Abstract

Breast cancer is one of the health problems in the world. In Indonesia, it washighest prevalence of cancerin 2018. CD44, a non-kinase transmembrane glycoprotein, plays an important role in cancer development and progression.It isresponsible for mediating of the adhesion between adjacent cells andalsobetween cells and the extracellular matrix. The aim of study was to investigatethe relationship between the CD44 expressionand histopathological grading, metastasis, overall survival (OS) as well as disease free survival (DFS)in breast cancer patients.Thiswas a retrospective cross sectional studyusing data on medical records of breast cancer cases at Dr. Wahidin Sudirohusodo General Hospital, Makasarfrom 2016 to 2018. Examination of CD44 expression were performed from paraffin block samples.Data of the CD44 expression and medical records obtained were then presented in tables and statistically analyzed using chi-squared and Kaplan–Meier survival analysis.No significantly relationship between the CD44 expression and histopathological grading was reported (p>0.05). However, significantly relationship between the CD44 expression and metastasis war reported (p<0.01). Patients with metastasis havinghigh CD44 expression was significantly higher (90%) than those patients without metastasis (60%) (p<0.01). No significantly relationship between the CD44 expression and OS or DFS was reported (p>0.05). In conclusion, there isrelationship between CD44 expression and metastasis in the breast cancer patients. However, there is norelationship between CD44 expression and histopathological grading, OS as well as DFS.

Abstract

Fracture is defined by the loss of bone or cartilage continuity, either total or partial. Diabetes mellitus (DM) is a group of metabolic diseases characterized by hyperglycaemia resulting from defects in insulin secretion, insulin action, or both. HbA1c could be used as a diagnostic tool and determines the glycaemic control of DM patients. This study aimed to investigate the correlation between serum HbA1c levels and the healing process of postoperative fractures in DM patients. This was an observational study with a retrospective cohort design. The study population was DM patients who were admitted to the Department of Emergency, Dr. Wahidin Sudirohusodo General Hospital, Makassar due to long bone fracture within the period of September 2019-February 2020. The bone fractures healing was evaluated based on the radiographic union scale in tibial fractures (RUST) scores. The results are analysed using the Chi-square test. A total of 70DM patients who underwent fracture reduction surgery were involved in this study. Out of 35 patients with controlled DM (HbA1c <7%), we obtained 24 (68.6%) patients and 11 (31.4%) patients with good and delayed union, respectively. The other 35 patients had uncontrolled DM (HbA1c >7%), and there were 13 (37.1%) patients and 22 (62.9%) patients with good and delayed union, respectively. The Chi-square test showed a significant relationship (p= 0.008). It can be concluded that the increase in HbA1c levels was related to the obstacles in bone fractures healing. Moreover, an infection contributes to the healing status of bone fractures.

Abstract

Pituitary macroadenomas are benign tumors in the pituitary gland measuring more than 1 cm in diameter. Pituitary adenomas account for 8 to 10% of all intracranial tumors and occur primarily in the anterior pituitary lobe and rarely in the posterior lobe. Most pituitary macroadenomas are non-functioning tumors, which show clinical symptoms due to tumor pressure and do not increase hormonal activity. In this case, a 40 y.o. woman was diagnosed with a pituitary macroadenoma after showing clinical symptoms of a functioning tumor, namely acromegaly, prognathism, and large, beefy hands and feet, for the past 4 years. Complaints were accompanied by amenorrhea, intermittent cephalgia, and hemianopsia in the right eye. Blood laboratory tests revealed increased levels of prolactin and growth hormone, while the results of computed tomography (CT) scans and magnetic resonance imaging (MRI) of the head indicate the presence of a pituitary macroadenoma. Endoscopic transsphenoidal surgery, a minimally invasive procedure, was performed with minimal complications.

