Abstract

Sindrom Koroner Akut (SKA) sebagian besar disebabkan aterosklerosis. Inflamasi kronis berperan dalam pembentukan plak aterosklerosis. Leukosit dan trombosit berperan pada proses inflamasi. Peningkatan leukosit dan Mean Platelet Volume (MPV) (penanda aktivasi trombosit) menjadi prediktor prognosis kurang baik pada SKA. Tujuan: Membuktikan hubungan rasio Leukosit MPV dengan skor GRACE sebagai prediktor prognosis pasien SKA. Metode: Penelitian potong lintang terhadap 51 pasien SKA onset 4-6 jam di RSUP Dr. Kariadi Semarang telah dilakukan selama bulan Maret sampai April 2019. Perhitungan rasio leukosit MPV dari hasil pemeriksaan hematologi pasien pertama kali saat rawat inap dan pengukuran skor GRACE dari rekam medis. Uji analisis dengan korelasi Pearson. Hasil: Uji korelasi Pearson untuk melihat hubungan antara rasio Leukosit MPV dengan skor GRACE, didapatkan r=-0,088 dan p=0,538 (p>0,05). Analisis subgrup risiko kematian rendah, sedang, tinggi, didapatkan masing-masing r=0,980 p=0,129; r=0,142 p=0,629; r=-0,112, p=0,527. Terdapat korelasi lemah, dapat diabaikan dan tidak signifikan antara rasio leukosit MPV dan skor GRACE (r=-0,088, p=538) pada pasien SKA dengan onset 4-6 jam. Hal ini mungkin disebabkan pengaruh faktor usia dan beberapa komponen dalam skor GRACE tidak berhubungan dengan inflamasi. Simpulan: Tidak terdapat hubungan antara rasio leukosit MPV dan skor GRACE. Abstract

Latar belakang: Operasi tiroid dapat menyebabkan rasa sakit sehingga untuk mencegah masalah ini dengan berbagai modalitas, seperti anestesi regional bilateral superficial cervical plexus block (BSCPB) dengan menggunakan bupivakain 0,25% yang dikombinasikan dengan anestesi umum.Metode: Penelitian ini merupakan penelitian klinis acak tersamar ganda dengan jumlah sampel 36 pasien. Sampel dibagi kedalam 2 kelompok yang diberikan anestesi regional melalui teknik BSCPB dengan kelompok perlakuan diberikan bupivakain 0,25% dibandingkan dengan kelompok kontrol yang diberikan NaCl 0,9% terhadap kadar substansi P serum pre dan post operasi pada pasien yang menjalani operasi tiroid.Hasil: Data penelitian diperoleh subjek laki-laki sebanyak 7 (19,4%) orang dan subjek  perempuan  sebanyak  29  (80,6%)  orang. Data substansi P post operasi kelompok  control dibandingkan kelompok perlakuan  didapatkan perbedaan bermakna  (p = 0,001). Substansi P pre operasi dibandingkan substansi P post operasi kelompok perlakuan didapatkan perbedaan bermakna (p = 0,004). Simpulan: Pemberian bupivakain konsentrasi 0,25% melalui teknik BSCPB terbukti menurunkan kadar substansi P serum post operasi.

Abstract

ABSTRACT Background: expression of VEGF-C and CA 15-3 may be useful to differentiate between malignant and benign breast tumour because VEGF-C plays a role in promoting angiogenesis and lymphangiogenesis in malignant processes and CA 15-3 is the soluble form of transmembrane protein MUC1, a tumour marker which shows higher expression in breast cancer.Objective: to determine the diagnostic value of VEGF-C and CA 15-3 as tumour markers in patients with breast cancer.Methods: this diagnostic study recruited 76 patients that underwent surgical biopsy procedures at Dr. Kariadi and Pantiwilasa Citarum Hospitals Semarang. The VEGF-C and CA 15-3 levels in blood specimens taken before surgical biopsy procedure was determined using ELISA method. An ROC curve and AUC were used to establish the cut-off points and diagnostic value. Pathology examination results from the biopsy specimens were used as the gold standard.Results: the cut-off value for VEGF-C and CA 15-3 were 989.50 pg/mL and 74.00 U/mL. Sensitivity for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 were 76.6%, 64.1% and 89.1%. Specificity for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 were 75.0%, 75.0% and 50.0%. The AUC for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 was 0.831 (95% CI = 0.727-0.934), 0.742 (95% CI = 0.628-0.856) and 0.840 (95% CI = 0.742-0.938).Conclusion: VEGF-C in combination with CA 15-3 is the best diagnostic parameter for breast cancer and has the best accuracy as a tumour marker for breast cancer.

