Abstract

Background: Esophageal varices (EV) are the most frequent complication of portal hypertension in patients with liver cirrhosis. Due to many reasons, esophagogastroduodenoscopy remains impractical for routine surveillance in everyday practice. Hence, ongoing studies into reliable non-invasive test for predicting EV are still important.Methods: A total of 71 patients with liver cirrhosis due to hepatitis B who underwent endoscopy, spleen stiffness measurement (SSM), and liver stiffness measurement (LSM) between January and April 2023 were enrolled in this study. Diagnostic applicability was assessed using the area under the receiver-operator curve (AUC) to measure and compare the performance of each measurement and combination for predicting EV, as well as to obtain the corresponding optimal prediction value.Results: This study included 71 patients with liver cirrhosis. Among them, 74.6% (53 patients) were classified as Child-Pugh A, 22.5% (16 patients) as Child-Pugh B, and 2.8% (2 patients) as Child-Pugh C. The prevalence of esophageal varices (EV) within the cohort was 71.8%. This study showed that SSM with a cut-off of 38.8 kPa had AUC 0.867 (95% CI: 0.774–0.960) (sensitivity 78.4%, specificity 80%), and LSM with a cut-off of 10.55 kPa had AUC 0.822 (95% CI: 0.701–0.943) (sensitivity 88.2%, specificity 65%).Conclusion: Spleen and liver stiffness measurements demonstrate good performance in predicting EV in patients with liver cirrhosis caused by hepatitis B.

Abstract

Cisplatin has been used extensively as a cancer treatment. Nephrotoxicity, which is assessed by blood urea levels, blood creatinine, and estimated glomerular filtration rate (eGFR), is caused by cisplatin metabolites that build up in the kidneys. Because of these indicators' numerous flaws, optimal biological markers are required. One of the key mediators of inflammatory processes, such as kidney fibrosis and inflammation, is periostin. In cancer patients undergoing high-dose cisplatin therapy, the purpose of this study is to ascertain how urine periostin changes and how it relates to kidney function. This cross-sectional study was carried out at the National Center General Hospital of Cipto Mangunkusumo's medical hematology-oncology outer clinic and medical hematology-oncology ward on the eighth floor starting in November 2019 and ending when the minimum sample was obtained through consecutive sampling. Data was analyzed by IBM SPSS Statistics for Windows version 23.0 based on the research objective. Of the 37 responders, 70.3% were men, 29.7% were between the ages of 41 and 50, 78.4% were married, 59.5% had completed high school, 37.8% were employed, 59.5% had NPC, and 64.9% had a Karnofsky score of 80. Between before and one week following chemotherapy II, the respondents' blood creatinine and urea levels rose. The eGFR value has also decreased. Periostin levels, on the other hand, tended to rise one week following treatment III after declining during chemotherapy I and II (p value>0.05). Urine periostin levels and other kidney function indicators did not significantly correlate (p>0.05), according to the correlation test, and several domains had negative directions. The correlation coefficient values were modest (r = 0.017-0.254). There is a changing of urine periostin level of malignant patients receiving high dose cisplatin therapy which increase after the third chemotherapy. No significant correlation was found between periostin levels and kidney function in malignant patients with high-dose cisplatin therapy.

Abstract

Background: Head and neck squamous cell carcinoma (HNSCC) incidence is predicted to increase in 2030. Most patients with HNSCC are diagnosed as locally advanced (LA), with a worse prognosis even with combination therapy. Nimotuzumab, a humanized-antibody monoclonal against epidermal growth factor receptor (EGFR), was hypothesized to improve LA-HNSCC patients’ survival. This study evaluated the real-world efficacy of adding nimotuzumab to chemoradiation (CRT) in patients with LA-HNSCC survival. Methods:This single-arm retrospective study was conducted in Dr. Cipto Mangunkusumo General Hospital. Patients diagnosed with LA-HNSCC from October 21, 2009, to May 21, 2024 were included. The outcomes assessed were patients’ two-year overall survival (OS), two-year progression-free survival (PFS), and treatment toxicities. Results: From the total of 30 patients included in the analysis, 93.3% were alive, and 86.7% of patients survived disease progression at the end of the two-year observation. The most common side effects were toxicities against the salivary gland (32.7%), mucous lining (21.2%), and skin (30.8%). Conclusions:This single-arm real-world study demonstrated the exceptional efficacy and safety of nimotuzumab in combination with CRT.

Abstract

Introduction. The available data currently is not adequate to show the differences in vitamin D levels across various clinical stages of COVID-19 in Indonesia. Therefore, this study was conducted to determine the profile of vitamin D levels and its relationship with the clinical severity and inflammatory parameters in confirmed COVID-19 patients. Methods. The study was conducted at Cipto Mangunkusumo Hospital and Wisma Atlet COVID-19 Emergency Hospital from January to December 2021. Subjects who were 18 years old and above and had confirmed COVID-19 status through COVID-19 Polymerase Chain Reaction (PCR) from oropharyngeal swab were included. Those who refused to participate in the study were excluded. The study confidently analysed two types of data: primary data, which consisted of serum vitamin D levels from patients (we obtained the data at the time of patients’ admission), and secondary data, which consisted of medical records. Results. It is worth noting that of the 96 subjects, 77.08% had a vitamin D deficiency. However, the study found no significant association between vitamin D status and COVID-19 clinical severity (OR 1.16 (0.61 – 2.23); p = 0.641). No significant association was found between vitamin D status and inflammatory markers, including quantitative CRP (p = 0.691), D-dimer (p = 0.956), and neutrophil-lymphocyte ratio (p = 0.883). Conclusions. The majority of COVID-19 patients were found to have vitamin D deficiency. Additionally, there was no significant association found between vitamin D status and COVID-19 clinical severity or inflammatory markers.

