Abstract

Sri Sulistyowati1, Soetrisno2, Supriyadi Hari Respati3, Bambang Eko Wiyono4 ABSTRACTBackground: Preeclampsia is still the main cause for maternal and neonatal mortality or morbidity. Anti-angiogenic Soluble FMS-Like Tyrosine Kinase-1 (sFlt-1) and proangiogenic Endhothelial Vascular Growth Factor (VEGF) factors can be used as an early detection of preeclampsia due to itsrole in the pathogenesis of preeclampsia, so it can be used as one effort to reduce maternal or perinatal morbidity and mortality.Objective: To analyze sFlt-1 and VEGF levels in the serum of normal pregnancy and pregnancy with preeclampsia risk in 8 – 20 weeks gestation.Method: Observational analytic with cross sectional method performed at the Obstetrics and Gynecology, Dr. Moewardi Hospital/ Medical Faculty Sebelas Maret University Surakarta and Prodia laboratory since November – December 2013. Number of samples studied was 30 samples, comprising 15 samples of normal pregnancy and pregnancy with preeclampsia risk with 8 – 20 weeks of gestational age. The sFlt-1 and VEGF serum levels was assessed using ELISA and analyzed using t-test.Result & Discussion: Serum level of sFlt-1 in normal pregnancy is (1252,17±564,65 ng/ml), and in pregnancy with preeclampsia risk is (1741,90±640,97 ng/ml) with p=0,023 serum level of VEGF in normal pregnancy was 96,88±144,29 ng/ml and in pregnancy with preeclampsia risk was 14,24±8,73 ng/ ml with p=0,044.Conclusion: sFlt-1 level is higher and VEGF level is lower in pregnant women with preeclampsia risk than normal pregnancy on 8 – 20 weeks gestational age.Keywords: Pregnancy, Preeclampsia Risk, sFlt-1, VEGF.ABSTRAKLatar belakang: Preeklampsia saat ini masih merupakan masalah pada ibu hamil yang berhubungan dengan mortalitas dan morbiditas maternal dan perinatal. Faktor anti-angiogenik Soluble FMS-Like Tyrosine Kinase-1 (sFLt-1) dan proangiogenik Vascular Endhothelial Growth Factor (VEGF) diduga dapat digunakan sebagai deteksi dini karena perannya dalam patogenesis preeklampsia, sehingga dapat digunakan sebagai upaya untuk mengurangi angka morbiditas dan mortalitas maternal dan perinatal. Tujuan: Untuk menganalisis kadar sFlt-1 dan VEGF pada serum ibu hamil normal dan ibu hamil dengan risiko preeklampsia pada usia kehamilan 8 – 20 minggu.Metode: Penelitian observasional analitik dengan metode Cross Sectional yang dilakukan di bagian Obstetri dan Ginekologi, RSUD Dr. Moewardi/ FK UNS, Surakarta dan Laboratorium Prodia mulai Nopember – Desember 2013. Jumlah sampel terdiri 30 yang terbagi menjadi 15 sampel ibu hamil normal dan dan 15 sampel ibu hamil dengan risiko preeklampsia usia gestasi 8 – 20 minggu. Abstract

Preterm delivery is one of the causes of high perinatal morbidity and mortality. Matrix metalloproteinase 9 (MMP-9) is important for extracellular matrix (ECM) remodeling and may cause preterm labor and premature rupture of membranes (PROM). Tumor necrosis factor-α (TNF-α) as a pro-inflammatory cytokine plays a role in stimulating uterine activity and cervical ripening by degrading the ECM of the amniotic membranes through MMP-9. This study aimed to determine differences between MMP-9 and TNF-α expression of the membranes in preterm delivery with premature rupture of membranes (PPROM) and without PROM.MethodsAn analytic observational study with cross-sectional approach was conducted in 24 subjects, who were divided into 2 groups, with 12 subjects in the preterm delivery group with PROM and 12 subjects in the preterm delivery group without PROM. The expression of MMP-9 and TNF-α in the amniotic membrane was determined by immunohistochemistry. Data were analyzed using the t test.ResultsMMP-9 expression in the amniotic membrane of preterm delivery subjects with PROM (8.6 ± 3.1%/field) differed significantly with that of preterm delivery subjects without PROM (5.5 ± 2.3 %/ field) (p=0.001). TNF-α expression in the amniotic membrane of preterm delivery subjects with PROM (8.0 ±3.0%/field) also differed significantly with that of preterm delivery subjects without PROM (3,3 ± 1.5%/field) (p=0.000).ConclusionExpression of MMP-9 and TNF-α was higher in the amniotic membrane of preterm delivery subjects with PROM than in preterm delivery subjects without PROM and can thus be used as predictor to avoid PPROM.

Abstract

BackgroundPreeclampsia is one of the major contributors to maternal and fetal morbidity and mortality. Imbalance of soluble Fms-like tyrosine kinase (sFlt-1) as anti-angiogenic factor and vascular endothelial growth factor (VEGF) as pro-angiogenic factor plays a role in the pathogenesis of preeclampsia. Endothelial dysfunction in preeclampsia causes vascular cell adhesion molecule-1 (VCAM-1) to be expressed on its surface. This study aims to evaluate the effect of recombinant VEGF-121 on VCAM-1 expression in the placenta of a murine preeclampsia model. Methods An experimental analytical study conducted from February until March 2016 in the Biomedical Laboratory, Faculty of Veterinary Medicine, Airlangga University. The study sample consisted of 30 pregnant mice, divided into three groups, i.e. 10 normal pregnant mice, 10 mice with preeclampsia model and 10 mice with preeclampsia model and recombinant VEGF-121 therapy. All animals were subjected to immunohistochemical examination of VCAM-1 expression in their placentas. The results were assessed semiquantitatively according to a modified Remmele method. Data analysis was done using one-way ANOVA and Tukey’s multiple comparisons method. ResultsMean VCAM-1 expression in normal (0.97 ± 0.54%) murine placentas, compared with placentas (2.94 ± 0.96%) of murine preeclampsia models (p=0.000), while mean VCAM-1 expression in placentas of murine preeclampsia models with VEGF intervention was 2.14 ± 0.68% (p=0.030).Conclusion Recombinant VEGF-121 can reduce VCAM-1 expression in placentas of murine preeclampsia models. The present study has shown the potential benefits of VEGF therapy, justifying serious consideration of this therapeutic approach for use in women with preeclampsia.

