Abstract

Asphyxia still remain a major causes of neonatal death per year. Apgar score provides information on neonatal vitality but has several limitations. Perinatal asphyxia can arise from a variety of intrapartum and postpartum risks which result in cessation of gas exchange. Anaerobic metabolism will cause lactate accumulation and pH decrease. Analysis of acid-base and lactate in cord blood can provide an objective assessment of the metabolic status of the neonate.

Abstract

Sepsis is a major cause of neonatal morbidity and mortality. Coagulation dysfunction is a complication that can manifest clinically or subclinically in intravascular coagulation. D-dimer, a cross-linked fibrin degradation product, is increased in neonates with sepsis.

Abstract

Introduction: Type 1 diabetes mellitus is a systemic disorder resulting from impaired glucose metabolism characterized by chronic hyperglycemia. Caused by damage to pancreatic β cells either by autoimmune or idiopathic processes resulting in reduced or even halted insulin production. Diabetes mellitus is associated with a decrease in calcium with impaired intestinal calcium absorption and hypercalciuria which causes a decrease in calcium absorption into the body. Method: This study used a prospective study method to assess the correlation between HbA1C levels and calcium levels in children with type 1 diabetes mellitus. This research was conducted  in the Endocrinology Outpatient Unit of Haji Adam Malik Hospital Medan, May 2022 to December 2022. The samples were children aged 1 month to 18 years. Results: Correlation is measured using Pearson correlation analysis for normally distributed data while Spearman correlation is used for data that was not normally distributed. There is a significant correlation between the HbA1c and calcium levels with a p value of 0.026 and a correlation value of (r= -0.445). The correlation value is negative, meaning that every increase in HbA1c levels will be followed by a decrease in serum calcium levels in children with type I diabetes, with moderate strength (r= 0.4 – 0.6). This effect is not influenced by diseases duration and onset of diabetes. Conclusion: There is a decrease in calcium in type 1 diabetes

Abstract

Latar belakang.

Abstract

Neonatal sepsis is one of the most common causes of neonatal death in the first month of life. Neonatal sepsis often shows atypical clinical features include temperature instability, lethargy, skin changes, feeding problems, and many others. Isolation of bacteria from the culture is the gold standard for diagnosing sepsis, but this approach is frequently time-consuming and even in cases of negative blood cultures, sepsis cannot be conclusively ruled out, posing a major challenge in its diagnosis and management. Early diagnosis and timely intervention are essential to improve the prognosis of neonatal sepsis. Aim: The aim of this study is to discuss how infection markers can aid in the early identification of neonatal sepsis and what are the best infection markers to predict neonatal sepsis mortality. Method: This study assessed the diagnostic accuracies of infection marker used in the diagnosis of neonatal sepsis. Included studies were retrieved by searching three major databases (Google Scholar, Pubmed, and Science Direct) using MeSH words and relevant references, and reviewed based on the inclusion/exclusion criteria. The article discusses potential advancements and future prospects for biomarkers. An excellent marker can guide antibiotic administration and duration if sepsis is confirmed, reducing unnecessary use. The marker should have high sensitivity, specificity, positive predictive value, negative predictive value and accuracy.
