Abstract

Pengkajian pertama yang dilakukan pada bayi saat lahir yaitu dengan menggunakan nilai Apgar melalui pemeriksaan fisik singkat. Nilai Apgar yang menurun dapat menyebabkan asfiksia pada neonatus. Keadaan asfiksia dapat menyebabkan ketidakseimbangan suhu tubuh dan terjadi hipotermia yang mengakibatkan berkurangnya aliran oksigen ke jaringan dan neonatus akan mengalami hipoglikemia. RSUP Dr. Mohammad Hoesin Palembang sebagai rumah sakit pendidikan dan rujukan nasional belum memiliki data mengenai hubungan asfiksia dan hipoglikemia ini. Penelitian ini dilakukan untuk mengetahui lebih jauh mengenai hubungan derajat asfiksia dan kejadian hipoglikemia pada neonatus di RSUP Dr. Mohammad Hoesin Palembang. Penelitian ini adalah penelitian observasional analitik dengan menggunakan rancangan potong lintang (cross-sectional study). Sampel penelitian adalah seluruh rekam medik pasien neonatus di RSUP Dr. Mohammad Hoesin Palembang dari Januari 2015 sampai Desember 2015. Analisis pada penelitian ini menggunakan uji Chi-square, bermakna apabila nilai p<? (?=0,05). Dari 106 neonatus, 84,9% adalah asfiksia ringan, diikuti berturut-turut oleh asfiksia sedang (13,2%), dan asfiksia berat (1,9%). Dari 90 neonatus tidak asfiksia dan derajat asfiksia ringan didapatkan 35,6% neonatus hipoglikemia dan 64,4% neonatus tidak hipoglikemia. Derajat asfiksia sedang dan berat didapatkan 37,5% neonatus hipoglikemia dan 62,5% neonatus tidak hipoglikemia. Dari hasil penelitian, tidak terdapat hubungan bermakna antara derajat asfiksia dan kejadian hipoglikemia pada neonatus dengan p=1,000.  Tidak terdapat hubungan bermakna antara derajat asfiksia dan  kejadian hipoglikemia pada neonatus di RSUP Dr. Mohammad Hoesin Palembang.

Abstract

In patients with Chronic Kidney Disease (CKD), renal function in maintaining the homeostasis of various metabolic systems of the body is impaired, one of which is the production of erythropoietin hormones. Anemia is a frequent complication of the CKD and contributes to the increasing morbidity and mortality of the patients. Diagnosis or treatment of anemia in PGK is still a challenge due to multifacorial. Hepcidin, a peptide that has been known as an acute phase protein, plays an important role in the regulation of iron metabolism. Hepcidin is known to have a correlation with ferritin, a parameter of anemia screening which is commonly examined during this time. In addition to the role of pathophysiology anemia in CKD, hepcidin is currently widely researched because it is suspected to contribute also to the pathophysiology of bone mineral disorders in PGK. Current research shows that the concentration of vitamin D is inversely proportional to the concentration of hepcidin and is positively associated with the concentration of hemoglobin and iron. The latest report invitro and invivo support the existence of the interaction between vitamin D and anemia in patients with PGK through the role of Hepcidin. Abstract

Background Chronic kidney disease (CKD) has become a global burden on the healthcare system and significantly impacts the quality of life of children with the condition. Objective To assess quality of life in children with CKD as well as its relationship with sociodemographic, medical, and psychosocial factors. Methods This cross-sectional analytic study was conducted from June to November 2021 at Dr. Moh Hoesin Hospital, Palembang. Children with CKD aged 2–18 years were included by consecutive sampling. Parents and patients were asked to complete the PedsQL™ generic score scale version 4.0 questionnaire. Results We assessed quality of life in 112 children with CKD from parents’ and children’s reports in the PedsQL™ questionnaire. Physical and emotional parameters had the lowest scores. Based on parental reports, quality of life was significantly associated with disease severity (P=0.002), behavioral disorders (P=0.007), and sleep disturbances (P=0.001). Based on the children’s reports, the factors significantly associated with quality of life were anemia (P=0.044), sleep disturbances (P=0.024), and behavioral disorders (P=0.002). Almost one-third of children with CKD had general impairment of quality of life, both from parental reports (32.1%) and children’s reports (33.0%). Conclusion Disease severity, anemia, sleep disturbance, and behavioral disorders were all associated with poorer quality of life in children with CKD.

Abstract

Reducing the incidence of stunting is one of the goals of the 2025 Global Nutrition Targets. The prevalence of stunting in children in Indonesia is still high over the last decade. Lack of knowledge on nutritional intake, maternal nutritional status factors, breastfeeding practices, complementary feeding practices, and exposure to infections as well as other factors including education, food system, health care, and sanitation are important determinants of child stunting in Indonesia. This community service activity is carried out with the aim of providing counseling about stunting to mothers and measuring the nutritional status of toddlers as an effort to prevent stunting. The activity was coordinated by students who are members of the Student Executive Board of the Faculty of Medicine, Sriwijaya University (FK Unsri) and Muhammadiyah University (FK UM), involving FK Unsri lecturers, in collaboration with staff at the Puskesmas Taman Bacaan Palembang. At the end of the activity, participants became more aware of the importance of measuring children's nutritional status and stunting prevention measures. There were 6 toddlers who were indicated to be malnourished and reported for management by the Puskesmas team so that they did not progress to stunting.

