Abstract

Objective: Aim of this study was to describe urodynamic utility pattern among Urologists in Indonesia especially in treating lower urinary tract symptoms (LUTS), urinary retention, overactive bladder (OAB), stress urinary incontinence (SUI) and urologic pediatric cases. Materials & methods: Subjects were Indonesian Urologists who attended urological scientific meetings or workshop in Jakarta between February-July2014. They were given questionnaires about urodynamics and its indications. Results: One hundred and eight Urologists completed and returned the questionnaires out of 303 Urologists in Indonesia. Thirty eight Urologists worked at hospitals where urodynamic machine is available, the rest sent their urodynamic cases elsewhere. Most of Urologists ordered urodynamics for LUTS patients with neurological deficit (84.3%) and weak anal sphincter tone/bulbocavernosus reflex (62.0%). In OAB cases, urodynamics was used in cases with failure of medical therapy (70.4%) and neurological deficit (68.5%). Two most common indication criterias in SUI cases were failure of conservative therapy (70.4%) and mixed incontinence cases (SUI with OAB) (60.2%). Neurological deficit (66.7%) and urinary incontinence (26.9%) were the most frequent urodynamic indications applied in children. Conclusion: We described the urodynamic utility pattern among Indonesian Urologists. Availability of urodynamic machine, patient economic capabilities, guideline availability on urodynamics could be the factors affecting this pattern.

Abstract

Objective: Urodynamic test is a series of test to evaluate the bladder function including voiding and storage physiology. Several previous studies have found that urodynamic testing had association with physical and emotional discomfort. The primary goal of this study was to determine the parameters of anxiety, pain, and shame related to urodynamic testing. Material & Method: From June 2014 to June 2015, a total of 57 consecutive patients who underwent urodynamic study were included in the present study, consisting of 20 men and 37 women. They have completed the questionnaire before and after urodynamic study. Patients were requested to answer four questions related to pain, embarrassment, anxiety, and knowledge related to urodynamic test. Results: Overall, most of the patients felt not at all and nearly not anxious before urodynamic study. Thirty percent of patient thought that there would be moderate pain when the urodynamic test is performed. Majority of patients (68% of patients) felt no embarrassment prior to the test. Overall, patients felt better after urodynamic study in terms of anxiety and pain. Parameter of embarrassment significantly increased after the test. Patient perception of anxiety and pain before urodynamic test are not significantly different with perception after urodynamic test. Conclusion: Urodynamic tests are very well tolerated by men and women. Majority of patients felt better after urodynamic test according to pain and anxiety score. No statistical difference in level of pain and anxiety between before and after the procedure, while parameter of embarrassment was significantly higher after the test was done.

Abstract

Objective: The purpose of this study is to examine the difference of lower urinary tract (LUT) symptoms in anuric and non-anuric individuals after renal transplantation (RT). Material & Methods: LUT function and symptoms were assessed in subjects who had undergone RT at Cipto Mangunkusumo General Hospital, Jakarta, from November 2016 to June 2017. Subjects were divided into anuric and non-anuric groups. We excluded patients with surgical complications that could not undergo uroflowmetry. Results: Thirty-two (21 male, 11 female) subjects were recruited in this study. The anuric subjects were younger than the non-anuric ones (47 ± 12.82 vs. 51.31 ± 16.33, p < 0.001) There were no significant differences in the International Prostate Symptoms Score (IPSS) (6.5 ± 3.67 vs. 6.25 ± 2.95, p = 0.567), Overactive Bladder Symptoms Score (OABSS) (4.06 ± 2.01 vs 4.12 ± 2.39, p = 1.000), maximum urinary flow rate (Qmax) (20.32 ± 9.04 vs. 22.32 ± 10.31, p = 0.956), post-void residual (PVR) (41.12 ± 37.63 vs. 47.62 ± 38.63, p = 0.361) and voided volume (227.88 ± 112.30 vs 251.06 ± 126.75, p = 0.588) between anuric and non-anuric subjects, respectively. IPSS-voiding symptom, IPSS-storage symptom, and OAB symptom did not differ significantly between both groups (p > 0.05). Thirteen (13/16) and seven (7/16) subjects of the anuric and non-anuric groups were both pleased with their quality of life, respectively. Conclusion: LUT symptoms do not differ between anuric and non-anuric patients.

