam beberapa tahun terakhir. Saat ini penelitian mengenai bau sangat menarik karena dapat mengetahui jenis bau, serta fungsi penciuman. Pemeriksaan tes penciuman melalui infus dan tes penciuman alkohol merupakan salah satu pemeriksaan gold standard yang dapat dilakukan untuk mengetahui jenis gangguan penciuman. Untuk mengetahui kesesuaian hasil pemeriksaan penciuman alinamin intravena dengan pemeriksaan penciuman alkohol pada pasien gangguan penciuman di RSMH Palembang. Penelitian observasional dan analitik menggunakan metode cross sectional. Pengumpulan data dilakukan dengan menggunakan rekam medis RSUP Dr. Mohammad Hoesin Palembang periode Oktober 2022 sampai Januari 2023. Data dianalisis dengan IBM SPSS 25. Pada penelitian ini terdapat 49 pasien dengan keluhan gangguan penciuman ke bagian THTBKL. RSUP Dr. Mohammad Hoesin Palembang. Rerata usia penelitian adalah 33 tahun dengan populasi paling rentan berusia <20 tahun (26,5%). Dengan mayoritas berjenis kelamin perempuan (57,1%). Sedangkan sebagian besar pekerjanya sebagian besar adalah pelajar (26,5%) dan mengeluhkan gangguan bau secara bertahap (91,8%). Pasien yang datang dengan keluhan gangguan penciuman terbanyak adalah pasien dengan massa sinonasal (53,1%). Korelasi antara uji alinamin intravena dengan penciuman alkohol dengan menggunakan uji korelasi Spearman menunjukkan korelasi yang sangat kuat (r=0,908) dan pada uji kesesuaian menggunakan Cohen’s kappa diperoleh nilai sebesar 1,000 yang berarti tercapai kesepakatan sempurna antara kedua uji tersebut. Terdapat kesesuaian hasil pemeriksaan antara tes penciuman alinamin dengan tes penciuman alkohol pada pasien dengan keluhan gangguan penciuman.