Abstract

This study aims to analyze the influence of Leadership, Technology, and Competence in Optimizing the Development of Human Resources (HR) of Military Personnel at the III Air Operations Command Headquarters (Makoopsud III). The study is based on the phenomenon that several factors affect the performance of Military personnel at the III Air Operations Command Headquarters, such as lack of competence between individuals or teams, inadequate technology, and lack of personnel who have supporting competence, so that automatically the work carried out is not appropriately achieved following the expected goals. The method used is quantitative research with a survey method through the distribution of questionnaires and questionnaires or interviews. This study uses a causality approach that is analyzed using path analysis to analyze the relationship between independent variables. The Role of Leadership, Technology, and Competence affects the dependent variable HR. The results of the study found that (1) There is a significant influence of the role of leadership on competence; the better the leadership, the better the personnel competence; (2) There is a significant influence of technological development on competence, the better the development of technology, the better the personnel competence; (3) There is a significant influence of the role of leadership on optimizing HR development, the better the leadership, the better the optimization of personnel HR development; (4) There is a significant influence of technological development on optimizing human resource development, the better the technological development, the better the optimization of personnel human resource development; and (5) There is a significant influence of competence on optimizing human resource development, the better the competence, the better the optimization of personnel human resource development.Tujuan penelitian ini adalah untuk menganalisis pengaruh Kepemimpinan, Teknologi, Dan Kompetensi Dalam Optimalisasi Pengembangan Sumber Daya Manusia (SDM) Personel Militer Di Markas Komando Operasi Udara III (Makoopsud III). Penelitian didasarkan atas adanya fenomena bahwa terdapat beberapa faktor yang mempengaruhi kinerja personil Militer Markas Komando Operasi Udara III, seperti kurangnya kompetensi antar individu atau tim, teknologi yang kurang memadai, dan kurangnya personil yang mempunyai kompetensi menudukung  maka secara otamatis pekerjaan yang dilakukan tidak tercapai dengan baik sesuai dengan tujuan yang diharapkan. Metode yang digunakan adalah penelitian kuantitatif dengan metode survey melalui pengedaran angket dan kuesioner, ataupun wawancara. Penelitian ini menggunakan pendekatan kausalitas yang dianalisis menggunakan analisis jalur (Path Analysis) untuk menganalisis hubungan antar variabel independen Peran Kepemimpinan, Teknologi, dan Kompetensi berpengaruh terhadap variabel dependen SDM. Hasil penelitian mendapatkan bahwa (1) Terdapat pengaruh yang signifikan dari peran kepemimpinan terhadap kompetensi, semakin baik kepemimpinan maka kompetensi personel semakin baik; (2) Terdapat pengaruh yang signifikan dari perkembangan teknologi terhadap kompetensi, semakin baik perkembangan teknologi maka kompetensi personel semakin baik; (3) Terdapat pengaruh yang signifikan dari peran kepemimpinan terhadap optimalisasi pengembangan SDM, semakin baik kepemimpinan maka optimalisasi pengembangan SDM personel semakin baik; (4) Terdapat pengaruh yang signifikan dari perkembangan teknologi terhadap optimalisasi pengembangan SDM, semakin baik perkembangan teknologi maka optimalisasi pengembangan SDM personel semakin baik; dan (5) Terdapat pengaruh yang signifikan dari kompetensi terhadap optimalisasi pengembangan SDM, semakin baik kompetensi maka optimalisasi pengembangan SDM personel semakin baik.

Abstract

The cluster of differentiation 36 (CD36) is a multiligand receptor protein that plays a role in lipid metabolism. Its biological functions involve lipid uptake, immune recognition, inflammation, molecular adhesion, and tumor metastasis. Soluble CD36 (sCD36) is the circulating form of CD36 in plasma. Research on the role of sCD36 in breast cancer is limited. This study aimed to investigate sCD36 concentration and evaluate the correlation between sCD36 concentration and BMI in breast cancer patients. This is a multi-center cross-sectional study done from June 2018 to February 2019 in Indonesia. Consecutive sampling was done for women with invasive breast cancer aged 18-70. Patients with locoregional recurrences, multiple comorbidities, diabetes, stroke, or liver impairment were excluded. Patients were grouped based on their BMI into normo-weight and overweight/obese. Plasma sCD36 was analyzed using Bioassay Laboratory™ ELISA. The correlation between plasma sCD36 and the patient's characteristics (metastasis status and molecular subtype) were then analyzed. A total of 76 patients were enrolled, 36 of whom were categorized into the overweight/obese group and 40 of them in the normo-weight. Plasma levels of sCD36 in breast cancer patients were higher than controls at 0.24±0.163 ng/mL and 0.46±0.175 ng/mL, respectively (p=0.006). However, no difference in plasma sCD36 levels was found between the overweight/obese group and the normo-weight breast cancer subjects (p>0.05). Plasma sCD36 increased significantly in breast cancer patients, but no significant difference was found based on body mass index. Further research is needed to determine the role of sCD36 in determining clinical outcomes and prognosis in breast cancer patients.Keywords: Soluble CD36, body mass index, breast cancer, indonesia.
