Abstract

The Indonesian Government established referral hospitals for COVID-19 as an effort in combating the pandemic. Referral hospitals for COVID-19 submit their services’ claims to the Ministry of Health. Hospital X in Bantul experienced challenges in submitting COVID-19 claims and underwent claim disputes which caused delays in payment process. This research aimed to explore the factors that led to the disputes over the COVID-19 claims. This descriptive research was conducted with a qualitative approach, using in-depth interviews and observations. The additional descriptive analysis used secondary data COVID-19 claim dispute file reports from 2020. The results showed that the highest criteria of disputed claims at Hospital X were the criteria for non- compliant guaranteed participants, incomplete claim files, non-compliant comorbid diagnoses, and identities that did not comply with the provisions. The causes of the disputes over claims for patients with COVID-19 included inaccurate history taking, differences in regulation perceptions between the provider and payer, PCR results were not provided, and doctors had a lack of understanding regarding the technical guidelines for COVID-19 claims. In addition, there were technical problems faced by the hospital during the process of submitting claims, including regulations were changed frequently, errors in applications, incomplete medical resumes, and unreadable doctors’ writings. Disputed claims did not affect the hospital cash flow, yet delayed the payment process to health workers, which might harm the quality of services.

Abstract

Policy has focused on the unequal distribution of health workers, while the potential for improvements in efficiency to address this problem has been neglected. This study aims to measure and compare the level of efficiency in the use of the available health workforce for the delivery of selected primary healthcare services among districts/cities in Indonesia, and to identify factors influencing that efficiency. The Data Envelopment Analysis method with output orientation to measure efficiency and Tobit regression analysis was performed to determine the effect of contextual factors. The average score for the technical efficiency of primary health care service delivery throughout Indonesia was 1.29, indicating the potential to achieve on average 29% higher coverage of the selected primary health services if all were as efficient as the most efficient. The average efficiency score in the Eastern Indonesia region was 1.62 and the average in the Java-Bali region was 1.05, indicating higher potential efficiency gains in the Eastern Indonesia region. Access to health care facilities was a consistently significant factor influencing the efficiency of primary health care in almost all regions. This study has demonstrated the potential for significant gains in coverage of key primary care services through improvements in the efficiency of use of the existing health workforce.

Abstract

Purpose: This research aims to get an overview of Programmatic Management of Drug-Resistant Tuberculosis (PMDT) Financing in Lampung in 2021. Methods: This is a mixed-methods research study aimed at determining the amount of financing for PMDT from a payer perspective and assessing the local government's commitment. Financing calculation using Lampung Provincial Health Office, Global Fund, and BPJS Kesehatan financial data. The local government's commitment was evaluated by a focus group discussion with the Lampung Provincial Health Office officer. Results: The total financing for the PMDT in 2021 was Rp 6,072,456,520, with 92.12% of funding sources coming from donors, 6.17% from APBN, 1.71% from JKN, and 0% from the APBD. The activities with the highest costs are the provision of anti-tuberculosis drugs at 35% and additional logistics at 34%. Conclusion: The main challenge in financing PMDT is the high-cost requirement, which the local fiscal capacity doesn't support. It should be an essential concern for local governments to ensure the program's sustainability after the termination of Global Fund funding. The government must enhance local fiscal capacity in the post-pandemic era and promptly develop and implement a national health insurance scheme to finance PMDT.