Abstract

BACKGROUND: Some of colorectal cancers (CRCs) are familial, however, heterozygote relatives have approximately 80% lifetime risk of cancer. Risk assessment of CRC’s family could be calculated by direct measurement of mRNA gene expression and Bayesian theorem which is modifying initial background of pedigree risk with additional conditional information. This application has not been reported.METHODS: The cross-sectional translational sequential studies were performed: (1) adenomatous polyposis coli (APC) and MutS homolog (MSH)2 mRNA quantitative RT PCR gene expressions in tissue and whole blood CRC patients; (2) gene expression was determined in matched controls; and (3) pedigree and Bayesian analysis was calculated in the patient’s family of Proband.RESULTS: Fourty CRC and 31 control subjects were enrolled. The mean blood APC level control’s group was 13,261±670 fold-change (fc) and blood MSH2 level was 12,219±756 fc. The cut-off points for hereditary APC was 12,195 fc and MSH2 was 11,059 fc. The mean APC blood level in CRC subject was 11,578±2,638 fc and MSH2 blood level was 11,411±2,912 fc. There were significant differences APC and MSH level between tissue and blood level in CRC. Eight of 40 CRC subjects had a history of familial CRC. Four patients and 10 Probands were available for recurrence risk evaluation of pedigree analysis, RNA PCR quantitative and Bayesian calculation.CONCLUSION: There was determined a cut-off point of hereditary mRNA quantitative expression. The APC and MSH2 levels in CRC subjects were significantly lower than controls. Bayesian analysis allowed for the calculation of relative risk in CRC family members and considered in clinical practice.KEYWORDS: hereditary CRC, APC gene, MSH gene, Bayesian analysis

Abstract

Introduction:  Breast cancer treatment is carried out at the early stage of the disease, and a comprehensive assessment of the subtype, stage, and incidence of metastases is required. This study aims to determine breast cancer patients' overall survival (OS) and disease-free survival (DFS) based on subtype, stage, and incidence of metastases. Methods:  An observational analytic study with a retrospective cohort design was performed on eligible breast cancer patients at Wahidin Sudirohusodo Hospital from 2016 to 2019. Univariate analysis, and the Kaplan-Meier survival log-rank method (Mantel-Cox), were used to determine differences in survivability rates (OS and DFS) based on subtype, stage, and incidence of metastases. Results:   A total of 172 breast cancer patients from 32 years to 84 years with a mean age of 53.5 years. The most subtypes were Luminal A (34.9%) patients, and the least was Luminal B (15.7%) patients. A total of 85 (49.4%) patients survived during these three years, while 87 (50.6%) died. Based on the results of statistical tests in this study, there were no significant differences between the subtypes with DFS and OS, but on the contrary, there was a significant difference between the stage and incidence of metastases with OS and DFS (p<0.05). Conclusions:  There were no statistically significant differences between the subtypes with OS and DFS. This study showed a significant difference between the stage and incidence of metastases with OS and DFS

Abstract

Kanker payudara merupakan penyebab kematian akibat kanker terbanyak pada wanita di seluruh dunia dengan angka kematian sebesar 458.400 wanita setiap tahunnya. Poly-(ADP ribose) polymerase (PARP) dan phosphatidylinositol 3-kinase (PI3K) adalah dua molekul yang memainkan peran penting pada patofisiologi kanker payudara. Metastasis kanker payudara merupakan sebuah proses berkesinambungan yang melibatkan beberapa tahapan penting seperti invasi, angiogenesis, hingga replikasi sel. Sel yang mengalami mutasi pada kanker payudara akan menyebabkan ekspresi yang berlebihan dari PI3K sehingga terjadi replikasi sel yang tidak terkontrol disertai dengan peningkatan matrix metallopeptidase 9 (MMP-9). MMP-9 memiliki peran dalam degradasi matriks ekstraseluler yang penting untuk proses invasi tumor. Di sisi lain, PARP membantu reparasi sel yang mengalami kerusakan sehingga proses replikasi sel kanker dapat terus berlangsung. Terlebih lagi, PARP diketahui memiliki andil besar dalam proses angiogenesis sel kanker. Gabungan dari kedua fenomena tersebut menghasilkan sel kanker dengan kemampuan replikasi dan metastasis yang tinggi.