Abstract

Objective: This study aimed to describe the urodynamic characteristics of post-transplant kidney patients with LUTS who were indicated for urodynamics. Material & Methods: This research is a cross-sectional study conducted at Cipto Mangunkusumo General Hospital between 2011-2017. Data were collected from patients who had undergone urodynamic examination after kidney transplantation due to LUTS/urinary retention. Data were collected from the patient’s medical record. Results: A total of 536 patients underwent kidney transplants at Cipto Mangunkusumo General Hospital from 2011-2017. Eleven patients (2%) developed LUTS and then underwent urodynamic examination with an average age of 41.4 (30.1 ± 52.6) years. Six patients (55%) had type 2 diabetes mellitus (DM) and 5 patients (45%) had hypertension (HT). A total of 6 out of 11 patients (54%) experienced urinary retention of which 4 subjects (67%) had decreased bladder compliance, 4 (67%) patients experienced detrusor overactivity (DO), 3 patients (50%) had bladder outlet obstruction (BOO), while 2 patients (33%) experienced detrusor underactivity (DU) respectively. Of 5 patients without urinary retention, decreased bladder compliance was found in 1 patient (20%), DO in 2 patients (40%), BOO in 1 patient (20%), and no subject experienced DU. In both groups, no subject was discovered to experience any urinary incontinence. Conclusion: Small number of post renal transplantation patients developed LUTS and half of which accompanied by urinary retention. Among these patients, urodynamic examination revealed detrusor overactivity as the most common underlying problem followed by decreased bladder compliance, BOO, and detrusor underactivity

Abstract

Objective: This study aims to see the correlation between occupational burnout and sexual dysfunction in Indonesian female nurses. Material & Methods: A cross sectional study was conducted in Kardinah General Hospital, Tegal, Central Java, Indonesia between January 2022, and March 2022. An anonymous, self-administered online questionnaire was used. Our study subjects are female nurses from outpatient clinics, inpatient wards, high/intensive care unit, emergency unit, and operating theater. We distributed online questionnaires to female nurses who matched our inclusion and exclusion criteria. Occupational burnouts among nurses were assessed using Copenhagen Burnout Inventory (CBI), while Female sexual dysfunction (FSD) was assessed using Female Sexual Function Index (FSFI). Statistical analysis was conducted using IBM SPSS software ver 25.0. Results: A total of 285 nurses participated as samples of this study, 164 nurses (57.54%) were in the low workload group and 121 nurses (42.46%) in the high workload group. The prevalence of sexual dysfunction in female nurses in this study was as high as 87.7% While occupational burnout in high and low workload nurses in our study was 42.2% and 19.5%, respectively. The analysis shows a significant negative correlation between CBI score, sub scores, and burnout status to FSFI score (p < 0.05) although these correlations were weak. Our data proved that there were no independent variables that can become a predictor variable. Conclusion: Married female nurses have a relatively high occupational burnout and are prone to sexual dysfunction. This study showed statistically significant but weak correlation between occupational burnout with sexual dysfunction in married female nurses from the CBI total score, subscores and burnout status with FSFI total score and subscores in terms of lubrication, orgasm, satisfaction, and pain. Abstract

BACKGROUND Stress urinary incontinence (SUI) is uncontrollable urine leakage when intra-abdominal pressure increases. Behavioral therapy with Kegel exercise is currently the best conservative management recommendation for treating SUI patients. This study aimed to investigate the success of supervised Kegel exercises using the Kegel exercises guidebook in women with SUI. METHODS This quasi-experimental study involved both groups being taught the same regiment of Kegel exercises, but the intervention group was given the Kegel exercises guidebook. After 12 weeks, the compliance, subjective, and objective success rates were evaluated through the patient’s notes, Urinary Distress Inventory-6, Incontinence Impact Questionnaire-7, perineometer, and pad test. RESULTS Patients with a higher level of education had lower compliance to Kegel exercises (p = 0.01; odds ratio [OR] 0.38; 0.18–0.79). No significant difference was observed in the subjective symptom improvement between the two groups. The intervention group had a significantly higher success rate based on objective success by analyzing the 1-hour pad test results. The intervention group was also more adherent (p<0.001; OR 4.78; 2.51–9.0). CONCLUSIONS Patients who received the Kegel exercises guidebook were more compliant and more objectively successful than those who did not receive the manuals.