Abstract

Depresi pasca persalinan (DPP) merupakan isu kesehatan penting yang berdampak pada kesejahteraan ibu, perkembangan anak, dan dinamika keluarga. Berbagai intervensi psikologis non-farmakologis, termasuk Terapi Perilaku Kognitif (CBT), Terapi Kognitif Berbasis Mindfulness (MBCT), serta intervensi berbasis mindfulness, telah diteliti sebagai pendekatan alternatif. Meskipun menunjukkan potensi manfaat, masih terdapat perbedaan dalam desain penelitian, karakteristik responden, serta metode intervensi yang memengaruhi hasil efektivitas. Penelitian ini bertujuan menilai kelayakan dan efektivitas CBT, MBCT, dan mindfulness pada ibu dengan DPP melalui tinjauan sistematis menggunakan daftar PRISMA-SR dan kerangka PICOs. Literatur ditelusuri dari PubMed, ScienceDirect, EBSCO, dan Research Rabbit dengan rentang tahun 2014 2024. Dari 576 artikel, 12 studi kuasi-eksperimental dan uji coba terkontrol acak memenuhi kriteria inklusi, meliputi penelitian di negara maju maupun berkembang. Hasil menunjukkan intervensi CBT, MBCT, dan mindfulness efektif menurunkan gejala DPP, dengan Edinburgh Postnatal Depression Scale (EPDS) sebagai instrumen utama. CBT lebih efektif pada tahap awal, sedangkan MBCT berfungsi sebagai terapi lanjutan dan pemeliharaan. Secara keseluruhan, intervensi psikologis individu lebih hemat biaya dibanding perawatan standar. CBT dan MBCT terbukti sama-sama layak sebagai pilihan terapi, namun diperlukan penelitian lanjutan untuk menilai efektivitas jangka panjang serta integrasinya dalam layanan kesehatan ibu.

Abstract

Purpose: JKN has been implemented since 2014; however, there has been no change in the value of access to and utilization of medical rehabilitation services, particularly physiotherapy, in FKRTLs in Indonesia. This research aims to identify the factors that influence the pattern of outpatient physiotherapy services received by stroke patients at BPJS Kesehatan during the treatment period. Methods: This study employs an analytical approach using secondary data from the 2015-2016 BPJS Health Data Sample, with a cross-sectional study design. The data were analyzed using univariable and bivariable analyses with the chi-square test, as well as multivariable analysis with logistic regression, with an α level of 0.05. Results: The analysis of patients with stroke cases yielded only 1,641 participants who visited physiotherapy services, and 3 participants from stroke cases who visited rehabilitation services, out of the total participants of BPJS Health in 2015-2016. Frequency of visits to health services for stroke cases among BPJS Kesehatan participants, based on gender, age, province of residence of the participant, type of participation, type of stroke, stroke comorbidity, type of hospital, and cost of claims. Conclusion: This study showed that only a very small number of stroke patients attend rehabilitation services. The utilization services are related to sex, living area, type of stroke, and comorbidities.

Abstract

Purpose: This study describes the utilization of healthcare facilities before and during the COVID-19 pandemic and the expenditure of national health insurance at primary and secondary healthcare facilities. Methods: This is a descriptive study with a cross-sectional design. We utilized claim data from the National Health Insurance from January 2018 to June 2021. The population included in this study were all Indonesian citizens who contracted COVID-19. Both the Ministry of Health and National Health Insurance expenditure for COVID-19 services were captured. The data were analyzed in frequency and proportions. Results: There was a consistent utilization trend across all types of healthcare facilities (primary vs. secondary; public vs. private), with an upward trend before the Covid-19 pandemic, a downward trend in 2020, and a slight increase before June 2021. Primary healthcare utilization jumped by 19% during the 2018-2019 period and dropped by 34% from March to April 2020. A similar trend was observed in the secondary healthcare facilities, with the utilization cost being lower in the 2020 and 2021 periods than in previous years. Hospital visits also showed a decreasing trend during the start of the pandemic (both public and private-owned), which was linear to the expenditure of inpatient and outpatient visits. The total claim for COVID-19 health care increased to IDR 77.8 trillion by August 2021 (inpatient and outpatient), with the highest cost per patient for inpatients with comorbidities. Conclusion: Healthcare utilization decreased significantly during the COVID-19 pandemic period. On the other hand, since COVID-19 increased expenditure, particularly due to inpatient services and comorbidities, National Health Insurance needs to prepare a health financing strategy to anticipate the shift of financing in time of the endemic.