Abstract

Chemotherapy is one of the modality therapies given to breast cancer patients to inhibit cancer cell metastases. The side effects of chemotherapy can suppress the hematopoietic system in the bone marrow and progressively affect the patient's prognosis. The aim of study was to analyze routine blood profiles as supporting data in developing nursing care for breast cancer patients undergoing chemotherapy at the RSU. Dr. Wahidin Sudirohusodo Makassar, Indonesia, from June to September 2018. This research was a descriptive research with descriptive survey method. This research was conducted at the RSU Surgical Polyclinic. Dr. Wahidin Sudirohusodo. The sampling technique used was Non-Probability Sampling a purposive sampling method. The participants of this study were patients with breast cancer who undergo chemotherapy treatment with a number of samples that meet the inclusion criteria as many as 32 people. of the 17 routine blood components which were assessed as one of the investigations in 32 patients were found to be 4 abnormal components, red blood cells mean ± SD (3.99 10^6/ uL ± 0.51), hemoglobin mean ± SD (11.18 gr/l ± 1.09), hematocrit mean ± SD (34.96% ± 3.96), and mean ± SD monocyte (15,043 10^3/uL ± 8.95). due to metastasis and side effects of chemotherapy treatment decrease red blood cells, hemoglobin, hematocrit, and increase in specific immune response through increase of monocytes indicator that performing phagocytosis.

Abstract

Cyclooxygenase-2 (COX-2) berperan dalam pertumbuhan tumor dan metastasisnya yang berdampak pada buruknya prognosis. Tujuan penelitian ini untuk mengetahui hubungan ekspresi COX-2 terhadap disease free survival dan overall survival pada pasien kanker payudara. Penelitian ini bersifat observasional menggunakan desain kohort dengan periode follow up selama 24 bulan. Hasil: Ekspresi COX-2 ditemukan pada 21 sampel (42%) dari 50 pasien kanker payudara. Selama follow up 24 bulan, lama DFS pada COX-2 negatif (20,1 bulan) lebih lama dibandingkan pada COX-2 positif (14,0 bulan) dan menunjukkan hubungan yang signifikan (p<0,01). Lama OS pada COX-2 negatif (22,6 bulan) lebih lama dibandingkan pada COX-2 positif (17,8 bulan) yang juga menunjukkan hubungan yang signifikan (p<0,01). Terdapat hubungan yang signifikan antara ekspresi COX-2 dengan disease free survival dan overall survival pada pasien kanker payudara. COX-2 bisa dijadikan salah satu faktor prognostik kanker payudara.

Abstract

Kehamailan merupakan pertumbuhan dan perkembangan janin mulai sejak konsepsi sampai permulaan persalinan. Kehamilan akan berlangsung selama 40 minggu yang dihitung dari menstruasi terkahir. Penatalaksanaan mual dan muntah pada masa kehamilan dapat dilakukan secara farmakologi dan non-farmakologi, diantaranya penggunaan aromaterapi. Papermint memiliki kandungan mentol yang dapat meringankan mual dan muntah dan kandungan minyak esensial lemon dapat meringankan gejala mual dan muntah. Penelitian ini bertujuan untuk mengetahui efektifitas pemberian terapi kombinasi peppermint dan lemon dalam menurunkan intensitas mual dan muntah pada ibu hamil trimester I dan II. Penelitian yang digunakan adalah quasi-experimental dengan menggunakan Teknik pre-posttest design. Sampel penelitian sebanyak 32 orang ibu hamil trimester I dan II, denggan Teknik purposive sampling. Instrument penelitian dengan mengukur nilai indeks PUQE-24.  Uji yang digunakan dalam penelitian ini yaitu uji Wilcoxon dan dilanjutkan uji mann-whitney. Hasil uji didapatkan nilai maximunm dan minimum sebelum dilakukan tindakan adalah 12 dan 15 sedangkan nilai maximunm dan minimum sesudah dilakukannya tindakan adalah 1 dan 6, sehingga didapatkan hasil setelah menggunakan uji Wilcoxon menunjukkan bahwa terdapat penurunan mual dan muntah sebelum dan sesudah tindakan dengan P-value=0,000 (ɑ≤0,05) yang artinya adanya pengaruh setelah diberikan kombinasi aromaterapi papermint+lemon.