Abstract

Child development disorders can affect the next stage of growth and development and can have an impact on the future. Good and proper care for children who are in the growth and development period will help children to grow healthy and be able to achieve their optimal abilities so that they can contribute better to society. The main objectives of this community service are to improve the skills of Early Childhood Education (ECE) teachers to detect early growth and development disorders, reduce the number of advanced growth and development disorders, and optimize child growth and development with early intervention. The method of service carried out in this activity is in the form of training on the use of the PRIMAKU application for 1000 ECE teachers in Palembang City and assessed using pre-test and post-test questionnaires. The results of the counseling showed an increase in results in two benchmarks, an increase in participant knowledge based on the results of the pre-test and post-test correct answers by 17.03%. Furthermore, it is expected that PAUD teachers who have attended the training can apply the use of the PRIMAKU application for early detection of growth and development disorders or delays in early childhood.

Abstract

Thalassemia is a genetic disorder that requires regular blood transfusions, often resulting in iron overload. Iron chelation therapy with deferiprone is commonly used to manage the iron overload, yet the relationship between deferiprone dosage and ferritin level reduction remains unclear. This study aimed to find out how the dose of deferiprone affects the reduction of serum ferritin levels in children with thalassemia major. A cross-sectional analytical study was conducted involving 69 paediatric patients with thalassemia major receiving deferiprone therapy at Dr. Mohammad Hoesin Central General Hospital, Palembang. Data on patient demographics, transfusion frequency, deferiprone dose, and serial ferritin levels (3–6 months apart) were collected. The optimal dose cut-off was determined using ROC analysis, and statistical association was tested using the Chi-Square test. Most patients (56.5%) received deferiprone doses ≤77.95 mg/kgBW. Only 36.2% achieved a ferritin reduction of ≥20%, while 50.7% experienced increased ferritin levels. No statistically significant association was found between deferiprone dose and ferritin reduction (p = 0.051), although a trend toward lower success in the high-dose group was observed (PR 0.355, 95% CI: 0.124–1.019). The average dose of deferiprone administered in this study was below the minimum dose recommended by the FDA, which may explain the lack of significant association with serum ferritin reduction. Further research with controlled dosing and exclusion of confounding variables is recommended to evaluate deferiprone effectiveness more accurately

Abstract

The canteen is one of the supporting facilities on campus. The availability of nutritious menus is of great assistance in meeting the nutritional needs of the academic community in support of its learning activities. However, not all campus canteens meet the criteria for healthy canteens. Students' input is required for the establishment of a healthy canteen, especially regarding the menu. This study aims to explore the canteen menu preferences of medical students at Universitas Sriwijaya to develop a healthy canteen. This study is an observational survey, and Google Form is used to collect data through an online questionnaire. The majority of the 646 samples (72.4 %) were females, lived within the family house (52.8 %), had meals from the canteen (70.1 %), were likely to buy ready-to-eat food (65 %), and spent their daily pocket money between IDR 20,000 to 30,000 (34.1 %). A part of them was pleased with the former canteen's menu, including flavor, presentation, menu variants, accessibility to desired menus, pricing, and nutrition content. The majority of students prefer to purchase the main menu (85.1 %), drink without added sugar (74.8%), get the seller's predetermined food portion (54%), and select a canteen that regulates unhealthy menu limitations (46.9 %), which provides a complete menu list with prices and nutritional content (49.8 %). Most students prefer menus that fill them up, no added sugar drinks, predetermined menu portions, limiting menus that are considered unhealthy, and clear nutritional content and price information on the menu list.

Abstract

Background Children with thalassemia are at risk for kidney dysfunction due to chronic anemia, frequent blood transfusions, iron retention, and use of iron chelating agents. Cystatin C, an endogenous marker for assessing estimated glomerular filtration rate (eGFR), a novel biomarker for the early detection of kidney failure, has been reported to be potentially superior to the commonly used serum creatinine. Objective To evaluate various creatinine- and cystatin C-based formulas for eGFR to detect kidney dysfunction in children with thalassemia. Methods This was a cross-sectional study on children (age <18 years) with thalassemia. Kidney dysfunction was defined as eGFR <90 mL/minute/1.73 m2. Hyperfiltration was defined as eGFR >150 ml/minute/1.73 m2. This study compared the proportion of kidney dysfunction as determined using various creatinine- and cystatin C-based eGFR formulas, comprising the creatinine-based Schwartz formula, the cystatin C-based Filler formula, the creatinine-cystatin C-based New CKID formula, and the creatinine-cystatin C-based Schwartz formula. Results The median age of the 152 study subjects was 11.0 (range 2.0-18.0) years. When using the creatinine-based Schwartz formula, none of the subjects had kidney dysfunction. Kidney dysfunction was found in 21.7% of subjects when using the cystatin C-based Filler formula, in 26.3% of subjects when using the creatinine-cystatin C-based (New CKID) formula, and 59.9% of subjects using the creatinine-cystatin C-based Schwartz formula. When using the creatinine-based Schwartz formula, 38.2% subjects experienced hyperfiltration no hyperfiltration was found by when using other eGFR formulas. There was low correlation between creatinine and cystatin C (r=0.195; P=0.016). There was only mild agreement in eGFR between the creatinine-based Schwartz formula and the cystatin C-based Filler formula (k=0.195; P<0.001). Conclusion The proportion of kidney dysfunction in children with thalassemia based on eGFR calculation using cystatin C- and creatinine-cystatin C-based formulas ranged from 21.7% to  59.9%. No kidney dysfunction was found using a creatinine-only-based eGFR formula, whereas hyperfiltration was a common finding. Hence, more than one parameter should be considered for early detection of kidney dysfunction in thalassemia.
