Abstract

Introduction: Open-angle glaucoma is a chronic, progressive, and irreversible multictorial optic neuropathy with open angle of the anterior chamber, accompanied by visual field disturbances, with increased intraocular pressure as the main risk factor, caused by trabecular meshworks resistance. The incidence of open-angle glaucoma is 2.4 million people each year. This study aims to determine the characteristics of open-angle glaucoma patients at the eve polyclinic Prof Dr IGNG Ngoerah Hospital Denpasar in 2022. Methods: This research is a descriptive analytical research with retrospective analysis. The study was conducted by collecting data based on observations of the medical records of patients who visited the Eye Polyclinic at Prof Ngoerah General Hospital in the period of January 1, 2022 until December 31, 2022. A total of 38 people (64 eyes) who met the inclusion and exclusion criteria were analysed using SPSS windows version 26.0. Results: The majority of subjects aged > 60 years (63.2%), male (68.4%). POAG (76.6%), SOAG (23.4%), the most common etiology of SOAG was lens induce, advanced stage (40.6%), and all had chronic onset. The median of visual acuity (logMar) pre operation vs three months post operation 1.0 vs 0.7, IOP 23 mmH vs 16 mmH, the mean of anti glaucoma medication was 1.37 vs 0.6. Conclusion: In patients with open-angle glaucoma, there is a good response to all treatment modalities (medical, trabeculectomy, phacoemulsification-IOL, phacoemulsification- IOL-trabeculectomy, implant glaucoma).

Abstract

ABSTRAK Lens induced glaucoma (LIG) merupakan salah satu jenis glaukoma sekunder yang diakibatkan oleh kelainan pada lensa mata. Menurut data, kasus LIG menjadi kasus glaukoma sekunder terbanyak yang ada di rumah sakit. Namun hingga saat ini masih jarang ditemukan penelitian epidemiologi mengenai LIG di Indonesia maupun di Bali. Penelitian ini bertujuan untuk mengetahui karakterisik LIG berdasarkan usia, jenis kelamin, pekerjaan, tipe LIG, status mata, manifestasi klinis, tekanan intraokuler (TIO), tajam pengelihatan, dan terapi. Penelitian ini dilakukan di RSUP Sanglah tahun 2019-2020 dengan metode penelitian cross sectional dengan metode pengambilan sampel consecutive sampling. Berdasarkan hasil penelitian didapatkan 23 pasien LIG di RSUP Sanglah tahun 2019-2020 yang memenuhi kriteria inklusi. Hasil penelitian menunjukkan pasien usia >59 tahun dengan presentase terbanyak 47,8%. 73,9% berjenis kelamin laki-laki dan 34,8% bekerja sebagai petani. Tipe LIG fakomorfik (56,5%) menjadi tipe yang paling sering dijumpai yang didominasi status fellow eye katarak imatur (43,5%). Seluruh pasien (100%) datang dengan manifestasi pandangan buram. TIO sebelum terapi terbanyak pada rentang >39 mmHg (52,5%) dan pasca terapi pada rentang <21 mmHg (65,2%). Didapatkan hasil tajam pengelihatan sebelum terapi dan sesudah terapi terbanyak yaitu hand movement masing-masing 30,4% dan 26,1%. Terapi yang paling sering diberikan adalah fakoemulsifikasi dengan implan intraocular lens sebanyak 60,9%. Dapat disimpulkan pentingnya pemberian terapi yang tepat di awal perjalanan penyakit agar bisa mendapatkan prognosis yang baik. Kata kunci : lens induced glaucoma, karakteristik, tekanan intraokular

Abstract

Glaukoma primer sudut terbuka dikarakterisitikan sebagai suatu neuropati saraf optik yang bersifat kronis dan progresif, dengan morfologi tertentu tanpa disertai penyakit atau kelainan kongenital dengan sudut bilik mata terbuka yang menyebabkan defek penglihatan permanen. Kerusakan saraf optik pada glaukoma primer sudut terbuka terus terjadi walaupun peningkatan tekanan intraokular tidak signifikan atau bahkan normal. Penelitian ini memiliki tujuan untuk mengetahui gambaran glaukoma primer sudut terbuka pada pasien di Poliklinik Mata Rumah Sakit Umum Pusat (RSUP) Sanglah Denpasar Periode Januari 2020- Desember 2020. Penelitian deskriptif retrospektif dengan metode cross-sectional. Data penelitian berupa rekam medis yang masuk dalam kriteria inklusi dan eksklusi melalui teknik total sampling. Data diolah menggunakan perangkat komputer SPSS versi 21 untuk mencari distribusi frekuensi dan persentase dari karakteristik usia, jenis kelamin, mata yang terlibat, tekanan intraokular, cup/disc ratio, keluhan utama, dan lapang pandang. Penelitian memperoleh 25 sampel terdiri atas 42 mata yang terdiagnosa glaukoma primer sudut terbuka. Hasil penelitian, glaukoma primer sudut terbuka terbanyak berusia 60-80 tahun sebesar 17 orang dengan persentase 68%. Kejadian pada pria lebih tinggi daripada wanita. Sebagian besar glaukoma primer sudut terbuka terjadi secara bilateral (68%). TIO ?21 mmHg normal lebih tinggi (57,2%) dibandingkan TIO >21 mmHg(42,8%). CDR terbanyak dengan nilai 0,6-0,8 dan 0,9-1 (35,7%). Glaukoma primer sudut terbuka dengan keluhan utama pandangan kabur terbesar dengan persentase 72%. Abstract

