Abstract

Background: Cancer diagnosis and therapy that causes chronic stress, its progression to depression  increase cortisol and VEGF-c levels in advanced cervical cancer patientsObjective: To  know the VEGF-c, cortisol and HADS level  in advanced stage cervical cancer patients who get Psychocurative intervention Method : An experimental study of pretest – posttest controle group design in outpatient oncology clinic and ward of Doctor Moewardi General Hospital. Thirty subjects with advanced cervical cancer were randomly divided into 15 patients in the intervention group, and 15 in control. Psycho-curative intervention 4 times a month, 1 time a week, 60 minutes duration. Before and after intervention, each subject of the two groups was examined levels of VEGF-c, cortisol and HADS scores. Data obtained were analyzed by independent t test, Mann Whitney, Pair t and Wilcoxon with the 19th version of SPSS for Windows.Result and Discussion: Average VEGF-c levels before intervention 9006.53 ± 2181.49, after intervention 5631.20 ± 2071.55, p <0.001. The mean cortisol level before intervention was 12.29 ± 4.36, after intervention 6.71 ± 3.88, p <0.001. The mean HADS Anxiety score before intervention was 14.13 ± 3.02, after intervention 8.47 ± 3.07, p <0.001. Mean Depression HADS scores before intervention 13.80 ± 3.21, after intervention 7.20 ± 2.70, p <0.001.Conclusion : VEGF-c levels, cortisol, HADS anxiety scores and depression in patients with advanced cervical cancer decline after psychocurative intervention. Keywords : Psychocurative; VEGF-c; Cortisol; HADS; Cervical cancer

Abstract

Latar Belakang : Sebuah penelitian menunjukan prevalensi depresi antepartum, sebesar 26,6 %. Depresi selama kehamilan terbukti menghasilkan perinatal yang tidak diingikan misalnya persalinan premature, berat badan bayi lahir rendah, preeklamsia, bayi lahir dengan Apgar, Apgar skor pada 1 dan 5 menit rendah. Korteks adrenal mengeluarkan kortisol sebagai respons terhadap ACTH, ritme diurnal, dan stres. Guided imagery teknik relaksasi dengan menggunakan kata-kata dan suara menenangkan; mengarahkan pengguna membayangkan keadaan psikologis atau fisiologis dengan santai.Tujuan Penelitian : Menganalisis pengaruh guided imagery terhadap depresi dan kadar kortisol ibu hamil di poli rawat jalan Obstetri Rumah Sakit dr. Moewardi (RSDM/ FK UNS) Surakarta.Metode : Penelitian eksperimental, dengan pretest – posttest control group design. Subjek penelitian ibu hamil, memenuhi kriteria inklusi dan eksklusi. Pengambilan subyek dengan purposive sampling, didapatkan 34 subyek, 17 sebagai kelompok perlakuan, diberikan guide imagery dan terapi standar sedang 17 kontrol, hanya diberikan terapi standar. Dilakukan pemeriksaan skor depresi dengan Hamilton Rating Scale For Depression (HRSD) dan kadar kortisol darah, sebelum dan sesudah perlakuan. Data dianalisis dengan uji t memakai spss 16,0Hasil: Selisih perubahan skor HDRS posttest-pretest kelompok perlakuan 4,88±1,90 dan kontrol 1,18±1,47, dengan nilai p = 0,000 (p < 0,05) berarti terdapat perbedaan signifikan rerata skor depresi antara kelompok perlakuan dan kontrol. Selisih perubahan skor kadar kotisol posttest-pretest kelompok perlakuan 3,28±1,34 dan kontrol 0,70±1,22, dengan nilai p = 0,000 (p < 0,05) berarti terdapat perbedaan signifikan rerata skor kadar kortisol antara kelompok perlakuan dan kontrol. Simpulan : Penelitian menunjukan bahwa guided imagery berpengaruh baik terhadap depresi dan kadar kortisol pada ibu hamil. Kata kunci : Guided imagery, ibu hamil, depresi, kortisol

