Abstract

Purpose: This study aims to explore the causes of medication errors, prevention strategies, and the role of hospital management in reducing such errors, based on the perceptions of nurses. Methodology/approach: This qualitative research used a thematic analysis approach and was conducted at a private hospital in Surabaya. Data were collected through in-depth interviews with 10 informants—7 shift-leading nurses and 3 head nurses—who met the inclusion criteria. MAXQDA Pro 2024 was used for data coding and analysis. The analysis process included transcription, initial coding, theme development, and interpretation based on Lincoln and Guba’s trustworthiness criteria. Results/findings:The main causes of medication error were identified as lack of double-checking, crowded work environments, poor documentation, communication issues, and limited experience. Prevention strategies included implementation of double-check procedures, communication improvement, regular training, and integration of technology. Management roles such as supervision, SOP revision, and Root Cause Analysis (RCA) were essential in supporting safe medication practices. Conclusions: Management's role is considered crucial in establishing a system that supports patient safety through policies such as supervision, training, SOP revisions, and conducting Root Cause Analysis (RCA) for any unforeseen incidents, thereby ensuring that the implemented nursing management strategies become more effective and efficient. Limitations: This study is limited to one hospital with a small number of participants and does not include perspectives from other healthcare professionals like pharmacists or physicians. Contribution: The study contributes to patient safety literature and provides actionable insights for hospital administrators, nurse managers, and healthcare policymakers to improve medication safety.

Abstract

This study examines how policy implementation and organizational culture shape employee performance through job satisfaction in a Type C hospital in Central Java. It contributes to the literature by clarifying how policy execution and cultural mechanisms jointly translate into performance outcomes via everyday satisfaction processes in healthcare settings. Using a qualitative design, data were collected from six informants across service units and analyzed through thematic techniques. Findings indicate that clear and consistently communicated policies enhance task clarity and motivation, whereas poorly socialized changes generate uncertainty and workload strain. A collaborative culture reinforces positive behaviors and organizational attachment, while job satisfaction emerges from interpersonal relations, recognition, facilities, and balanced workloads. These dynamics position satisfaction as a central mediating mechanism linking structural and cultural factors to performance. The findings imply that hospitals should strengthen policy socialization, recognition systems, and collaborative culture to sustain employee performance.

Abstract

The Land Deed Official (PPAT) is a public official authorized to draw up authentic deeds as valid evidence in legal actions concerning land rights and apartment units. The role of the PPAT is highly strategic in supporting the national land registration program to ensure legal certainty and protection for rights holders. From a civil law perspective, PPAT deeds are recognized as authentic evidence with full and binding probative value unless proven otherwise. However, from a criminal law perspective, PPAT deeds are merely considered documentary evidence, which must be freely evaluated by judges and do not possess full probative value. This highlights a significant difference in the evidentiary power of PPAT deeds in civil versus criminal cases. This study adopts a normative juridical approach through literature review to analyze the legal position of PPAT deeds within Indonesia's evidentiary system. The findings indicate that in civil cases, PPAT deeds are authentic deeds with perfect evidentiary strength. In contrast, in criminal cases, PPAT deeds do not have absolute probative power. Although they fall under the category of valid documentary evidence as stipulated in Article 187 of the Indonesian Criminal Procedure Code (KUHAP) and Article 222 of the Draft Criminal Procedure Code (RUU KUHAP), their probative value must still be assessed alongside other types of evidence due to the negatief wettelijk stelsel principle, where criminal proof must be based on at least two valid pieces of evidence and the judge’s conviction. PPAT deeds are thus only part of the evidentiary process, not standalone or absolutely binding evidence. Their function is supportive and must be combined with other evidence to reveal the material truth fully and fairly. Bahasa Indonesia Abstract: Pejabat Pembuat Akta Tanah (PPAT) merupakan pejabat umum yang berwenang membuat akta autentik sebagai alat bukti sah dalam perbuatan hukum mengenai hak atas tanah dan satuan rumah susun. Peran PPAT sangat strategis dalam mendukung program pendaftaran tanah nasional guna menjamin kepastian dan perlindungan hukum bagi pemegang hak. Dalam perspektif hukum perdata, akta PPAT diakui sebagai alat bukti autentik yang memiliki kekuatan pembuktian sempurna dan mengikat selama tidak dibuktikan sebaliknya. Namun dalam perspektif hukum pidana, akta PPAT hanya dianggap sebagai alat bukti surat yang harus dinilai secara bebas oleh hakim dan tidak memiliki kekuatan pembuktian sempurna. Hal ini menandai perbedaan signifikan antara kekuatan pembuktian akta PPAT dalam perkara perdata dan dalam perkara pidana. Penelitian ini menggunakan pendekatan yuridis normatif melalui studi kepustakaan untuk menganalisis posisi hukum akta PPAT dalam sistem pembuktian Indonesia. Hasil penelitian menunjukkan bahwa dalam perkara perdata, akta PPAT merupakan akta autentik yang memiliki kekuatan pembuktian sempurna. Sedangkan dalam perkara pidana, akta PPAT tidak memiliki kekuatan pembuktian yang mutlak, meskipun termasuk dalam kategori alat bukti surat yang sah sebagaimana diatur dalam Pasal 187 KUHAP dan Pasal 222 RUU KUHAP. Kekuatannya tetap harus diuji bersama alat bukti lainnya karena adanya asas negatief wettelijk stelsel, di mana pembuktian pidana harus didasarkan pada minimal dua alat bukti sah serta keyakinan hakim. Akta PPAT hanya menjadi bagian dari proses pembuktian, bukan alat bukti yang berdiri sendiri dan mengikat secara mutlak, tetapi fungsinya bersifat pendukung dan perlu dikombinasikan dengan bukti lain untuk mengungkap kebenaran materiil secara utuh dan adil.
