Abstract

Developments in the industrial world cause many problems that are important to study considering the potential hazards that can result in the risk of health problems and even work accidents.PT. Sami Surya Perkasa is a factory engaged in manufacturing. An industry requires an Occupational Health and Safety (K3) system with the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method which, if applied, can easily find hazardous work activities and can be carried out immediately. control according to the level of risk (urgent, high, medium, and low). This research was conducted at PT.Sami Surya Perkasa and aims to know the identification of hazards, risk assessment, and risk control, especially in the Cutting process. The results showed that there were 18 potential hazards in 13 types of work in each. Controls that can reduce the level of risk are by using Personal Protective Equipment (PPE) for workers and a safe work attitude, so that workers can safely perform work activities.The purpose of this study was to determine whether the shift workers of PT.Sami Surya Perkasa's production unit violated the rules for using PPE. Abstract

Usaha milik desa sebagai salah satu kegiatan bagi tingkat desa, juga dapat menjadi sumber pemasukan bagi peningkatan fasilitas maupun layanan pada tingkat desa. Badan usaha milik desa merupakan badan usaha selayaknya badan usaha milik negara atau BUMN. Permodalan badan usaha milik desa berasal dari pemerintah kabupaten yang disalurkan melalui pemerintah desa. Desa Pudungsari, sebagai desa yang memiliki mata pencaharian utama petani, pedagang dan pegawai. Sebagai wilayah yang mata pencaharian utama sebagai petani, Desa Pudungsari memanfaatkan kesempatan dan peluang dalam pendirian BUMDes dalam bidang pertanian. Abstract

PT XYZ is a metal industry company that produces drinking water pipe joints with specifications of gray cast iron and round graphite cast iron. The problems that exist in PT. XYZ are the lack of implementation of occupational safety and health in each process area of the production floor with the aim of research analyzing potential hazards, knowing risk level assessments, and controlling recommendations. The HIRA method calculates that there are 3 risk categories for occupational accident risk assessment with 10 high risk categories with a percentage of 41.66%, 12 moderate risk categories with a percentage of 50% and 2 low risk categories with a percentage of 8.33%. The JSA method results from calculating KPIs for low risk categories with a range of 0 to 2.33, medium risk categories with a range of 2.33 to 4.66, high risk categories with a range of more than 5.25. Calculation of the T-Test Test obtained the difference value before the control recommendation and after the control recommendation was 7.875 and the t value was 7.814 by obtaining a p-value of 0.00 so it is less than an alpha value of 0.05.

Abstract

Azhari Bakar Bread Factory is an MSME domiciled in Rejowinangun, Kotagede, Yogyakarta. In the production process, this factory can produce 7807 pieces of bread in a month, of the total production that month 57 pieces of bread are defective. To reduce defects, quality control is needed. Quality control aims to produce products in the form of goods or services that comply with the desired and planned standards, as well as improve the quality of products that do not comply with predetermined standards and possibly maintain appropriate quality. This research uses the Six Sigma and Kaizen methods by showing factual data and comparing it with existing theories to find things that need to be added, improved, or maintained. This research found that the sigma process capability calculated for 1 month was 2.97 with a possible damage of 3661.21 for one million production. From these results, it can be said that the company has not implemented quality control completely properly and correctly because it is still far from the target level of 6 sigma, therefore the company needs to carry out quality control on defective products. The factor that causes production defects is the human factor. machines, methods, materials, and environment.

Abstract

. PT. IGP Internasional Bantul is a company engaged in manufacturing. This company produces several custom barbies. Product production level at PT. IGP International Bantul is very high. The problem is that the company has not achieved the production targets that have been set. In 2022 to 2023, in several months, namely August 2022, the target number was 4,454,588 pcs for a month, but 4,333,029 pcs were produced. This research aims to determine the level of productivity measurement at PT. IGP International Bantul uses the OMAX method and identifies factors that can influence productivity in the production of Barbie clothes at PT. IGP International Bantul using the RCA method. The data processing stage is carried out at PT. IGP International Bantul by identifying types of problems using the Objective Matrix (OMAX), Root Cause Analysis with weighting based on the Analytical Hierarchy process (AHP) method. So the largest productivity value was obtained in July 2022, namely 135%, while the lowest productivity index was in February and April 2023, namely -35%. Then to find the root of the problem in the company using the Root Cause Analysis (RCA) method. The tools to find the root of the problem used in the RCA method are the 5-why and fishbone diagrams which produce two main problems, namely lack of training for operators and lack of maintenance on machines.

