Abstract

ABSTRACTIntroduction: The issue of oral health within the prison's environment remains a conspicuously neglected aspect of prisoner welfare. The underutilization of access to oral care constitutes a significant barrier. The necessity for effective oral health promotion programs tailored to this vulnerable population is further underscored by the high rates of oral disease and the lack of preventive measures. The objective of this study was to evaluate the extant literature on oral health programs among prisoners. Tujuan penelitian ini bertujuan untuk melakukan kajian pustaka mengenai program promosi kesehatan di lembaga pemasyarakatan. Methods: A comprehensive search was conducted in various databases, including PubMed, Scopus, Cochrane, ScienceDirect, ProQuest, EbscoHost, Nature, JSTOR, and Sage Journal, to identify oral health promotion programs in correctional institutions. The data were meticulously mapped and thematically analyzed to distill the key findings by the PRISMA-ScR guidelines. Results: the study is as follows: A review of the extant literature reveals that while many programs focus on improving oral health knowledge and behaviors, few report long-term outcomes or address systemic barriers such as inadequate access to dental care. Interventions that are tailored to the unique needs of correctional populations, such as culturally appropriate education and peer-led programs, have shown promising results. Nevertheless, the extant evidence regarding the effectiveness of these measures remains limited. Conclusion: The prisoner training program through peer coaching can be applied to improve prisoners' knowledge and literacy of dental and oral health. Prisoners who act as peer coaches for other prisoners facilitate the effective development of the program in accordance with the conditions of the correctional institution. It is hoped that improving inmates' oral health literacy will increase their motivation to access dental services and their commitment to maintaining oral health.KEY WORDS: oral health promotion, prisons, correctional populations, preventive dentistry, public health.Program promosi kesehatan gigi dan mulut di lembaga pemasyarakatan: Scoping reviewABSTRAKPendahuluan: Kesehatan gigi dan mulut merupakan masalah yang terabaikan di lingkungan Lembaga Pemasyarakatan. Kurangya pemanfaatan perawatan gigi dan mulut menunjukkan program promosi kesehatan gigi dan mulut yang efektif perlu untuk disikapi lebih lanjut dengan pertimbangan tingginya tingkat penyakit gigi dan mulut dan minimnya tindakan pencegahan terhadap populasi ini. Tujuan penelitian ini bertujuan untuk melakukan kajian pustaka mengenai program promosi kesehatan di lembaga pemasyarakatan. Metode: Pencarian komprehensif dilakukan di berbagai database seperti PubMed, Scopus, Cochrane, ScienceDirect, ProQuest, EbscoHost, Nature, JSTOR, dan Sage Journal untuk mengidentifikasi program promosi kesehatan gigi dan mulut di lembaga pemasyarakatan. Data dipetakan dan dianalisis secara tematik untuk merangkum temuan-temuan utama dengan pedoman PRISMA-ScR. Hasil: Banyak program yang berfokus pada peningkatan pengetahuan dan perilaku kesehatan gigi dan mulut, namun hanya sedikit yang melaporkan hasil jangka panjang atau mengatasi hambatan sistemik seperti akses yang tidak memadai keperawatan gigi. Intervensi disesuaikan dengan kebutuhan populasi di lembaga pemasyarakatan, seperti program pendidikan yang dipimpin oleh teman sebaya menunjukkan hasil yang menjanjikan, akan tetapi bukti tentang efektivitasnya masih terbatas. Simpulan: Kajian pustaka ini menunjukkan bahwa program promosi kesehatan gigi dan mulut di lembaga pemasyarakatan sebagian besar berfokus pada peningkatan pengetahuan dan perilaku narapidana, dengan pendekatan pendidik sebaya sebagai strategi yang paling banyak dilaporkan dan berpotensi meningkatkan partisipasi. Namun, bukti mengenai efektivitas jangka panjang serta kemampuan program dalam mengatasi hambatan sistemik, terutama keterbatasan akses layanan perawatan gigi, masih terbatas. Temuan ini menegaskan perlunya pengembangan program promosi kesehatan gigi dan mulut yang terintegrasi dengan sistem pelayanan kesehatan pemasyarakatan dan didukung oleh penelitian berbasis bukti yang lebih kuat.KATA KUNCI: promosi kesehatan gigi dan mulut, lingkungan lembaga pemasyarakatan, populasi yang di lembaga pemasyarakatan, kedokteran gigi preventif, kesehatan masyarakat.

