Abstract

Childhood is a continuous process of growth and development. Every parent hopes that their child will grow and develop optimally. To meet these expectations, regular growth and development monitoring are required. In support of this, the Indonesian Ministry of Health provides expectant mothers with the Maternal and Child Health (MCH) book, containing comprehensive guidance on maternal and child health. The present health education program aimed to assess the adequacy of MCH book and its practical application. Health workers (n=50) and mothers (n=50) received guidance on using the MCH book and the importance of taking anthropometric measurements to monitor growth in early childhood. Forty-one mother and toddler pairs participated in the MCH book assessment using a validated and reliable questionnaire. Also, they participated in child health consultations and growth monitoring via anthropometric measurements. Categorical data were expressed as proportions, while factors associated with the completeness of the MCH book were analyzed using the chi-square test. Ten toddlers did not have an MCH book, while 18 (58.1%) had a book with a low level of completion. No statistically significant factors influencing the completeness of MCH books were identified. Within the MCH books, growth and immunization charts were most likely to be completed. During the program, we explained that mothers are encouraged to read the MCH book and can enter measurements themselves so that their children’s growth and development are well documented. After the education and assessment sessions, mothers and health workers were aware of which pages of the MCH book should be completed when toddlers are brought to health facilities, whether healthy or symptomatic. This health education program benefited health workers and mothers as it improved their knowledge and ability to use and complete MCH books for monitoring children’s growth and development.

Abstract

The technology development for care and management in intensive care units reduces mortality and increases survivors' survival rate and life expectancy. A significant decrease in mortality rate in patients was related to mechanical ventilation. The hypoxia degree and severity of the respiratory distress are the increase of PaO2/FiO2 ratio and decrease of oxygenation index as predictors of oxygenation sufficiency in arterial blood supply to body tissues. The critical parameter of intensive care unit survivors' quality of life is the Pediatric Quality of Life (PedsQL) Inventory score after survival from the pediatric intensive care unit (PICU). During six months of research observation, eight patients died, and 13 patients survived among 68 patients admitted to pediatric intensive care unit who have ventilatory support. The mean PaO2/FiO2 ratio (PFR) in pediatric intensive care survivors who underwent mechanical ventilation was 285, and the mean Oxygenation Index (OI) was 5.07. There was no relationship between PFR and OI with PedsQL score in this study, but we have found a significant correlation between PedsQL and the duration of mechanical ventilation and length of stay in the intensive care unit. The longer mechanical ventilation duration and length of stay in PICU, the lower the pediatric quality of life score.

Abstract

Background: Internet use by adolescents has increased rapidly in recent decades. This has been particularly evident during the Covid-19 pandemic due to the shift from face-to-face to online learning. Internet use has become convenient and commonplace. However, negative use can cause internet addiction, which interferes with brain or mental function in everyday life. Purpose: The aim of this survey is to analyze the prevalence of internet addiction and the factors that affect it. Methods: This cross-sectional analytical survey was conducted online with adolescents living in the city of Medan, North Sumatra, from August to November 2021. The demographic data came from a self-reported structured questionnaire, and internet addiction was assessed using the Internet Addiction Diagnostic Questionnaire (KDAI) in Indonesian. The data were processed with descriptive statistics and presented in the form of frequency distribution. Factors affecting the occurrence of internet addiction were analyzed using nonparametric tests. Results: Among the 110 adolescent respondents, the prevalence of internet addiction was 40.0%. The majority of those experiencing internet addiction had a high-school education (56.8%) and were girls (52.3%), firstborn (41.0%), and aged 17–19 (40.9%). The total daily and weekend internet use levels were medium (81.8% and 61.4%, respectively). Total daily internet use time was a statistically meaningful factor affecting the occurrence of internet addiction in adolescents (p = 0.014). Conclusion: The prevalence of internet addiction in adolescents is quite high, and total daily internet use was a factor affecting it.

Abstract

Background: Valvular heart disease is a major contributor to increased mortality. The HARKIT score has been validated to predict mortality after specific valve surgery and the STS score to predict mortality rates in patients after cardiac surgery. Haji Adam Malik General Hospital Medan has never used the HARKIT score and the STS score to predict mortality in patients with post valve surgery. Methods: A cross-sectional study from January 2022-April 2023 at Haji Adam Malik General Hospital Medan. The data used is obtained from patient medical record who had heart valve surgery. The data were analyzed with descriptive statistics and presented the form of frequency distribution. Comparison of the HARKIT score with the STS score to predict mortality rates were analyzed using chi-square test. Results: Among 63 patients who underwent valve surgery, 8 patients died after surgery. The HARKIT score system has good analytical power and very strong discriminant (H-L test P=0.646; AUC=0.916; 95% CI=0.794-1.000), while the STS score has poor analytical power (H-L test P=0.002) but has moderate discriminant (AUC=0.798; 95% CI=0.632-0.964). Conclusion: The HARKIT score is more accurate than the STS score in predicting the mortality rate of post-valve surgery patients at Haji Adam Malik General Hospital Medan.