Abstract

Occupational safety and health (K3) programs are important in maintaining the physical condition and cooperative attitude of employees within the company. Law No. 1 of 1970 regulates the importance of protecting work safety for every worker. UMKM tofu owned by Mr. Kadek, a micro and small industry, faces challenges in implementing OSH for its workers. The humid and slippery working environment, the risky waste disposal system, and the lack of use of personal protective equipment (PPE) cause accidents and illnesses for workers. The results of the interviews indicated that there were significant work accident cases, which had an impact on decreasing employee productivity and income levels. The rate of work accidents at MSMEs knows Mr Kadek has increased significantly from 2021 to 2022. The frequency of accidents has increased from 577 to 915 per million hours worked, while the severity of disability injuries has increased from 2,090 to 2,820. The impact of work accidents also affects work productivity. In 2022, there will be an increase in the number of working hours lost due to accidents, namely 154 hours. This shows that frequent work accidents have a negative impact on productivity levels. The high rate of work accidents also negatively affects the company's downfall. Improvements are needed in the application of OSH, such as ergonomics training, active supervision, waste management, personal protection, facility improvements, and safety awareness. In this case, Fault Tree Analysis is used to identify the causes of work accidents. In order to increase the level of productivity and employee safety, it is necessary to have a K3 improvement program in MSMEs, Mr. Kadek knows.

Abstract

Dipo Bakery is a small and medium business engaged in the food sector, namely the sweet bread processing industry. The average number of production during October 2022 - March 2023 was 48,941.68 pcs, the average overproof defective product during October 2022 - March 2023 was 2,688.16 pcs, the average defective product size did not match during October 2022 - March 2023 as many as 4,949.33. Statistical Quality Control is a technique used in controlling and managing both manufacturing and service processes using statistical methods. Failure Mode and Effect Analysis (FMEA) to find out the main causes of product defects and provide appropriate improvement suggestions. Based on the results obtained, it shows that there is no data that crosses the upper control limit (UCL) for overproof defects at 0.168844 and for defects of unsuitable size at 0.251876 as well as the lower control limit (LCL) for overproof defects. at point – 0,. So, it can be concluded that the damage to the production process of the sweet bread variant is still under control within the control limits. obtained the highest RPN value of 105 with the cause of haste.

Abstract

Menurut data dari BPJS, kecelakaan kerja yang terjadi di Indonesia pada bulan Januari hingga November 2022 sebanyak 265.334 kasus. Jumlah ini meningkat 13,26 persen dibandingkan 234.270 kasus pada tahun 2021. Data tersebut menjadi peringatan keras bahwa pelaksanaan keselamatan dan kesehatan kerja (K3) harus semakin menjadi perhatian. UD Cantenan menggunakan beberapa mesin, bahan-bahan kimia dan suhu yang tinggi, dan hampir seluruh proses dilakukan masih secara manual tanpa alat bantu dan dioperasikan langsung. Berdasarkan obeservasi yang dilakukan terdapat 34 risiko kecelakaan kerja yang terdapat pada bagian proses produksi dan tercatat ada 14 kasus kecelakaan kerja yang terdapat pada bagian produksi di Ud Cantenan, data tersebut diambil dari bulan Febrari tahun 2020 hingga bulan Juli tahun 2023. Metode JSA digunakan untuk menentukan pekerjaan yang dianalisis serta mengidentifikasi risiko pada masing-masing pekerjaan, Metode FMEA difungsikan untuk mengidentifikasi tingkat risiko kecelakaan kerja yang mengukur dari aspek dampak, peluang kejadian dan pencegahaannya dilakukan, sedangkan metode FTA digunakan untuk mengdentifikasi potensi penyebab kecelakaan. Dari hasil analisis data diketahui bahwa 5 failure mode dengan nilai RPN terbesar yaitu, terkena tumpahan logam cair,, tergulung putaran chuck mesin turning, tersengat listrik, tergulung putaran mata pahat milling dan mata terkena sekrap logam sisa produksi, ke lima failure mode dengan nilai RPN tertinggi yaitu fm 14, fm 27, fm 31 dengan nilai RPN 75 dan fm 22, fm 28 dengan nilai RPN 64. Nilai RPN tertinggi nantinya akan dilakukan analisis FTA untuk mengetahui akar penyebab masalah dan diberikan rekomendasi untuk perbaikan, untuk FM 14, 21, dan 38 memiliki tiga akar masalah, sedangkan fm 27 dan 28 memiliki 4 akar masalah.

