Abstract

Prevention of stunting is a priority development program in Indonesia until 2024. Nutrition in the first 1000 days of life (HPK), which is from the fetus is in the womb until 2 years old, is very important to consider as an effort to prevent the risk of stunting. Health cadres play an important role in self-care management of maternal nutrition during pregnancy as an effort to prevent stunting. The problem faced by health cadres in Talang Benuang Village and Lokasi Baru Village of Air Priukan Subdistrict as a village of 20 special locations (locus) for stunting in Seluma Regency in 2020 is a lack of knowledge about stunting and 1000 HPK so that they do not have sufficient provisions to carry out their role as perpetrators driving the community in realizing health, including the prevention and management of stunting. The goal of PKM is to empower cadres as the front guard in improving nutrition self-care management for pregnant women to minimize the risk of stunting in children under five. The method was to conduct socialization, training, and mentoring cadres in self-care management of nutrition for pregnant women. The result of the activity was an increase in knowledge, skills, and independence of cadres in providing education and assistance to pregnant women in nutrition management self-care. Cadres were expected to apply the knowledge and abilities they have acquired to provide education and assistance to pregnant women in their area. Counseling has proven effective in increasing understanding of parents and cadres regarding stunting. Parents and cadres responded enthusiastically to the material because they gained new knowledge. Therefore, it is necessary to increase socialization and training activities for parents and cadres. Based on the monitoring and evaluation that has been carried out by the implementation team, the recommendation for the follow-up plan proposed by the team is that similar activities must be carried out continuously in the context of health promotion for the elderly.

Abstract

This study aims to provide an overview of the application of diabetic foot spa procedures to optimize foot function after healing diabetic foot wounds. This qualitative research method uses a descriptive case study design. The results of the study indicate that the diabetic foot spa procedure can be applied to people with diabetes mellitus by using therapeutic communication stages which consist of a pre-interaction phase for conducting assessments, diagnoses, and learning action procedures. The introduction orientation phase builds trusting relationships, explanations of actions, and approval of activities. The interaction phase is to carry out the preparation of equipment, environment, patients, and nurses, as well as all implementation of action procedures, and the termination phase is for evaluation, as well as future contracts and follow-up plans. In conclusion, diabetic foot spa can be used as a nursing intervention in foot care to fulfill security and protection needs, focusing on nursing problems and the risk of disrupting skin/tissue integrity. Abstract

Uremik yang dialami pasien chronic kidney diseases (CKD) berisiko menimbulkan keluhan gatal dan kerusakan pada kulit. Edukasi perawatan kulit menjadi intervensi penting dalam penanganan gangguan integritas kulit yang dialami. Tujuan penelitian ini untuk memberikan gambaran penerapan edukasi perawatan kulit untuk mengatasi ganguan integritas kulit pada pasien chronic kidney disease. Metode: Penelitian ini mengunakan desain studi kasus deskriptif dengan mengunakan pendekatan proses keperawatan. Studi kasus ini berfokus pada masalah kolaboratif chronic kidney disease dan fokus pada kebutuhan kemananan dan proteksi dnegan fokus diagnosa keperawatan gangguan integritas kulit serta fokus pada pengembangan standar intervensi keperawatan edukasi perawatan kulit. Partisipan yang terlibat seorang penderita CKD end stage renal diseases (ESRD) yang mengalami gangguan intergitas kulit di RSUD dr. M Yunus Bengkulu. Hasil: Pengembangan aktivitas keperawatan pada edukasi perawatan kulit diawali dengan fokus pengkajian pada kebutuhan proteksi dan keamanan pada diagnose keperawatan gangguan integritas kulit, dengan standar luaran integritas kulit dan jaringan. Didapatkan kulit tampak lebih lembab dan tidak terlalu kering, kemerahan pada kulit sudah jauh berkurang. Simpulan: Edukasi perawatan kulit dapat diterapkan guna mengoptimalkan integritas kulit dan jaringan pada pasien CKD.