Abstract

ABSTRACT Background: An increase in fat accumulation in obesity has been suggested to link with an increase in inflammation. This inflammation may be associated with an elevated of Lipoprotein-Associated Phospholipase A2 (Lp-PLA2), Apolipoprotein B (Apo B), and Low-Density Lipoprotein (LDL), thereby associated with the risk of atherosclerosis.Objective: To investigate the differences between Lp-PLA2, Apo B, and LDL levels in obese and lean men.Methods: A cross-sectional study was conducted on 74 men (obese and lean) at the Faculty of Medicine, Diponegoro University, Indonesia, in 2020. The concentration of LDL was measured using the homogenous enzymatic colourimetric method, whereas the levels of Lp-PLA2 and Apo B were determined using the ELISA method. Data were analyzed using an Independent t-test, setting statistical significance at p <0.05.Results: This study showed that Lp-PLA2 levels were significantly different between obese and lean men (p = 0.039). Furthermore, LDL levels were also significantly different between obese and lean men (p = 0.002). However, we did not find any differences in Apo B between obese and lean men (p = 0.640).Conclusion: Lp-PLA2 and LDL levels were slightly higher in obese compared to lean men, but no difference of Apo B.

Abstract

Diabetic Nephropathy (DN) is becoming the most serious microvascular complication, which be marked by the presence of persistent albuminuria. N–asetil–β–glucosaminidase is dominant lyzosom enzyme in the renal tubule epitel. β2 microglobulin is low molecular weight protein which produced by major histocompatibility complex class 1 (MHC-1) expressed cell in all nucleated cell. N–asetil–β–glucosaminidase and β2 microglobulin could be new usefull marker for early DN. Analytic observational study with cross sectional approach was conducted in May – July 2019 involving 27 non diabetic patients (K1), 27 diabetic patients without DN (K2) and 27 diabetic patients with early DN (K3) at the Clinical Pathology department of Faculty of Medicine, Diponegoro University and Diabetic Clinic. Data include age, gender, fasting blood glucose, blood preasure and urine albumin creatinine ratio. N–asetil–β–glucosaminidase level between groups were analyzed using Anova, β2 microglobulin level between groups using Kruskal Wallis, p<0.05 were considered significant. There are significant differences in levels of N–asetil–β–glucosaminidase between K1 and K2 (p =0.01), K1 and K3 (p =< 0.01), K2 and K3 (p = 0.03) and β2 microglobulin  between K1 and K2 (p = 0.02), K1 and K3 (p =< 0.01), K2 and K3 (p< 0.01). N-acetyl-β-glucosaminidase and β2 microglobulin levels were higher in K2 compared to K1 and increased higher in K3 compared to K1 and K2. N-acetyl-β-glucosaminidase and β2 microglobulin can be used as an alternative marker for early DN.

Abstract

Penelitian ini bertujuan untuk menganalisis bagaimana pemenuhan hak atas informasi dan hak kesehatan reproduksi perempuan penyandang disabilitas di Kota Semarang. Terbatasnya akses informasi dan sempitnya ruang gerak bagi pendidikan kesehatan reproduksi serta kurangnya fasilitas kesehatan yang ramah bagi penyandang disabilitas di Indonesia berdampak terhadap terjadinya kekerasan seksual bagi perempuan penyandang disabilitas. Kota Semarang merupakan salah satu kota dengan jumlah kasus ketidakadilan penyandang disabilitas yang tinggi. Penelitian ini merupakan penelitian yuridis sosiologis dengan spesifikasi penelitian deskriptif analitis dengan data yang digunakan adalah data primer dan sekunder yang dianalisis secara kualitatif. Hasil penelitian ini menunjukkan bahwa hak atas informasi bagi perempuan penyandang disabilitas di Kota Semarang sudah terpenuhi namun, hak kesehatan reproduksi bagi perempuan penyandang disabilitas di Kota Semarang belum terpenuhi karena kurangnya anggaran dana dari pemerintah, tidak adanya sosialisasi seputar kesehatan reproduksi dari lembaga terkait, kurangnya kompetensi tenaga kesehatan untuk berkomunikasi dengan penyandang disabilitas, kurang berpartisipasinya masyarakat dalam mendukung penyandang di disabilitas, tidak adanya informasi kesehatan reproduksi yang diberikan kepada para penyandang disabilitas, serta tidak terlaksananya kebijakan yang telah dibuat.

Abstract

Thalassemia major patients with routine transfusions are at risk of experiencing iron overload and bone turn over disorders which will increase the risk of osteoporosis. Community training activities with BMD examinations and osteoporosis exercise training in thalassemia major patients are expected to identify the prevalence of bone fragility in thalassemia major patients, increase the awareness of thalassemia major patients and their families to the risk of osteoporosis in thalassemia major patients and osteoporosis exercise training as an effort to prevent osteroporosis. The results of BMD on the spine showed 57.1% osteoporosis, 28.6% normal and 14.3% osteopenia. The results of BMD examination on the cervical spine showed 71.4% osteopenia, 21.4% normal and 7.2% osteoporosis. The results of BMD examination on the femur found 50% normal, 42.8% osteopenia and 7.2% osteoporosis. The bone location with the highest percentage of osteoporosis was the spine (57%), and the bone location with the highest percentage of osteopenia was the neck bone (71.4%). The high percentage of osteopenia locations in the cervical spine of thalassemia major patients with repeated transfusions should be become a concern. Based on these findings, we held education and counselling on the risk of osteoporosis in thalassemia patients with repeated transfusions, osteoporosis exercise training and distribution of osteoporosis exercise compact disk (CD) for at home osteoporosis exercise training.