Abstract

Pekerjaan pemotongan hewan merupakan sebuah pekerjaan yang memiliki pontensi bahaya dan dapat menimbulkan terjadinya kecelakaan kerja. Kecelakaan kerja tersebut dapat menimbulkan bahaya bagi pekerja, perusaahaan, dan bahkan masyarakat sekitar. Pemotongan Ayam UD. XYZ ini perharinya dapat menerima pesanan sekitar 100 hingga 250 ekor ayam dan pada saat proses produksi tentunya diperoleh sejumlah risiko kecelakaan bagi pekerja yang terlibat. Oleh karena itu, tujuan dari penelitian ini adalah untuk mengetahui hasil analisis data kecelakaan kerja dengan metode JSA dan HIRARC serta untuk mengetahui rekomendasi pengendalian bahaya untuk mengurangi tingkat kecelakaan kerja yang terjadi. Dalam penelitian ini yang menjadi subjek penelitian adalah bahaya yang terdapat di lokasi kerja dan bahaya yang terjadi pada saat proses produksi berlangsung. Sedangkan yang menjadi objek penelitian adalah seluruh kegiatan proses produksi, kondisi lingkungan didalam pabrik dan kinerja pekerja. Hasil dari penelitian ini adalah diketahui persentase tingkatan resiko berdasarkan skala tingkat kemungkinan kejadian (likelihood) dan keparahan yang dapat ditimbulkan (concequences). Nilai presentase untuk masing masing tingkat risikonya adalah Low Risk sebesar 27,78%, Medium Risk sebesar 50%, High Risk sebesar 16,67% dan Extreme Risk sebesar 5,55%. Rekomendasi pengendalian yang dapat diterapkan berdasarkan segitiga hirarki pengendalian adalah menghilangkan (elimination), rekayasa teknik (engineering control), administrasi (administrative control), dan pemberian APD (PPE)

Abstract

PT XYZ merupakan perusahaan di wilayah Yogyakarta yang memproduksi saos sebagai produk utamanya. Saat ini sistem perencanaan kebutuhan bahan baku perusahaan belum optimal karena tidak memperhitungkan kebutuhan masa depan, sehingga perlu melakukan perencanaan tingkat produksi dan pengelolaan bahan baku baik dalam hal pembelian maupun pemakaian yang didahului dengan perhitungan peramalan permintaan konsumen. Titik fokus atau yang menjadi objek penelitian adalah tingkat pemakaian dan kebutuhan bahan baku saos. Pengumpulan data dilaksanakan dengan observasi dan wawancara serta pengolahan data dilakukan dengan metode Material Requirement Planning (MRP) dengan membandingkan teknik lot sizing seperti Lot for Lot (LFL) dan Economic Order Quantity (EOQ). Teknik lot sizing yang menghasilkan biaya lebih rendah adalah teknik LFL, sehingga keputusan yang diambil adalah perencanaan kebutuhan bahan baku di PT XYZ dengan metode MRP akan menggunakan teknik LFL. Perhitungan MRP dengan teknik LFL dapat diimplemetasikan karena lead time bahan baku tidak lama serta pemasok bahan baku tidak mensyaratkan ukuran lot tertentu. Perencanaan kebutuhan bahan baku dengan metode MRP lebih optimal dibandingkan metode konvensional. Penggunaan metode MRP dapat menghemat biaya persediaan hingga sebesar Rp 1.983.657,33 atau hampir 2 kali lipat dari metode konvensional.