Abstract

Introduction: The Covid-19 pandemic has had a comprehensive impact, one of which is the availability of food which has an impact on family food preparation and nutritional needs. Nutritional problems in adolescents arise due to inadequate nutritional intake, namely an imbalance between nutritional intake and recommended nutritional adequacy. Nutritional problems that can occur in adolescents are underweight, obesity, and anemia. This study was conducted to determine the description of balanced nutrition knowledge during the pandemic in grade XI & XII students of SMAN 4 Medan. Method: This study is descriptive with a cross-sectional research design. The instrument used in the study was a questionnaire. The questionnaire was made by the researcher himself which was taken from related theories or references. The measuring instrument used in this study was a questionnaire using a google form. Results This study found that the level of knowledge about balanced nutrition during the pandemic in grade XI and XII students of SMA Negeri 4 Medan was in the good category of 194 people (73.5%), the sufficient category of 65 people (24.63%), and the lacking category of 5 people (1.89%). Discussion: Students of grade XI and XII of SMA Negeri 4 Medan have good knowledge about balanced nutrition during the pandemic, this is because students have obtained related theories from lessons at school and students' activeness in exploring related knowledge from the internet. Conclusion: Students of SMA Negeri 4 Medan have good knowledge about balanced nutrition during the pandemic.

Abstract

Drug-resistant epilepsy (DRE) is found in up to one-third of individuals with epilepsy who have received appropriate therapy and this condition causes significant child morbidity and mortality. Epilepsy that appears at an early age is at higher risk of developing DRE. Objective to assess the effect of age on the characteristics of seizure severity in children with DRE. This study used a cross-sectional design involving 36 DRE patients aged 2-18 years at the Child Neurology Polyclinic, Adam Malik Hospital Medan, from September to October 2024. Seizure severity characteristics were assessed using the Global Assessment of the Severity of Epilepsy (GASE) questionnaire instrument. The categorical data are presented in the form of proportions and the effect of age on seizure severity characteristics was analyzed using the chi-square test. There were 36 children with DRE in this study, predominantly aged ≥ 10 years. Bivariate analysis showed a significant effect of age in children with drug resistant epilepsy only on the disruption of activity experienced by children with a p value = 0.036 (p <0.05). Most patients aged ≥ 10 years did not experience activity disorders or experienced mild activity disorders (73.9%), while in patients aged <10 years, most patients experienced severe activity disorders (61.5%). The age of children with drug resistant epilepsy have a significant effect on daily activity disorders.

Abstract

Neonatal sepsis is one of the most common causes of neonatal death in the first month of life. Neonatal sepsis often shows atypical clinical features include temperature instability, lethargy, skin changes, feeding problems, and many others. Isolation of bacteria from the culture is the gold standard for diagnosing sepsis, but this approach is frequently time-consuming and even in cases of negative blood cultures, sepsis cannot be conclusively ruled out, posing a major challenge in its diagnosis and management. Early diagnosis and timely intervention are essential to improve the prognosis of neonatal sepsis. Aim: The aim of this study is to discuss how infection markers can aid in the early identification of neonatal sepsis and what are the best infection markers to predict neonatal sepsis mortality. Method: This study assessed the diagnostic accuracies of infection marker used in the diagnosis of neonatal sepsis. Included studies were retrieved by searching three major databases (Google Scholar, Pubmed, and Science Direct) using MeSH words and relevant references, and reviewed based on the inclusion/exclusion criteria. The article discusses potential advancements and future prospects for biomarkers. An excellent marker can guide antibiotic administration and duration if sepsis is confirmed, reducing unnecessary use. The marker should have high sensitivity, specificity, positive predictive value, negative predictive value and accuracy.

Abstract

In children and adolescents, depressive symptoms often presenting as irritability, anhedonia, or behavioral changes. Despite significant advancements in survival rates due to improved therapeutic protocols, the psychological burden experienced by children with ALL.Objective: To evaluate the relationship between age and the presence of depressive symptoms in children diagnosed with ALL.Methods: A cross-sectional study was done using consecutive sampling with 40 children aged from 7 to 18 years old acquired, conducted from August to October 2024 at the outpatient unit of Adam Malik Hospital, Medan. Forty children aged 7–18 years with a confirmed diagnosis of ALL were included. Depressive symptoms were assessed using the Patient Health Questionnaire-9 (PHQ-9). Categorical variables were analyzed using the chi-square test, followed by bivariate to identify significant predictors of depression.Results: Among the 40 participants, the most age group were children ≤ 10 years old (65%). A statistically significant association was found between age group and the presence of depressive symptoms (p = 0.001, OR= 33,3). Conclusions: Age is significantly associated with the severity and presence of depressive symptoms in pediatric patients with ALL. Older children were more likely to report higher levels of depression. Psychological evaluation and targeted mental health support may be especially important in older age groups.