Abstract

Background: Obesity is one of global epidemic health problems and its prevalence is higher among women. Obesity can cause low grade chronic inflammation mechanism in adipose tissue, which is characterized by the increase of proinflammatory cytokines and adipokines. Neutrophil lymphocyte ratio (NLR) is a simple inflammatory marker which can be reliable in evaluating the inflammatory status occurring in obese women. Waist to height ratio (WHtR) and waist to hip ratio (WHR) are anthropometric measurements, have been reported to be associated with obesity and risk of metabolic syndrome.Objective: This study aimed to determine the correlation of WHtR and WHR with NLR in population of obese women.Materials and Methods: This was a cross sectional study enrolling 80 obese women with Body mass index (BMI) > 27 aged 30 - 50 years in National Diponegoro Hospital Semarang, Indonesia. WHtR was determined by dividing waist circumference by height and WHR was determined by dividing waist circumference by hip circumference. NLR was examined manually from automatic hematology analyzer by dividing absolute neutrophil count (ANC) and absolute lymphocyte count (ALC). Spearman correlation test was performed, p<0.05 was considered as statistically significant.Results: There was significant weak positive correlation between WHtR and NLR in obese women (p = 0,046; r = 0,224). There was no significant correlation between WHR and NLR in obese women (p = 0,961; r = 0,006).Conclusion: The present study showed that WHtR is one of better anthropometric measurement because it is associate with NLR as a simple marker of inflammation in obese women.

Abstract

Background: The most restoration material used currently in dentistry was composite resin. The newest generation of composite resin was bulk-fill composite resin. The bulk-fill composite resin had advantages of lower polymerization contraction and a more straightforward and faster restoration technique. One of the factors which could affect the hardness of composite resin was the consumption of acidic drinks. The acidic beverage that people often consumed is coffee. A few coffee brewing methods were espresso and drip brewing.Objective: This research aimed to identify the effects of coffee soaking through espresso and drip brewing methods on bulk-fill composite resin's hardness.Methods: his research was experimental research, with a post-test control group design and a total sample of 27 samples. The soaking process was taken for seven days at a temperature of 37˚C on artificial saliva, espresso, and drip brewing coffee methods. The hardness of bulk-fill composite resin was measured after seven days by using Vickers hardness. The statistic test exerted One-Way ANOVA.Results: This research found a significant difference in bulk-fill composite resin's hardness among all groups, p-value = 0.00 (p<0.05). The result of the post-hoc LSD test showed a significant difference in the change of bulk-fill composite resin hardness between the artificial saliva group and espresso group (p=0,00) and no significant difference found between the artificial saliva group and drip brewing group (p=0.85).Conclusion: Soaking with espresso coffee has the smallest hardness value compared to soaking with drip brewing coffee and artificial saliva, so soaking with espresso coffee is the most influential on the hardness of the bulk fill composite resin.

Abstract

BACKGROUND: Transthyretin protein experiences misfolding and aggregation in preeclampsia due to placental ischemia and inflammation. Placental endothelial damage occurring in preeclampsia stimulates the production of larger young platelets in the bone marrow and can be reflected by an increase in the immature platelet fraction (IPF) and mean platelet volume (MPV). Since the change in these markers in preeclampsia remains controversial, this study was conducted to analyze the differences in levels of transthyretin, IPF, and MPV as easily accessible markers in normotensive and preeclampsia pregnancies.METHODS: Total subjects included in this study were 32 normotensive and 26 preeclampsia pregnant woman. The measurement of serum transthyretin level was using enzyme-linked immunosorbent assay (ELISA) method. Meanwhile, IPF and MPV levels were measured by using a hematology analyzer with flowcytometry method. Data was statistically analyzed using unpaired T-test and Mann-Whitney, with significancy of p<0.05.RESULTS: Median value of transthyretin in normotensive and preeclampsia pregnancy were 11 (6-30) mg/dL and 9 (5-18) mg/dL. The mean value of IPF in normotensive and preeclampsia pregnancy were 4.56±2.19% and 6.33±2.99%. Mean value of MPV in normotensive and preeclampsia pregnancy were 10.41±0.76 fL and 11.32±1.2 fL. There were significant differences in levels of transthyretin, IPF, and MPV between normotensive and preeclampsia pregnancies (p=0.008, p=0.017, and p=0.002, respectively).CONCLUSION: There are significant differences in transthyretin levels, IPF, and MPV values in normotensive and preeclampsia pregnancies. Therefore, serum transthyretin, IPF, and MPV can be developed as affordable parameters for the diagnosis of preeclampsia.KEYWORDS: preeclampsia, transthyretin, IPF, MPV
