Abstract

Background: Tuberculosis is a contagious infectious disease that attacks various organs, especially the lungs. Chest CT scan modalities have a role in helping the diagnosis of tuberculosis and lung cancer. This case report presents a rare case of two women diagnosed with tuberculosis and lung cancer. Cases Presentation: Two women aged 43 and 42 years came to the radiology department with a history of chest pain, loss of appetite, and cough, and underwent tuberculosis therapy by taking medication for two months. The patient underwent chest x photo thorax, abdominal ultrasound, thoracic CT scan, and a biopsy was performed with CT scan guiding. Discussion:  The findings of this case are consistent with previous studies which explained that radiological CT scans found tuberculosis with lung cancer. In the lung window, CT scan chest contrast found cystic bronchiectasis. Other results showed an encapsulated pleural effusion in the right hemithorax, compression atelectasis, and multiple lymphadenopathies. Conclusion: CT scan imaging simultaneously can show the occurrence of tuberculosis and lung cancer. Lung cancer that worsens can cause adenocarcinoma with metastasis spreading to other organs.

Abstract

Background: COVID-19 is a disease caused by SARS-CoV-2 and has numerous clinical spectrums. Mild respiratory infection is the common clinical manifestation of COVID-19, and the less common is pneumonia accompanied by fever, cough, and breathing difficulty. Long COVID can be defined as prolonged signs and symptoms which cannot be explained for other reasons 4 weeks after being diagnosed with SARS-CoV-2. This study aimed to describe the cause of illness is confirmed or suspected COVID-19 patients, specifically on long COVID.Subjects and Method: We performed literature searches of the latest articles with Medline, CINAHL (EBSCO), Global Health (Ovid), WHO Global Research on COVID-19 database, LitCovid and Google Scholar databases published from 2019 to 2020. Two reviewers searched all articles independently (P and W, with 7 and 10 years of experience, respectively). We conducted a systematic review to provide recent evidence of symptoms and complications in long COVID. We followed PRISMA guidelines. Results: A total of 22 papers was identified and screened for eligibility from medical databases. There were 15 papers included in this review. Reason for the continuous symptoms covid an extent of organ destruction, continuous response of chronic inflammation or immunology reaction, non-specific effect of hospital admission, some critical disease, post ICU syndrome, complications from COVID-19 infection,  morbidities and adverse effects of medications used. Common symptoms are fatigue, shortness of breath, cough, chest pain, palpitations, dizziness, arthralgia, myalgia and weakness, sleep problems, sharp pain, gastrointestinal problems, rash and hair loss, impaired balance and ataxia, neurologic problems such as dementia, concentration disorders and poor quality of life. Conclusion: The incidence of long-term manifestations of COVID-19 has been increasing and systemic clinical symptoms affect many organs and systems. This can be due to numerous reasons like post-ICU syndrome, post-viral fatigue syndrome, permanent organ deterioration or others. Correspondence: Widiastuti Soewondo. Abstract

Abstract

Background:  Germ mediastinum tumors grow in the mediastinum and consist of teratoma, seminoma, and nonseminomatous. Cases of germ mediastinum tumors are rare. During the last decade, only two cases have been found at Dr. Ario Wirawan Lung Hospital, Salatiga, Indonesia. Objective: Germ mediastinum tumor is a rare case. This paper aims to present a case report that would benefit the radiology database.Case Presentation: Two male patients aged 18 and 19 were referred to radiology complaining of continuous shortness of breath and coughing. The shortness of breath does not decrease daily, so the patient uses a breathing apparatus to help reduce shortness of breath. The patient underwent chest X-rays, ultrasonography (USG), and computerized tomography (CT). Pleural effusion is seen, the COR of the right heart border is good, the heart border is covered with a homogeneous covering, and the location of the heart is pushed to the right. The USG results showed a lot of pericardial effusion, a hyperechoic lesion with clear boundaries, a round, oval shape, lobulated with enormous size, vertical, right vertical, left atrium, and right atrium, aortic pressing lesion. The CT scan showed isohypodense lesions in the form of lobulated, partially indistinct boundaries, forced injection of contrast appeared inhomogeneous enhancement, the right lung had an increased bronchovascular pattern, and patches appeared on the dextra lung. Conclusion: The radiological imaging with x-ray, USG, and CT scan revealed that the patient found a mass in the mediastinum with firm boundaries, a round oval, homogeneous structure, fluid in the pericardial effusion, flat edges, and a homogeneous structure. Based on the findings, it can be concluded that radiological imaging can play a role in detecting germ mediastinum tumors.