Myopia is the main cause of visual impairment suffered by school-age children in the world. Myopia causes children unable to see clearly at a distance so that if not corrected it can affect children's activities and achievements. This study aims to determine the characteristics of myopia in pediatric patients who came at Eye Polyclinic Prof. Dr. I.G.N.G. Ngoerah in 2020/2021. This study is a descriptive retrospective cross-sectional design with total sampling from secondary medical record which analyzed and processed using SPSS. Based on the study results, it was found that 32 pediatric patients suffered myopia. The results showed that myopia was more common in women (59.4%) with an age range 6-12 years (62.5%). Most of the patients were from Denpasar (62.5%). All cases of myopia were present in both eyes or bilateral (100.0%). The most common classification of myopia found compound myopic astigmatism in right eye (53.1%) and left eye (65.6%). Patients most often experienced moderate visual impairment with visual acuity <6/18 – 6/60 in right eye (46.9%) and left eye (40.6%). The highest degree of myopia was low myopia (40.6%) in right eye with a correction lens <S-3.00 and high myopia in left eye (43.8%) with >S-6.00. Most of patients experienced improvement to normal vision 6/6 – 6/12 after correction in right eye (59.4%) and left eye (65.6%). Amblyopia was found (56.3%) myopia in children. The results of this study are expected to be basis for further research and to realize the importance of preventing and proper management myopia in children. Abstract

Introduction: Corneal perforation is an ophthalmic emergency and requires immediate medical and surgical treatment. The most common causes of corneal perforation are infection, inflammation, or trauma. Case Report: A 9-year-old boy came with complaints pain in his right eye for 2 days of BATH, he had history of hit by a sharp tip wire. Visual acuity on the right eye was 6/60 PH 6/30, revealed ciliary injection, corneal rupture with the avulsion, size 6x3x1mm, stromal oedema, iris prolapse, shallow anterior chamber, eye pressure was n-1/p. Patient was diagnosed with corneal rupture and iris prolapse. Patient was treated by penetrating keratoplasty. The last condition of the graft is quite good and the visual acuity was 6/15. Discussion: Perforation cause by the traumatic more than 3 mm can be treated by the penetrating keratoplasty as an emergency situation. Tissue adhesives, such as cyanoacrylate glue, amniotic membrane multilayer transplantation, conjunctival flap, or patch graft, can be used to treat small corneal defects. The success of keratoplasty is measured by corneal clarity and functional refractive outcomes. Conclusion: The success rate of emergency keratoplasty in corneal perforations reported by various studies and one of these case reports reinforces the importance of eye banking in corneal graft supplies.

Abstract

Introduction & ObjectivesGlaucoma is an eye disorder characterized by glaucomatous optic neuropathy with a typical visualfield defect and increased intraocular pressure (IOP).The incidence of glaucoma in dialysis patient is8.18/10,000 person. The dialysis process may change the IOP and Mean Arterial Pressure (MAP)which may lead to glaucoma. The aims of this study are to determine whether there was a change inIOP and MAP and to determine the correlation between changes in IOP and MAP due to dialysis. MethodsThis is an observational study with cross sectional approach using stratified sampling method. Thesubjects were all regular dialysis patient at the Dialysis Unit in Sanglah Hospital period April-May2021 with total 63 people. The data taken are age,gender ,duration of dialysis, fasting blood sugar,urea and serum creatinine level before dialysis. Subjects were measured IOP and calculated MAPbefore and after dialysis. ResultsMean of age of this study was 47,03±12,06 years old and dominated by male. Paired T-Test showedno significant changes IOP after dialysis but there were significant decreased on systolic, diastolicand MAP. Pearson correlation showed no correlation between changes of IOP and MAP afterdialysis. Multivariate analysis shows significant correlation between changes of IOP with urea levelbefore dialysis. ConclusionIOP after dialysis was not increased in this study. Systolic, diastolic and MAP were decreased afterdialysis compared to before dialysis. There is no correlation between changes in IOP and MAP afterdialysis compared to before dialysis. Urea level before dialysis may be associated with increased IOPafter dialysis.

