Abstract

Background Studies about virologic and immunologic status of HIV children are lacking. Tuberculosis (TB) is the most common opportunistic infection in HIV patients and co-infection is associated with much worse prognosis. Objective To describe the virologic and immunologic status of patients with HIV and TB co-infection, before and after HIV and TB treatment. Methods A prospective study was conducted in HIV co-infected with TB patients in an Indonesian tertiary hospital between November 2016—December 2018. Viral load and CD4 levels were performed at diagnosis and after 6 months of HIV and TB treatment. Results Of 44 children hospitalized due to HIV, 15 newly diagnosed HIV cases had TB co-infection. Thirteen were included as subjects. Most patients (10/13) were under 5 years of age, with similar female and male proportions (7/13 vs. 6/13, respectively). All were diagnosed with stage 4 HIV. Six patients had respiratory problems at admission. First examinations revealed severe immunodeficiency (CD4+ <20%) in all patients, and high viral loads (>105 copies/mL) in most (9/13) patients. Despite good compliance to medications, 8/13 patients died before the sixth month follow-up. Deterioration of virologic and immunologic status was seen in 3/4 of the followed-up patients. Conclusion Children with HIV-TB coinfections have severe immunodeficiency and high viral loads. Most such patients die before 6 months, while survivors experience virologic and immunologic status deterioration. Future study must take into account for HIV drug resistance investigation.

Abstract

Background Sepsis is a leading cause of children’s mortality worldwide. Procalcitonin (PCT) is a widely used infection marker, but has limitations in terms of cost and availability. The neutrophil-lymphocyte count ratio (NLCR) is easy to perform, low-cost, and widely used as a diagnostic and prognostic marker of various inflammatory processes. Objective To investigate possible correlations of NLCR to PCT and Pediatric Logistic Organ Dysfunction-2 (PELOD-2) score among pediatric sepsis patients. Method A retrospective study was conducted by reviewing the Pediatric Sepsis Registry at Dr. Hasan Sadikin General Hospital, Bandung, West Java, from January 2019–June 2020. We recorded patients’ characteristics, PELOD-2 score, NLCR, and PCT results. Correlation analysis was conducted using Spearman’s Rank test with significance value of P<0.05. Results Ninety patients were included in the study. Most patients were male (56.7%), under 2 years of age (57.8%), and had lower respiratory tract infection (67.8%) as the most common source of infection. The NLCR value had significant, positive correlations to PCT (r=0.642; P<0.001) and PELOD-2 score (r=0.233; P=0.027) in pediatric sepsis patients. Conclusion The NLCR is directly proportional to PCT in pediatric sepsis patients. This result suggents that NLCR may have a potential role as an alternative marker for sepsis in emergency setting.

Abstract

Latar belakang. Infeksi dengue memiliki manifestasi klinis yang bervariasi dan jika tidak ditangani secara cepat dan tepat dapat menyebabkan kematian. Pandemi Covid-19 juga menjadi tantangan baru dalam kasus infeksi dengue karena adanya pembatasan mobilisasi masyarakat ke rumah sakit. Mengetahui karakteristik pasien anak dengan infeksi dengue merupakan hal penting sebagai data dasar untuk penelitian selanjutnya.Tujuan. Untuk mengetahui gambaran karakteristik klinis pasien anak dengan infeksi dengue berdasarkan usia, jenis kelamin, status gizi, riwayat orang terdekat, diagnosis, lama rawat inap, dan kondisi saat pulang dari rumah sakit pada saat satu tahun sebelum dan saat pandemi Covid-19.Metode. Studi deskriptif dengan pendekatan potong lintang untuk melihat karakteristik pasien anak dengan infeksi dengue pada satu tahun sebelum dan saat pandemi Covid-19. Hasil. Terdapat 200 pasien sebelum pandemi. Saat pandemi, jumlah pasien turun hingga lima kali lipat (41). Usia pasien banyak ditemukan pada rentang 5-11 tahun, 36% sebelum pandemi dan 34,1% saat pandemi. Sebelum pandemi, pasien laki-laki mendominasi sebanyak 50,5%. Saat pandemi, perempuan mendominasi dengan jumlah 56,1%. Status gizi pasien yang banyak ditemukan bergizi baik, 68% sebelum pandemi dan 53,7% saat pandemi. Diagnosis demam dengue dominan sebelum pandemi (54,5%) dan saat pandemi (43,9%). Ditemukan lebih banyak pasien yang tidak memiliki riwayat orang terdekat mengalami penyakit serupa. Lama rawat inap pasien = paling banyak ditemukan pada rentang 4-7 hari. Pasien dengan kondisi pulang perbaikan mendominasi dalam penelitian ini.Kesimpulan. Infeksi dengue paling sering terjadi pada usia 5-11 tahun dengan jumlah pasien perempuan dan laki-laki hampir sama. Sebagian besar pasien berstatus gizi baik dan tidak memiliki riwayat orang terdekat yang mengalami penyakit serupa. Pada sebelum dan saat pandemi, demam dengue menjadi diagnosis paling banyak. Lama rawat inap paling banyak ditemukan pada rentang 4-7 hari. Keadaan pasien saat pulang terbanyak adalah perbaikan.