Abstract

ABSTRACTIntroduction: The World Health Organization (WHO) reported in 2022 that approximately 3.5 billion people worldwide suffer from oral diseases. Poor oral health not only leads to clinical problems but also results in social disadvantages and reduced quality of life. In health epidemiology, religion is considered a social determinant of health. Higher levels of religiosity are believed to contribute to better oral health through social support and psychological well-being. Religiosity functions as a psychosocial determinant of health that enhances emotional stability and preventive behaviors. Therefore, examining the relationship between religiosity and oral health status, as well as oral health-related quality of life (OHRQoL), is important as a basis for developing promotive and preventive interventions based on a bio-psycho-social-spiritual approach. Methods: A literature search was conducted in PubMed, ScienceDirect, EBSCOhost, and SpringerLink using the keywords “belief,” “religion,” “religiosity,” “oral health,” “quality of life,” and “oral health-related quality of life”. Manual searching was performed through the reference lists of relevant articles. This study employed a scoping review method based on the framework proposed by Arksey and O’Malley. Results: Five publications met the inclusion criteria. Three studies demonstrated a positive relationship between religiosity and OHRQoL, indicating better oral health status and lower mean DMFT scores. Two other studies reported negative associations: individuals without religious affiliation had better OHRQoL, and the prevalence of dental diseases among clergy and monastic communities was higher than that of the local population. Conclusion: Religiosity is associated with oral health status and OHRQoL, acting both as a protective and a risk factor. These findings emphasize the importance of considering spiritual aspects in planning community oral health programs. Practical implications include the integration of faith-based oral health education, behavioral counseling, and collaboration between health professionals and religious leaders.KEYWORDS: Religiosity, oral health status, oral health-related quality of life (OHRQoL)Hubungan antara religiusitas dan status kesehatan oral serta oral health related quality of life (OHRQoL): Scoping reviewABSTRAKPendahuluan: Laporan World Health Organization (WHO) tahun 2022 menyebutkan sekitar 3,5 miliar penduduk dunia mengalami penyakit gigi dan mulut. Kondisi kesehatan rongga mulut yang buruk tidak hanya berdampak klinis, tetapi juga menimbulkan kerugian sosial dan menurunkan kualitas hidup. Dalam epidemiologi kesehatan, agama dipandang sebagai determinan sosial kesehatan. Tingkat religiusitas yang tinggi diduga berkontribusi pada kesehatan oral melalui dukungan sosial dan kesejahteraan psikologis. Religiusitas berperan sebagai penentu kesehatan psiko-sosial yang meningkatkan stabilitas emosional dan perilaku preventif. Kajian hubungan religiusitas pemeluk agama dengan status kesehatan oral dan oral health related quality of life (OHRQoL) diperlukan sebagai dasar pengembangan intervensi promotif dan preventif berbasis pendekatan bio-psiko-sosial-spiritual. Metode: Pencarian literatur dilakukan pada PubMed, ScienceDirect, EBSCOhost, dan SpringerLink menggunakan kata kunci belief, religious, religions, oral health, life quality, dan oral health related quality of life. Penelusuran manual dilakukan melalui daftar referensi artikel relevan. Penelitian menggunakan metode scoping review menurut Arksey dan O’Malley. Hasil: Diperoleh lima publikasi yang memenuhi kriteria. Tiga penelitian menunjukkan hubungan positif antara religiusitas dengan OHRQoL, status kesehatan mulut lebih baik, serta rerata DMFT lebih rendah. Dua penelitian lainnya menunjukkan hubungan negatif: individu tanpa afiliasi agama memiliki OHRQoL lebih baik, dan prevalensi penyakit gigi pada komunitas pendeta dan biarawan lebih tinggi dibandingkan penduduk lokal. Simpulan: Terdapat hubungan antara religiusitas dan status kesehatan oral serta (OHRQoL) sebagai faktor protektif dan faktor resiko. Temuan ini menegaskan pentingnya mempertimbangkan aspek spiritual dalam perencanaan program kesehatan gigi masyarakat. Implikasi praktis mencakup integrasi edukasi kesehatan mulut berbasis komunitas keagamaan, konseling perilaku, serta kolaborasi antara tenaga kesehatan dan pemuka agama.KATA KUNCI:  religiusitas, status kesehatan oral, kualitas hidup terkait kesehatan oral (OHRQoL)