Abstract

Latar belakang. Buku Kesehatan Ibu dan Anak merupakan salah satu upaya pelayanan kesehatan promotif bagi masyarakat yang berfungsi sebagai alat komunikasi, informasi dan edukasi terkait kesehatan ibu dan anak. Pemanfaatan buku tersebut mencakup membawa buku saat pemeriksaan kesehatan, membaca, dan mengisi buku yang dapat meningkatkan pengetahuan ibu mengenai kesehatan anak. Studi mengenai pemanfaatan Buku Kesehatan Ibu dan Anak dan pengetahuan ibu mengenai kesehatan anak masih sedikit.Tujuan. Mengetahui hubungan antara pemanfaatan Buku Kesehatan Ibu dan Anak dengan pengetahuan kesehatan anak pada ibu.Metode. Penelitian cross-sectional dengan wawancara menggunakan kuesioner terstruktur yang valid dan reliabel kepada orangtua dari anak usia 0-59 bulan yang datang ke Posyandu di praktek bidan mandiri periode Maret sampai Desember 2023 secara consecutive sampling. Kriteria inklusi adalah ibu yang memiliki buku dan anak usia 0-59 bulan, dan ibu menyetujui mengikuti penelitian. Kriteria eksklusi adalah anak tidak datang bersama ibu. Hasil. Dari 144 reponden penelitian, pemanfaatannya terbanyak dengan kategori baik (60,4%) dan pengetahuan ibu mengenai kesehatan anak terbanyak dengan kategori cukup (43,8%). Terdapat hubungan bermakna antara pemanfaatan Buku Kesehatan Ibu dan Anak dengan pengetahuan kesehatan anak pada ibu (p=0,001).Kesimpulan. Hubungan antara pemanfaatan Buku Kesehatan Ibu dan Anak dengan tingkat pengetahuan ibu mengenai kesehatan anak bermakna. Oleh karena itu, ibu harus memanfaatkannya lebih sering untuk meningkatkan pengetahuan mengenai kesehatan anak.

Abstract

ABSTRACT Obesity is a public health concern that its prevalence and intensity on children have been increasing with alarming status. Breastfeeding has been considered as one of protective factors that can prevent childhood obesity. The aim of this study is to discover the relationship between history of breastfeeding and obesity on elementary school children. This is an analytical study with cross sectional design. All children in first and second grade of Namira Islamic Elementary School and their mothers were included as population. Ninety samples were recruited randomly. Data on breastfeeding history were collected by using questionnaire guided interview from the mothers. The BMI-for-age curve CDC 2000 was used to determine obesity status of the children (≥ 95th percentile). Data were analyzed using chi-square test. This study found percentage of children with and without history of breastfeeding consecutively is 92.2% and 7.8%. Prevalence of children with obesity is 12.2% and without obesity is 87.8%. Eight of 83 children with history of breastfeeding (72.7%) were obese, and 3 of 7 children without history of breastfeeding (27.3%) were obese (PR=0.2; 95% CI=0.27-0.752;p<0.05). Based on the study, there is a relationship between history of breastfeeding and obesity in elementary school children. Keywords: Breastfeeding, Elementary School Children, Obesity ABSTRAK Obesitas merupakan masalah kesehatan yang prevalensi dan intensitas kejadiannya pada anak terus meningkat serta telah mencapai status yang mengkhawatirkan. Pemberian ASI telah dipertimbangkan sebagai salah satu faktor yang dapat mencegah terjadinya obesitas pada anak. Tujuan dari penelitian ini adalah untuk mengetahui adanya hubungan antara riwayat pemberian ASI dengan kejadian obesitas pada anak sekolah dasar. Penelitian ini dilakukan melalui pendekatan analitik dengan desain cross sectional. Populasi penelitian ini adalah anak Sekolah Dasar Swasta Islam Namira yang berada di kelas satu dan dua dan Ibu nya. Jumlah sampel dalam peneltian ini adalah 90 orang yang dipilih secara acak. Data mengenai riwayat pemberian ASI didapatkan melalui wawancara dengan kuesioner dari Ibu. Kriteria obesitas pada anak ditentukan dengan menggunakan kurva indeks massa tubuh menurut usia dari CDC 2000 (persentil ≥95). Data yang didapat dianalisis dengan uji kai kuadrat. Pada penelitian ini ditemukan 92.2% anak mendapat ASI dan 7.8% anak tidak mendapat ASI saat bayi. Anak yang mengalami obesitas adalah 12.2% sedangkan anak yang tidak mengalami obesitas adalah 87.8%. Anak yang mendapat ASI dan mengalami obesitas adalah 72.7% sedangkan anak yang tidak diberi ASI dan mengalami obesitas adalah 27.3%. Berdasarkan hasil uji hipotesis didapatkan (RP=0.2; 95% CI=0.27-0.752: p<0.05). Berdasarkan hasil penelitian didapatkan bahwa terdapat hubungan antara riwayat pemberian ASI dengan kejadian obesitas pada anak sekolah dasar. Kata Kunci: Anak Sekolah Dasar, ASI, Obesitas