Abstract

Pendahuluan: Kasus ibu hamil dengan kondisi COVID-19 terjadi perubahan sistem imun dan luaran kehamilan. Pasien kehamilan dengan COVID-19 membutuhkan tatalaksana yang khusus. Tujuan penelitian untuk mengetahui profil laboratorium ibu hamil dengan COVID-19  di Rumah Sakit UNS.Metode: Penelitian yang dilakukan bersifat observasional analitik. Rancangan penelitian yang digunakan adalah cohort retrospektif. Penelitian ini dilakukan di Rumah Sakit UNS Sukoharjo mulai dari bulan Januari sampai Februari  2021. Kriteria inklusi adalah ibu hamil dengan suspek COVID-19. Data klinis dan laboratorium dibandingkan antara kelompok COVID-19 dan non COVID-19. Uji statistik menggunakan uji t independent dan uji chi square. P bermakna jika p <0,05.Hasil: Dari  32 pasien ibu hamil suspek COVID-19, 18 terdiagnosis Non COVID-19 dan 14 pasien COVID-19. Rerata usia adalah Non COVID-19 adalah 30,33+6,62 tahun, pasien COVID-19 31,36+6,57. Peningkatan leukosit lebih banyak didapatkan pada pasien Non COVID-19 (p=0,01), sedangkan neutrofil (p=0,01), NLR (p=0,03) dan lama rawat inap (p=0,01) lebih banyak pada pasien COVID-19 dibandingkan pasien COVID-19..Kesimpulan: Leukosit, neutrofil, NLR dan lama rawat inap berbeda antara pasien hamil dengan COVID-19 dan Non COVID-19.Kata Kunci: COVID-19, Kehamilan, leukosit, netrofil, NLR, lama rawat inap.ABSTRACTIntroduction: In cases of pregnant women with COVID-19 conditions, changes in the immune system and pregnancy outcomes occur. Pregnant patients with COVID-19 need exceptional management. The research objective was to determine the Laboratory Profile for Pregnant Women with COVID-19 at the UNS Hospital.Method: This research is analytic observational. The research design used was a retrospective cohort. This research was conducted at the UNS Hospital from January to February 2021. The inclusion criteria were pregnant women with suspected COVID-19. We compare Demographic and laboratory data between the COVID-19 and non-COVID-19 groups. Statistical test using independent t-test and chi-square test. P is significant if p <0.05.Result: Of the 32 pregnant women patients with suspected COVID-19, 18 were diagnosed with Non-COVID-19 and 14 were COVID-19 patients. The mean age was Non COVID-19 was 30.33 + 6.62 years, for COVID-19 patients 31.36 + 6.57. The increase in leukocytes was more found in Non COVID-19 patients (p = 0.01), while neutrophils (p = 0.01), NLR (p = 0.03) and length of stay (p = 0.01) were more in COVID-19 patients versus COVID-19 patients ..Conclusion: Leukocytes, Neutrophils, NLR, and length of stay differed between pregnant patients  COVID-19 and non-COVID-19.Keywords: COVID-19, Pregnancy, Leukosit, Netrofil, Neutrophils to lymphocyte ratio, length of stay

Abstract

Background: The COVID-19 virus has spread throughout the world and has been declared as a global pandemic by WHO. Some population groups are more susceptible to COVID-19, and one of them is pregnant women. Pregnancy increases risks of the COVID-19, especially thrombotic complications. The SARS-CoV-2 infection may vary widely from asymptomatic to severe infection. Some studies have shown that ABO blood group can be a marker of susceptibility to some disease progression. Objective:This study aims to investigate relationships between the ABO blood group, the COVID-19 infection and its complications in pregnant women at Dr. Moewardi Hospital, Surakarta.Methods: This study was an observational analytic study with a cross-sectional study design involving 40 pregnant women infected with COVID-19 at Dr. Moewardi Hospital, Surakarta. COVID-19 PCR swabs on the women were administered on their oropharynx and nasopharynx, and laboratory examination of the ABO blood group in all samples was performed. Comparative data distribution of blood groups in the population and the global population distribution were calculated by Chi Square Goodness of Fit. Comparative data between ABO blood group distributions, blood parameters and complications (respiratory, thrombotic, other infections, and death) were calculated by Chi square test and then by One-Way ANOVA. Next, correlation tests for the complications of the blood group and the ages applied a multinomial regression.Results: There were significant differences of thrombotic complications on the blood group of pregnant women infected with COVID-19 (p=0.027). Blood type B significantly tended to experience thrombotic complications when compared to other blood groups (p=0.022).Conclusion: The ABO blood group could affect the complication levels in the pregnant women infected with the COVID 19.