Abstract

Introduction: Periodontal disease during pregnancy continues to be a significant worldwide health concern with wide-ranging systemic effects. The association between maternal periodontitis and unfavourable pregnancy outcomes (APOs) has been shown by numerous systematic reviews. Studies conducted worldwide reveal significant variations in periodontitis prevalence and associated risk factors. This literature review aims to determine the prevalence and risk factors of periodontal disease during pregnancy in Indonesia. Article searches were conducted in 7 databases: PubMed, ProQuest, ScienceDirect, Scopus, Garuda, EBSCOhost, and Google Scholar. Review: This study followed the predefined PEOS eligibility criteria, with a population consisting of pregnant women of all ages and trimesters in Indonesia, various risk factors associated with periodontal disease as the exposure, and periodontal disease as the outcome. Inclusion criteria were original research published between January 2015 and April 2026, written in English or Indonesian. A total of 312 studies were identified, and 18 studies were included. The reported prevalence of periodontal disease during pregnancy in Indonesia ranged from 4.4% to 100%. Several associated risk factors found are oral hygiene, particularly calculus accumulation, crowded teeth, frequency of toothbrushing, betel nut consumption, smoking status, level of education, living area, gestational age, stress, health motivation, and contraceptive use history. Conclusion: The prevalence of periodontal disease during pregnancy in Indonesia has remained quite high over the past decade. The majority of pregnant women in Indonesia need intervention in the form of Dental Health Education (DHE) and dental scaling.

Abstract

Introduction: Gingivitis, a reversible inflammatory condition, is the second most prevalent oral health issue in children, after dental caries. A national health survey in 2018 revealed a high prevalence of gingivitis among Indonesian children, with 5.8% among 5-year-old and 14.3% among 12-year-old children. Childhood is a critical period for the development of oral health behaviors, which are influenced by genetic, familial, and environmental factors. Research highlights the intergenerational transmission of gingivitis, linking parental oral health to child’s outcomes. This study aimed to analyze the relationship between the occurrence of gingivitis among parents and their 3--14-year-old children in Indonesia. Materials and Methods: Cross-sectional study using secondary data from the 2018 Riskesdas. The data were collected by the Republic of Indonesia Ministry of Health's Research and Development in March 2018. The subject criteria were 3--14-year-old children and their biological parents who had clinical data on gingivitis status and interviews for behavioral variables. Data analysis was performed using SPSS ver.27 for univariate and bivariate analyses. Results and Discussion: The relationship between the occurrence of gingivitis in parents and children was significant (p< .001). The parental risk factors included parents’ perceptions of oral health problems. Parents’ habits were also significantly related to their children’s habits. Conclusion: The significant relationship between the occurrence of gingivitis among parents and children, and the significant role of parents in shaping their children’s oral health behaviors were in accordance with the concept of the intergenerational life course.

Abstract

Introduction: Oral diseases remain a major global public health challenge, contributing to disability, reduced quality of life, and increased healthcare costs. Although regular dental attendance improves oral health outcomes, the utilization of dental health services remains unequal across population due to complex individual, social, structural, and policy-related factors. Theoretical models are essential for understanding these determinants and informing interventions to improve access and equity, yet a comprehensive synthesis of these models is lacking. This scoping review aimed to identify, map, and synthesize theoretical models used to explain dental health service utilization. Methods: A scoping review was conducted in accordance with the PRISMA Extension for Scoping Reviews (PRISMA-ScR). Eight electronic databases (PubMed, Scopus, ProQuest, Cochrane Library, ScienceDirect, SpringerLink, JSTOR, and Emerald Insight) were searched, supplemented by manual screening of reference lists. English-language publications from 1960 to 2025 that proposed, adapted, or conceptualized theoretical models of dental health service utilization were included. Study selection, duplicate removal, and screening were performed using Rayyan.ai, followed by a narrative synthesis of study characteristics, theoretical foundations, and model components. Results: Of the 1,037 records identified, 25 studies met the inclusion criteria. Most were published after 2000 and originated from high-income countries, particularly Europe and the United States, with limited evidence from middle-income settings. Models were applied across diverse populations, including children, adults, older adults, women, migrants, rural communities, and other vulnerable groups. Most were adaptations of established health service utilization frameworks, while fewer proposed novel or context-specific models. Determinants were consistently grouped into five domains: individual characteristics, enabling and structural factors, need-related factors, psychosocial influences, and broader contextual and policy determinants. Conclusion: This review provides the a comprehensive synthesis of theoretical models explaining dental health service utilization. The findings offer a conceptual foundation for future research and support evidence-informed policies and interventions to improve equitable access to dental health services.