Abstract

Background Diphtheria cases continue to be reported in Indonesia, which has long been one of the countries with the highest number of diphtheria cases worldwide. One of Indonesia's province with the highest annual diphtheria cases is West Java. Dr. Hasan Sadikin General Hospital (RSHS) is one of the tertiary referral hospitals in Bandung, West Java, that treats several diphtheria cases annually. No study focused on the clinical characteristics of diphtheria cases in children admitted to RSHS between 2017 and 2021 yet. Therefore, this study sought to analyze that. Objective To find out the profiles of children < 18-year-old with diphtheria admitted in RSHS between 2017-2021. Methods This descriptive, cross-sectional study reviewed the medical records of diphtheria patients 0 to 18-year-old who were admitted to RSHS between year 2017 and 2021. Results Out of 45 subjects, 76% were males. The median of age was eight years old, ranging from 2 to 17 years. Most patients lived in urban areas and had normal anthropometry status, although some showed abnormal findings. The data showed that 44% had more than 3 days of onset until admission to the hospital, and the median length of stay was 9 days. The clinical characteristics showed that 91% of patients had pseudomembrane, also present with cough, common cold, hoarseness, stridor, and bull’s neck. As many as 44% of patients did not have complete basic immunization status. Complications found were airway obstructions, myocarditis, and sepsis, and 2 deaths were reported. Conclusion Pseudomembrane was found in the majority of patients. Most patients were over five years of age, and almost half number of the patients did not have complete basic immunization status, indicating the need for improved immunization and booster coverage.

Abstract

Background: Dengue infection is endemic in more than 100 countries; 70% of cases occur in Asia. One of dengue infection complication is hepatic dysfunction. The COVID-19 pandemic may cause a delay in seeking treatment and affect severe case of dengue infection when admitted to the hospital. This study aimed to analyze the liver function profile in dengue pediatric patients during the COVID-19 pandemic.Methods: All patients under 18 with confirmed dengue serology (NS-1 immunochromatography or IgM anti-Dengue (ELISA) test and IgG anti-Dengue (ELISA) test) in Dr. Hasan Sadikin General Hospital from 2021–2022 were included in this retrospective study. The patients were categorized based on the modified WHO classification of 2009. Data were processed with SPSS® ver. 25 and analyzed using Chi-Square and One Way-ANOVA.Result: In total, 85 patients were tested for the liver function; most severe dengue patients had abnormal SGOT and SGPT levels (100% vs. 64%).  The SGOT and SGPT levels during the initial admission were higher in the severe dengue group (634 U/l and 271 U/l) and significantly different among groups (p=0.001 and p=0.032). The elevated SGOT (1,339 U/l vs. 203 U/l vs. 87.3 U/l; p=0.014) and SGPT (438 U/l vs. 100 U/l vs. 42.8 U/l; p=0.005) levels were higher in the severe dengue group.Conclusion: The severity of dengue is in line with the increase in SGOT and SGPT levels. During the COVID-19 pandemic, the liver dysfunction persists and may be interfered with by delays in dengue treatment. Early recognition and prompt treatment are needed to decrease morbidity and mortality.

Abstract

Indonesian government launched a triple elimination program to eliminate mother-to-child transmission (MTCT) of human immunodeficiency virus (HIV), syphilis, and hepatitis B in 2018, aiming to increase screening uptake among pregnant women during antenatal visits and to reduce the rates of these infections in children less than 50 per 100,000 live births. Despite this initiative, a thorough assessment of its effectiveness, particularly in Bandung, the capital city of West Java, as one of the most densely populated cities in Indonesia with a high HIV incidence, has yet to be conducted. The aim of this study was to analyze the impact of this triple elimination program in Bandung by assessing the data between 2017 and 2020. Monthly data was obtained from the Health Office of Bandung for four years, including number of screenings done for HIV, syphilis, and hepatitis B, number of confirmed cases and number of pregnant women treated for those infections. Additionally, data on children under 24 months old afflicted by these infections were also collected. Our data indicated an increase in screening coverage for HIV, syphilis, and HBV among pregnant women; however, it remained below the national set benchmarks for screening coverage. Only 59.5% of HIV-positive pregnant women received anti-retroviral therapy in 2020, while merely 25% of syphilis-positive cases were administered benzathine penicillin G. Syphilis screening was correlated with an increase in positive cases among children, suggesting missed opportunities in managing syphilis-positive pregnant women. Furthermore, management of HIV- and syphilis-positive cases had suboptimal outcomes. Data on hepatitis B was not evaluated since it was not available. To achieve the triple elimination program goals, comprehensive coordination among all relevant stakeholders is required, as is continuous monitoring and evaluation.

Abstract

Soil-transmitted helminth (STH) infections remain a major public health concern in developing countries, particularly among lower- to middle-income populations with poor sanitation, limited access to healthcare, and inadequate clean water. Preschool-aged children are especially vulnerable due to their developing immune systems and increased nutritional needs. This study aimed to identify factors associated with STH infections among children aged 24–59 months in Bandung District, Indonesia. A case-control design was employed using secondary data from helminthiasis surveillance conducted between October 2019 and January 2023, involving a total of 261 children. The Kato-Katz technique was used to examine stool samples for the detection of STH species. Among them, 30 (11.49%) were infected with soil-transmitted helminths (STH), comprising ascariasis (11.11%), trichuriasis (0.38%), and hookworm infection (0.38%). Bivariate chi-square analysis revealed significant associations with fathers’ education level (p = 0.0003), BPJS-Healthcare participation (p = 0.015), water source (p = 0.015), distance from the water source to the latrine (p = 0.003), and nail hygiene (p = 0.018). Multivariate logistic regression confirmed that distance between water source and pit latrine (OR = 0.265; 95% CI: 0.76–0.92), nail hygiene (OR = 0.318; 95% CI: 0.13–0.76), and participation in BPJS-healthcare program (OR = 0.364; 95% CI: 0.15–0.91) were key determinants of STH infection. These findings highlight the importance of addressing environmental and behavioral factors through public health interventions, including improving access to clean water, promoting personal hygiene, and enhancing parental, particularly paternal education, as essential strategies for reducing the risk of STH in children.