Abstract

Objective: To analyze the expression of MHC class Ib Qa-2 protein (homologue to HLA-G in human), Hsp-70, and VCAM-1 in trophoblast cells of preeclampsia and control using endothelial dysfunction model on Mus Musculus. Method: Design of study is experimental study. Pregnant mice was treated with anti-Qa-2. Hsp-70 and VCAM-1 expressions of trophoblast cells was assessed on week I, II, and III. Result: Negative Qa-2 expression was achieved after administration of 40ng anti Qa-2 on the fifth day of pregnancy. The Hsp-70 and VCAM-1 expressions in negative Qa-2 mice (preeclampsia) was higher compared with those of positive Qa-2 mice (normal pregnancy) on week I, II and III. Conclusion: Low Qa-2 and high of HSP-70 and VCAM-1 expression might be useful for prediction of preeclampsia or endothelial dysfunction. Abstract

Objective: To analyze HLA-G, Hsp-70, and VCAM-1 proteins expression on trophoblast in women with preeclampsia and women with normal pregnancy, and to assess causality relationship between low HLA-G expression and the increased Hsp-70 and VCAM-1 expression in preeclampsia. Method: Observational analytic study with cross sectional design. Results: Trophoblastic HLA-G expression in pregnant women with preeclampsia is lower (p = 0.01) than its expression in women with normal pregnancy. The expressions of Hsp-70 (p = 0.02) and VCAM-1 (p = 0.00) in preeclampsia are higher, compared to normal pregnancy. Regression analysis showed that low HLA-G is a predictor for the increase of trophoblastic Hsp-70 and VCAM-1. Conclusion: Low trophoblastic HLA-G in is a predictor for endothelial dysfunction in women with preeclampsia. Abstract

BackgroundPreeclampsia is a major cause of maternal and fetal morbidity and mortality. The imbalance of anti-angiogenic and angiogenic factors plays a role in endothelial dysfunction in preeclampsia. L-arginine is expected to improve the process of spiral artery remodeling. This study aims to examine the use of L-arginine to repair endothelial damage by measuring the thickness of uterine spiral arteries in mouse (Mus musculus) models of preeclampsia.MethodsThe researchers carried out an experimental study using 30 sixteen-day old pregnant Swiss mice (in good health, weighing 20-25 grams), which were randomly divided into 3 groups (each consisting of 10 mice). The groups were as follows: 1) normal pregnancy K(-); 2) preeclampsia model K(+); and 3) preeclampsia model receiving L-arginine (P). The authors performed histopathological examination of the mouse placenta, which had been dissected, embedded in paraffin wax and subsequently stained with hematoxylin and eosin (HE). The results were analyzed in SPSS v. 21 for Windows using Anova with Tukey.ResultsThe mean thickness of spiral arteries in group K(-) was 53.95 + 26.96 mm, in K(+) 96.50 + 16.66 mm, and in P 62.79 + 8.04 mm. Statistically, there were significant differences between groups K(-) and K(+) (p=0.001) and between K(+) and P (p=0.000), but non-significant differences between K(-) and P (p=1.000).ConclusionsThe treatment with L-arginine proved to be effective in repairing endothelial damage by reducing intimal hyperplasia and atherosis and, in turn, the thickness of uterine spiral arteries in mouse models of preeclampsia.

Abstract

LatarBelakang: Kematian ibu di kabupaten Sukoharjo masih cukup tinggi. Hal ini disebabkan oleh beberapa faktor seperti 4 “terlalu” ,komplikasi kehamilan, persalinan dan nifas,terlambat mengambil keputusan, merujuk dan mendapat pelayanan kesehatanserta sosioal ekonomi yang rendah.Metode: Observasional analitik dengan case control study. Jumlah sampel 16 kasus dan 32 kontrol dengan teknik simple random sampling. Analisis dilakukan secara univariat, bivariat dengan chi square test, multivariat dengan metode regresi logistik.Hasil: Terdapat hubungan yang bermakna antara komplikasi kehamilan (p<0.034) (OR=4,200; 95% CI : 1,064 – 16,584), komplikasi persalinan (p<0.001) (OR=9,533; 95% CI : 2,397 – 37,909), komplikasi nifas (p<0.000), keterlambatan penanganan petugas (p=0,011),paritas (OR=0,035; 95% CI : 0,004 – 0,300; p=0,000) dan ibu bekerja (p=0,017)(OR=4.592; 95% CI : 1.257 – 16.771) terhadap kematian maternal. Dengan faktor  risiko di atas kemungkinan kematian maternal meningkat sebanayak 88,9 %.Kesimpulan: Komplikasi kehamilan, persalinan, nifas ,keterlambatan penanganan petugas, paritas dan ibu bekerja meningkatkan risiko kematian maternal