Abstract

Background: Diabetic foot ulcers pose a severe complication for individuals with diabetes mellitus, necessitating effective therapeutic support such as albumin nutrition throughout the wound healing process. Derived from Channa striata, a commonly consumed source of albumin in Indonesia, this extract can potentially expedite wound healing.Objective: This study aims to evaluate the efficacy of Channa striata extract nutrition in accelerating the inflammatory, proliferative, and maturation phases of diabetic foot ulcer healing.Methods: This quantitative study employed a pra-experimental design using a single-group pretest-posttest approach. The study involved 30 diabetic patients who were administered a daily dose of 550 grams of Channa striata extract over four weeks. The Bates Jensen Assessment Tool (BWAT) assessed wound progress.Results: Results revealed that the average participant age was 60.2 years, with an average diabetic history of seven years. The average wound duration was 8.7 weeks; 66.7% of the participants were female; 70% had Non-Insulin Dependent Diabetes Mellitus (NIDDM); and 56.7% were at stage IV. The study found a significant difference in BWAT scores before and after the administration of Channa striata extract in diabetic foot ulcer patients.Conclusion: This study supports the effectiveness of Channa striata extract as an adjunct therapy in expediting the wound-healing phases in diabetic foot ulcers.

Abstract

The evolution of wound dressings has shifted from simply covering wounds to maintaining a moist environment. Chitosan, derived from chitin, and Zinc Oxide (ZnO) are well-known for their roles in wound healing. Chitosan is used due to its natural polymer properties, and ZnO is used to promote cell growth. This study investigates the effectiveness of chitosan zinc oxide cream as a primary dressing to enhance wound healing in diabetic ulcers. This quantitative research method uses a clinical retrospective cohort design. The sample used 235 verified medical records from 2018 to 2023, namely medical records of patients with stage III-IV diabetic wounds who received a primary dressing of chitosan zinc cream every 2-3 days and there was also documentation of the results of the bates-Jensen wound assessment at the first visit and a minimum of four weeks of treatment. The analysis included univariate and bivariate analyses to correlate treatment with healing progress. The results showed significant improvement (p<0.001) in wound condition after treatment with chitosan zinc oxide cream, including a reduction in wound size and depth, faster removal of necrotic tissue, and increased granulation and formation of epithelial tissue. These findings highlight the efficacy of the cream in treating diabetic ulcers, although further research is needed for other types of cancerous wounds, pressure ulcers, and chronic wounds.

Abstract

Abstrak: Masalah: Perbedaan standar pelayanan dan masih banyaknya penerapan metode konvensional dalam perawatan luka tidak sesuai dengan tuntutan peran layanan primer dalam upaya pencegahan infeksi luka operasi. Tujuan: Pengabdian Masyarakat ini bertujuan meningkatkan pengetahuan dan keterampilan Perawat dan Bidan dalam emmberikan pelayanan perawatan luka paska operasi di wilayah kerja Puskesmas Jalan Gedang, Puskesmas Lingkar Timur, Puskesmas Lingkar Barat, dan Puskesmas Sidomulyo. Metode: Kegiatan Pengabmas dilakukan dengan memberikan pelatihan 15 JPL dengan metode teori, praktik laboratorium, dan praktik lapangan melalui pendampingan langsung kepada 15 perawat dan bidan Puskesmas yang berada di wilayah kerja Kecamatan Gading Cempaka. Evaluasi terhadap hasil capain kegiatan yaitu peningkatan pengetahuan yang diukur dengan kuisioner sedangkan indicator ketepatan keterampilan yang diukur dengan metode observasi menggunakan daftar tilik, sedangkan kepuasaan pasien diukur secara kualitatif dengan metode wawancara. Rerata nilai pengetahuan setelah pelatihan dan pendampingan yaitu 92 dengan SD 3,723, dan rerata nilai keterampilan 93,87 dengan SD 4,172, serta sebanyak empat pasien menunjukkan proses penyembuhan luka yang sangat baik tanpa terjadi komplikasi dan mengaku puas dengan perawatan yang diberikan.Abstract: Problem: The difference in service standards and the number of applications of conventional methods in wound care are not to the demands of the role of primary services in efforts to prevent surgical wound infections. Objective: This Community Service aims to improve the knowledge and skills of Nurses and Midwives in providing postoperative wound care services in the work area of the Jalan Gedang Health Center, East Ring Health Center, West Ring Health Center, and Sidomulyo Health Center. Method: Community Service activities were carried out through 15 hours of training with theoretical methods, laboratory practices, and field practices through direct assistance to 15 nurses and midwives of the Health Center in the work area of Gading Cempaka District. The evaluation of the activity results was an increase in knowledge, which was measured by a questionnaire. In contrast, the indicator of skill accuracy was measured by the observation method using a list of points, and patient satisfaction was measured qualitatively by the interview method. The average value of knowledge after training and mentoring was 92 with SD 3,723, and the average skill score was 93.87 with SD 4,172. As many as four patients showed an excellent wound-healing process without complications and were satisfied with the treatment.