Abstract

Introduction & ObjectivesGlaucoma is one of the leading causes of irreversible global blindness and the second most commoncause of blindness in the world after cataracts. Depression and anxiety often occur in glaucomapatients when glaucoma is first diagnosed, the effect of decreased vision function in glaucoma, sideeffects of glaucoma therapy both medically and surgically and financial effects due to length oftreatment and loss of job due to limited vision. This study aims to determine the relationship of thedegree of optic nerve damage to the level of depression and anxiety in patients with primaryglaucoma. MethodsThis research is an analytic, prospective observational research with a cross-sectional studyapproach. The relationship between independent variables and dependent variables was analyzed. ResultsThe Man Whitney test found a significant difference in anxiety scores between the severe and mildto moderate groups with a p value of 0.020 . The ANCOVA test found that there was a relationshipbetween the degree of optic nerve damage in primary glaucoma and the level of anxiety aftercontrolling for control variables. ConclusionThis study can be concluded that there is a difference in the level of anxiety between the degree ofsevere and mild to moderate optic nerve damage. The level of anxiety itself corresponds to thedegree of damage to the optic nerve. The more severe the damage to the optic nerve, increased ofthe patient's anxiety score.

Abstract

Introduction & ObjectivesMicro-invasive glaucoma surgery with goniotomy is a recent surgical treatment modality that targetsthe trabecular outflow to maintain intraocular pressure. This study aims to describe the clinicaloutcomes of excisional goniotomy using KDB and to review the published literature regarding amodified goniotomy technique as a potential surgical treatment modality in Indonesia. MethodsA cross-sectional descriptive study was conducted in patients that underwent goniotomy stand-aloneor combined with cataract surgery. Main outcome parameters consist of visual acuity, intraocularpressure (IOP), number of glaucoma medications, and complications during post-operative follow-up. ResultsA total of 7 patients (8 eyes) were included in the study, consisting of 5 (71,4%) males and 2 (28,6%)females. The mean age of the subjects was 47.7 years. Subjects consisted of primary,pseudoexfoliative, steroid-induced, and angle-recess glaucoma. Mean pre-operative IOPmeasurements showed 20.74 mmHg with visual acuity consisting of 1 (14.3%) mild visualimpairment and 7 (85.7%) severe visual impairment. Postoperatively there was progressiveimprovement in the mean IOP to 13.81 and 11.31 mmHg on first-day and one-week follow-up,respectively. Visual improvement was observed in 2 (28.5%) with mild visual impairment, 3 (42.6%)with severe visual impairment, and 2 (28.5%) with hand movement due to complications of cornealedema following phacoemulsification. A week later, visual acuity improved to 4 (57.1%) with mildvisual impairment and 3 (42.9%) with moderate visual impairment. ConclusionGoniotomy is a simple surgical method that may be applied in Indonesia as a novel technique toprovide an effective intraocular pressure-lowering effect.

Abstract

Introduction & ObjectivesPrimary open angle glaucoma (POAG) is a chronic optic neuropathy characterized by slowlyprogressive visual function loss. Platelet activation as well as its interaction with vascularendothelium and plasma components, play an important role in the vascular pathophysiology ofPOAG. Platelet aggregation in glaucoma tends to increase. This change has negative impact on smallvessel circulation in the eye, such as branches of short ciliary arteries supplying the optic nerve. Inthis study, we will assess the correlation between platelet parameters and the severity of POAG. MethodsThis study is an analytical observational study. Data were obtained retrospectively and prospectivelywith a cross sectional study approach conducted at Sanglah Hospital, Denpasar. The study samplewas 62 eyes selected consecutively. Data was analyzed using SPSS. Descriptive data presentednumerically or categorically, bivariate analysis using Pearson and Spearman correlation tests andmultivariate analysis using logistic regression. ResultsResults showed that the mean age of the research subjects was 66.71 ± 9.795 years, with themajority was male (70.6%). Bivariate analysis, showed no significant correlation between plateletparameters (PLT, PDW, MPV) and the severity of POAG. Multivariate analysis found that age ? 68years, 9,850 times more likely to develop moderate to severe glaucoma, compared to patients < 68years old (OR 9,850, 95% CI, p = 0.009). ConclusionPlatelet parameters showed no significant correlation with the severity of POAG, meanwhile olderage is more at risk of developing moderate to severe POAG.