Abstract

Purpose: Participation in JKN (Jaminan Kesehatan Nasional) is one of the factors that has a positive effect on healthcare utilization. The utilization of JKN has continued to increase, reaching 276.1 million visits in 2019. However, the COVID-19 pandemic has caused disruptions in the utilization of healthcare. Visits to health facilities are reported to have decreased, with 224.7 million visits in 2020. This research aims to describe utilization patterns based on the type of health facilities visited and determine the relationship between JKN participation and other factors on health service utilization during the COVID-19 pandemic. Methods: Descriptive analytical study with a cross-sectional design using secondary data from the Health and Demography Surveillance System (HDSS) 2020 cycle 6. The quantitative data analysis was conducted using univariate, bivariate, and multivariate analyses. Results: 69.77% of respondents used healthcare, while 30.23% did not. Other health facilities, particularly pharmacies, were the most commonly used choices by respondents to address their health problems. Not being sick enough to receive treatment (83.94%) and fear of contracting COVID-19 (11.92%) were the primary reasons respondents cited for not accessing healthcare. There was no significant correlation between the variables of JKN involvement, age, gender, education level, marital status, economic status, and residence location and the use of healthcare services (p > 0.05). The most critical variable was a history of NCD (p<0.05). Conclusion: The need factor, combined with the history of NCD, is the primary predictor of health service utilization among the people of Sleman Regency during the COVID-19 pandemic.

Abstract

Purpose: Identify the effectiveness between Yoga and birth ball in decreasing labor pain intensity. Methods: This study used a systematic review design with the PRISMA-SR checklist. This systematic review referred to three steps, namely, (1) search strategy, (2) selection criteria, and (3) data extraction and quality assessment. The article search used 2 databases: ScienceDirect and Pubmed and ResearchRabbit. Results: Of 14 articles, 7 discuss Yoga's effectiveness in diminishing labor pain, and another 7 discuss birthing balls' efficacy in alleviating labor pain. Yoga and birth balls effectively reduce childbirth pain intensity. Conclusion: Besides, a combination of Yoga and birth ball or a combination of birth balls and counterpressure or free position when entering childbirth reduces labor pain. Yoga could also be combined with back massage, aromatherapy, or hypnosis. Future studies are expected to analyze the level of effectiveness between birthing balls and Yoga in reducing labor pain intensity in both developing and developed countries.

Abstract

Purpose: The use of long-acting reversible contraception (LARC) methods, implants, and IUDs, are assessed for cost-effectiveness, which compares cost-effectiveness in technology, environment, and health. The aim is to critically evaluate the economic evaluation of IUDs and implants based on previous scientific evidence, with a review to discuss cost-effectiveness. Methods: The literature review used was a systematic review, and PRISMA-SR 2020 was chosen as a reference for preparing the literature study. The authors used three databases, PubMed, Science Direct, and Wiley, to find scientific evidence of the cost-effectiveness of IUDs and implants. Results: The search in Pubmed (n=119), ScienceDirect (n=1,228), and Wiley (n=83) databases included (n=1,510) articles. Eighty-two articles were assessed for eligibility, and only seven were eligible according to the inclusion criteria for further review. Conclusion: IUDs and implants are two highly effective long-term contraceptive methods for preventing pregnancy, so using IUDs is more cost-effective than implant contraception based on scientific articles that meet the inclusion criteria.

Abstract

Purpose: This study aimed to elucidate the knowledge, attitudes, and practices of older adult caregivers in Ngestiharjo, Bantul, Yogyakarta, Indonesia, towards older adult care and to discern the role of Community Health Workers (CHWs). Methods: A mixed-methods approach was employed between April and June 2023, encompassing quantitative data collection with a cross-sectional approach from 253 older adult caregivers via a pretested questionnaire and qualitative insights with a descriptive approach from focus group discussions with seven CHWs. The data were analyzed using descriptive analysis and thematic analysis. Results: The caregivers were predominantly female (67.2%) and over the age of 50 (46.6%), with the most common occupation being housekeeping (34.8%). Knowledge deficits were noted in several areas, with mean scores of 0.53 for depression signs, 0.56 for physical exercise, 0.73 for sleep quality, and 0.79 for fall risk. Caregivers' sleep quality and fall risk knowledge were better than depression and physical exercise. Attitude scores averaged 82.42 ± 20.42 (out of 125), showing moderate confidence in caregiving activities. Practices related to older adult care showed promising results, especially in maintaining a clean living environment (mean Score: 4.08 ± 0.86). The pivotal role of CHWs in bridging this knowledge gap and facilitating healthcare access was underscored. Conclusion: The study highlights the need for targeted educational programs to improve the knowledge and practices of caregivers, particularly in physical exercise and mental health areas. Strengthening the role of CHWs is essential to enhancing caregiving outcomes.