Abstract

Introduction: One of the most common cardiovascular diseases with a high morbidity and mortality rate is congestive heart failure (CHF). Patients with CHF often experience leg edema, which can cause discomfort and increase the risk of skin health issues. Therefore, nursing interventions are necessary to address the problem of leg edema in CHF patients. Methods: This study aims to provide a framework for the application of contrast bath therapy and 30-degree foot elevation procedures. This qualitative study employs a descriptive case study design with a therapeutic communication process approach, involving one participant with leg edema caused by CHF. Results: The implementation of the procedure begins with the pre-interaction phase, which involves assessing the patient's safety and protection needs and formulating a diagnosis regarding the risk of skin integrity damage. During the orientation phase, nurses provide therapeutic greetings, introductions, and obtain informed consent. In the interaction phase, the patient’s feet are soaked in warm water at 39°C for 3 minutes, followed by a soak in cold water at 16°C for 1 minute, then another soak in warm water at 39°C for 3 minutes. Afterward, the feet are dried with a towel, and the patient’s feet are positioned at a 30-degree elevation. The termination phase involves evaluating the patient's complaints and measuring the degree of edema. Conclusions: The contrast bath procedure and the 30-degree foot elevation, when combined with the therapeutic communication stages, can be effectively applied to treat edema in patients with congestive heart failure.

Abstract

The most common complication of diabetes is neuropathy. Diabetic foot screening includes secondary prevention efforts against further complications from diabetes. The need for practitioners to have practical, simple, and affordable sensory neuropathy screening techniques that can be independently is a demand in today's health services. A study aimed to assess the sensitivity and specificity of the ipswich touch test (IPTT) to the 10g monofilament test in the early detection of diabetic neuropathy. Methods: This quantitative observational research design used a cross-sectional method with 61 respondents who had diabetes mellitus for more than one year in the working area of the Pasar Ikan Health Center, Bengkulu City. Samples were taken by consecutive sampling. The researcher conducted an ipswich touch test (IPTT) first and then an examination with a monofilament test of 10g as the gold standard. The data is processed and analyzed with a computer application to see the sensitivity and specificity values. Ipswich touch test (IPTT) had a sensitivity value of 91.3%, specificity of 92.1%, positive predictive value of 87.5%, negative predictive value of 94.59%, positive probability ratio of 11.55, negative probability ratio of 0.09 and AUC value of 91%. The ipswich touch test (IPTT) has the same sensitivity, specificity, and accuracy as the monofilament test.

Abstract

People with type 1 diabetes mellitus need insulin therapy to maintain blood glucose stability and improve their quality of life. Nurses have an important role in insulin medication management interventions to achieve therapeutic goals. Objective: This study aims to provide an overview of the application of evidence-based practice nursing activities for developing insulin medication management interventions to improve blood glucose stability in patients with type I diabetes mellitus. Method: This descriptive case study uses a nursing process approach carried out on one young person with type 1 diabetes mellitus at the Regional General Hospital (RSUD) Dr. M Yunus Bengkulu. The nursing process is carried out for three days while the patient is treated in the internal medicine room. Data were collected through interviews and observation. Results: Nursing activities started with assessing the patient's nutritional and fluid needs and continued to data analysis. The analyzed data shows that >80% of the major and minor data are listed in the nursing diagnosis guidelines, namely the Indonesia Nursing Diagnostic Standards (SDKI). From the data obtained, the diagnosis that can be raised is the instability of blood glucose levels with the criteria of nursing outcomes of the level of knowledge with increased expectations. On the last day of the nursing evaluation, there was an increase in the level of knowledge from moderate to increase in the respondents. Conclusions: Applying nursing activity development to insulin medication management intervention effectively achieves blood glucose stability in patients with type 1 diabetes mellitus.
